Abortifacient effect and uterine cervix-dilating action of 16, 16-dimethyl trans delta 2 PGE1 methyl ester (ONO 802) in the form of a vaginal suppository (a randomized, double-blind, controlled study in the second trimester of pregnancy).

A newly synthesized 16, 16-dimethyl trans delta 2 PGE1 methyl ester (ONO 802) was clinically applied in the form of a vaginal suppository for therapeutic abortion of second trimester pregnancies. Its effects were studied in a double-blind test comparing it with an inactive placebo suppository. The study was conducted at 12 Japanese university hospitals. The number of patients was 125 in total, i.e. 63 receiving ONO 802 vaginal suppository (containing 1.0 mg ONO 802) and 62 receiving inactive placebo suppository. ONO 802 was more effective than placebo with a success rate of 87% (complete (71%) and incomplete (16%) abortions). The onset of uterine contractions was observed in 154.3+/-18.1 min. and the onset of uterine bleeding in 323.6+/-41.0 min. The expulsion of the fetus and placenta was observed in 955.4+/-97.0 and 961.6+/-97.0 min., respectively. The cervix dilating effect of ONO 802 was observed in 63.5% of the patients at 3 hours after the start of administration. Nausea, vomiting, abdominal cramps, diarrhea and pyrexia were noticed. However, all these side effects were transient and mild, requiring no treatment. No abnormality was observed in the puerperal course, duration of uterine bleeding or onset of subsequent menstruation following the therapeutic abortion. Therefore, the present study demonstrated that ONO 802 vaginal suppository was an effective and valuable drug for therapeutic abortion of second trimester pregnancies.

Computed tomography in moyamoya disease.

Eighteen patients with moyamoya disease were examined by computed tomography (CT). At the first examination, CT findings were normal in one patient and abnormal in 17 patients. Significant abnormalities on precontrast CT were characterized by low density lesions in 14, ventricular dilatation in 9, and cerebral atrophy in 11 patients. Postcontrast CT showed the presence of contrast enhanced lesions in three patients. Follow-up examinations were performed in 13 patients, and the following changes on CT were found in 10 of them: increase or development of low density lesions in 10, ventricular dilatation in 4, cerebral atrophy in 5, appearance of a contrast enhanced lesion in 1, and disappearance of the enhanced lesion in 3. The patient with normal findings at the first examination showed a low density lesion on the second examination. The abnormal vascular networks in the region of the basal ganglia were visualized successfully in six examinations of five patients by higher concentration of contrast medium in blood than usual. Abnormal CT findings consisted of those seen in multiple cerebral infarctions and the visualization of the abnormal vascular networks. Computed tomographic findings on these patients showed evolution as seen in clinical and angiographic findings.

Asplenia and polysplenia syndrome.

This report described the morphological characteristics of seven cases of asplenia syndrome and three of polysplenia syndrome. Each syndrome has been characterized by a tendency for symmetric development of normally asymmetric organs, with varying degrees of cardiovascular anomalies. These latter anomalies are usually present in asplenia syndrome to a greater extent than in polysplenia syndrome. While, as observed in our material, the conotruncal anomalies were present more commonly in cases with asplenia, and absence of inferior vena cava with azygos continuation was seen specifically in all the cases with polysplenia. This evidence implied the presence of some pathogenetic distinction between the two syndromes.

Articulatory parameters for the perception of bilabials.

In order to investigate articulatory cues for the perception of bilabial consonants, a production model was set up whose control parameters were converted into acoustic parameters by which a terminal analogue speech synthesizer was driven to synthesize test sounds for the perception experiments. Experiments concerning discrimination between (b) and (w) and between (m) and (w) were then performed with these test sounds. Statistical testing of the results showed that discrimination among the bilabials in an intervocalic context entirely depended on the minimum value of lip opening and that, contrary to the facts hitherto known, it was independent of the rate of lip opening/closure. At the same time, the features [+ anterior, - coronal] and [+ nasal] turned out to be perceptually independent phonological attributes of bilabial and nasal consonants, respectively.

Immunofluorescence studies on C1Q in mast cells.

Evidence has been obtained of deposition of complement component C1q in mast cells, by the immunofluorscence technique, using biopsy specimens from the scalp from 5 of 7 normal subjects and 14 of 17 patients with alopecia areata and also from skin lesions of 1 of 2 patients with urticaria pigmentosa studied. The fluorescence in mast cells was of a granular pattern in most instances. No deposition of immunoglobulins (IgG, IgA, IgM, IgE) or other complement components (C3, C4, C5, C9) was demonstrable in the mast cells. The specificity of the C1q deposition in mast cells was confirmed by various tests, viz., the blocking test, prerinsing of tissue sections with bovine serum albumin-containing phosphate-buffered saline, absorption test of conjugates, C1q destruction test with urea or by heating, and the Ouchterlony method.

Outbreaks of type C botulism in waterfowl in Japan.

Four outbreaks of botulism in waterfowl were encountered over a five-year period of 1973 to 1977 in Japan. In all the outbreaks toxin was detected from all 12 sera, twenty-three of 24 gizzard contents from diseased or dead birds and one of three maggots. It was neutralized with Clostridium botulinum type C antitoxin serum, regardless of its origin. By using CO2 gas jet method, C. botulinum was isolated from four of 11 gizzards from diseased birds, five of 7 ones from dead birds, one of one maggot and one of one sludge sample, that is, eleven of 20 specimens in total. All 20 strains were identical with C. botulinum type C in biological properties. Most of the isolates showed a toxin titer ranging from 1,000 to 200,000 LD50 for mice. Four of them were identified as type C by mouse neutralization tests with antitoxin sera. The toxic suspensions of a strain 1-15 were administered orally to Chinese spot-billed ducks, which died when more than 200,000 LD50 mouse toxin was administered. Environmental conditions for occurrences of waterfowl botulism were discussed.

[Clinical trial of cisplatin in the treatment of ovarian carcinoma].

Cisplatin (cis-diamminedichloroplatinum) was administered to 20 patients with histologically proven ovarian cancer. Ten of 20 patients were evaluable. They consisted of 1 patient by single use and 9 by combined use. The dose of cisplatin in single administration was 50 mg/m2, and the schedule for combined administration was as follows: cisplatin (50mg/m2, i.v. day 1), adriamycin (40 mg/m2, i.v. day 1), cyclophosphamide (350 mg/m2, i.v. day 1) and 5-FU (350 mg/m2, i.v. day 1-5). This was repeated every three weeks. The side effects of cisplatin and in its combination use were relatively severe. All of 20 patients showed severe nausea and vomiting, but fortunately there were no signs of hepatotoxicity, bone marrow or renal impairments during these courses. As the result, according to the response criteria of Koyama-Saito's group, out of 10 evaluable patients, the response rate consisting of partial and complete responses was 50% (CR: 3 cases, RP: 2 cases). It is shown that cisplatin is effective and useful in chemotherapy for the patients with ovarian carcinoma.

[Relationship between circulating androgens and ovarian hyperstimulation syndrome in anovulatory patients treated with hMG-hCG].

In order to evaluate relationship between circulating androgens levels and development of ovarian hyperstimulation syndrome (OHS) in anovulatory patients treated sequentially with hMG and hCG, serum concentrations of androstenedione (A), testosterone (T) as well as estradiol (Ed) were measured serially in a total of 17 anovulatory patients including 9 who did not develop OHS and 8 who developed mild or moderate OHS. Increase of Ed levels during the period of hMG treatment varied remarkably in individual patients with OHS ranging from 7.2 to 190.1 times as much the pretreatment value. On the other hand, increase of A levels during the hMG treatment was recorded in the range from 2.0 to 3.2 times as much the pretreatment value in 4 patients with mild OHS, and from 1.5 to 5.9 in 4 patients with moderate OHS. However, the ratio of A increase remained within 1.4 times in patients without OHS. A transient increase of circulating T was observed in 2 days after commencement of hCG treatment, ranging between 1.4 and 3.2 times in patients without OHS, between 1.8 and 2.4 times in patients with mild OHS and between 3.8 and 6.8 times in patients with moderate OHS. It is concluded that serial measurements of A and T during the course of hMG and hCG treatments respectively appear to be an additional index other than Ed to predict development of OHS.
