Voiding inability after transurethral resection of a bladder diverticulum.

Following transurethral resection of the opening of a large bladder diverticulum a 62-year-old man was in complete urinary retention. The reason was the considerable vent created by altering the hydrodynamics. The complication was resolved only after conventional vesical diverticulectomy. A warning is issued against the use of transurethral treatment of large bladder diverticula.

Results of surgical treatment for lung cancer.

A review is presented of 233 patients who were surgically treated for bronchial carcinoma during a five-year period. The survival time was determined in relation to operative procedure and to cell type and anatomic extent of the tumours. Staging according to the TNM classification gave five-year survival rates of 51.6% in Stage I, 12.0% in Stage II and 8.4% in Stages III-IV. Among the Stage I cases, tumours classified as T1N0M0 were associated with longer survival than were T1N1M0 or T2N0M0 tumours. It is concluded that the stage of the disease as defined by TNM classification should be regarded as the most important prognostic factor.

Iatrogenic perforation of the esophagus. Causes and management.

A series of 13 perforations of the esophagus in 12 patients is presented. All the perforations were caused by instrumentation. Minimal delay in treatment is advocated. Most importance is attached to gastric decompression, adequate drainage of the mediastinum with parenteral hyperalimentation and, if possible, suture of the lesion.

Imipramine cardiotoxicity: an electrocardiographic and haemodynamic study in rabbits.

The purpose of the present study was to investigate the cardiotoxicity of the tricyclic antidepressant imipramine. The experiments were carried out in rabbits during continous intravenous infusion of imipramine, and electrocardiographic and haemodynamic changes were observed. The blood flows were measured using the radioactive microsphere method based upon the principles of Fick and Stewart Hamilton. It was found that the decrease in heart rate and the changes in heart rhythm were always preceded by a fall in arterial blood pressure and cardiac contractility, expressed by a fall in dp/dtmax. On the basis of the results it is concluded that a direct depressing action on the myocardium is of importance in the development of cardiac complications, although both a depressing influence on cardiac conduction, an anticholinergic effect, and an influence on adrenergic factors may also contribute. The possibility of positive correlations between the changes in plasma-imipramine concentration and the changes in the dp/dtmax and the QRS complex cannot be excluded.

[Studies on the cultivation of isolated murine renal glomeruli (author's transl].

The rate of growth of isolated murine renal glomeruli was studied in vitro. Separation by screening was followed by cultivation in Carrel flacks using Eagle's medium, and the number of utilizable tests was 104. It was possible to cultivate glomeruli for up to 27 days and to observe the growth of cells. These were epithelial capsule cells, endothelial capillary cells and fibroblasts. The existence of mesangial cells not be convincingly demonstrated experimentally. The problems associated with the cultivation of isolated renal glomeruli are discussed with reference to data reported in the literature. Possible uses of this particular model are pointed out in some detail.

[Examinations with the modified VNPI].

The revised design of a psychometric procedure (VNPI) is described. Additional factors were extracted by intercorrelations and factor analysis: increased perseveration, inhibited contact, defective flexibility, instability of moods. They provide information, additionally to the evaluation of the factors of vegetative lability, neuroticism, and introversion. A random test in 360 persons from a screening series is used for the presentation of the relations between VNPI values and findings of social and internal medicine. The possibilities of individual diagnosis of the procedure are demonstrated by VNPI profiles of patients.

Increased collagen deposition in an uncomplicated surgical wound compared to a minimal subcutaneous test wound.

Little information is currently available concerning the relationship between results obtained in humans from surgical test wounds and results from wound models. Therefore, to evaluate human wound healing parameters, tubings of expanded polytetrafluoroethylene were implanted in a subcutaneous test wound in the arm of 47 volunteers and 20 patients undergoing hernia repair. The surgical patients also had implants left in the surgical wound cavity. After 10 days the deposition of collagen in the tubings as expressed by hydroxyproline content was 30% higher in the surgical wound than in the test wound, p < 0.05. The amount of collagen deposited in the tubing within the surgical wound did not correlate with measurements in the test wound, whereas a significant correlation of proline levels was found between the two sites, p < 0.05. Deposition of proline and total protein in the model was equivalent. In both wound types age negatively correlated with levels of protein, but not collagen. The variability of the results was 40% lower in the subcutaneous test wound than in the surgical wound. There was no significant difference in hydroxyproline deposition between the volunteers and the patients undergoing hernia repair. In patients undergoing minor surgery without signs of compromised healing the expanded polytetrafluoroethylene test wound in the arm reflects the deposition of non-collagenous protein, but not collagen, within the surgical wound.
