Congenital pyloric atresia, a rare malformation producing gastric outlet obstruction during the neonatal period, was found in three patients. Sixty per cent of these malformations are membranous in type. Excision of the membrane and Heineke-Mikulicz pyloroplasty, which has been advocated as the procedure of choice, inevitably results in postoperative duodenogastric bile reflux. We suggest that to minimize reflux and hence possible increased risk of future development of gastric ulcer, maximal preservation of pyloric function in the operative management of the malformations is important.
Cutaneous delayed hypersensitivity (CDH) tests, using 6 antigens (PHA, PPD, SK-SD, Candida, Mumps and DNCB) were performed in 260 patients with cervical cancer and the following results were obtained. 1) Decrease in incidence of positive CDH (PHA, PPD and DNCB) was found as the clinical stage advanced. Especially, decrease in incidence of positive CDH (DNCB and PHA) was found in stage III and IV. 2) There was no relationship between the incidence of positive CDH (SK-SD, Candida and Mumps) and clinical stage but, decrease in incidence of these tests was found in stage III and IV. 3) In comparison on numbers of positive CDH, there was no significance in control groups and cervical cancer patients. 4) Incidence of advanced invasion of primary lesion (gamma and delta type) was 83.7% in DNCB-negative group who were treated with curative operation. 5) Incidence of regional lymph node metastasis was 42.9% in Candida-negative group, whereas, 16.2% in Candida-positive group who were treated with curative operation.

Five distance runners (H groups) performed a 60 min bicycle exercise at a load of 60--70% VO2max in a moderately hot environment (Ta: 33.5 degree C, 60% RH). Following a period of heat acclimation with bench-stepping at a load equal to about 25--30% VO2max, in a hot environment (Ta: 45--50 degree C, 30--40% RH) for 9 days, the work test was repeated. Two control subjects (R) performed the same work tests with no heat acclimation. Heat acclimation increased performance time. Rectal temperature, mean skin temperature, heart rate, and Na+ concentrations in sweat were lower in H and, with one exception, sweat rate was higher after heat acclimation. All H subjects demonstrated that the linear relationship between sweat rate and rectal temperature was shifted to a lower temperature (threshold shift). This shift correlated with a lowering of resting rectal temperature. The magnitude of the reduction in those two temperatures due to heat acclimation was identical. The observed improvement of work performance in moderate heat following heat acclimation to a higher temperature is attributed to a more efficient thermoregulatory mechanism.
Extraluminal force transducers were chronically implanted onto the serosal surface of the gastrointestinal tract in dogs and the interdigestive migrating contractions (IMC) were studied in conscious dogs. When the IMC occurring periodically in the stomach reaches the distal ileum, the next IMC develops in the stomach. We therefore studied the effect of isolation or resection of the lower third of the small bowel on the periodic occurrence of IMC in the stomach. Transducers were sutured onto the antrum, the upper jejunum, the mid intestine and the terminal ileum. After control records on gastrointestinal motility were made for these dogs, the abdominal cavity was again opened to prepare a Thiry loop in the lower third of the small bowel. (isolation group) Next, the loop was removed (resection group) and gastrointestinal motility was recorded. In these groups, we 1) measured the time from the feeding to the initiation of the first IMC. 2) measured the time from the termination of one IMC to the termination of the next IMC. 3) measured the migrating time of the IMC. 4) and measured the duration of the IMC in the stomach. As a result, it was found that the time interval between the termination of one IMC in the stomach and the termination of the next was almost constant throughout all experiments. The migrating time of the IMC from the stomach to the terminal ileum in isolation and resection groups was slightly shortened. These findings indicate that the existence of the lower gut has no significant influence upon the regular periodic occurrence of the IMC in the stomach and the migrating time.
We have studied a patient with congenital cystic dilatation of the common bile duct who had undergone ultrasonography five times in 6 mo before operation. Ultrasonography revealed that the duct was distended with inflammation, and was deflated when inflammation was not present.
Our study of the sequence of morphological events leading to the formation of hydatidiform mole (HM) led us to this study. Our object in this paper is to present evidence supporting our 1971 hypothesis with results obtained by various immunological methods in our laboratory, and to propose the importance of an immunoregulatory mechanism by blocking antibody (BA). Our hypothesis is that the conceptus, after the early demise of the embryo, may be dropped out in most cases as spontaneous abortion (SpA), and the trophoblast in the remaining cases continues to grow, developing into HM by an "immune selection" mechanism. Our conclusion is that the trophoblast will be damaged by immune cytotoxic mechanism through a decrease of BA in SpA, and continue to grow in HM through continuous production of BA after the demise of the embryo.
We have previously reported that during the interdigestive state repeated episodes of high-amplitude contractions interrupted by long-lasting motor quiescence were seen to occur regularly until the next meal in the dog. However, none of the dogs used in our laboratory showed such regular changes in gastric motor pattern all the time during the interdigestive state. In the present study, we measured plasma motilin concentrations and intraduodenal pH together with gastric motor activity in conscious dogs. It was found that as long as intraduodenal pH remained between 7.0 and 8.5 the regular occurrence of the interdigestive contractions was closely associated with increases in the plasma immunoreactive motilin (IRM) concentration. However, when intraduodenal pH became acid, there were no typical interdigestive contractions even though plasma IRM concentration was elevated. We consider that high-amplitude contractions like the interdigestive contractions could not be evoked in the acid-secreting stomach even if the plasma IRM concentration was elevated during the interdigestive state. Acid secretion during the fasted state, the cause of which is not known, seems to be one of the main factors in disturbing the regularity of the interdigestive motor pattern.

Ultrasonography was applied to 140 pediatric patients who were diagnosed with histopathological confirmation of masses by either surgical or autopsy methods during the past nine years in the National Children's Hospital and the Hospital of Toho University. The masses were classified into three ultrasonic patterns; solid, cystic, and mixed. A solid pattern was obtained in 47 cases, a cystic pattern in 67, and a mixed pattern in 26. Of the cases of the solid pattern, a benign tumor was seen in 32 per cent, and a malignant tumor in 68 per cent. Each 50 per cent of the cases of the mixed pattern had malignant or benign tumors. All the cases of the cystic pattern had benign tumors. Ultransonics was used as a supplement to routine clinical studies.
