[Aneurysm of the anterior inferior cerebellar artery at the internal auditory meatus: report of two cases].

Aneurysm of the vertebrobasilar system is not a rare lesion but constituting 5-10% of all intracranial aneurysms. Aneurysm at the peripheral part of the anterior inferior cerebellar artery (AICA), however, is rare and only 20 cases have been reported previously. In this report, two cases of aneurysm arising at the junction between the AICA and internal auditory artery and extending into the internal auditory meatus are reported. The first patient was a 51-year-old male who was admitted because of sudden onset of dizziness, rt. tinnitus and deafness. Left vertebral angiograms demonstrated an AVM in the rt. cerebellar hemisphere and two aneurysms on the feeding arteries, one on the superior cerebellar artery and the other on the meatal loop of the rt. AICA. Removal of the AVM and neck clipping of the aneurysms, which were found unruptured, were performed through the rt. suboccipital approach. The second patient was a 42-year-old female who was admitted because of sudden onset of severe headache and vomiting followed by rt. tinnitus, deafness and double vision. CT scan showed subarachnoid clot mainly located in the rt. cerebellopontine angle. Repeated vertebral angiograms revealed an aneurysm at the meatal loop of the rt. AICA. Complete neck clipping was carried out. The clinical features of the aneurysms of this location can be divided into three subgroups according to the characters of onset: sudden onset with subarachnoid hemorrhage, insidious onset of the VII th, VIII th, nerve palsies by the mass effect of the aneurysm, intermittent episodes of the VIII th nerve disfunction due to the insufficiency of the internal auditory artery.(ABSTRACT TRUNCATED AT 250 WORDS)

[Pituitary abscess: report of two cases and a review].

Two cases of pituitary abscesses are reported. The first case was a 22-year-old female who was admitted with 14 months history of intermittent fever and headache followed by amenorrhea and polyuria. Plain craniograms showed no remarkable changes on the sella turcica. Direct coronal CT demonstrated a cystic lesion with a ring-like enhanced capsule in the pituitary complex with the apparent bulging of the pituitary top surface. On dynamic CT, the cystic lesion showed no remarkable increase in density. Transsphenoidal surgery was performed and a purulent materials was drained by cutting the underlying fibrous pituitary tissue. Pathological specimen revealed a marked inflammatory reaction around the degenerative pituitary gland. The second case was a 27-year-old female who underwent transsphenoidal excision of craniopharyngioma on May 12, 1983. Postoperatively she did well with the hormonal replacement therapy until 9 months after surgery when a follow up direct coronal CT revealed a cystic lesion with ring-like enhancement in the pituitary fossa. Purulent material was found at transsphenoidal surgery. Pathological specimen from the hard capsule was a chronic inflammatory fibrous tissue without tumorous component. Over seventy cases of pituitary abscesses have been reported previously, but the majority of them were secondary infections associated with systemic sepsis, chronic sphenoid sinusitis or pituitary tumors. Primary pituitary abscesses were rare and would be difficult to be diagnosed preoperatively without an aid of recent CT technology. Thirty cases in the literature since 1970 are reviewed and its clinical pictures and radiological findings are discussed.

[A case of pineal teratoma with intraventricular free fat seen in CT scan].

Detection of an intraventricular or intratumoral fat-fluid level on the plain craniograms has been known as a characteristic sign indicating the presence of intracranial teratomatous tumors. On CT scans, however, only thirteen cases have been previously reported to be found an intraventricular and/or subarachnoid free fat associated with spontaneous ruptures of these tumors. We reported a case of pineal teratoma with intraventricular free-fat seen on CT scans. A nine-year-old male with precocious puberty was admitted to our hospital complaining a moderate nonpulsatile headache. Neurological examinations were normal without signs of meningeal irritation. The serum and CSF titer of HCG were raised markedly. The laboratory data of the CSF were normal and there were no pathological cells in the CSF. The CT scans revealed a large heterogeneous mass containing multiple areas of negative density in the pineal region. There were negative density droplets in the bilateral frontal horn on the same CT scans indicating a presence of free fats. At surgery, an yellowish oily material was drained from the tumor, but there was no sign of meningitis over the cortical surface of the occipital lobe. An intraventricular free fat on CT scan have been reported in fourteen cases including ours following the first case described by Fawcitt in 1976. Although most of the cases presented headache, only two cases was diagnosed clinically as chemical meningitis. Pathological changes indicating granulomatous meningitis, however, were noted in five cases, all of them presenting seizure attacks.

The effects of alpha-adrenergic stimulation on the regulation of the pyruvate dehydrogenase complex in the perfused rat liver.

The regulation of the pyruvate dehydrogenase multienzyme complex was investigated during alpha-adrenergic stimulation with phenylephrine in the isolated perfused rat liver. The metabolic flux through the pyruvate dehydrogenase reaction was monitored by measuring the production of 14CO2 from infused [1-14C] pyruvate. In livers from fed animals perfused with a low concentration of pyruvate (0.05 mM), phenylephrine infusion significantly inhibited the rate of pyruvate decarboxylation without affecting the amount of pyruvate dehydrogenase in its active form. Also, phenylephrine caused no significant effect on tissue NADH/NAD+ and acetyl-CoA/CoASH ratios or on the kinetics of pyruvate decarboxylation in 14CO2 washout experiments. Phenylephrine inhibition of [1-14C]pyruvate decarboxylation was, however, closely associated with a decrease in the specific radioactivity of perfusate lactate, suggesting that the pyruvate decarboxylation response simply reflected dilution of the labeled pyruvate pool due to phenylephrine-stimulated glycogenolysis. This suggestion was confirmed in additional experiments which showed that the alpha-adrenergic-mediated inhibitory effect on pyruvate decarboxylation was reduced in livers perfused with a high concentration of pyruvate (1 mM) and was absent in livers from starved rats. Thus, alpha-adrenergic agonists do not exert short term regulatory effects on pyruvate dehydrogenase in the liver. Furthermore, the results suggest either that the rat liver pyruvate dehydrogenase complex is insensitive to changes in mitochondrial calcium or that changes in intramitochondrial calcium levels as a result of alpha-adrenergic stimulation are considerably less than suggested by others.

Determination of cyanide and thiocyanate in blood plasma and red cells by high-performance liquid chromatography with fluorometric detection.

A method for the determination of cyanide and thiocyanate in blood plasma and red cells of humans was established. It involved high-performance liquid chromatography and fluorometric detection by the König reaction. Calibration curves for cyanide and thiocyanate were linear in the range 1-200 pmol and 2-300 pmol, respectively. Clean-up methods for the determination of cyanide and thiocyanate in red cells were also developed. These methods were applied for the determination of cyanide and thiocyanate in the blood of smokers and non-smokers.

A case report of pancreatic metastasis of an intracranial angioblastic meningioma (hemangiopericytoma) and a review of metastatic tumor to the pancreas.

The authors report a case of solitary metastasis to the pancreas from an intracranial angioblastic meningioma (hemangiopericytoma) and discuss secondary (metastatic) tumors of the pancreas. A 65-year-old man developed a pancreatic tumor 15 years after the craniotomy was performed for a parasagittal meningioma. Histological investigation of the pancreatic tumor excised as a biopsy on exploratory laparotomy revealed a tumor identical to the intracranial meningioma. It was concluded that the intracranial angioblastic meningioma had recurred and metastasized to the pancreas.

A case of 17 alpha-hydroxylase deficiency syndrome associated with right adrenal tumor.

A 35-year-old woman, who had been hypertensive for about 17 years and had lacked menarche, showed hypokalemia, low plasma cortisol and aldosterone levels, suppressed renin activity, and marked elevation of plasma corticosterone. The patient was diagnosed as having 17 alpha-hydroxylase deficiency from functional studies. In addition, a right adrenal tumor was found by adrenal venography. Adrenal venous sampling showed that this tumor might be secreting corticosterone and possibly also deoxycorticosterone (DOC). The genotype was 46,XY, so she was diagnosed as having male pseudohermaphroditism. Right adrenalectomy and contralateral adrenal biopsy were done. The retained testicles were removed. Dexamethasone administration normalized the blood pressure and serum potassium. This is the first report of 17 alpha-hydroxylase deficiency with a right adrenal tumor.

[Diagnosis of pituitary microadenoma by magnification carotid angiography and intercavernous sinus venography].

Carotid angiography was performed on 12 cases of pituitary microadenomas at 2.3 X magnified frontal projection and 3.0 X magnified lateral projection. Magnification carotid angiograms were analyzed to determine the pathologic findings in 12 cases on 23 sides. Carotid angiograms demonstrated compressed posterior pituitary gland in 3 of 12 cases and 4 of 23 sides which resulted from the expansion of pituitary microadenomas. But carotid angiogram failed to demonstrate any evidence of tumor stain, abnormal capsular arteries or hypertrophied inferior hypophyseal arteries. These results suggested that magnification carotid angiography was not a useful diagnostic procedure for the detection of pituitary microadenomas. Intercavernous sinus venography was performed on 12 cases of pituitary microadenomas. The technique consisted in catheterization into the inferior petrosal sinus after puncture of the femoral vein. Injection of 10 ml. of contrast medium into the inferior petrosal sinus at a rate of 5 ml. per second. The base projection is most demonstrable and routinely used. Intercavernous sinus venography identified the presence of microadenoma in 10 of 12 cases and false negative in two cases. Nine of 12 cases demonstrated narrowing, interruption and disappearance of the anterior and inferior intercavernous sinuses. Only 3 of 12 cases demonstrated narrowing of the posterior intercavernous sinus and nine cases demonstrated compression signs of the venous strips of the cavernous sinus.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of pH on the formation of Tc(NaBH4)-MDP radiopharmaceutical analogues.

Anion exchange high performance liquid chromatography has been employed to separate components of 99mTc(NaBH4)-MDP formulations prepared as a function of pH, the presence or absence of 99Tc carrier, and the presence or absence of air. The formulation pH is an effective variable for controlling the generation and interconversion of 99mTc-MDP components, and for allowing the preparation of large quantities of specific 99mTc-MDP complexes for biological evaluation.

The biological distributions of some technetium-MDP components isolated by anion exchange high performance liquid chromatography.

Individual components of 99mTc(NaBH4)-MDP mixtures, separated by anion exchange HPLC, have been evaluated as skeletal imaging agents in rats. Biodistribution data show that the evaluated components exhibit markedly different bone uptakes and soft tissue localizations. The component with the highest bone uptake and the highest bone/muscle and bone/blood ratios, is the major component of carrier-added 99mTc(NaBH4)-MDP formulations prepared at pH 8.2. Comparative scintiphotos illustrate the enhanced imaging performance of this component vs unseparated reaction mixtures.
