Prognostic factors and therapeutic results in multiple myeloma.

Prognostic factors in 68 consecutive patients with myeloma treated at the National Cancer Center Hospital from 1962 to 1984 were analyzed. Median survival time from onset was 100 months for stage I, 72 months for stage II, and 26 months for stage III of the Durie and Salmon's clinical staging system. It was 55 months in patients with normal renal function and 18 months in those with abnormal renal function. All early deaths occurred in patients with stage III disease. Hemoglobin level, bone lytic lesions and presence of Bence Jones protein were also significant prognostic factors. On the other hand, heavy chain as well as light chain subtypes of monoclonal immunoglobulin (M-component) and M-component production rate did not influence the survival of myeloma patients. The analysis of chemotherapeutic responses and survival curves according to the chemotherapy used in this study (alkylating agent vs Vinca-alkaloid plus alkylating agent) did not disclose any significant difference between the two groups. The overall response rate was 67%. The survival time from the initial chemotherapy of responding patients was significantly longer than that of nonresponders.

Differential sensitivity to ethidium bromide of replicative DNA synthesis and bleomycin-induced unscheduled DNA synthesis in permeable mouse sarcoma cells.

Replicative DNA synthesis in permeable mouse sarcoma cells was more sensitive to ethidium bromide (EtBr) than bleomycin-induced unscheduled DNA synthesis (UDS). A similar difference in sensitivity to EtBr was observed between DNA polymerases alpha and beta. The difference in sensitivity to EtBr of replicative DNA synthesis and UDS in the present system seems to reflect mainly the sensitivity difference between DNA polymerases alpha and beta.

Gas exchange and facilitation of high-frequency ventilation in intrathoracic surgery.

High-frequency ventilation (HFV) of the jet type was evaluated for facilitation of intrathoracic operations because HFV does not necessitate ventilatory movement of the lung and reduces the volume of the lung. The feasibility of HFV as a substitute for the current respiratory management of intermittent positive-pressure ventilation (IPPV) was determined in 12 patients with cancer of the lung by employing two types of HFV-HFV alone (Group 1) and HFV on physiological dead-space volume (VD) (Group 2). For the 6 patients in Group 1, HFV was set at a frequency of 3 Hz and a driving pressure of 0.5 kg/cm2. For the 6 patients in Group 2, HFV was set at 6 Hz with the same driving pressure but was superimposed on a small tidal volume equal to a dead space. High-frequency ventilation facilitated intrathoracic operations because of the disappearance of ventilatory movement and the reduced volume. No significant difference between the groups was found for arterial oxygen tension. Arterial carbon dioxide tension (PaCO2) in patients in Group 1 remained in the normal range, although it was slightly acidotic. The PaCO2 in Group 2 was acidotic. Therefore, it was concluded that HFV alone can be substituted for IPPV in gas exchange and provides good accessibility to the operative field.

Differential sensitivity to aphidicolin of replicative DNA synthesis and ultraviolet-induced unscheduled DNA synthesis in vivo in mammalian cells.

In vivo in mammalian cells, ultraviolet-induced unscheduled DNA synthesis was less sensitive to aphidicolin than was replicative DNA synthesis. Replicative DNA synthesis in HeLa, HEp-2, WI-38 VA-13 and CV-1 cells was inhibited more than 97% by aphidicolin at 10 micrograms/ml, whereas aphidicolin inhibition of DNA synthesis in ultraviolet-irradiated cells varied between 30% and 90% depending on cell types and assay conditions. Aphidicolin inhibition of unscheduled DNA synthesis (UDS) in HeLa cells increased gradually with increasing aphidicolin concentration and reached approximately 90% at 100 micrograms/ml aphidicolin. A significant fraction of UDS in ultraviolet-irradiated HEp-2 cells was resistant to aphidicolin even at 300 micrograms/ml. Considered along with related information reported previously, the present results suggest that both aphidicolin-sensitive and insensitive DNA polymerases, DNA polymerase alpha and a non-alpha DNA polymerase (possibly DNA polymerase beta), are involved in in situ UDS in these ultraviolet-irradiated cells. Comparison of staphylococcal nuclease sensitivity between DNAs repaired in the presence and in the absence of aphidicolin in HEp-2 cells suggested that the involvement of DNA polymerase alpha in UDS favored DNA synthesis in the intranucleosomal region.

Establishment of human glioblastoma multiforme cell line, G5A.

A new cell line, G5A, was established from a human glioblastoma multiforme of the primary site and maintained for 25 months, subcultivated 36 times. The cloned cells showed morphologically epithelial-like patterns and loss of glial filaments. The cells inoculated in the nude mice produced tumors in 100% of the mice. The plating efficiency of the cell line was 20.9% in a standard culture. The modal chromosome number was 63 with many morphological abnormalities. The generation time was 22.9 hours and the parameters of the cell cycle were clarified by the percent labeled mitosis analysis. No morphological differentiation of the cell line was induced by the chemical differentiation-inducers.

Clinical implications of anti-HTLV antibody titer, abnormal lymphocytes in HTLV carriers and HTLV genome negative adult T-cell leukemia-lymphoma.

Abnormally large lymphocytes with Pelger-like nuclear lobulation have been found in peripheral blood of smoldering adult T-cell leukemia-lymphoma (ATL) patients. They are also found in quite low percentage in peripheral blood of healthy HTLV carriers. Therefore, it has become difficult to discriminate smoldering ATL having only abnormal lymphocytes from a healthy human T-cell leukemia virus (HTLV) carrier only by morphology and serology. We found that patients with smoldering ATL with visceral organ involvement such as skin or lung lesions had much higher titer (about 1 : 1,000) of anti-HTLV antibodies than those with other subtypes of ATL (about 1 : 250); the difference was highly significant (p less than 0.001). In addition, the period of survival of patients with smoldering ATL having antibody with high titer was much longer than that of patients with other subtypes of ATL, suggesting the role of a host immune response. Four cases of HTLV-negative ATL were found to have the same chromosomal abnormality as that has been seen in HTLV-positive typical ATL; the situation was quite similar to Burkitt's lymphoma. Since it has been considered that HTLV does not include oncogenes, a common oncogenic mechanism, ATL specific cellular oncogenes, might exist: they might be activated by HTLV or a mechanism other than a virus.

[Multidisciplinary treatment for small cell lung cancer].

The effect of multidisciplinary treatment of small cell carcinoma of the lung was tested. The response rates to induction chemotherapy were 69.2% (9/13) and 57.1% (12/22) in small cell carcinoma of the lung with limited and extensive disease, respectively. The response rates of oat and intermediate cells were 63.6% (14/22) and 50% (5/10), respectively. Radiation therapy to primary site and mediastinum after induction chemotherapy was also effective, producing the response rates of 87.8% (7/8) and 58.3% (7/12) in limited disease and extensive disease, respectively. The non-responder and recurrent cases responded to chemotherapy with epipodophyllotoxin and/or cisplatin but not with mitomycin C and/or adriamycin. The median survival of all 37 cases was 12.2+ months, and those with limited and extensive cases were 14.8+ and 8.7 months, respectively.

[Effect of CDDP administration on NK activity of human peripheral lymphocytes and its modification by corticosteroid as anti-emetic agent].

The effect of cDDP (cis-diamminedichloroplatinum) on NK (natural killer) activity of peripheral blood lymphocytes in 12 patients (7 primary lung cancer, 3 metastatic pulmonary tumor and 2 malignant mediastinal tumor) was examined with emphasis on the combination of corticosteroid. To all patients, 80 mg/m2 of CDDP was administered intravenously every 3 weeks. Four patients were treated with CDDP alone, and 8 patients received 375 mg of methylprednisolone on the same day of CDDP administration and 125 mg on each of following consecutive 5 days respectively. 1) NK activity was not suppressed for 3 weeks after CDDP administration, in the group of 4 patients without receiving corticosteroid. 2) Significant NK suppression was found 1 week after CDDP administration, and recovered 2 weeks later, in the group of 8 patients who were treated for their emesis by corticosteroid. It can be concluded that 80 mg/m2 of CDDP does not reduce NK activity at all. However, the additional administration of corticosteroid strongly inhibited NK activity. Therefore, one should be very careful when combines the corticosteroid in order to relieve emesis induced by CDDP treatment, even if it has some antiemetic effect.
