[Diagnosis of pituitary adenoma by dynamic CT scanning: concerning direct sign and indirect sign for diagnosis of pituitary microadenoma].

The advantage of high resolution CT in the diagnosis of pituitary microadenomas has been established, but the diagnosis becomes more difficult when the pituitary microadenoma is less than 5mm in diameter. We have studied the usefulness of dynamic CT scans particularly for diagnosis of small microadenomas. The dynamic CT scans were performed for 61 normal pituitary glands and 68 pituitary adenomas (36 microadenomas, 32 macroadenomas) with a GECT/T 9800 scanner. Coronal sections of 1.5mm thickness were taken at the plane just in front of the pituitary stalk of the pituitary gland. Following a bolus intra-venous injection of 40 - 60 ml of contrast media using an automatic injector, ten consecutive CT scans of 2 seconds scan time were obtained, beginning 2 seconds from the start of intravenous injection. The first seven scans were taken with an interval of 2.3 seconds, and the last three scans with an interval of 10 seconds. Then, time-density curves were obtained at the ROI which were set on the anterior pituitary gland, the vascular bed of the pituitary gland, the pituitary stalk and the area of the pituitary adenoma respectively. In a normal pituitary gland, the density increases gradually and makes an S shaped curve, then attains the maximum density value (92.3 CT number) approximately 60 seconds after the administration of contrast media. The pituitary vascular bed is located in midline on the upper surface of the pituitary gland, and shows a symmetrical square, triangular or rhomboid shape.(ABSTRACT TRUNCATED AT 250 WORDS)

[Extracorporeal shock-wave lithotripsy for radiolucent calculi in the upper urinary tract].

Eight years have passed since extracorporeal shock-wave lithotripsy (ESWL) was used clinically for the first time, and now its usefulness is recognized all over the world. At first, variable cases were considered to be contra-indicative, one of which was the radiolucent calculi. This time for two cases of the radiolucent calculi we performed ESWL under retrograde pyelography. Using this technique, we brought the filling defect of RP into the focus of the shock waves. The stones were effectively disintegrated and these patients became stone-free in five weeks.

[A case of adenomatoid odontogenic tumor with intracranial extension].

A 49-year-old female of adenomatoid odontogenic tumor with intracranial extension was reported. She had 3 months history of left nasal obstruction and visual disturbance. Endocrinological examinations revealed a low basal level of serum cortisol and low level of urinary 17-OHCS. Neurological examinations revealed left anosmia and left visual disturbance (hand movement, marked afferent pupillary defect, and lower hemianopsia). Plain X-ray films and frontal tomograms of the skull showed a mass of soft tissue density in the left upper maxillary, left posterior ethmoid, and sphenoid sinuses with extensive destruction of the sella turcica, dorsum sellae, and anterior clinoid process. Plain axial CT scans demonstrated a low density mass filling up the left maxillary, left posterior ethmoid, and sphenoid sinuses and extending into the left middle fossa with bony destructions. Contrast-enhanced coronal CT scans demonstrated an extradural extension of the tumor into the left middle fossa and upward displacement of the pituitary complex. Left carotid angiograms showed a faint narrowing of the left internal carotid artery between the cisternal portion and the origin of the ophthalmic artery. Subtotal removal of the tumor and decompression of the optic canal and superior orbital fissure were performed via a left subtemporal extradural approach. Adenomatoid odontogenic tumor is a comparatively rare benign lesion which exhibits a slowly expansive growth without recurrence or metastasis after the surgical removal. No other case that recurred several times and extended into the intracranial space has been reported in the literature.(ABSTRACT TRUNCATED AT 250 WORDS)

Highly toxic coplanar PCBs: occurrence, source, persistency and toxic implications to wildlife and humans.

Isomer-specific determinations of PCB congeners in a wide variety of animal species such as fish, marine mammals (whale, dolphin and porpoise) and terrestrial mammals (dog, cat and human) revealed the environmental occurrence of highly toxic coplanar 3,3',4,4'-tetrachlorobiphenyl (T(4)CB), 3,3',4,4',5-pentachlorobiphenyl (P(5)CB) and 3,3',4,4',5,5'-hexachlorobiphenyl (H(6)CB) within a range of few pg g(-1) to several ten ng g(-1) in fat tissues (except fish) on a wet weight basis. Detection of these toxic residues in wild specimens collected from remote areas such as the North Pacific suggests the already widespread distribution of coplanar PCBs as in the case of general PCB pollution. The clear positive correlations between concentrations of total PCBs and each of the three coplanar PCBs obtained in all mammals analysed suggest that the sources of coplanar PCB contamination to the environment are mainly commercial PCB preparations. Comparison of the composition of three toxic coplanar PCBs in commercial PCB mixtures and in the various animals indicates the relative metabolisability of these congeners as follows: 3,3',4,4'-T4CB>3,3',4,4',5-P5CB>3,3',4,4',5,5'-H6CB. Moreover, marine mammals seem to have lower potency to metabolise the coplanar PCBs in comparison with terrestrial mammals. In human adipose tissues, the concentrations of coplanar PCBs were found to be much higher than 2,3,7,8-tetrachlorodibenzo-p-dioxin (T(4)CDD), 2,3,4,7,8-pentachlorodibenzofuran (P(5)CDF) and other toxic congeners. 'T(4)CDD-equivalent' analysis based on the enzyme induction potencies and the residues of these toxic chemicals indicates that 3,3',4,4',5-P(5)CB may impose a greater toxic threat than dioxins and furans to the humans and probably to wildlife also.

Mussels as bioindicators of PCB pollution: a case study on uptake and release of PCB isomers and congeners in green-lipped mussels (Perna viridis) in Hong Kong waters.

The uptake and release of PCB isomers and congeners were examined in green-lipped mussels (Perna viridis) through a transplantation experiment in two locations in Hong Kong waters. Rapid rates of uptake and release of relatively less lipophilic lower-chlorinated PCBs were observed in the mussels, indicating that the primary mechanism of bioaccumulation of lipophilic pollutants in P. viridis complies with the concept of equilibrium partitioning. Thus, data for contaminant concentrations are most appropriately based upon lipid weights of samples when using mussels as bioindicators of aquatic PCB pollution. Considering the kinetic parameters of PCBs based on lipid weight-related data, it is concluded that P. viridis has the ability to respond rapidly to changes in ambient levels of PCBs. This is significant in determining the usefulness and limitations of mussels as bioindicators for monitoring programmes investigating aquatic pollution by PCBs.

Gain and loss rates and biological half-life of PCBs and DDE in the bodies of adelie penguins.

The gain and loss rates and the biological half-life of polychlorinated biphenyls (PCBs) and p,p'-DDE in wild birds were calculated using the values on burdens of these compounds in the bodies of adelie penguins in a previous report (Subramanian et al., 1986). The daily loss rate of PCBs (0.26%) was found to be higher than DDT compounds (0.12%) resulting in the longer biological half-life of the latter compounds (580 days) than PCBs (270 days). Hence, once DDT compounds are absorbed they are more persistent in the organisms' bodies due to their high lipophilicity and comparatively less metabolisable nature than PCBs.

Method for the determination of three toxic non-ortho chlorine substituted coplanar PCBs in environmental samples at part-per-trillion levels.

A method has been devised combining alkali digestion, carbon chromatography and high-resolution gas chromatography for the determination of 3,3',4,4'-tetra, 3,3',4,4',5-penta, and 3,3',4,4',5,5'-hexachlorobiphenyl, the biologically active congeners of PCBs and approximate isostereomers of 2,3,7,8-TCDD. This method permits determinations of parts-per-trillion levels of these toxic residues in biological samples. Interference both from biogenic and from xenobiotic substances was reduced to extremely low levels. Using this method, 13.5 ng of 3,3'4,4'-tetrachlorobiphenyl/g, 0.89 ng of 3,3'4,4',5-pentachlorobiphenyl/g and 0.64 ng of 3,3',4,4'5,5'-hexachlorobiphenyl/g were detected on wet weight basis in the blubber of a finless porpoise. To our knowledge this is the first report on the three toxic non-ortho chlorine substituted PCB residues detected in a higher mammal in the wilderness.

[A case of gastric cancer with peritonitis carcinomatosa which improved upon oral administration of UFT, mitomycin C and lentinan].

A 29-year-old male visited our hospital because of lower abdominal pain. Acute appendicitis was suspected, and surgery was performed. However, no abnormality was found in the vermiform appendix. Instead, atrophy of the greater omentum, numerous nodes of varying sizes, and a small amount of ascites were observed. On the basis of histological examination, a diagnosis of metastatic glandular cancer was made. Gastric fluoroscopy performed 2 months earlier in another hospital had revealed irregularity of the stomach wall and a large concave area on the side of the greater curvature. Photogastroscopy and CT demonstrated progressive Borrmann type III cancer at the corresponding site and metastasis to regional lymph nodes. Biopsy findings were similar to the histological findings. After 1 week, treatment with UFT (400 mg/day), MMC (10 mg/month) and LNT (1 mg/week) was initiated. After about 3 months, i.e., following administration of 40 g UFT, 30 mg MMC and 8 mg LNT, gastric fluoroscopy and photogastroscopy revealed complete disappearance of the tumor. No abnormality was found by laboratory studies. The patient has since returned to a normal life.

[Vascular compression neuropathy of the optic nerve by sclerotic internal carotid artery].

Although mechanical compression of the optic nerve by a dilated and/or tortuous sclerotic intracranial internal carotid artery is occasionally reported as an etiology for impairment of vision and visual field, early diagnosis of such condition has been hampered by the lack of characteristic neuro-ophthalmological and neuroradiological findings. The review of 35 previously reported cases (60 eyes) including ours showed that 41% out of the 34 operated eyes were improved neurologically by the decompressive procedure, while it was in 27% out of the 26 undecompressed or non-operated eyes. These unsatisfactory operative results are considered mainly due to the failure of early diagnosis. In order to investigate the relationship between the carotid artery and optic nerve, we proposed a method of angiographical projection consisting of the straight view along the long axis of the optic canal. By this projection, it is possible to detect a supero-medial enlargement and/or dislocation of the cisternal portion of the internal carotid artery. The addition of metrizamide CT cisternography which clarifies the relationship of the optic nerve, chiasm and internal carotid artery in the supersellar cistern may further provide an useful clue for the diagnosis of this condition.

[Comparison of urinary and seminal N-acetyl-beta-glucosaminidase isoenzymes].

N-acetyl-beta-glucosaminidase (NAG) and its isoenzymes were measured in the urine and seminal plasma of healthy volunteers. Urinary NAG level was 2.62 +/- 1.30 U/L (mean +/- standard deviation), 1.99 +/- 0.77 U/g creatinine and seminal NAG level was 2370 +/- 1007 U/l. Urinary NAG level was 2.59 +/- 1.44 U/l, 1.93 +/- 0.80 U/g creatinine in males and 2.67 +/- 1.15 U/l, 2.08 +/- 0.78 U/g creatinine in females, and there was no significant sex difference. NAG isoenzymes in the urine and seminal plasma were divided into two major peaks, A and B. The A:B ratio was 78.0 +/- 6.5: 21.9 +/- 6.5 in the urine and 24.7 +/- 3.2: 75.2 +/- 3.2 in the seminal plasma, and was significantly different. Urinary NAG isoenzyme was 80.3 +/- 6.7: 19.6 +/- 6.7 in males and 74.1 +/- 4.3: 25.9 +/- 4.3 in females, and there was no significant difference between the sexes. These results indicated that urinary and seminal NAG can be differentiated by measuring the isoenzymes. Furthermore, the comparison of seminal NAG isoenzymes before and after vasectomy indicated that seminal NAG may be affected not only by the sperm but also by the prostatic fluid.

The effect of formulation conditions on the distribution of 99mTc(NaBH4)-HEDP components separated by anion exchange HPLC.

Reduction of 99mTcO4- by NaBH4 in the presence of HEDP leads to a mixture of 99mTc-containing complexes which can be separated by anion exchange high performance liquid chromatography (HPLC). The distribution of complexes within this mixture can be varied by controlling pH, the concentration of HEDP, the concentration of technetium, and the presence of air. Suitable control of these formulation conditions can yield mixtures which consist of essentially (greater than 85%) one component. Arguments are presented to support the view that the components of 99mTc(NaBH4)-HEDP and 99mTc(NaBH4)-MDP mixtures are in fact oligomeric or polymeric complexes that can contain technetium centers in at least two different oxidation states.

Squamous cell carcinoma with hypercalcemia and leukocytosis.

Squamous cell carcinoma with hypercalcemia and leukocytosis arising from burn scars in a 45-year-old man is reported. Hypercalcemia and leukocytosis improved with pepleomycin treatment and worsened with recurrence of the tumor. Serum levels of parathyroid hormone, prostaglandin E and 25-OH-Vitamin D were within normal limits. Autopsy did not disclose any bone metastases or abnormalities of the parathyroid glands. It is suggested that hypercalcemia and leukocytosis were due to factors produced by the squamous cell carcinoma. This is the fifth reported case of cutaneous squamous cell carcinoma associated with hypercalcemia in the absence of bone metastasis or parathyroid gland abnormalities.
