Plasma cholecystokinin responses after ingestion of liquid meal and intraduodenal infusion of fat, amino acids, or hydrochloric acid in man: analysis with region specific radioimmunoassay.

A highly sensitive and precise radioimmunoassay system for plasma cholecystokinin (CCK) was developed with the anti-CCK-8 specific antiserum which raised against N-terminal amino acids residue of sulfated CCK-8 and reacted with CCK-8, CCK-33, and CCK-39 but not with gastrin and its related peptides. Mean concentration of the fasting plasma CCK determined with this method using CCK-8 as standard was 12.9 +/- 5.9 pg/ml in normal subjects (n = 26), and in patients with hepatic cirrhosis it was significantly higher (36.7 +/- 16.9 pg/ml, n = 9, p less than 0.01) than in normal subjects. In six young healthy volunteers, intraduodenal infusion of fat caused a significant increase (p less than 0.05) of plasma CCK from a basal level of 8.0 pg/ml to a peak of 43.0 +/- 12.0 pg/ml at 20 min after starting of infusion. In the same subjects, a significant increase of plasma CCK was also observed by amino acids infusion, but no elevation of plasma CCK level was found during intraduodenal acidification.

A novel technique for the determination of body fat by computed tomography.

We have developed a new method of measuring fat volume of the human body, using computed tomographic scanning (CT scan) - no ideal or direct method has been available for this calculation. In our study a human body is divided into 11 cylindrical shapes, ie a head, a chest, an abdomen, both sides of forearms, upper arms, thighs and calves to be subjected to the measurement. Eleven computed tomographic sections were examined at the middle point of each segment to obtain the areas of fat tissue. Volume of each cylindrical segment was calculated by multiplying a cross-sectional area of fat tissue by the height of each part. Then the volume of fat tissue of the whole body was determined by the sum of fat tissue volume of eleven parts (as above). By this method, nine massive obese subjects with 147-188 percent of ideal body weight were found to possess fat tissue occupying 30-63 percent of total body volume. We also found female obese patients had a significantly higher percentage of fat tissue than male obese patients when they had same degree of obesity. In addition to the study above, we examined 18 control and 19 obese subjects and measured subcutaneous fat, muscle plus bone and visceral fat separately by CT scan at the umbilicus level. In obese subjects, increased abdominal fatty mass was mainly due to the accumulation of subcutaneous fat, although substantial increase of visceral fatty mass was demonstrated in a few cases.

Radioimmunoassay for antibodies to human skeletal muscle myosin in serum from patients with polymyositis.

Antibodies to human skeletal muscle myosin were detected in 90% of sera from patients with polymyositis by radioimmunoassay using purified human myosin as antigen, and the mean titre was 4.24 X 10(-10)M. The incidence and the mean titre of anti-myosin antibodies were significantly higher than in patients without polymyositis. Anti-myosin antibody titres correlated with steroid therapy in polymyositis patients. Titres in untreated patients with polymyositis correlated with the severity of muscle weakness. Radioimmunoassay for anti-myosin antibodies should prove to be useful in the diagnosis of polymyositis and the evaluation of clinical state.

"Nonalcoholic steatohepatitis" induced by massive doses of synthetic estrogen.

We have noticed that functional disorders of the liver characterized by hepatomegaly and an increase in serum gamma-glutamyl transpeptidase develop in patients with prostatic cancer who are placed under longterm therapy with massive doses of estrogen after castration. We performed laparoscopy in six cases of prostatic cancer with hepatomegaly so that we could study the morphology of the liver. Our findings were as follows. In five, the histological features of the liver biopsies were very similar to those seen in alcoholic hepatitis. In spite of this fact, two of the five had no history of alcohol consumption. Furthermore, in one other case, liver damage resembling alcoholic hepatitis developed during abstinence. The findings in these three cases suggested that long-term, massive doses of synthetic estrogen may lead to liver injury similar to alcoholic hepatitis in nonalcoholics. The ultrastructural findings of the liver cells were also suggestive of the adverse effect of treatment. All cases were negative for hepatitis B surface antigen. Recent reports have demonstrated some nonalcoholics with histological features of the liver indicative of alcoholic hepatitis. This particular condition was termed "nonalcoholic steatohepatitis" by Ludwig et al. It is quite likely that synthetic estrogen is also responsible for "nonalcoholic steatohepatitis" when it is used in massive doses.

Immunoglobulin production and secretion in Bence Jones protein myeloma and "nonsecretory" myeloma. Ultrastructural and immunofluorescence study.

Using our electron microscopic method for polysome analysis and an immunofluorescent technique we studied Ig production and secretion by tumor cells in seven BJP myeloma patients and seven "nonsecretory" myeloma patients. In BJP myeloma Ig production and secretion is of three types: Type 1, only L-chains are synthesized and secreted; Type 2, the myeloma cells show fluorescence for H-chains, but upon polysome analysis there is no peak at polysomes corresponding H-chain production; Type 3, the myeloma cells show fluorescence for H-chains, and polysome analysis shows two peaks corresponding to L- and H-chain production. Polysome analysis of "nonsecretory myelomas show the presence of only very few membrane-bound polysomes and their distribution curves are entirely different from those of "ordinary" myeloma. Furthermore, the distribution patterns vary among seven cases. Results obtained by polysome analysis and immunofluorescent technique suggest that the "nonsecretory" myeloma could be divided into several subtypes.

Hypertrophic and hypoactive smooth endoplasmic reticulum in hepatocytes uremic patients. A morphometric and biochemical study.

Morphometric analysis of the hepatic endoplasmic reticulum and assays of drug-metabolizing enzymes in microsomes prepared from needle biopsy specimens were performed with samples from uremic patients on chronic hemodialysis and normal controls. The smooth endoplasmic reticulum (SER) morphometrically increased in the uremic patients, whereas the cytochrome P-450 content and the activity of p-nitroanisole O-demethylase per mg microsomal protein decreased in the uremic patients. This condition of SER corresponds to that of the so-called hypoactive hypertrophic smooth endoplasmic reticulum, which could suggest lower activity of hepatic microsomal drug oxidation in uremic patients. On the other hand, the P-450 content and the activity of p-nitroanisole O-demethylase per g liver were not significantly different between the uremic and the normal subjects. This seems to indicate that the capacity of drug oxidation is retained in whole livers of uremic patients. However, since some patients in this study showed markedly low activity of p-nitroanisole O-demethylase per mg microsomal protein and per g liver, lipophilic drugs metabolized in the liver as well as hydrophilic ones eliminated by the kidney should be carefully administered to uremic patients.

Coronary artery disease in heterozygous familial hypercholesterolemia.

Serum lipids, lipoproteins and Achilles tendon thickness in 52 patients with heterozygous familial hypercholesterolemia (FH) were investigated in order to clarify what are the important factors for the development of coronary artery disease (CAD) in heterozygous FH patients. There were no significant differences in the average concentration of total cholesterol and triglyceride between the patients with and those without CAD. The HDL cholesterol (HDL-C) level was significantly lower in patients with CAD than in those without, and the HDL-C value was within the normal range in most of the patients with heterozygous FH, if not associated with CAD. Although most of the males aged over 50 years had CAD and a decreased level of HDL-C, many of the aged females were without signs of CAD. The HDL-C value of heterozygous FH patients with CAD was significantly lower compared with the age-matched group without CAD. The Achilles tendon was thicker in patients with CAD than in those without CAD, both for males and females, although it was less closely correlated with the incidence of CAD than HDL-C or the atherogenic index. A forecast concerning the development of CAD in heterozygous FH may be possible if we consider multiple parameters, such as HDL-C, atherogenic index, Achilles tendon thickness, etc.

Comparison of diurnal serum insulin levels during short term treatments with sulfonylurea and with insulin in non-insulin dependent diabetes.

Nine non-insulin dependent type diabetic patients were hospitalized and place on the standard diabetic diet throughout this study period. (25 cal/kg ideal weight/day). After three days of diet therapy alone, 5 mg of glibenclamide was given orally for three days, followed by an interval of four days without medication. 16 units of lente insulin was then administered subcutaneously for three days. The diurnal levels of plasma glucose (PG) and serum immunoreactive insulin (IRI) were determined on the third day of each treatment, and the integrated areas below the curves of the diurnal profile of PG and IRI were compared. The mean fasting PG level before treatment was 200 +/- 21 mg% (mean +/- SE). The initial PG area during diet therapy alone, (4,245 +/- 669 mg . h/dl) decreased significantly to similar levels both by glibenclamide (3, 317 +/- 384 mg . h/dl, P less than 0.05) and by lente insulin (3,177 +/- 552 mg . h/dl, P less than 0.01). The IRI area increased from 187 +/- 24 micromicron . h/ml during diet therapy alone to 296 +/- 65 micromicron . h/ml by glibenclamide (P less than 0.01), and to 267 +/- 43 micromicron . h/ml by lente insulin (P less than 0.05). There was no significant difference between glibenclamide and lente insulin treatments either in PG area or IRI area. These finding suggest that the hypoglycemic effect of glibenclamide treatment in the short term is mainly, if not entirely, due to augmented endogenous insulin secretion.

Laparoscopic findings of the liver in the diagnosis of primary biliary cirrhosis: "reddish patch", a laparoscopic feature in the asymptomatic stage.

The laparoscopic findings of the livers of 12 patients with primary biliary cirrhosis (PBC), including six asymptomatic and six symptomatic cases, were investigated. On the liver surface of asymptomatic PBC, we found a laparoscopic appearance, consisting of reddish patches of irregular shape, ranging from 0.5 to 3 cm in diameter, and not evenly distributed. Five of six cases of asymptomatic PBC had reddish patches in our series. From serial laparoscopic observations, the patches were found to lose some of their reddish tone and to develop protuberances with vaguely outlined white markings in each reddish areas. The protuberances and white markings are characteristic appearances of the liver surface in more advanced PBC cases. The transition of reddish patches to protuberances with white markings coincided with progression among histopathological stages in PBC classified by Scheuer. A histometrical examination showed that areas with reddish patches had less advanced changes of intrahepatic bile ducts. As a result, we propose that laparoscopic findings of the liver have a value in the diagnosis of PBC and "reddish patch" is considered a laparoscopic feature which has no been described in asymptomatic PBC.

Stimulatory effect of cytochrome b5 induced by p-nitroanisole and diisopropyl 1,3-dithiol-2-ylidenemalonate on rat liver microsomal drug hydroxylations.

The relationship between the changes in the amount of the components of the liver microsomal electron transport systems and drug hydroxylase activities on administration of p-nitroanisole was investigated. The content of cytochrome P-450 in the p-nitroanisole-treated rats was not significantly different from that in the controls during the treatment. The cytochrome b5 content increased 2.2-fold over that of the controls after 14 days of treatment. Demethylation activities per mg microsomal protein of p-nitroanisole, aminopyrine, and benzphetamine were enhanced 2.7-, 2.4-, and 2.8-fold, respectively, but aniline hydroxylase activity was not enhanced. Similar results were obtained with microsomes from diisopropyl 1,3-dithiol-2-ylidenemalonate (NKK-105)-treated rats. The time-course study of changes in the amount of the components of drug hydroxylase systems suggested that the increase in cytochrome b5 content enhanced the demethylation activities. In the reconstituted system containing cytochrome P-450 partially purified from p-nitroanisole- or NKK-105-treated rats, cytochrome b5 was required for the maximal activities of the demethylation reactions, but did not participate in aniline hydroxylation. These findings showed good correlation between the change of cytochrome b5 content and the demethylation activities and suggested that the increase in cytochrome b5 content might enhance the demethylation activity by stimulating supply of a second electron to cytochrome P-450.

Mechanism of inhibition of hepatic triglyceride lipase from human postheparin plasma by apolipoproteins A-I and A-II.

The present data describe the mechanism of the inhibitory effects of human plasma apolipoproteins A-I and A-II on hydrolysis of triglyceride catalyzed by hepatic triglyceride lipase using a substrate of triolein particles stabilized with gum arabic in vitro. The experimental data could well be described by a model in which apolipoproteins bound to the surface of lipid substrate particles inhibited the enzyme reaction. The values of Km obtained were similar with or without inhibitors and the calculated saturation levels of apolipoprotein binding to the lipid were in good agreement with those obtained in independent binding experiments.

Effects of iodide and propylthiouracil on the release of 3,5,3'-triiodothyronine and of cyclic adenosine 3',5'-monophosphate from perifused rat thyroids.

We have established a perifusion system using rat thyroid. With this system, we analyzed the effects of TSH, 3-isobutyl-1-methylxanthine (IBMX) and iodide on the release of T3 and of cAMP, paying special attention to the relation between the release of the two substances. During perifusion, which was continued for 7 h, T3 release increased progressively with time and in a dose-related manner when TSH was added at concentrations of 0.1-10 mU/ml. However, cAMP release was unappreciable even in the presence of 10 mU/ml TSH. The release of T3 and cAMP was markedly enhanced by 3 x 10(-4) M IBMX. When iodide was added, a marked increase in cAMP release was unexpectedly observed. However, a slight but significant suppression of TSH-stimulated T3 release was shown with 1 x 10(-3) M iodide. TSH-stimulated T3 release was almost completely inhibited by 1 x 10(-3) M 6-n-propyl-2-thiouracil, but such a complete inhibition did not occur with 2-mercapto-1-methylimidazole. The cAMP release stimulated by IBMX was not affected by 6-n-propyl-2-thiouracil, but that stimulated by iodide was effectively inhibited. The present studies indicate that TSH and IBMX enhance T3 release, but only IBMX increases cAMP release. Iodide also results in a marked increase in cAMP release but does not affect T3 release from unstimulated thyroid and inhibits T3 release from TSH-stimulated thyroid. We suggest that there is not necessarily any close correlation between T3 release and cAMP release into perifusates of the rat thyroid.
