Plasma prolidase activity as a possible diagnostic index of chronic hepatic abscess in cattle.

The usefulness of prolidase as a biochemical parameter to represent the chronic state of hepatic abscess was discussed in eight cattle experimentally inoculated with Fusobacterium necrophorum and 18 spontaneously affected cattle. Blood was daily collected to measure the plasma prolidase activity and sialic acid level from the experimental cattle for 90 days after inoculation. In three out of four cattle affected with hepatic abscess, prolidase activity began to rise about 40 days after inoculation, and maintained high activity till 90 days. In the same cattle the sialic acid concentration increased from 7 to 10 days after inoculation, and gradually returned to the normal value 50 days after it. Another cow showed a similar change in early stage of experiment, but prolidase activity decreased after 70 days and sialic acid concentration maintained high level till 90 days. In two cattle, which showed scars but no abscess on autopsy, the prolidase activity increased temporarily from 40 to 55 days after inoculation. In the control cattle inoculated with an inactivated bacterial suspension, neither the sialic acid level nor the prolidase activity showed any large variation in the experimental period. Among the spontaneously affected cattle, those with a high sialic acid level revealed normal prolidase activity and those with a normal sialic acid level had high prolidase activity.

Serum content of zinc and vitamin C in severely handicapped children.

As fracture is known to occur more easily in severely handicapped children (SHC), bone mineral content, serum zinc level and serum vitamin C level were determined in 65 SHC. These parameters were found to be greatly reduced in all patients. The decreases in zinc and vitamin C in the serum appeared to be associated with that in bone mineral content. Supplements of zinc and vitamin C may be important to decrease the chance of fracture due to diminished bone mineral content in SHC.

Regional cortical blood flow during extra-intracranial bypass surgery in young patients with moyamoya disease.

Regional cortical blood flow (rCBF) was measured with a thermal diffusion flow probe in 14 cortical regions of 12 young patients with moyamoya disease before and after completion of superficial temporal-middle cerebral artery anastomosis. The prebypass rCBF value was low in most patients, especially in the frontal regions. On temporary occlusion of the cortical artery during the surgical procedure, no significant drop in rCBF occurred. rCBF increased immediately after anastomosis in all but one region and continued to increase for 5-10 minutes in four of the eight regions measured. In the other four regions, rCBF declined gradually after the initial increase, and the final increase was slight. The average increase in rCBF was significant at 1-2 minutes and at 5-10 minutes after anastomosis. However, the latter increase did not bring the flow into the normal range. It may be that the initial increase in postbypass rCBF is determined solely by the pressure gradient between the donor and recipient arteries, and that subsequent rCBF is controlled by other factors within the brain. In one patient who underwent double anastomoses to the frontal and temporal lobes, neither anastomosis increased rCBF in the non-corresponding lobe to recipient artery. This suggests that there is no direct connection between supra- and infra-Sylvian arteries and supports the concept of nonuniform epicerebral microcirculation in moyamoya disease.

[Reconstructive vascular surgery in the territory of the anterior cerebral artery in children with moyamoya disease].

The hemodynamics of the anterior cerebral arteries (ACAs) were studied in 28 children with moyamoya disease. In 39% of 56 cerebral hemispheres, antegrade filling of the ACAs was not verified by preoperative angiography. On the other hand, adequate collateral pathways to the ACA territory were generally not achieved by encephalomyosynangiosis (EMS) or superficial temporal artery-middle cerebral artery (STA-MCA) anastomosis. The authors have devised new surgical methods for the revascularization of the ACA territory, which they term "encephaloarteriosynangiosis" (EAS) and "encephalogaleosynangiosis" (EGS). In these procedures, one or several burr holes are made in the frontal skull, the dura mater is incised, and either the frontal branch of the STA or the pedicled galea aponeurotica stump is placed on the surface of the frontal cortex. In addition to EMS or STA-MCA anastomosis, these methods were applied to 23 cerebral hemispheres in 14 pediatric patients. In 14 cerebral hemispheres (61%) of 10 patients, postoperative external carotid angiograms showed some cortical branches in the ACA territory, and clinical improvement was achieved in patients with transient ischemic attacks affecting the lower extremities. The results of this study demonstrate that EAS and EGS are simple and useful techniques for revascularization of the ACA territory. In addition, EGS can be applied to the territory of the posterior cerebral artery.

[Some phenolic compounds stimulate the proliferation of human pulpal fibroblasts].

Phenolic compounds are widely used in dental clinics especially for the treatment of inflammatory responses of the dental pulp. However, the role of these agents in the repair of pulpal connective tissue is unclear. In the present study, an effect has been identified in several phenolic compounds that can stimulate active proliferation of pulpal fibroblasts. Human pulpal fibroblasts (HPF) were obtained from subcultures of between 5 and 15 passages. HPF in tissue culture plates were incubated in serum-free medium with several phenolic compounds at concentrations of 10(-8) M to 10(-4) M for 4 days. After incubation, cells were fixed, stained in culture plates and the number of nuclei counted. Phenol (10(-8) M to 10(-4) M) stimulated proliferation in a quiescent population of HPF, and the number of cells increased 27-41% compared with unstimulated cells. When HPF were incubated with p-chlorophenol, guaiacol, thymol or eugenol, there was a significant activation of cell proliferation (5 to 22%). Moreover, cell viability of the HPF was not influenced at all, except in the cases of p-chlorophenol and eugenol at concentrations of 10(-4) M. These observations may contribute to an understanding of the relationship between the stimulative effects of phenolic compounds and pulpal tissue repair.

[In vitro cultivation of human pulpal fibroblast strains--permanent and deciduous teeth].

We succeeded in separating and the cultivating stable monolayer cultures of dental pulp fibroblast strains derived from permanent and deciduous human teeth. Human permanent (n = 67) and deciduous teeth (n = 26) were extracted under acupuncture anaesthesia for the correction of malocclusion. After splitting the teeth, the pulp tissues were carefully removed, placed in tissue culture flasks, and grown in Dulbecco's modified Eagle medium supplemented with 10% fetal calf serum (FCS). The human pulpal fibroblasts (HPF) of permanent teeth and deciduous teeth (DHPF) were subcultured. Both the HPF and DHPF appeared to migrate from adherent tissues within 24 to 48 hr after explanation. They proliferated in the pulp explants, and lined up in parallel rows of cells closest to the explant tissue within 7 to 10 days in all of the experimental cases. The outgrowing cells were subcultured at 1.3 x 10(4) cells/cm2 in tissue culture flasks every 4-11 days. They showed vigorous proliferation. The average number of cells in the 6-7 day cultures of HPF were 5.6 x 10(4) cells/cm2 from 3 to 16 passages. It was 4.7 x 10(4) cells/cm2 from 3 to 10 passages with DHPF. However, no difference was observed between HPF and DHPF in the amount of synthesized protein in culture flasks. Furthermore, the growth rate of DHPF was more sensitive than that of HPF to the FCS percentages of the culture media.

Establishment and characterization of a subline predisposed to pulmonary metastasis from a human gestational choriocarcinoma cell line in nude mice.

A subline predisposed to pulmonary metastasis was successfully derived by repeating in vivo selection of GCH-1, a human gestational choriocarcinoma cell line, in nude mice. While the parent line GCH-1, transplanted subcutaneously to nude mice, induced pulmonary metastasis in a few of the host animals, the newly established subline GCH-1(m) successfully metastasized to the lungs in 100% of them. Compared with GCH-1, GCH-1(m) exhibited a higher degree of cell atypia, a lower capacity for cell growth and markedly higher productivity for human chorionic gonadotropin (hCG). Another characteristic of this subline was its enhanced growth after the addition of a pulmonary extract obtained from nude mice. The relationship of these findings to the mechanisms of metastasis of this cancer was discussed.

[Translumbar radical nephrectomy of renal cell carcinoma].

We invented a new surgical approach to the kidney through the flank to perform a radical nephrectomy for renal cell carcinoma. With the patient in usual lateral decubitus position keeping the dorsum vertical to the operating table, a skin incision is made over the XII rib from the posterior axillary line to the lateral edge of the rectus muscle. After the tip of XII rib is resected by about 5 cm, the retroperitoneal space is entered. Blunt dissection of the posterior aspect of Gerota's fascia from fasciae of the quadratus lumborum and psoas muscle is easily carried out with a liver retractor or intestinal spatula. The pulsating renal artery can be identified through Gerota's fascia when the renal hilus is exposed. Following ligation and division of the artery, renal vein is exposed. On the left side, adrenal, gonadal and occasionally lumbar veins are also ligated and severed in addition to the renal vein. Then, the kidney in Gerota's fascia is removed en bloc with perinephric adipose tissue and adrenal gland. Of 21 patients with renal cell carcinoma seen during 1 year and 3 months from June 1987, 11 underwent this operation, and other 10 patients transperitoneal radical nephrectomy because of the possible tumor extension into the renal vein, inferior vena cava or adjacent organ, the severe spinal deformity or metastases and the necessity of additional surgical procedures for concomitant benign intraperitoneal diseases. The blood loss was smaller and operating time was shorter significantly in the translumbar group than the evaluable transperitoneal group. None of those in the translumbar group received blood transfusion. As complications, pneumothorax due to pleural injury during operation and postoperative incisional hernia occurred each in one patient, but no other serious one was found. From the above results, this approach seems to be one of choices for the surgical treatment of renal cell carcinoma, as long as the tumor is not likely to extend to adjacent organs, ipsilateral nodes or the inferior vena cava.

[Fundamental and clinical aspects of adoptive immunotherapy with tumor-infiltrating lymphocytes].

Fundamental analysis was conducted on the phenotype and function of autologous tumor-infiltrating lymphocytes (TIL) in terms of their role as effector cells in specific immunotherapy for malignant tumors. Their effects were further examined in a preliminary clinical study. Analysis of the phenotype of lymphocytes which infiltrate tumor tissue and proliferate in the presence of IL-2, by using cell-surface subset markers, revealed proliferation of two types of lymphocytes: CD8-positive cytotoxic/suppressor T-cells and CD4-positive helper/suppressor inducer T-cells. In vitro cytotoxicity tests on these lymphocytes for antitumor activity showed that only CD8-positive lymphocytes had a much stronger antitumor effect on autologous tumor cells than LAK cells. In contrast, they showed no antitumor effect on autologous normal cells or homologous tumor cells of the same histological type, indicating specificity of their antitumor activity. Furthermore, in vitro treatment of tumor cells with interferon was found to increase both expression of MHC class I antigens and sensitivity to TIL cytotoxicity. In vitro of the close relationship between induction of CD8-positive TIL and expression of HLA (A, B, C) antigens in tumor tissue, the above findings suggest that the presence or absence of expression of MHC class I antigens plays an important role in the efficacy of adoptive immunotherapy with TIL. Significant antitumor activity was observed in patients given CD8-positive cells in a preliminary clinical study, the same as in in vitro assay. No patient showed serious side effects such as the pulmonary edema or body fluid retention observed in LAK therapy. Specific immunotherapy using TIL seems to be a promising modality in multidisciplinary treatment of cancer. Further developments from research on this subject are expected.

[Limb-saving surgery of malignant bone tumors using massive bone grafts].

The artificial materials in limb-saving surgery pose problems such as the age of patient, tolerance and stability. Thus, massive bone grafting should be reconsidered. In our clinic, out of 78 patients having malignant bone tumors, 29 received limb-saving surgery, and 10 of these cases received massive bone grafting using autografts and/or frozen allografts between 1968 and 1987. There were 7 cases with osteosarcoma, 1 case with mesenchymal chondrosarcoma, 1 case with leiomyosarcoma and 1 case with malignant fibrous histiocytoma. There were 5 males and 5 females ranging in age from 11 years to 44 years, and averaging 21 years. According to Enneking's staging system, these were all II-B cases. The median follow-up period was 71 mos, ranging from 13 to 239 mos. According to the modified Amago evaluation method, 80% of these cases had an excellent prognosis, one had a recurrence, but in all cases good bone union and stability were achieved without immunological reaction. Therefore we conclude that massive bone grafts in cases of malignant bone tumor surgery is a very beneficial method.

A method for differentiating between monozygotic and dizygotic twins.

It has become possible to accurately differentiate monozygotic from dizygotic twins. In this study DNA fingerprinting was performed with new mini-satellite DNA as a probe. Identical DNA fingerprints from each twin indicate a monozygotic relationship and different fingerprints indicate a dizygotic relationship. In a case of dizygotic twins, the DNA fingerprints digested with HinfI indicated a monozygotic relationship, whereas the DNA fingerprints digested with MboI showed a dizygotic relationship.

[Effectiveness, accuracy, and acceptability of breast self-examination as a breast cancer screening method].

(Breast self-examination) BSE was reviewed in terms of effectiveness, accuracy, and acceptability. Evidence on the effectiveness of BSE is inconsistent. Some researches show the effect of BSE for early detection and better prognosis of breast cancer, but others do not. Sensitivity and specificity of BSE has not been determined yet, but they are assumed to be lower than those of the clinical examination and mammography. Education of BSE for women consistently attained an improvement in frequency and skill of BSE practice. BSE may be a useful method for detecting breast cancer at the interval of periodic clinical examination.

[Immunohistochemical analysis of the expression of HLA on the trophoblast of normal pregnancy].

The expression of MHC class II antigens (HLA-DR, -DP, -DQ) on trophoblast was analyzed by an immunoperoxidase technique using monoclonal antibodies. Tissues were obtained after pregnancy hysterectomy with the medical indications and after normal deliveries. They were snap-frozen in liquid nitrogen, cut in a cryostat and fixed in acetone for 10 minutes. Mouse monoclonal antibodies used in this study were anti-HLA-ABC (W6/32, Sera lab), anti-HLA-DR (Cappel), anti-HLA-DP (B7/21, Becton Dickinson) and anti-HLA-DQ (anti-Leu-10, Becton Dickinson) in addition to anti-cytokeratin (CAM5.2, Becton Dickinson) to recognize trophoblast. In the first trimester, HLA-DP could be identified on cytotrophoblastic cell column, cell island, extravillous trophoblast invading the decidua basalis, which were positive for HLA-ABC, and villous cytotrophoblast. In term placenta, villous syncytiotrophoblast and extra-villous trophoblast found in the decidua were positive for HLA-DP. In contrast, no trophoblast reacted with anti-HLA-DR or anti-HLA-DQ.

[Immunohistochemical study on the MHC class I antigen expression on squamous cell carcinoma of the uterine cervix].

Immunohistological analysis was performed to detect MHC Class I antigen expression on uterine cervical squamous cell carcinomas of 23 patients using a monoclonal antibody (W6/32) to recognize a monomorphic determinant of HLA-ABC and an antibody (MA2.2) to react only with cells typed as HLA-A2. The results were as follows. 1. In 2 of 23 cases examined, cancer cells lacked reactivity with W6/32, although normal epithelial cells and connective stromal cells surrounding cancer nests were clearly positive for the antibody in all cases. 2. Of the remaining 21 cases, fourteen cases were judged to be HLA-A2 positive, based on the obvious staining of the stromal cells with MA2.2. Of these 14 only 5 carcinomas were positive for HLA-A2, and the cancer cells in the other 9 cases failed to bind MA2.2. These data suggest that in most clinically diagnosed patients, cancer cells do not have their own MHC Class I products on their cell surfaces phenotypically, and this unique property may be related to the biological characteristic that permits cancer cells to survive and proliferate unrestrictedly in vivo, mainly escaping from the immune-surveillance system under cytotoxic T lymphocytes (CTLs), which need to recognize not only cancer-specific (associated) antigen(s) but also the self-MHC Class I products to kill the cancer cells efficiently.

[Somatosensory evoked potentials over the unaffected hemisphere in patients with unilateral intracerebral hematoma].

Somatosensory evoked potentials (SEPs) recorded from the unaffected hemispheres were studied in patients of less than one month after the onset of unilateral intracerebral hematoma. We examined 66 SEPs obtained from 49 patients with putaminal hemorrhage (midline shift 0-15 mm), 38 SEPs from 25 patients with thalamic hemorrhage (midline shift 0-8 mm), and 14 SEPs from 10 patients with subcortical hemorrhage (midline shift 0-12 mm). These examinations were made after electrical stimulation of the median nerve at the wrist by using square waves of 0.2 msec in duration delivered at a rate of 3 Hz. We analyzed the first cortical potential N 20 recorded from the contralateral scalp over the unaffected hemisphere with reference on Fpz. N 20 peak latency was compared with the maximum shift of the midline structure such as in foramen of Monro or third ventricle in computed tomogram. Also with the site of the hematoma and the disturbance of consciousness. N 20 peak latency was not correlated with the degree of the midline shift in all the patients. There was abnormal prolongation of N 20 peak latency in one SEP of one patient, and disappearance of N 20 in two SEPs of two patients with putaminal hemorrhage. Abnormal prolongation of N 20 peak latency was found in 3 SEPs of 3 patients with thalamic hemorrhage. There was no disappearance of N 20 in patients with thalamic hemorrhage. There was no abnormal prolongation or disappearance of N 20 peak latency in patients with subcortical hemorrhage. In two of five patients showing stuporous state, disappearance of N 20 was found.(ABSTRACT TRUNCATED AT 250 WORDS)
