[Analysis of GM2 and anti-GM2 antibody in normal pregnancy].

Oncofetal antigen immunogenic (OFA-I), an antigen common to the fetal brain and some malignant tumors (derived from ectoderm), is immunogenic in man, and anti-OFA-I antibody appears in the sera of not only malignant patients but also pregnant women. Biochemical analysis revealed that OFA-I is ganglioside GM2 and GD2. In the present study, GM2 and anti-GM2 antibody were investigated as to the kinetics in pregnant sera and the tissue localization in placenta and decidua. The results obtained were as follows: 1. The concentration of GM2 was 76.7 pmol/ml (mean) in pregnant sera and 10.8 pmol/ml (mean) in cord sera. GM2 was not measurable in the sera of non-pregnant women. 2. GM2 was detected by thin layer chromatogram-immunostaining methods in the gangliosides extracted from decidua and term placenta. GM2 was, however, undetected from villi at an early stage. 3. In immunohistological analysis for GM2, positive findings were observed in decidua and in villous stroma of term placenta. In contrast, no distinct positive findings were observed in trophoblast at any stage. 4. Anti-GM2 antibody appeared in pregnant sera at an early stage and was detectable up to the 5th day postpartum.

[Middle term follow-up and comparative study of valve replacement with four kinds of new model bovine pericardial xenografts].

Our early clinical experience (up to 4 year's follow-up) with four new pericardial xenografts were reviewed. During the period July 1983 to December 1986, 148 Ionescu-Shiley Pericardial Xenografts (ISL) in 130 patients, 68 Carpentier-Edwards pericardial xenografts (CEP) in 65, 32 Mitroflow pericardial xenografts (MF) in 29 and 36 Hancock pericardial xenografts (HP) in 29 have been implanted. The actuarial survival rates at 3.3 years are 89.9 +/- 2.7% for ISL, 92.3 +/- 3.3% for CEP, 93.1 +/- 4.7% for MF and 93.1 +/- 4.7% for HP. Fifteen cases of primary tissue failure (PTF) were caused in all groups but CEP. The actuarial free rates from PTF at 3.3 years were 92.9 +/- 2.4% for ISL, 100% for CEP, 95.5 +/- 4.4% for MF, 82.6 +/- 7.9% for HP. All bioprostheses explanted because of PTF showed commissural tears occurred at the top of the stent posts or at the edge of the stent. In this respect, it is the reason why the cases with CEP were free from PTF that CEP has been achieved to be improved its frame design. The incidences of prosthetic valve endocarditis were not different among these kinds of bioprosthetic valves. The free rates from thromboembolism at 3.3 years were 97.5 +/- 1.5% for ISL, 98.3 +/- 1.7% for CEP, 96.0 +/- 3.9% for MF and 88.7 +/- 6.1% for HP. There was no patient with CEP and MF suffered from thromboembolism with sinus rhythm. In comparison of these 4 valves, we conclude that CEP is useful clinically because of its satisfactory durability and antithrombogenicity.

[Continuous intra-arterial infusion chemotherapy for malignant bone and soft tissue tumors in pelvis and trunk areas].

We evaluated the effectiveness of intra-arterial infusion chemotherapy for 21 cases of malignant bone and soft tissue tumors in the pelvis and trunk areas. Eight cases are alive for one or two years without operation. In all cases, pain was decreased and quality of life improved. We concluded that intra-arterial infusion chemotherapy is one of the useful treatments for malignant bone and soft tissue tumors in the pelvis and trunk areas.

Increased intraglomerular thrombin formation in diabetic microangiopathy.

Estimations of soluble fibrin monomer complexes (SFMC) in plasma are a convenient index of thrombin activation. Renal venous-arterial differences in plasma SFMC concentrations were determined in 16 randomly chosen diabetic patients by sampling directly and simultaneously from the renal artery and vein according to the method of Seldinger. In all subjects, SFMC concentrations were higher in the renal vein than in the renal artery, indicating that the kidney is an important source of SFMC. Venous-arterial differences were markedly elevated in patients with severe renal and retinal microangiopathy coupled with hypertension. The hypothesis is advanced that elevated plasma SFMC levels lead to abnormal fibrin deposits in lesioned glomeruli and retinal vessels. It is postulated that plasma SFMC may be a useful parameter for the assessment of diabetic vascular complications.

Ovarian Sertoli-Leydig cell tumor with elevated serum alpha-fetoprotein.

An 18-year-old girl with virilization and an elevated level of serum alpha-fetoprotein (AFP) was found to have a Sertoli-Leydig cell tumor (SLCT) of the ovary. The tumor had a heterologous element of gastrointestinal type epithelium, retiform pattern, and multilocular cysts, many of which were lined by Sertoli-like cells. Alpha-fetoprotein was detected immunohistochemically in cells with the histologic appearance of Sertoli cells, and testosterone was detected in the Leydig cells. Results of lectin affinity chromatography indicated that AFP produced by this tumor was different from the AFP to be found in the liver.

Type A behavior pattern in Japanese employees: cross-cultural comparison of major factors in Jenkins Activity Survey (JAS) responses.

The responses to the Japanese edition of the Jenkins Activity Survey (JAS) (Form C) were analyzed in 1682 male employees of a Japanese enterprise to investigate the characteristics of the Type A behavior pattern (TABP) in Japan. When the occupational level was controlled, the mean values of the Type A score were found to be quite similar to those of the Western Collaborative Group Study (WCGS), although those of factor H scores were considerably lower than those of the WCGS. Additionally, the results of a factor analysis conducted on the responses to JAS questions showed that the factor structure of the responses was basically equal to that found in the WCGS. In contrast to factor J of the WCGS, however, Japanese factor J did not comprise the items which signaled rises in positions or incomes in recent years. The results of this study indicate that the JAS is able to measure the TABP in Japan, at least concerning its three component factors. Furthermore, they suggest that the TAPB is not necessarily reinforced in Japanese occupational environments.

Intravesical combination chemotherapy with mitomycin C and doxorubicin for carcinoma in situ of the bladder.

We treated 30 patients with carcinoma in situ of the bladder via intravesical combination chemotherapy. As an induction therapy 20 mg. mitomycin C on day 1 and 40 mg. doxorubicin on day 2 were instilled into the bladder once a week for 5 consecutive weeks. Patients who achieved complete response were assigned a maintenance instillation of mitomycin C alone every 2 to 4 weeks for 1 year. A total of 19 patients achieved complete response following an initial course of induction therapy and 2 partial responders to initial therapy also achieved complete response after repeated induction therapy, resulting in a 70 per cent over-all complete response rate. Toxicity was considerable with moderate to severe bladder irritation occurring in 20 patients but it was tolerable in the majority. Of 21 complete responses 13 remained free of disease for 6 to 43 months (average of 23 months), 5 had recurrent carcinoma in situ and 1 had invasive urethral cancer. In contrast, of 9 nonresponders 2 and 1 had invasive cancer of the bladder and ureter, respectively.

Relationship of changing delta 4-steroid 5 alpha-reductase activity to [125I]iododeoxyuridine uptake during regeneration of involuted rat prostates.

To elucidate the phenotypic expression of proliferating prostatic cells, rats were castrated, and the regenerating process of involuted ventral prostates during testosterone propionate (TP) administration was investigated by examining morphology, [5-125I]iododeoxyuridine (125I-UdR) uptake, DNA content, weight, acid phosphatase, and delta 4-steroid 5 alpha-reductase (5 alpha-reductase) activities. Morphologically, TP treatment initially increased the number of epithelial cells lining glandular lobules and subsequently restored the shape of epithelial cells. 125I-UdR uptake peaked on Day 3 of TP treatment and stayed at higher levels than for uncastrated controls until Day 14 of treatment. Prostatic weight, protein content, acid phosphatase, and DNA content returned to uncastrated control levels by Day 14 of TP treatment. TP administration markedly stimulated prostatic 5 alpha-reductase activity, which peaked on the Day 5 of treatment and decreased to uncastrated control levels by Day 14 of treatment. It is concluded that TP administration to castrated rats initially induced active mitotic division of the remaining stem cells, followed by formation of differentiated functional epithelial cells. Prostatic 5 alpha-reductase was highly active at the initial phase of active mitotic cell division. The major portion of the increased enzyme activity can be regarded as a phenotypic expression of stem or transient cells of prostatic epithelium.

A correlation between the expression of CD 8 antigen and specific cytotoxicity of tumor-infiltrating lymphocytes.

Tumor-infiltrating lymphocytes (TIL) from six gynecologic malignant tumors (two uterine cervical cancers, two ovarian serous cystadenocarcinomas, and two uterine corpus cancers), cultured in the presence of recombinant interleukin 2, were assayed for their cytotoxic activities against various fresh tumor cells including autologous tumors. A clear correlation between phenotype and cytotoxic activity of TIL was observed. Four of six TIL preparations exhibited strong cytotoxic activity against autologous fresh tumor target cells, and were all CD8+. In contrast, cytotoxic activity was not detected in any of the CD4+ TIL preparations. The cytotoxic activities of the CD8+ TIL preparations were highly specific; only autologous fresh tumor cells were lysed. This result is consistent with the notion that TIL are of a different cell lineage from lymphokine-activated killer cells which are antigen-nonspecific and CD8-. Instead, TIL appear to be of cytotoxic T cell lineage that is highly antigen-specific and CD8+. To explore the potential for clinical use, we have attempted to augment the cytotoxic activities of these CD8+ TIL by treatment of the target tumor cells with gamma interferon (IFN) in vitro, hoping that elevated expression of MHC class I gene products on the cell surface would enhance their recognition. It was observed that brief treatment of freshly prepared tumor cells in vitro with gamma-IFN resulted in augmentation of the expression of MHC class I gene products, and the treated tumor cells were more susceptible to lysis by TIL than untreated cells.

Is immunotherapy for habitual aborters an immunologically hazardous procedure for infants?

Physical development and tests of immunologic function are reported from the first year of life for 13 infants born to mothers who were habitual aborters and who had undergone subcutaneous vaccination with their husband's lymphocytes. The mean weight of the infants at birth was 2,975 +/- 540 g, including one infant who was small for dates. Physical development parameters for the first year were all within normal range. Immunologic studies were performed at ages 0, 1, 3, 6, 9, and 12 months. The studies included the following: (1) serum levels of immunoglobulins and complement components; (2) the number of peripheral blood lymphocytes (PBL) and their subpopulations; and (3) proliferative responses by PBL against unrelated lymphocytes and some mitogens (pokeweed, phytohemagglutinin, and concanavalin A). All were normal when compared with studies of infants of nonimmunized mothers. These observations suggested that subcutaneous vaccination of women with their husband's lymphocytes did not result in any adverse effects on their infants' physical or immunologic development.

MLR-blocking antibodies are directed against alloantigens expressed on syncytiotrophoblasts.

In this study, the reactivity of mixed lymphocyte reaction-blocking antibodies toward trophoblasts was investigated. Nineteen sera that had a significant level of MLR-blocking effect were obtained from normal pregnant women and from primary habitual aborters after immunotherapy. The blocking effects of these sera were studied before and after absorption with syncytiotrophoblast plasma membrane (STPM) from the respective mothers' placenta (autologous) as an experimental group and from unrelated mothers' placenta (allogeneic) as a control. The HLA types of cord blood lymphocytes corresponding to ten of these STPM were then determined. In the study with the 11 normal pregnancy sera, the blocking effect of every serum was partially removed by autologous STPM absorption. This revealed a significant difference between the preabsorption and postabsorption MLR-blocking effect. Nevertheless, absorption by the allogeneic STPM either partially removed or did not remove the blocking effect. Furthermore, there was a statistically significant difference in the elimination of the MLR-blocking effect between the autologous STPM and the allogeneic STPM. Studies with the sera from the immunized aborters revealed similar results. There was no statistically significant difference in the elimination of the antibodies between absorption with autologous and with allogeneic STPM. In these habitual aborters' sera, comparison of paternal stimulator and cord blood lymphocytes HLA types showed that the positive allogeneic STPM absorptions could not be explained sufficiently by coincidence of HLA types. Further studies are needed to determine whether the alloantigens are HLA-linked or not.

Angiographic follow-up study of cerebral arteriovenous malformations with reference to their enlargement and regression.

Twenty patients with cerebral arteriovenous malformations (AVMs) were followed up by angiography. Of the 20 AVMs, 16 were nontreated and the remaining 4 were residual. The follow-up periods between the two examinations were from 5 to 28 years, with a median interval of 15 years. At follow-up, the size of the AVM had increased in 4 patients (20%), decreased in 4 (20%), totally regressed in 4 (20%), and was unchanged in 8 (40%). In cases of enlarged AVMs, the patients were all young: from 0 to 11 years old at first angiography and from 6 to 30 at second angiography. The decrease or total regression of the AVM occurred in older patients (over 30 years old at second angiography), and the AVMs were relatively small and fed by single or a few feeders. The patient's age and the size of AVM should be considered when operative indications are discussed.

Intracranial extradural pressure monitoring after direct operation on ruptured cerebral aneurysms.

Intracranial extradural pressure (ICP) was monitored by using a miniaturized transducer for an average period of 8 days after direct operation in 55 patients with ruptured cerebral aneurysms. Acute stage operation and drainage of cerebrospinal fluid were simultaneously performed in most of the patients. In many grade I patients with satisfactory cisternal drainage, ICP was monotonously stable, with faint pressure waves. When the drained fluid volume decreased or drainage was removed, however, ICP was elevated moderately, with associated pressure waves present. There were no A-waves observed in any of the patients. Decreases of mean ICP and disappearance of pressure waves were found 2 to 3 hours after infusion of 200 or 300 ml of 10% glycerol or 20% mannitol in patients without drainage or with an inadequate drain, but were not found in patients with a good drain. In a patient showing diffuse severe vasospasm, a rapid elevation of ICP caused by marked brain edema was observed. In patients with residual aneurysms or incompletely clipped aneurysms, ICP increased immediately after rupture of these aneurysms. The daily mean ICP was higher in patients with a clinically poor condition and/or severe subarachnoid hemorrhage on admission than in those with a good condition and/or mild subarachnoid hemorrhage, in spite of a functional drain. There was a poor outcome in about half of the patients showing maximum daily mean ICP greater than 30 mm Hg or frequent B-waves. No complications caused by ICP monitoring were found.(ABSTRACT TRUNCATED AT 250 WORDS)

Melanin production in cultured albino melanocytes transfected with mouse tyrosinase cDNA.

An attempt was made in the present study to express mouse tyrosinase cDNAs fused with the authentic genomic 5' non-coding flanking sequence in cultured albino melanocytes. One of the cDNA sequences, which expressed successfully and produced melanin pigments, was analyzed with respect to deduced amino acid sequence. Sequencing of the tyrosinase genomic gene revealed the existence of several sets of a characteristic structure which consists of a chain of two successive stem structures, CCAAT-homology and TATA box at its 5' non-coding region. It seems possible that this region represents the regulatory element of the tyrosinase gene. Unusually long GA cluster at 5' upstream region was also found.
