Immunofluorescent studies of complement C3 in the hair follicles of normal scalp and of scalp affected by alopecia areata.

The deposition of complement C3 in the hair follicles of scalps of 4 normal subjects and of the affected parts of 9 patients with alopecia areata was found by the direct immunofluorescent methods. No deposition of immunoglobulins (IgG, IgA, IgM, IgD and IgE) was found. In the normal scalps, complement C3 was found to be deposited in the hyaline membrane of the hair bulb and in the internal layer of connective tissue sheath at the lower about 2/3 transient portion of anagen follicles, and in the thickened hyaline membrane of the catagen follicles. The deposition of complement C3 was also found in the anagen and catagen-like hair follicles even in the patients with alopecia areata, which was similar to the finding in the normal scalps. From the fact that the deposition of complement C3 was found chiefly in the transient portion of the hair follicles of the normal scalps, and that the behavior of deposition of complement C3 reflected the morphological state of hair follicles in each stage of the hair cycle, even in alopecia areata, complement C3 may be intimately related to the hair cycle, even though its significance is as yet unknown.

[Cellular localization of alpha-fetoprotein (AFP), human chorionic gonadotropin (HCG), and carcinoembryonic antigen (CEA) in malignant germ cell tumors of the ovary using immunoperoxidase technique (author's transl)].

An immunohistologic study of germ cell tumors of the ovary was undertaken to investigate a correlation of the histology with the presence of alpha-fetoprotein (AFP), human chorionic gonadotropin (HCG), and carcinoembryonic antigen (CEA) in the tissue specimens. A positive reaction for AFP was consisted of dark brownblack granular intracytoplasmic deposits and some of hyaline globules were also positive in some instances. There was tissue localization of AFP in 9 of 10 embryonal carcinomas (endodermal sinus tumor, Teilum) and in 4 of 5 solid teratomas. Two patients with embryonal carcinoma positive for AFP had normal serum level in AFP. A positive staining for HCG was found in 4 embryonal carcinomas and in 2 solid teratomas; it was consisted of dark brown cytoplasmic deposits localized exclusively to syncytiotrophoblast giant cells. The tumor cells positive for AFP but negative for HCG, and vice vasa indicated that these two carcinoembryonic proteins were produced by different cells. The localization of CEA was also studied in 3 embryonal carcinomas. None of embryonal carcinomas and 3 solid teratomas were reacted to CEA; it was consisted of a dark brown deposit localized in cell surface of stratified squamous cells and tall columnar cells forming glandular structure. All of the histologically immature neural components were negative for these carcinoembryonic proteins.

[Studies on treatment of invasive mole and subsequent reproductive performance, a report of 39 cases (author's transl)].

Thirty-nine invasive mole patients treated during the last 6 years (1971-1976) in our clinic, were studied. The data were summarized in the following items. 1. Surgical treatment was done in 31 cases (79.5%), and methods of operation were removal of focus in 13 cases and hysterectomy in 18. Chemotherapy was performed in 31 cases (79.5%). 2. In 31 operated patients, all of invasive lesions were in uterine bodies. A degree of chorial invasion into myometrium was shallow in 12 cases, medium in 12 and deep in 7. Molar vesicles were observed in 17 cases and trophoblastic tissues were microscopically confirmed in 14. 3. Anti-cancer agents, such as ACTD and MTX, were required 1-4 courses in 8 patients received chemotherapy alone and 1-6 courses in 23 patients received operation combined with chemotherapy. 4. Up to the present time, 14 pregnancies in 11 cases out of these patients occurred, but 2 of them ended in spontaneous abortion and one was molar pregnancy. 5. Malignant sequela, that is, choriocarcinoma was occurred in 2 cases. But, these 2 cases are in remission. 6. Thirty-seven patients, including 2 choriocarcinoma cases, are healthy at present.

[Immunological analysis of hydatidiform mole and invasive mole (author's transl)].

Thirty-two cases of hydatidiform mole without endometrial invasion of chorial tissue and 17 of invasive mole were studied to ascertain whether these two diseases can be differentiated by various immune parameters. All of hydatidiform 11 of invasive mole were sampled at a state of "mole in utero" (pre-evacuation), but 6 of invasive mole were at a post-evacuation state, respectively. The results were summarized in the following items; 1. Serum total protein, especially albumin decreased with no significant difference in both groups. Globulin fractionation (alpha 1, alpha 2, beta, gamma-globulin) showed no characteristic changes. 2. A variation of immunoglobin (IgG, IgM, IgA) was similar in both. 3. Beta 1 C (a component of complement) increased in two groups, but no difference was revealed. 4. Leucocytosis was observed, but lymphocytosis was not detected in both groups. 5. Active E-RFC slightly increased, although total E-RFC and EAC-RFC were within normal range. But these findings were not statistically different in both. 6. In vitro lymphocyte-PHA-response was fairly depressed without a significant difference in two groups. 7. Suppressive effect of serum on lymphocyte-response was also slightly strengthened, but a difference in both was not significant. 8. The results of immune skin tests (PHA, PPD, SK-SD, Candida) could not be debated with liquid numbers studied.

[Studies on mixed lymphocyte-tumor cell culture reaction (MLTR) in gynecological malignant tumors by the microplate methods (author's transl)].

For the purpose of observing the cellular immune response to their own tumor cells, MLTR was performed on 16 cases of cervical cancer, 3 cases of corporeal cancer and 6 cases of ovarian cancer. The results of MLTR was presented as a stimulation index (S.I.) by the following formula. (Formula: see text). Results of S.I. were as follows; 1.37 +/- 1.47 in cervical cancer, 0.65 +/- 0.21 in corporeal cancer and 1.12 +/- 0.74 in ovarian cancer. A positive S.I. case which was more than 2.0, was found in 3 cases with cervical cancer (18.8%), in none with corporeal cancer (0.0%) and in 2 with ovarian cancer (33.3%). Thus, the cellular immune response to their autologous tumor cells was considered to be very slight in this assay system.

Antitumor activity of benzaldehyde.

Ninety patients with inoperable carcinoma in the terminal stages and 12 patients in serious condition with other tumor types were given benzaldehyde in the form of beta-cyclodextrin benzaldehyde inclusion compound (CDBA) orally or rectally at a daily dose of 10 mg/kg divided in four doses. Toxic effects, including hematologic or biochemical disturbances, were not seen during long-term successive administration of CDBA. Fifty-seven of the patients treated were evaluable; 19 patients responded completely and ten patients responded partially (greater than 50% regression). For all responding patients longer response durations were associated with longer CDBA treatment periods. Treatment of squamous cell carcinoma induced the cancer cells to change into a conglomeration of pearls (the well-known product of differentiation) which consisted of keratinized normal squamous cells.

[Clinical studies on cases of cardiovascular disease during pregnancy (author's transl)].

The long term follow up studies on cases of cardiovascular diseases during pregnancy or delivery were reviewed in 181 patients administrated in our clinic. The results as follows: 1. An incidence of patients with cardiovascular disease during pregnancy was 1.46 per cent as a whole, and there was increasing tendency in congenital heart disease. 2. An accidental heart failure during pregnancy was seen in 7.7 per cent and all cases with NYHA III grade had an incidence of 100 per cent. 3. The occurrence of toxicosis and SFD pregnancy were more frequently seen in group of heart disease than in controls. But, no difference was observed in spontaneous abortion or neonatal asphyxia. 4. 28 patients with Starr-Edwards prosthetic heart valves became pregnant in our clinic. In these pregnancies, 5 cases receiving with warfarin therapy resulted in 3 anomalies of new born and in 2 spontaneous abortion. So, the use of warfarin seemed to have the risk to the fetus. But omission of the anticoagulant therapy on the other hand, increased the danger of systemic embolization to the mother. 5. The follow up study by mail in some cases revealed that, as a general, pregnancy and delivery may aggravate the condition of patients, especially who had mitral heart failures.

[A correlation between the histological parameters such as stromal reaction and cell-mediated immune-structure of the regional lymph nodes, and the delayed cutaneous hypersensitive reaction toward DNCB in cervical cancer (author's transl)].

We have studied a correlation between the histological parameters such as stromal reaction to cervical cancer and cell-mediated immune response of the regional lymph nodes designated as lymphocyte depletion, unstimulated, and lymphocyte predominance pattern which were thought to reflect the cell-mediated immune reaction to cervical cancer and the DNCB skin reaction in 84 patients. 1) Group with DNCB skin reaction negative (20 cases): Eight showed no or only the slightest trace, 7 moderate degree, and 5 marked degree of the stromal reaction. The immune response of the regional lymph nodes were 4 of lymphocyte depletion, 6 of unstimulated and 10 of lymphocyte predominance pattern. 2) Group with DNCB skin reaction positive (64 cases): This group showed 7 cases with no or only the slightest trace, 30 cases with moderate degree and 27 cases with marked degree of the stromal reaction. In this group, 6 had the regional lymph nodes of lymphocyte depletion, 10 unstimulated and 48 lymphocyte predominance pattern. 3) There was a significant correlation between the DNCB skin reaction and the degree of stromal reaction (p less than 0.05), but no correlation between the DNCB skin reaction and the regional lymph node morphology was found in this study (p greater than 0.05). 4) The discrepancy between the stromal reaction, the lymph node morphology and the DNCB skin reaction was discussed in relation to the significance of these parameters.

Evaluation of 3-minute semiquantitative test kit for urinary estrogens of pregnant women.

A 3-minute rapid slide assay of total urinary estrogens is available for testing in pregnant women. This is a semiquantitative, immunologic competitive latex agglutination inhibition slide test, which is reported in levels (level 1: less than 5 mg/liter, level 2: greater than or equal to 5 mg/liter, level 3: greater than or equal to 10 mg/liter, level 4: greater than or equal to 20 mg/liter). With the use of this kit, the results of 100 samples of pregnancy urine were compared with the results from a chemical colorimetric assay procedure. There was 94% agreement between these 2 methods. There was no 2-step difference in grading. The probable future usefulness of this kit is described because of its rapidity and simplicity in testing. Its limitations are also discussed.
