[The effects of DDP on the seminiferous epithelium of rats].

Adult male rats were injected intraperitoneally with 2 mg, 4 mg and 6 mg Cis-diamminedichloroplatinum (DDP) per kg body weight. The animals were then sacrificed on the 7th, 24th or 35th day. Two specimens of testis from each rat were stained by H.E. technique, then the Pachytene primary spermatocytes and Sertoli cells in the cross sectioned tubules at stage IV-V, IX-X and XIII-XIV were counted. Significant decrease of Pachytene/Sertoli(P/S) valueinstage IX-X tubules on the 24th day (p less than 0.01) suggests that DDP has a cytotoxic effect on early differentiated type A spermatogonia (A1).

Cerebral hemodynamics in patients with moyamoya disease. A study of the epicerebral microcirculation by fluorescein angiography.

Fluorescein angiography was performed in 12 hemispheres of eight young patients with moyamoya disease to study the epicerebral microcirculation. Regional circulation time and the time interval between carotid injection of the dye and first filling of the cortical arteries were various and prolonged in the most parts. There were nonfilling areas by carotid injection of the dye in six hemispheres. By venous injection, the dye appeared in all vessels of the exposed areas. Extravasation of the dye was seen in four hemispheres. The degree of these abnormalities of the epicerebral microcirculation was well correlated with the findings in conventional cerebral angiography.

[Clinical effects of human fibroblast interferon in advanced gynecological cancers].

Human fibroblast interferon (IFN-beta) was given 13 cases of advanced gynecological cancers. Eight patients, who were clinically evaluable, were reported as follows; Patients consisted of ovarian adenocarcinoma (5), cervical adenocarcinoma (1), endometrial carcinoma (1) and tubal carcinoma (1). Route of administration was intravenous in 5 cases and intratumorous in 3 cases. IFN-beta dose ranged from 2, 650 X 10(4) to 10, 620 X 10(4) units. Clinical effects according to Koyama - Saitoh 's category was progressive disease (PD) in 7 cases and minor response (MR) only in one case who received intratumorous injection for recurrent tumor mass of tubal carcinoma in vaginal stump. Side effects of IFN-beta were chill and fever, fatigue and anorexia, leucocyte--and thrombocyte-- penia and hepatic dysfunction, though they were mild in grade and not dose-limiting factors. No anti-IFN-beta-antibodies were detected in any cases.

[Study on the vertebro-basilar system in "moyamoya" disease].

Posterior circulation in 82 children of "moyamoya" disease are studied. Two aspects of "moyamoya" disease, (ie; occlusive lesion of cerebral vessels and the development of abnormal vascular network as collateral channel,) are also detected in the vertebrobasilar system. Among 82 cases, 49 cases showed the occlusion of posterior cerebral artery at their quadrigeminal segment. Twenty-three showed the more proximal occlusive lesions. Vertebral artery occlusion were found in 3 cases. As the occlusive lesion progresses, abnormal vascular network at the posterior portion of skull base developed. This network consists mainly of thalamogeniculate artery, posterior choroidal artery, and also of other thalamoperforators. Visual field defect as an ischemic symptom of occipital lobe was detected in 9 cases (11%). Superficial temporal artery-middle cerebral artery anastomosis and encephalo-myo-synangiosis (temporal muscle graft), which were not considered to be so effective to the ischemia of the posterior circulation, were shown to exert indirect redistribution effect upon the vertebrobasilar system. However, this effect is such an indirect one that these surgical treatments cannot prevent the occurrence of ischemic stroke in the vertebrobasilar system. For this purpose, omentum transplantation to the occipital lobe may be needed as a method of direct revascularization.

Wound healing in the cornea of the chick embryo. V. An observation and quantitative assessment of the cell shapes in the isolated corneal epithelium during spreading in vitro.

Spatio-temporal changes in the shapes of the epithelial cells in culture were followed with the aid of scanning electron microscopy. On a substratum that enables the epithelium to spread extensively, the first remarkable change in shapes of the cells occurred at the margin of epithelium at 12 h of culture. The marginal cells formed leading edges with filo- or lamellipodia, flattened, and lost microvilli on surface. In accordance with those changes, the borderlines among cells became almost indiscrenible. Flattening of the cells was the essential characteristic associated with active epithelial spreading throughout the culture period. Elongation of cells of intermediate zone at right angles to the direction of the locomotion of the marginal cells at 24 h of culture was the second significant change. As the third, the change from the ordinary pentagonal or hexagonal to extraordinary tetragonal or other polygonal shapes, with or without irregular margins, began in cells of the intermediate area at 24 h and propagated to those in inner area. The active deformation of the inner cells with no space in which to move was considered to play some role in the extensive epithelial spreading.

Perineal groove and perineal canal.

Perineal groove is a rare congenital wet sulcus extending from the fourchette to the anus. With awareness of the lesion, surgery can be avoided. Perineal canal is a congenital anorectovestibular fistula coexistent with normal anus. Recently, it has become evident that these lesions are relatively common and clinically important variants of anorectal anomalies. We now report one patient each with these anomalies and briefly review the surgical problems.

Neonatal gastric perforations; a diagnostic clue in pre-perforative phase.

This paper is a retrospective analysis of the pre-perforative clinical picture in twenty-seven neonates with gastric perforations. In fifteen there was a remarkably consistent progressive pattern in the preperforative clinical course. We separated the clinical course of the disease into three distinct stages in relation to pathological changes in the gastric wall i.e. gastric ischemia and dysfunction, transmural peritonitis and paralytic ileus, and actual perforation. Emphasis was placed on the existance of a clinically detectable pre-perforative phase which, if interpreted correctly, should lead to earlier diagnosis and consequently more satisfactory therapeutic results.

Hepatitis B immunoglobulin (HBIG) efficacy in the prevention of perinatal transmission of hepatitis B virus (HBV) infection.

Twelve out of 13 infants born from mothers having both HBsAg and HBeAg developed HBV carrier state within 4 months after birth. On the other hand, no babies from mothers having HBsAg and anti-HBe developed HBV carrier state. In order to prevent perinatal transmission of HBV, HBIG was administered into 14 babies born from mothers with positive HBeAg three or four times during 6 months after birth. During 12 months or more of observation period 5 out of 14 infants who received HBIG acquired active anti-HBs response after discontinuation of HBIG (passive-active immunization). However, 3 out of 14 infants unfortunately developed persistently positive HBsAg antigenemia at 12th, 14th and 14th month respectively after birth. Remaining 6 babies still have no virus markers, indicating not infected. These results indicates that HBIG administration was extremely effective for prevention of perinatal transmission of HBV. However, additional preventive measures with active immunization (HBV vaccine) seems to be necessary to prevent completely the perinatal transmission of HBV.

Regulation of interdigestive contractions in the denervated stomach.

The effects of thiopental sodium on the regulation of interdigestive migrating contractions (IMC) in the stomach were investigated in dogs with extrinsically denervated fundic pouches. During the interdigestive state, strong phasic contractions in the pouch took place consistently in accordance with the IMC in the main stomach. The cyclic increase in plasma motilin concentration was more closely correlated with the increase in contractile activity in the pouch than the increase in the main stomach. General anesthesia by thiopental sodium promptly inhibited the IMC in the main stomach but the contractile activity in the pouch was not affected but gradually decreased in proportion to the decrease in plasma motilin concentration. In conclusion, contractile activity in the extrinsically denervated pouch, which is quite independent of the CNS, is directly controlled by a humoral factor(s), motilin. However, interdigestive motor activity in the extrinsically intact stomach appears to be regulated by the CNS as well as peripheral circulation humoral factors.

Computed tomographic demonstration of the effect of proximal parent artery ligation for giant intracranial aneurysms.

Four patients with giant intracranial aneurysms, which measured over 2.5 cm in diameter, were treated by ligation of the proximal parent artery. The location of the aneurysms were intracavernous in two patients, paraophthalmic in one patient, and vertebral in the other patient. The effect of ligation was followed by repeated computed tomography scans with angiographic correlation. The aneurysms thrombosed soon after ligation, and with time, gradually organized with a reversal of the mass effect. Five years after the ligation, one aneurysm thrombosed completely and lost its mass effect upon the adjacent structures. For giant aneurysms treated with ligation of the proximal parent artery, computed tomography scans are a useful method for continuing review, producing precise information concerning actual size, thrombus formation, and various pathological changes.

Interrelation between luteal cell types in steroidogenesis in vitro of human corpus luteum.

Slices of human corpora lutea of menstruation at different stages, or of inner and outer layers consisting of pure granulosa lutein cells and combined theca and granulosa lutein cells respectively, were incubated with [4-14C]-pregnenolone or with [4-14C]-androstenedione respectively. Conversion of 14C-radioactivity to progestins, androgens and/or estrogens was assessed by the reverse dilution technique with recrystallization to constant specific activity. Although progesterone was the major steroid formed from pregnenolone, a transient increase in androgen formation with a concomitant decrease in estrogen formation in developing corpora lutea was followed by an increase in estrogen with a decrease in androgen formation in mature corpora lutea. The reciprocal relation between estrogen and androgen formation is associated with the behaviour of theca lutein cells since the cells disappeared at the former and clearly differentiated at the latter stages of corpora lutea. The outer layers consistently produced more 17-hydroxyprogesterone, androgens and estrogens from pregnenolone than the inner layers, while the inner layers converted constantly more androstenedione to estrogens than the outer layers. It is suggested that aromatase activity of granulosa lutein cells in co-existence with theca lutein cells is required by human corpora lutea for fulfillment of the function of estrogen synthesis.
