[Effect of hydrogenated ergot alkaloids on cerebral metabolism of the patients with neurological deficits after subarachnoid hemorrhage (author's transl)].

Cerebral blood flow and metabolism were measured in 9 patients with neurological deficits after subarachnoid hemorrhage using the 133Xe intra carotid injection methods. Regional cerebral blood flow (r-CBF) values were calculated with initial slope method (2 minutes flow index), their values were compared with the blood flow response to arterial PCO2 and to administration of hydrogenated ergot alkaloids (HEA). In case of repeated examination after short interval CBF values were corrected using the rate of reproducibility. The content of oxygen, glucose, lactate, and pyruvate in the arterial and the internal jugular blood were measured , and CMRO2, CMR glucose, CMR lactate, values were calculated: 1st step: The measurement of r-CBF was performed immediately after the infusion of 1.5 mg HEA for 15 minutes. The focal ischemic lesions decreased, mean CBF value increased by 8% significantly, arterio-venous difference of oxygen value increased by 20% significantly and CMRO2 value increased by 26% significantly after HEA administration. 2nd step: 1.8 mg HEA was infused for one week. EEG findings were improved after administration of HEA. The pressure and the lactate levels of cerebrospinal fluid decreased, but the effect of HEA must be carefully distinguished from natural recovery. Administration of HEA increased the CMRO2 values and consequently increased the mean CBF values.

Comparison of the effects of a multi-component vaccine and a formalin-killed cell vaccine on protection against enzootic of hemorrhagic pneumonia due to Pseudomonas aeruginosa in mink.

Effectiveness of a multi-component vaccine consisting of the common antigen (OEP) derived from Pseudomonas aeruginosa strain N 10 (serotype E) and toxoids of protease and elastase was compared with that of formalin-killed cells of strain N 10 on protection against enzootic of hemorrhagic pneumonia due to P. aeruginosa in mink. One administration of the multi-component vaccine (100 microgram each of OEP, protease toxoid and elastase toxoid) clearly prevented enzootic of hemorrhagic pneumonia due to P. aeruginosa (serotype G) in mink, while a vaccination of formalin-killed cells was much less effective in preventing an epidemic. The difference in mortality rates between two vaccines was remarkable.

Malignant ameloblastoma with metastasis to the skull: report of case.

An unusual case of ameloblastoma that underwent malignant change and metastasis during recurrence is described. The primary tumor of the mandible and two independent recurrent lesions found in the base of the coronoid process and in the mandibular notch were cystic ameloblastoma of follicular type, with no histological evidence off malignancy. The second recurrence developed in the soft tissues on the medial aspect of the ascending ramus and consisted of a large solid tumor mass with poorly differentiated ameloblastoma cells, which were seen clustered in blood vessels in close apposition to tumor nests. Apparently these metastasized to the temporal bone in five months. The metastatic tumor was composed of atypical follicles packed with undifferentiated hyperchromatic cells with nuclear atypia and abundant mitoses. The histological diagnosis was malignant ameloblastoma. The cerebral lesion that developed in the skull base, possibly by direct extension of the second recurrent tumor, was also regarded as malignant because of its rapidity and aggressive growth and its high sensitivity to radiotherapy.

Provocation experiment: Pneumocystis carinii in several kinds of animals.

Provocation experiments of P. carinii in several kinds of laboratory animals were carried out in an animal house where P. carinii of rat strain had been maintained for several years. The organism propagated in cortisone treated rats, rabbits and mice without difficulty as reported by some authors in the past. The organism also propagated in the lungs of rats which had been bred with protein free diet. The present paper first indicates that guinea pig is susceptible to P. carinii. On the other hand, it becomes evident by repeated experiments, that golden hamster seems to be non-susceptible to P. carinii of rat strain even if enough dose of corticosteroid hormones is given. The presence of non-susceptible animal to a certain strain of P carinii is quite interesting from an epidemiological point of view.

[Insulin secretion in the perfusion of rat pancreas in situ (author's transl)].

A new perfusion technique was applied to the rat pancreas to elucidate neural mechanisms related to insulin secretion. The pancreas and a part of the gastroduodenal tract were perfused in situ with an artificial perfusion fluid consisting of 5% of rat blood cells, 4% bovine serum albumin and 0.1% of glucose and Krebs-Ringer bicarbonate buffer. Stimulation (5-10V, 5 msec, 50 Hz for 5 min) to the right or bilateral cervical vagal nerves produced a significant increase in insulin activity in the perfusate. Acetylcholine (5-10 micrograms/0.1 ml) added to the perfusion fluid produced a dose-dependent increase in insulin activity in the perfusate, and the responses disappeared after pretreatment with atropine (100 micrograms/ml in perfusion fluid). After adding noradrenaline or isoproterenol (each 0.1 micrograms/0.1 ml) to the perfusion fluid, appreciable changes were not apparent.

An immune dependency of trophoblastic growth implied by the antithetic difference in immunology between spontaneous abortion and hydatidiform mole.

It may be difficult to demonstrate clearly an immune dependency of trophoblast growth in vivo. The author's standpoint, however, is that comparative study of the immunology of SpA and HM must attempt to demonstrate it. In this chapter, before entering the main topics, the genesis of HM in relation to early SpA, the dependency of trophoblastic cell growth on the presence of an embryo or fetus, as evidenced by animal experiments, and the pathology and cytogenetics of SpA were outlined in a summarized form. This summary was provided to stress that the comparative immunologic study of SpA and HM may be a very useful experimental model in the elucidation of an immune dependency of trophoblastic cell growth in vivo. Many immunologically antithetic differences between SpA and HM were found and the important role that BA plays in trophoblastic cell growth was strongly suggested by the considerations of immune injury to the trophoblast in SpA and of the immune survival of the trophoblast in HM. Thus, we have come to believe that our clinical and experimental model described here might illustrate a new aspect of reproductive immunology, while contributing to the elucidation of SpA and HM genesis. Further studies along these lines may be useful and, needless to say, should be performed.

[Pregnancy and delivery following cardiac valve replacement (author's transl)].

We describe 30 patients with Starr-Edwards cardiac valve replacements who underwent pregnancy. Fourteen pregnant women who received anticoagulants gave rise to 3 neonatal anomalies such as congenital heart disease, polydactylia and microphthalmus. And fetal losses were 5 spontaneous abortions. Women with artificial valves can tolerate the hemodynamic load of pregnancy well, but there is an increased fetal wastage in patients taking oral anticoagulants (coumarin: warfarin). Namely, there is probably largely attributable to fetal hemorrhage but there is also a risk of malformation caused by teratogenic effects of warfarin. In contrast, pregnant women who didn't receive anticoagulants had the risk of thrombo-embolismus. It is suggested that the best plan is to use long acting oral anticoagulants during antenatal period and heparin over the periods of labor.

[Application of two dimensional echocardiography for the intracardiac manipulation: the evaluation of atrial septal movement before and after balloon atrial septostomy (author's transl)].

Two-dimensional echocardiography was applied to intracardiac catheter manipulation (especially balloon atrial septostomy: BAS) in 6 babies with complete transposition of the great arteries and 2 with total anomalous pulmonary venous drainage. Under the usual roentgenoscopic investigation, intracardiac catheter manipulation was observed by two-dimensional echocardiography directly representing the right atrium, left atrium, atrial septum, right ventricle, left ventricle and interventricular septum by the subcostal approach. Also, formation and movement of the atrial septum and intra-atrial shunt before and after BAS was evaluated by contrast echocardiography. 1. It was confirmed that the catheter tip was inserted into the left atrium from the right atrium. 2. It was clearly demonstrated that the balloon was expanding in the left atrium. 3. When the balloon was pulled out to the right atrium from the left atrium, the movement of the atrial septum and the balloon was observed. 4. The formation and movement of atrial septal defect by BAS was demonstrated and the shunt of this site was estimated by using contrast echocardiography. The relation between the catheter tip and the intracardiac structures can be judged more correctly by two-dimensional echocardiography than roentgenoscopic investigation. Also the structural and hemodynamic changes by BAS can be clearly estimated by the observation of the atrial septum by two-dimensional echography.

[Two-dimensional echocardiographic diagnosis of aortic arch by suprasternal approach (author's transl)].

Two-dimensional echocardiographic studies of the aortic arch and proximal descending aorta were performed in 6 cases with coarctation of the aorta, 40 normal subjects and 9 cases of coarctectomy approached by the suprasternal notch. In cases with the normal aortic arch and proximal descending aorta, the ascending aorta, aortic arch, descending aorta and right pulmonary artery were visualized clearly. The diameter of the aortic lumen is consistent throughout the plane of the scan. The origins of the left carotid artery and the left subclavian artery were seen clearly. In 3 of 7 normal newborns a localized area of a slightly aortic narrowing (diameter of aortic narrowing area / diameter of proximal descending aorta greater than 0.75) was visualized. In 5 of 6 cases with coarctation of the aorta a localized area of an aortic narrowing distal to the origin of the left subclavian artery is visualized which corresponded to the angiographic appearance of the coarctation. In 1 of 6 cases with coarctation of the aorta a more diffuse area of aortic obstruction beginning from distal portion of the left carotid artery to distal portion of the left subclavian artery was visualized. In this case, aortic valve stenosis, ventricular septal defect, patent ductus arteriosus, and hypertrophy of the interventricular septum and left ventricular posterior wall were associated. In 9 coarctectomy cases a localized area of an aortic narrowing distal to the origin of the subclavian artery was enlarged after operation. Two-dimensional echocardiogram approached by the suprasternal notch may offer a useful noninvasive method for direct visualization of the aortic arch and proximal descending aorta.

[Studies on the appropriate frequency of the cervical cancer screening (author's transl)].

A total of 73,897 cases of women above 30 years old were screened for their cervical cancer by Pap smear in Niigata prefecture in 1978 and 1979. From them 37 cases of invasive cancer (0.05%) including a case of endometrial adenocarcinoma, stage II, 30 cases of carcinoma in situ (0.04%), and 52 cases of dysplasia (0.07%) were detected. From the correlation between the check up frequency of these patients and the cervical malignancy detected, the following were pointed out: 1) No invasive cancer but carcinoma in situ were detected from those patients who were cytoscreened by successive annual visits. 2) Stage Ia but not more advanced cervical cancer were detected from those patients who were screened at 2 years interval. 3) Stage Ib or farther advanced cervical cancer were detected from those patients who visited at 3 years or more years interval just like from those patients who visited for the first time. To detect cervical cancer in its preinvasive stage, annual Pap smear of adult women over 30 years old should be continued.

Effect of benzaldehyde on the proliferation of HeLa cells.

The effect of benzaldehyde on the proliferation of HeLa cells was studied in the present work. Benzaldehyde caused a dose-dependent reduction of the proliferation of HeLa cells in vitro. This effect of benzaldehyde might be caused principally by a reversible and mild growth modulation, since no serious morphological damage was observed. Delay of the S and G2 periods in the cell cycle upon addition of benzaldehyde was observed by the pulse-labeling technique using 3H-thymidine.
