Supratentorial primitive neuroectodermal tumors (stPNETs) are malignant tumors. We saw within three years six children with stPNETs. In four of the six children radical resection could be achieved. All had craniospinal irradiation and chemotherapy according to the HIT-91 protocol. The two children with incomplete resection died due to tumor progression after 7 and 10 months. Two of the 4 children with complete tumor resection had local relapses 8 months after diagnosis and died after 14 and 18 months. One child had a diffuse meningeal relapse 12 months after diagnosis. Despite (high-dose) systemic chemotherapy and intraventricular mafosfamide, he died 21 months after diagnosis due to tumor although remission could be achieved. Only one child is still in remission 86 months after diagnosis.
It is known from epidemiological studies, that high apolipoprotein B or low HDL-2 concentrations in serum are correlated with an increased risk for premature cardiovascular diseases. It was the aim of this study to generate reference values for apolipoproteins and HDL-cholesterol fractions to accomplish early diagnosis and therapy. All concentrations are age depending, being low at birth and increasing during the first years of life. This rise is more pronounced in apolipoprotein B than in apolipoprotein AI and HDL-2. Apolipoprotein II and HDL-3 reach later childhood values as early as at the end of the first month of life. The ratio of B and AI, which is a very good indicator for an increased risk for premature cardiovascular diseases, moves also distinctively upward during the first year of life.
OBJECTIVE: To assess measurement of gastric electrical potential difference, pH of gastric mucosa, and endoscopic findings in patients in intensive care units who are at risk of developing stress ulcers. DESIGN: Open comparison with age- and sex-matched control subjects. SETTING: Herlev Hospital, Denmark. SUBJECTS: Sixteen consecutive patients with no history of gastrointestinal haemorrhage, coagulopathy, or ulcer disease who had been admitted to the intensive care unit, and 16 age- and sex-matched outpatients with normal endoscopic findings. INTERVENTIONS: Upper gastrointestinal endoscopy, during which any lesions that were found were scored according to severity, the gastric potential difference, and the pH of gastric juice were measured. OUTCOME MEASURES: Correlation between the incidence of stress ulceration found at endoscopy, gastric potential difference, and gastric pH. RESULTS: Gastric potential difference was significantly reduced and gastric pH significantly increased in the patients in the intensive care unit (p less than 0.05 in both cases), all of whom had stress ulcers in more than one gastric segment. Nine of the patients had gastric pH readings of greater than 4. CONCLUSION: Gastric electrical potential difference may be useful measurement for the assessment of stress ulceration in patients in intensive care units.
Body temperature, P-cortisol, P-glucose, P-transferrin, P-orosomucoid, P-IgM, hematocrit and total and differential leucocyte counts were investigated in 16 men undergoing inguinal herniotomy. The patients were randomized to epidural analgesia (n = 8) or epidural analgesia + assumed blockade of prostaglandin synthesis, blockade of receptors for histamine1, histamine2 and serotonin2, and inhibition of fibrinolysis with indomethacin, astemizole, ranitidine, ketanserin and tranexamic acid, respectively (n = 8). The rectal temperature and blood granulocyte counts rose significantly and similarly after surgery in both groups. Acute-phase protein (transferrin and orosomucoid) changes were also similar in both groups, as were P-cortisol and P-glucose, which did not increase in response to the neural blockade. Other factors than neural stimuli, prostaglandins, histamine, serotonin and activation of fibrinolysis are concluded to be effective in releasing postoperative leucocytosis, rise in body temperature and changes in acute-phase proteins.
OBJECTIVE: To assess pulmonary function, pain, and fatigue after elective laparoscopic cholecystectomy. DESIGN: Prospective study. SETTING: University hospital. SUBJECTS: The first 53 patients listed to undergo laparoscopic cholecystectomy. MAIN OUTCOME MEASURES: Assessments of pain, peak flow, and the subjective feeling of fatigue before, and 6 hours after, operation, and then daily for the first week. RESULTS: Pain scores at rest and while coughing and fatigue score were less than have been reported after open cholecystectomy. Peak flow was reduced 6 hours after operation (p < 0.01) but less than has been reported after open surgery, and it returned to the preoperative value within the study period. CONCLUSION: The less disturbance of pulmonary function and the lower pain and fatigue scores may explain the observed early discharge from hospital and the rapid mobilisation, including ability to go to work, in patients who have undergone laparoscopic cholecystectomy.
Reference values for 26 plasma amino acids were determined by ion exchange chromatography in 29 healthy pregnant women (1st to 3rd trimenon). The results are given as P50 (P7-P93). In the course of pregnancy the concentrations of amino acids generally decrease. Threonine, however, shows a distinct, statistically significant increase. The interindividual concentration ranges are low and they show little change in the course of pregnancy. The data presented here are relevant for dietary treatment of metabolic disorders, e.g. maternal phenylketonuria in pregnants.
