Result of immunotherapy on patients with unexplained recurrent abortion: a beneficial treatment for patients with negative blocking antibodies.

Thirty-nine unexplained recurrent aborters underwent vaccination using husband's lymphocytes according to the previously reported protocol. No mixed lymphocyte culture reaction-blocking antibodies (MLR-BAbs) were observed in these patients prior to vaccination. Of 35 newly pregnant patients after vaccination(s), pregnancy successfully continued in 28 (80.0%) and have already been terminated with a liveborn offspring. Pregnancy outcome was also analyzed in unexplained recurrent aborters who revealed positive MLR-BAbs without immunotherapy. In this group, out of eight pregnancies in seven patients, five (62.5%) continued beyond their critical period of 14 wks of gestation. Three infants born from these pregnancies, however, presented severe abnormalities. Furthermore, outcome of 14 pregnancies in 12 unexplained recurrent aborters with negative MLR-BAbs was analyzed since they had become pregnant without immunotherapy; pregnancy was successfully continued in only four cases (28.6%). Thus, vaccination using husband's lymphocytes on unexplained recurrent aborters with negative MLR-BAbs is suggested to be effective. In addition, it is suggested that immunotherapy for patients with positive MLR-BAbs should be carefully followed.

[A clinical study on five cases of respiratory infections caused by Neisseria meningitidis].

Five cases of the respiratory infections, in which Neisseria meningitidis was isolated upon the examination of the transtracheal aspiration (TTA), were reported. Patients studied were four males at the ages of 18, 56, 66 and 78 years, and a 18-year old female. Five cases were bronchopneumonia (two cases), acute bronchitis (two cases) and diffuse panbronchiolitis (DPB) (one case). Underlying diseases were as follows: lung cancer or acute myocardial infarction in cases of bronchopneumonia, and interstitial pneumonia or pulmonary tuberculosis in cases of acute bronchitis. There was a case in which only N. meningitidis was cultured from specimens obtained by TTA, although another different organism, in addition to N. meningitidis, was recovered from the patient of other cases. Other organisms found together with N. miningitidis were H. influenzae (2 cases), S. dysgalactiae (1 case) and M. tuberculosis (1 case). Predisposing factors were common cold in the female patient and bronchoscopic examination in the cases of lung cancer and of interstitial pneumonia. Two of the five cases occurred consecutively in the same room and so they were considered as nosocomial infections. With these findings, it can be posturated that N. meningitidis might be one of the etiological agents of the respiratory infections.

Enhanced interleukin production after long-term administration of erythromycin stearate.

The effects of erythromycin stearate (10 mg/kg/day) were studied on productions of interleukin (IL)-1 and -2 in mice after a long-term treatment. A 28-day treatment resulted in higher levels of IL-1 production by macrophages and of IL-2 production by splenocytes, while a 7-day treatment did not increase them. T-cell growth factor activity of IL-2 preparation prepared on day 28 of treatment as determined by HT-2 cell proliferation was reduced by about 40% in the presence of anti-murine IL-4 monoclonal antibodies, while control IL-2 activity was not reduced. Furthermore, a 28-day treatment with erythromycin stearate increased concanavalin A-induced blastogenesis of splenocytes significantly. These results suggest that long-term treatment with erythromycin stearate can stimulate host defense by increasing interleukin production.

'Stereovectorcardiogram' made by stereolithography.

A method for making directly a single solid vectorcardiogram is reported. A solid vectorcardiogram was made by employing an apparatus for production of three-dimensional objects by stereolithography. Stereolithography is a computer-aided manufacturing technique that is practiced in conjunction with computer-aided design and engineering. To design the solid vectorcardiogram, voltages in the X, Y and Z axes obtained according to the Frank lead are memorized in the computer-aided design system. By use of the stereolithography apparatus, the designed solid vectorcardiogram is formed in a vat filled with liquid plastic. In the vat, the plastic changes rapidly from liquid to solid in the presence of ultraviolet light so that a solid vectorcardiogram made of solid plastic may be formed.

Melanization in albino mice transformed by introducing cloned mouse tyrosinase gene.

We introduced a mouse tyrosinase minigene, mg-Tyrs-J, in which the authentic genomic 5' non-coding flanking sequence was fused to a mouse tyrosinase cDNA, into fertilized egges of albino mice. Of the 25 animals that developed from the injected eggs, four mice exhibited pigmented hair and eyes. Histological analysis of the transgenic mice revealed that the melanogenesis was restricted to hair bulbs and eyes. These results suggest that this minigene encodes active tyrosinase protein and that its 5' flanking region contains the sequences regulating expression of mouse tyrosinase gene. This is the first report of a successful expression of tyrosinase gene and of pigment production in transgenic mice.

Distribution of catecholaminergic receptors in the rat's pancreas islet.

The effects of noradrenaline, adrenaline, isoproterenol, dopamine, apomorphine and their blockers on biphasic insulin secretion induced by 0.3% glucose were observed by means of a modified Lacy's perifusion method. Noradrenaline and adrenaline completely inhibit biphasic insulin secretion, and their effects disappeared completely after pretreatment with phentolamine. Dopamine inhibited the first phase completely and inhibited the second phase partially, and the inhibitory action of the dopamine disappeared after the pretreatment with phentolamine or propranolol. Isoproterenol had no effect on glucose-induced insulin secretion, whereas after treatment with phentolamine, isoproterenol produced a first-phase type of response. Domperidone blocked the effect of dopamine on the suppression of the first phase response. Apomorphine produced the second phase suppression slightly. It was concluded that the sympathetic alpha receptor and DA1 was distributed on the B-cells, the sympathetic beta 2 receptors on the D-cell and the DA2 on the varicosity of the sympathetic beta 2 neuron.

Reconstruction for the vertebral artery stenosis at its origin--bypass surgery using a saphenous vein graft with external shunting.

Eight patients with vertebral artery (VA) stenosis at its origin were surgically treated by a new reconstructive technique. Two patients suffered vertebrobasilar transient ischemic attacks, and five had brainstem and/or cerebellar infarction. All patients had multiple stenotic lesions, including bilateral VA stenoses in one and unilateral VA stenosis plus contralateral VA occlusion in seven. They underwent a subclavian artery to VA bypass using a saphenous short vein graft with external shunting. Postoperatively, no patients had aggravated neurological symptoms, and angiography showed all bypasses to be patent. One had an episode of transient loss of consciousness 8 months postoperatively, but the others showed no episodes of ischemia during an average follow-up period of 2 years and 10 months. The results indicate that this procedure is safer and more useful than other surgical procedures.

[Multiple urothelial cancers accompanied by acquired cystic disease of the kidney in a long-term hemodialysis patient].

A 41-year-old miller undergoing hemodialysis for 9 years presented with recurrent episodes of asymptomatic gross hematuria of a 7 years' duration and left flank pain with slight fever of a four months' duration. CT scans revealed bilateral acquired cystic disease of the kidney (ACDK) and a mass (phi 4 cm) in the medial portion of the left kidney, which was angiographically a hypovascular tumor. Cystoscopic examination revealed multiple papillary tumors arising from the entire bladder wall. These findings suggested that the renal mass was a renal pelvic tumor. Bilateral nephroureterectomy with left paraaortic lymph nodes dissection and radical cystectomy were carried out, followed by urethrectomy two weeks later. Histologically, multiple papillary transitional cell carcinomas with G2 greater than G3 anaplasia were found in the left renal pelvis, the lower portion of the left ureter and the bladder. The tumors were predominantly superficial, although the renal pelvic one focally invaded into the muscular layer. Lymph node metastasis and renal cell carcinoma were not recognized. Postoperative course was uneventful with normal blood pressure, though anemia was slightly progressive. He has been free of disease for six months.

[Late results after open repair of tetralogy of Fallot: a 13-24 year follow-up].

Late results in 139 patients undergoing open repair of tetralogy of Fallot from 1965 to December 1975 were analyzed. Their ages ranged from 1 year to 35 years. All patients were repaired through a right ventricular approach. One hundred two patients had infundibulectomy alone (non-patch group) and 14 required right ventricular outflow patch (RV-patch group) and 23 trans-annular patch. The hospital mortality rate was 18.7% (26 cases) and 103 patients were discharged and have been followed until death or for 13-24 years. Cardiac catheterizations were performed in 80 patients an average of 1.8 years after correction. Right ventricular systolic pressure was 49.8 +/- 22.4 mmHg in non-patch group, 41 +/- 16.5 mmHg in RV-patch group and 54.5 +/- 14.2 mmHg in PA-patch group. Seven in 11 patients of PA-patch group had over an 8 mmHg of right ventricular end diastolic pressure. Six patients died during the follow up: four deaths in the first 5 years, 2 between 5 and 15 years. The causes of death were congestive heart failure (3), sudden (1), pneumonia (1) and accidental (1). Late complications were 3 arrhythmias and 2 reoperations. Event free rate at 24 years was 74.8 +/- 3.8%.

[A report of successful treatment of mediastinitis and graft infection after ascending and aortic arch reconstruction].

A 43 years old male patient had composite graft replacement of ascending aorta and aortic arch with total reconstruction for his aortic valve regurgitation and ascending and aortic arch aneurysm. He developed high fever leucocytosis and in the early postoperative period and was diagnosed as aorta mediastinitis by proving bacterial culture positive in blood as well as in the drainage fluid. Re-exploration of mediastinal cavity was carried out and the necrotic tissues were removed as much as possible. The mediastinal cavity, especially around the graft was also thoroughly irrigated and washed with Iodine (Isozin) solution. Following this, mediastinal cavity was packed with non-diluted solution of Iodine sponge. Thereafter mediastinum was left open by applying sprint and patient was brought back to ICU. He was kept controlled ventilation and deep sedation. Mediastinal cavity was irrigated and re-packed with same solution every 8 hours for the following 48 hours so as to expect complete disinfection of the mediastinal cavity. He was brought back to the operation room again and midsternal incision was extended to the abdomen. Half portion of the major omentum was isolated from the stomach and transplanted into the mediastinal cavity with its vascular supply. Special care was taken to wrap around the graft with omentum. The sternum and skin were closed without any drainage tube. As postoperative complications most probably related to the application of high concentration of Iodine solution, he developed severe hepatic failure for which he was treated with hemofiltration. This was gradually improved and the infection was well controlled. He was discharged from the hospital on the 77th postoperative day.(ABSTRACT TRUNCATED AT 250 WORDS)

The charge-dependent materno-fetal barrier in the rat.

The charge-dependent materno-fetal barrier of near-term rats was examined with two pairs of radiolabelled proteins differently charged. When the proteins were injected intra-arterially, the maternal blood concentrations of the two cationized proteins (BSA and human IgG) exceeded those of native proteins in the chorioallantoic placenta and fetus during the period of examination (15-180 min). In the yolk sac placenta, these percentages were essentially the same as those of native proteins. Following intra-placental injection, the amounts of the cationized proteins in the chorioallantoic placenta and fetus were significantly (p less than 0.01) greater than those of native proteins at the particular times examined (5 and 60 min). In contrast, the amounts of cationized proteins in the yolk sac placenta were basically the same as those of native proteins. The data presented above indicate the charge on proteins that considerably influenced their materno-fetal transfer, particularly via the chorioallantoic placenta.

[Valve prostheses of mitral position in children].

Twenty children less than 15 years old of age who had undergone mitral (left-sided atrioventricular) valve replacement before August 1983 have been followed up until October 1988. Fourteen biological valves (B-group) and seventeen mechanical valves (M-group) were used in them. Preoperative conditions had been similar between B- and M-groups. For the B-group, there were one operative death and two late deaths. For the M-group there were no operative death and three late deaths. An actuarial survival rate at ten years was 53.5 +/- 23.3% for the B-group and 85.7 +/- 9.4% for the M-group (not significant). All biological valves were calcified and failed in 27 to 89 months. Three mechanical valves caused thrombosis. There was no other complication for both groups. An event free rate was 0% at eight years for the B-group, however it was 70.6 +/- 11.0% at ten years for the M-group (p less than 0.025). Our data indicates that the mechanical valve is more suitable than the biological valve for children.

[The effect of silicone oil on the ocular tissues].

It has been pointed out that the silicone oil tamponade can result in complications such as corneal damage, elevation of intraocular pressure and retinal toxicity against. The effect of silicone oil on the ocular tissues was investigated histopathologically by injecting silicone oil into the anterior chamber of the eyes of rabbits. In addition, in order to study its effect on the retina, silicone oil was injected into the vitreous cavity after vitrectomy. The eyes were extracted 3, 6, 9, 12 and 18 months after injection and various tissues were observed by light and electron microscopy. Silicone oil particles were first observed in the retro-corneal membrane 18 months after injection. In the trabecular meshwork silicone oil particles were seen for the first time 12 months after injection. Migrating cells engulfing silicone oil particles were attached to the internal limiting membrane of the retina three months after injection. Twelve and 18 months after injection, silicone oil particles passed through the internal limiting membrane and were engulfed by Mueller cells.

Sex differences in blood constituents of rats following tail suspension.

Tail suspension was utilized to study sex differences in the effects of hypokinesia on blood constituents of rats. Twelve male and twelve female rats were used. After four weeks of exposure to tail suspension or non-treatment, the rats were laparotomized under ether anesthesia and blood samples were obtained. Biochemical tests on the blood samples were then performed immediately. Hematocrit and serum iron were also estimated. The results were as follows: 1) The same tendencies were noted in blood constituents between male and female rats in the control group and the tail suspension group. 2) The average levels of blood albumin, creatinine, uric acid, glucose, triglyceride, serum iron and hemoglobin in the tail suspension group were lower than those in the control group, especially in male rats.
