Ectopic tonsillar tissue in the nasal septum.

A rare case of hypertrophied lymphoid tissue in the nasal septum considered to be an ectopic tonsil was reported. The patient was a 53-year-old male, and a polypoid mass attached to the septal wall by a stalk was removed. Histological examination showed that the tumour was an ectopic hyperplastic tonsil presenting in the nasal septum. The pathogenesis of this tumour was not clear, but it was considered as hyperplasia or a new formation of primary or secondary lymphoid follicles. This is the first case report of such a rare condition.

Comparison of Activities of the Host-Specific Toxin of Helminthosporium maydis, Race T, and a Synthetic C(41) Analog.

It previously had been proposed that the host-selective toxin of Helminthosporium maydis race T consists of a series of unusual linear (C(35) to C(45))polyketols, of equal toxicity on a weight or molar (10(-8)-10(-9)) basis. Previous laboratory synthesis of T-toxin analogs was limited to shorter (C(15) to C(26)) versions which possessed the requisite specificity for susceptible corn (Zea mays) but were less toxic on a weight or molar (10(-6)-10(-7)) basis. In the present study, a C(41) analog with four beta-ketol units spaced by CH(2) bridges as in native toxin has been synthesized. On a weight or molar basis, it is as effective as native toxin or its purified components in stimulating NADH oxidation of mitochondria from susceptible corn, thus providing firm evidence for the correctness of the proposed structures of T-toxin. Additional support derives from the observation that C(24) and C(26) analogs with -(CH(2))(4)- and -(CH(2))(6)- bridges between ketol groups are not as effective in stimulating NADH oxidation as are C(23) and C(25) analogs with the -(CH(2))(3)- and -(CH(2))(5)- bridges of native T-toxin.It was calculated that a single molecule of the C(41) analog is at least 300 times more effective in stimulating mitochondrial oxidation than a molecule of the C(23) or C(25) analogs. This emphasizes the importance of chain length for toxicity, perhaps through perturbation of membrane functions of mitochondria and/or chloroplasts.

Halophilic Nuclease of a Moderately Halophilic Bacillus sp.: Production, Purification, and Characterization.

A moderately halophilic bacterium, Bacillus sp., isolated from rotting wood on the seashore in Nauru, produced an extracellular nuclease when cultivated aerobically in media containing 1 to 2 M NaCl. The enzyme was purified from the culture filtrate to an electrophoretically homogeneous state by ethanol precipitation, DEAE-Sephadex A-50 column chromatography, and Sephadex G-200 gel filtration. The enzyme consisted of two charge isomers and showed both RNase and DNase activities. Molecular weight was estimated to be 138,000 by Sephadex G-200 gel filtration. The enzyme had marked halophilic properties, showing maximal activities in the presence of 1.4 to 3.2 M NaCl or 2.3 to 3.2 M KCl. The enzyme hydrolyzed thymidine-5'-monophosphate-p-nitrophenyl ester at a rate that increased with NaCl concentration up to 4.8 M. In the presence of both Mg and Ca, activity was greatly enhanced. The activity was lost by dialysis against water and low-salt buffer, but it was protected when 10 mM Ca was added to the dialysis buffer. When the inactivated enzyme was dialyzed against 3.5 M NaCl buffer as much as 68% of the initial activity could be restored. The enzyme exhibited maximal activity at pH 8.5 and at 50 degrees C on DNA and at 60 degrees C on RNA and attacked RNA and DNA exonucleolytically and successively, producing 5'-mononucleotides.

Treatment of moyamoya disease by temporal muscle graft 'encephalo-myo-synangiosis'.

10 young patients from 7 to 16 years of age with moyamoya disease were treated by temporal muscle graft (encephalo-myo-synangiosis: EMS) for the purpose of increasing extracranial and intracranial collateral circulation. Ischemic cerebrovascular symptoms and signs were encountered in all of them without intracranial hemorrhage. The patients were observed for 6-26 months after the first operation. Transient ischemic attacks disappeared in 4 out of 7 patients, and improved in frequency and duration in the remaining 3. Paresis of the limbs disappeared in 1 patient and improved in 2. In 2 patients, facial palsy disappeared and dysarthria improved as well. Ataxia disappeared in 2 patients, and involuntary movement of the extremities disappeared in 2. There were no ineffective cases clinically. In postoperative carotid angiography, the middle cerebral arteries were visualized clearly to the peripheral portion mainly via the thickened deep temporal arteries, and the abnormal vascular networks in the region of the basal ganglia reduced in size in most of patients. Improvements in intelligence quotient, electroencephalogram and cerebral blood flow were also obtained in several patients, respectively. EMS seemed to be an effective surgical treatment in young patients with moyamoya disease who suffered from cerebral ischemic symptoms.

[Combination chemotherapy with bleomycin, vincristine and methotrexate for stage I non-seminomatous germ cell testicular tumors].

Ten patients with Stage I non-seminomatous germ cell testicular tumors underwent orchiectomy and combination chemotherapy with bleomycin, vincristine and methotrexate with CF rescue (BOM) as an induction therapy followed by cyclophosphamide, vincristine and methotrexate as a maintenance therapy (COM). The results were as follows: BOM seemed to be effective as a further treatment for Stage I non-seminomatous germ cell testicular tumor patients. All 10 patients are alive and disease-free for a period from 1 year and 6 months to 5 years and 3 months (mean: 3 years and 9 months). The chemotherapy should be continued at least for a year, since relapse occurred in 2 of 4 patients who received only induction therapy. However, they completely responded to other chemotherapy regimens (BVP and BOAM ). BOM and COM therapy were scarcely toxic and no serious side effects were recognized. From the above results, it was considered that combination chemotherapy (BOM and COM) can be an alternative treatment to the conventional retroperitoneal node dissection and radiation therapy for Stage I non-seminomatous germ cell tumors.

[Effects of noradrenaline and isoproterenol on the insulin secretion from the rat pancreas perfused isolatedly in situ].

Effects of noradrenaline and isoproterenol on insulin secretion in rat pancreas have been studied by means of the in situ perfusion method described previously. Noradrenaline: The insulin secretion from the rat pancreas perfused with the high glucose buffer decreases after the administration of 1 microgram/0.1 ml of noradrenaline into the perfusion fluid. These noradrenaline-induced hyposecretions can be reversed by pretreatment with phentolamine (10 micrograms/ml), while they are potentiated after propranolol (10 micrograms/ml) pretreatment. Isoproterenol: The insulin secretion from the rat pancreas perfused with the same buffer increases after the administration of 1 microgram/0.1 ml of isoproterenol into the perfusion fluid. These isoproterenol-induced hypersecretions are found to be potentiated after pretreatment with phentolamine or propranolol, and atropine (100 micrograms/ml) pretreatment does not produce any changes in the isoproterenol-induced hypersecretion. These results suggest that the beta-receptors on the B-cells stimulate the insulin secretion and the beta-receptors on the D-cells decrease them. This communication gives a short discussion of whether alpha-receptors may be on D-cells or not.

[Studies of anti-lymphocyte antibodies, with special reference to anti-B cell antibodies in choriocarcinoma].

For the purpose of elucidation of the immunologic properties of anti-lymphocyte antibodies which appeared in sera of choriocarcinoma patients, HLA typing of lymphocytes, complement-dependent lymphocytotoxicity test (CT) and mixed lymphocyte reaction (MLR)-blocking assay with patients' sera were performed in 7 couples with the disease. The results obtained were as follows: HLA distribution characteristic of patients and their husbands was not observed. In 3 of 7 patients, their sera exhibited positive CT to their husband's lymphocyte. In MLR-blocking assay, a significant blocking effects (BE) with sera were detected in all patients. The serum BE tended to be sustained for 4 to 8 months even after the decline of serum beta-hCG into the negative range. It was also observed that a case in whom the BE was positive for a longer period of time in a state of remission eventually underwent recurrence. It was demonstrated that the BE and MLR was induced by serum IgG antibodies closely associated with surface antigens of the husband's B-lymphocyte. Consequently, it was strongly suggested that serum anti-lymphocyte antibodies immunologically reflect the tumour burden in choriocarcinoma patients.

[Inhibitory effects of OK-432 and PSK on cell growth of in vitro human choriocarcinoma cell lines].

The cancer chemotherapy combined with immuno-potentiators, OK-432 and/or PSK, had yielded an improvement in survival rate of choriocarcinoma patients in our clinic as previously reported elsewhere. In the present study, for the purpose of evaluation of direct antitumor activity of OK-432 and PSK, inhibitory effects of the drugs on cell growth of GCH-1 and GCH-2 (in vitro cell lines of human choriocarcinoma) were analyzed referring to those of MTX (methotrexate) and Act-D (actinomycin D). 1) IC 50 (50% inhibitory concentration) of OK-432 and of PSK were 0.56 KE/ml on GCH-1 and 0.48 KE/ml on GCH-2, and 310 micrograms/ml on GCH-1 and 460 micrograms/ml on GCH-2, respectively. 2) The morphological changes of the target cells, observed by light and electron microscope, induced by OK-432 and PSK seemed to be not different from those by MTX and Act-D. 3) Serial changes of hCG levels in the culture media in which OK-432 or PSK was added were roughly comparable with those in MTX or Act-D. Thus, the direct antitumor activity of OK-432 and PSK on GCH-1 and GCH-2 was exquisitely weak judging from their IC50 and it was suggested that their direct antitumor effects in vivo might be clinically negligible in choriocarcinoma.

[Efficacy of paromomycin sulfate in the elimination of Diphyllobothrium species and Taenia saginata in clinical cases].

Recently it has been known that paromomycin sulfate had marked anthelmintic activity against some species of tapeworms. In this investigation, paromomycin sulfate was used for treating 6 cases of diphyllobothriasis and 1 case of Taenia saginata infection. Paromomycin sulfate was administered orally in a single dose or 2 divided doses of 40--60 mg/kg, being followed by a purge about 1.5 hours later. Strobila or fragments of proglottids were expelled from 5 cases, of which only 1 scolex was found in a case of infection with marine species of Diphyllobothrium. In 1 case of diphyllobothriasis no worm or worm-fragment was found in the stool even after the second challenge of treatment. Nevertheless follow-up examination for more than 1 month showed no evidence of remaining infection in all cases. Transient dizziness was observed in only 1 case, disappearing within about 30 minutes. Serum- and urine-examinations detected no abnormal signs in all cases excepting 1 case of diphyllobothriasis with suspected hepatitis. It was concluded that paromomycin sulfate is effective therapeutic agent for the treatment of taeniasis and diphyllobothriasis including marine species.
