Effects of methylmercury chloride (MMC) on circadian sleep-waking rhythms were examined in rats which had been chronically implanted with EEG and EMG electrodes. Bihourly distributions of wakefulness (W), slow wave sleep (SWS) and paradoxical sleep (PS) and 12-h amounts of W, SWS and PS during light and dark periods were measured before and after MMC administration for 2 successive days at 3 dose levels. A total dose of 10 mg MMC/kg body wt was found to be the threshold for inducing reversible changes in the sleep-waking patterns. A total dose of 30 mg MMC/kg produced an increase in both dark-phase SWS and PS as well as a decrease in light-phase PS at the expense of an increase in light-phase W and a delayed phase of the circadian PS rhythm. The delayed phase of the PS rhythm tended to persist after the increased SWS during the dark period returned to normal. Brain mercury concentrations were measured in order to find the dose-response relationship and the time dependence of the MMC-induced sleep disorder. The sleep-waking disorder was found to appear at lower levels of brain Hg and shorter latency than behavioral disorders of movement and postural maintenance previously reported [5-8].
2 cases of verrucous carcinoma of the oral cavity are presented. In addition, 19 Japanese patients of verrucous carcinoma of the oral region from the literature are reviewed with respect to several clinical items.
Four men (M age 90.5 years) and four women (M age 91.2 years) in Vilcabamba, Ecuador (a community noted for people with extreme longevity) volunteered for sleep recordings using four-channel Medilog portable cassette recorder. Of the eight physically healthy persons recorded, only one man displayed sleep apnea syndrome. No nocturnal myoclonus was observed. Low incidence of sleep apnea of elderly persons in Vilcabamba may be due to a low barometric pressure and/or lack of drug abuse.

Of 10 epileptic children with IgA deficiency, one showed normal IgA synthesis and secretion on in vitro pokeweed mitogen (PWM) stimulated lymphocyte culture in contrast to IgA deficiency in vivo, two showed IgA synthesis in cytoplasm without any release of IgA into the supernatant and seven failed to synthesize IgA. Co-cultures with allogeneic T or B cells in various combinations with PWM showed intrinsic IgA-B cell defect without T cell defect in two of the second group affected at IgA secretion and in five of the third group, and intrinsic IgA-B defect with dysfunction of T cells in two of the third group. Thus, the IgA deficiency in these epileptic patients was demonstrated to be heterogenous.

The ability to photograph the interior of the bladder accurately just by pressing a button has great clinical merit. This approach, which has become a basic method in gastrointestinal endoscopy, is now possible in cystoscopy. We will call this technique which uses a newly designed telescope, Olympus A 3405 and Polaroid Instant Endocamera EC-3 with Polaroid High Speed Color Land Film 600, photocystoscopy . The telescope used in photocystoscopy has more improved resolution and homogeneous brightness through its field of view than the conventional telescope, Olympus O 3405. The photographic image of the bladder interior taken by the Polaroid camera has a good color and fine resolution but its size which is 2.5 cm in diameter is too small for clinical use. Color instantographs by the Polaroid camera are most appropriate for use in photocystoscopy , but many devices and improvements are necessary before it can be used routinely.

Vascular changes in rat skin after irradiation were examined microangiographically. Revascularization of the skin transplanted during the chronic stage after irradiation was also studied. The results obtained through these examinations revealed higher vascular densities at the acute and the subacute stages, and low values at the chronic stages compared with those of the control. Furthermore, when the skin grafts were transplanted to the irradiated beds in the chronic stage, primary revascularization was scant, and the inhibited capillary proliferation in the recipient sites prevented new vessel penetration. This explains to why grafts transplanted to previously irradiated beds fail to survive.

Recurrent ST-segment elevations in leads where new Q waves developed were repeatedly recorded in six patients during a recovery phase of acute myocardial infarction. Such ST-segment elevations were transient, occurred with or without chest pain, and returned to control levels. No enzymatic changes signifying recurrent myocardial necrosis were found after each episode. Selective coronary cineangiography in one patient demonstrated a mild segmental stenosis in the coronary artery perfusing the infarcted area; this artery became completely occluded after administration of i.v. ergonovine. Administration of calcium antagonists effectively reduced the frequency of postinfarction angina and ST-segment elevations. The clinical features suggest that the postinfarction angina in these patients is produced by coronary arterial spasm and that coronary arterial spasm may cause severe life-threatening dysrhythmias.
Angiographically demonstrable collateral flow occurred immediately when coronary arterial spasm was induced by ergonovine in two patients with angina who had organic stenosis of less than 40 percent. These findings suggest that coronary collaterals may develop in response to intermittent brief myocardial ischemia in man, and that collaterals may be preserved even if they are closed at rest and can immediately function when a coronary artery is acutely occluded.
