[Analysis of factors influencing early patency of saphenous vein grafts].

To determine factors affecting early patency of saphenous vein grafts, 140 grafts in 65 patients were studied angiographically within 2 months after operation. Twenty of the 140 grafts were occluded. Sixteen variables were extracted from the angiographic findings, intraoperative measurements, clinical characteristics, and biochemical data. The univariate and multivariate analyses were performed to assess their predictive value. Of 16 variables, 3 (coronary artery internal diameter, graft flow, and coronary artery resistance) correlated significantly with graft patency in the univariate analysis. The multivariate analysis selected coronary artery resistance, coronary artery internal diameter, and degree of proximal stenosis as predictors of early graft patency. Of these 3 factors, the coronary artery resistance influenced graft patency mostly. Therefore, the coronary artery resistance was considered to be the most reliable predictor of early graft patency.

[A case report of controlled aortic root reperfusion (CARP) following global myocardial ischemia during surgery for severe mitral stenosis with tricuspid regurgitation].

The use of controlled aortic root reperfusion (CARP) as additional myocardial protection in cardiac surgery has been employed at University of Alabama at Birmingham Since 1986. This technique has been applied to a patient in Japan undergoing mitral valve replacement and tricuspid annuloplasty and favorable results were obtained. The CARP method consists of two consecutive procedures following global myocardial ischemia, that is, terminal warm blood cardioplegic reperfusion and selective controlled coronary perfusion. When the repair was almost completed, the CARP technique was initiated with warm blood (37 degrees C) from an oxygenator, and the initial phase was hyperkalemic (K+ 20 mEq/1, 3 min). [Terminal warm blood cardioplegic reperfusion] This was followed by normokalemic warm blood (without interruption) from the pump oxygenator infused through a cardioplegic needle into the isolated aortic root with the aortic cross-clamp still in place (10 to 20 min). [Selective controlled coronary perfusion] Terminal warm blood cardioplegic reperfusion induced electromechanical quiescence initially which allowed rapid repletion of adenosine triphosphate and improved cardiac function. Selective controlled coronary perfusion with normokalemic warm blood permits necessary and sufficient coronary blood flow following global ischemia even when systemic arterial pressure fell and coronary vascular resistance rose. On considering these two points, we consider that the CARP technique offers excellent myocardial protection in accelerating the recovery of myocardial function following global ischemic damage. The CARP method offers useful assistance in cardiac surgery particularly for patients who have abnormal preoperative cardiac function and who have experienced long periods of global myocardial ischemia.

[Long-term follow-up results on Bentall's and our modified procedure].

Replacement of the ascending aorta and the aortic valve with composite graft, including reimplantation of coronary ostia, were performed in 84 patients from October, 1978 to December, 1989. Annuloaortic ectasia was the most common indication for surgery (76 patients). Thirty patients (36%) had the Marfan syndrome. Thirty-five patients were performed with original Bentall's procedure. Thirty patients were carried out with our modified technique like Inberg's, the coronary arteries were excised with attachment of the selected piece of the aortic wall, and implanted to the tube prosthesis with continuous suture. Nineteen patients were operated with modified technique by Cabrol or other interposition graft method. The actuarial free of death related to the operation of 4 years was 83%, 88%, and 74% (p = NS) in each methods respectively. The complications of anastomosis, for example, leakage and dilatation of coronary ostia were not seen in our modified Bentall's procedure. In conclusion, our modified method is proved to be reliable and competent.

[Clinical studies on preoperative continuous intra-arterial chemotherapy in the treatment of malignant soft tissue tumors].

Preoperative continuous intra-arterial chemotherapy combined with surgery in the treatment of malignant soft tissue tumors has been used since 1971 in our clinic. We treated thirty-eight patients, and twenty-five of them underwent preoperative intra-arterial chemotherapy and surgery. The diagnoses were liposarcoma (5), synovial sarcoma (4), rhabdomyosarcoma (2), angiosarcoma (2), malignant schwannoma (2), malignant lymphoma (2) and 8 other types of tumors. From the standpoint of local failures, there was only one recurrent case. Eighteen patients are alive now without evidence of disease. The 10-year cumulative survival rate for cases treated with intra-arterial chemotherapy was 69.5%, as compared with 24.9% of cases without intra-arterial chemotherapy. Therefore we conclude that preoperative continuous intra-arterial chemotherapy is a very effective method in the treatment of malignant soft tissue tumors.

Nucleolar organizer regions in bone tumors.

Silver-stained nucleolar proteins (AgNORs) were counted in a variety of bone tumors. In osteosarcomas, the number of AgNORs was also quantified before and after chemotherapy. Malignant bone tumor cells possessed more than five small AgNORs (5.85 +/- 1.39). Nuclei of benign bone tumor cells had less than three (2.61 +/- 0.51). A significant difference in the number of AgNORs between osteosarcomas before chemotherapy (6.10 +/- 1.22) and after chemotherapy (4.20 +/- 1.07) was observed. (p less than 0.001). The number of AgNORs in osteosarcoma patients with better prognoses was smaller than that of osteosarcoma patients showing poor prognoses, but without significant difference. The results of the present study indicate that the AgNOR count might help in determining malignancy, evaluating the effect of chemotherapy, and deciding the prognosis.

[The surgical treatment on thoracic and abdominal aortic aneurysm in elderly patients over age of 70 years].

For the last 5 years from 1986 to the end of 1990, 279 and 271 surgeries were undertaken on thoracic and abdominal aortic aneurysm, respectively. The highest age distribution of the former was between 60 to 69 years old comparing to 70 to 71 years old of later group. In thoracic aortic aneurysm, overall surgical results were inferior to elderly patients, however, this poor result was mainly attributed to the surgery for ruptured-emergency cases. In eliminating these situation, mortality of elderly in elective surgery was found to be equivalent to those of patients age of less than 69 years old. The same findings were also apparent for the surgery on abdominal aortic aneurysm where the mortality was totally the same (2.3% vs 2.4%) in elective surgery but high in ruptured cases (82% vs 30%). The long term survival rates were analysed in both groups in elderly patients. It was found the survival curve in thoracic aneurysm patients was lower especially for the first 5 years following surgery comparing to that of abdominal patients, however, because of relatively small number of patients listed in this late follow up, precise result has to be waited for the future analysis.

[A case of infantile Sertoli cell tumor].

A case of infantile Sertoli cell tumor of the testis is presented. A 2-year-old child visited our hospital with the complaint of painless swelling of the left scrotal content. A solid tumor was demonstrated in the left testis by scrotal echography. Physical and laboratory examinations were within normal limits and alpha-fetoprotein and beta-HCG were not elevated. The left testis was removed. The tumor sized 3.0 x 3.0 x 2.2 cm and weighted 14.5 g. Pathohistological diagnosis was benign Sertoli cell tumor. Tumor cells were vimentin positive and TM-1 reactive antigen positive and revealed electronmicroscopically some features of Sertoli cells such as intermediated microfilament, rich lysosomes and interdigitation between adjacent cells. The patient was well with no evidence of diseases one and a half years after the operation.

[An early phase II study of CPT-11 in gynecologic cancers. Research Group of CPT-11 in Gynecologic Cancers].

An early Phase II study of CTP-11, a new derivative of Camptothecin, in gynecologic cancers was carried out by a cooperative study group of 9 institutions. Forty-six patients were enrolled, and there were 14 cases of ovarian cancers, 7 of cervical cancer, 6 of uterine body cancers and 1 of endometrial stromal sarcoma which satisfied study criteria. The response rate in ovarian cancers was 21.4%, and in cervical cancers 42.9%, among an overall rate of 21.4%. Three out of 6 patients with objective response had undergone previous chemotherapies including cisplatin, suggesting that CPT-11 was effective for patients with no response or refractory to these therapies. Leukopenia was a major adverse reaction with an incidence of 60.0% (grade 2 or more). Gastrointestinal symptoms such as nausea vomiting, anorexia and diarrhea were frequently observed (grade 2 or more; 13.3-43.3%). These were generally tolerable except in a few cases. Besides these reactions, alopecia was also observed (33.3%), but severe adverse reactions such as nephropathy were not. These results suggested that CPT-11 was effective against ovarian cancer and cervical cancer. The recommended dose regimen for a late phase II study is considered to be 100 mg/m2 once weekly and 150 mg/m2 once every 2 weeks.

[Clinical evaluation of cefuzonam for bacterial pneumonia].

Ten patients with moderate or severe bacterial pneumonia with underlying diseases or complications were treated with cefuzonam (CZON) at a daily dose of 2 g to 4 g. The clinical effectiveness was good in 9 patients. No side effects or abnormal laboratory test results were observed in any patient. These results suggest that CZON may be useful in the treatment of bacterial pneumonia.

Cancer antigen 125, carcinoembryonic antigen, and carbohydrate determinant 19-9 in ovarian tumors.

The authors studied data of combination assays of tumor markers, because simultaneous elevation of different types of tumor markers in the serum was puzzling. They interpreted such phenomena regarding cancer antigen 125, carcinoembryonic antigen, and carbohydrate determinant 19-9 in ovarian tumors. The tissue expression of the antigens was compared with preoperative serum levels. Several different factors were found to cause the simultaneous elevation of two or three of these markers in the serum. Furthermore, even when the levels of some of the tumor markers were raised in the serum, the ovarian tumor did not always produce the marker by itself. This study indicates that immunohistochemical identification of a marker in tumor tissue is prerequisite to the use of that marker in the serum to monitor disease status.

[A case of young woman with giant ovarian tumor as the manifestation of leukemic recurrence].

Following rapid development of chemotherapy to leukemia, reports about tumor-forming-leukemia are on the increase. But ovarian tumor-forming-leukemia is relatively rare, hence not enough is known about it. Recently, the authors experienced a case of young woman with recurrent leukemia which manifested as a giant ovarian tumor. She suffered from acute lymphocytic leukemia (ALL) in February, 1986, and received chemotherapy (predonisolone and cytocine arabinoside) until complete remission in peripheral blood. But she developed a giant abdominal tumor in May, 1988, and admitted to our hospital for close examination and treatment. Preoperative blood examination was within normal range, and peripheral blood smear revealed no leukemic blasts. She had a giant abdominal tumor occupying whole abdominal cavity, and the tumor showed solid pattern in ultra-sonography, CT and NMR-CT. Malignant lower abdominal tumor was suspected, and simple abdominal hysterectomy, bilateral salphingo-oophorectomy and omentumectomy was performed on 27th. May, 1988. A giant abdominal tumor developed from left ovary, infiltrating to the uterus through left Fallopian tube, and metastatic lesion at omentum were present. Postoperative pathological findings in immunochemistry showed that the tumor was recurrence of ALL as tumor-forming-leukemia. After operation, she was treated in the department of internal medicine for postoperative chemotherapy (TCMP-therapy 1 kur) and discharged in December, 1988. She is alive and healthy 10 months after operation, and reveals no signs of recurrence. It was important to recognize lower abdominal tumor in leukemic patients as gynecologic problem, and significance of surgical approach to ovarian tumor-forming-leukemia was indicated.

Calcitonin gene-related peptide augments parasympathetic contraction of rabbit tracheal smooth muscle in vitro.

The effect of calcitonin gene-related peptide (CGRP) on parasympathetic contraction of rabbit airway smooth muscle was studied under isometric conditions in vitro. CGRP (10(-7) M) did not contract tracheal smooth muscle, but it potentiated the contractile response to electrical field stimulation (EFS) in a dose-dependent manner, the maximal increase from the baseline response being 14.4 +/- 6.2%. In contrast, the contractile responses to exogenously administered acetylcholine were not altered by this peptide. The CGRP-induced potentiation of the contractile responses to EFS was further augmented by physostigmine (123.1%), but not influenced by phentolamine, propranolol, indomethacin, or thiorphan, an enkephalinase inhibitor. These results suggest that CGRP augments the neurally mediated contraction of rabbit airway smooth muscle probably through a prejunctional mechanism and that this effect may not be modulated by tissue enkephalinase.

Aortopulmonary shunt with replacement of a common atrioventricular valve in a patient with double inlet ventricle.

Replacement of a common atrioventricular valve with a St. Jude medical prosthesis, together with construction of a central aortopulmonary shunt using a polytetrafluoroethylene graft, were performed on a patient with double inlet ventricle complicated by severe regurgitation across an atrioventricular valve and hypoplastic pulmonary arteries. No complications were noted during a postoperative follow-up of 58 months; indeed, a considerable increase was noted in postoperative exercise capacity. This simple surgical procedure should be considered as a mode of treatment in patients with this combination of lesions.

Assay of protein A in Staphylococcus hyicus subsp. hyicus by ELISA and immunoelectron microscopy.

The presence and quantity of protein A in Staphylococcus hyicus subsp. hyicus isolates were examined by an enzyme-linked immunosorbent assay (ELISA) and immunoelectron microscopy. Cell-bound protein A was demonstrated in 45 (94%) of 48 isolates from diseased pigs and in 113 (86%) of 132 isolates from healthy pigs by ELISA using peroxidase-conjugated rabbit antibody, but was not found in isolates from chickens and cows. Most of these swine isolates contained about 100 to 300 ng of cell-bound protein A/ml. Extracellular protein A was not detected in any isolates from pigs, chickens or cows. In the immunoelectron microscopy assay, swine isolates were labeled with goat anti-mouse IgG conjugated to colloidal gold particles, but chicken and cow isolates were not labeled.
