The incidence of simple renal cyst by computed tomography.

Five hundred and forty-two abdominal computed tomography (CT) scans, including the kidneys, were reviewed to investigate the incidence of simple renal cysts. Increasing frequency with age was clearly demonstrated. The incidence in patients 50 years of age and over was at least 27%. Two thirds of simple renal cysts were 2 cm or less in diameter. Cysts were usually observed as exo-endoparenchymal or endoparenchymal lesions on CT.

The determination of urinary oxalate by gas chromatography.

A new quantitative determination method for urinary oxalate by gas chromatography has been established. This method consists of concentration of urine to dryness followed by methylation of the oxalic acid with 7 per cent hydrochloric acid-methanol, subsequent extraction with chloroform and gas chromatographic analysis. The advantages of the technique are smaller sample sizes (only 5 ml.), rapidity (within 2 to 3 hours) and accuracy (99.14 plus or minus 1.34 per cent recovery). In addition, the treatment of the sample is simple and no specialized equipment is required. Therefore, this method is recommended to clinicians for routine determination of oxalic acid in urine. Urinary oxalate content was analyzed with this method in 62 male patients with calcium oxalate stones. Urinary oxalate concentration and excretion in 33 patients with recurrent stones were significantly higher than in the 22 controls using Welch's test.

Staging of renal cell carcinoma by computed tomography.

To evaluate the efficacy of computed tomography (CT) in the diagnostic workup of renal cell carcinoma, CT findings were correlated with surgical and pathological findings in 40 cases. Fifteen cases without obliteration of perinephric fat or thickening of Gerota's fascia were confined within the renal capsule. Eighteen lesions showing anatomic contact with adjacent organs were either within the renal capsule or Gerota's fascia but never beyond Gerota's fascia. Two cases (Stage II) showed thickening of Gerota's fascia and two cases (Stage IV) showed tumor infiltration into the spleen. Computed tomography was found to be a useful method for the investigation of tumor thrombosis and lymph node metastasis.

Metabolic deterioration in shock state and its modulation.

The initial cellular reaction against the deleterious effects of shock-inducing stimuli apparently elevates the energy-producing capacity so that the cell is able to sustain its normal function. Several endocrine events are fundamentally important as triggering factors for this kind of reaction. As the shock state advances, cellular metabolism deteriorates progressively and cellular energy is exhausted. Depression of intracellular cAMP may induce cellular metabolic unresponsiveness to hormonal stimuli. Consistent degradation of high-energy substances, extreme deviation of the redox state in the NAD+-NADH system, and decrease of endogenous key substances such as L-carnitine ultimately may lead to a standstill of cellular enzymatic reactions (Fig. 13). Methods intended to sustain cellular membrane and enzymatic systems may offer the best contribution to the improvement of shock therapy.
