[Nd:YAG laser treatment of cancer of the esophagus and cardia].

In 1988, Nd:YAG laser treatment was employed as palliative therapy for 23 patients with cancer of the oesophagus and cardia. After treatment, half of the patients could ingest light or normal diets and the ability to swallow improved in a total of 3/4 of the patients. Perforation of the oesophagus occurred in two patients and aspiration in two other patients. It is concluded that, in a great number of patients with inoperable cancer of the oesophagus and cardia, laser treatment is better than other palliative therapeutic measures. The indications are discussed.

Epidural bupivacaine and morphine plus systemic indomethacin eliminates pain but not systemic response and convalescence after cholecystectomy.

We studied 24 patients undergoing elective cholecystectomy and randomized to either conventional postoperative pain treatment, with intermittent nicomorphine (10 to 15 mg) and acetaminophen (1 gm) on request, or thoracic epidural analgesia with plain bupivacaine for 48 hours and epidural morphine 4 mg every 8 hours for 96 hours plus systemic indomethacin 100 mg every 8 hours for 96 hours. Epidural analgesia for pin prick extended from the fourth thoracic to the first lumbar nerve for 48 hours. Assessments of pain, various injury response parameters, peak flow, and subjective feeling of fatigue were performed preoperatively, 3 and 6 hours after skin incision, and 1, 2, 4, and 8 days postoperatively. The epidural analgesia-systemic indomethacin treatment eliminated postoperative pain during rest and coughing. In contrast, only a minor and clinically unimportant modulation of the conventional perioperative and postoperative changes in plasma cortisol, glucose, transferrin, orosomucoid, leukocyte and differential counts, rectal temperature, peak flow, and fatigue was observed. Our results suggest that factors other than pain per se must be controlled in order to reduce postoperative morbidity.

Metastasis size in pathologic femoral fractures.

Radiographic assessment was made of metastases in 69 consecutive patients with pathologic femoral fractures. The following variables were considered: width of metastasis, ratio between width of metastasis and bone, axial length of cortical destruction, and proportion of cortical bone destroyed of the circumference. Differences in size of the metastases were not related to location in the femur or histologic type. In 62 of 69 fractures the ratio between width of the metastasis and bone was greater than or equal to 0.60, axial cortical destruction in the neck was greater than or equal to 13 mm and in other parts of the femur greater than or equal to 30 mm, and cortical destruction of the circumference greater than or equal to 50 percent. Prophylactic internal fixation need not be considered for femur metastatic lesions that do not reach these limits.

Influence of neural blockade and indomethacin on leucocyte, temperature, and acute-phase protein response to surgery.

Body temperature, P-cortisol, P-glucose, P-transferrin, haematocrit and total and differential leucocyte counts were investigated in 24 men undergoing inguinal herniotomy, but otherwise healthy. The patients were randomized to general anaesthesia (GA, n = 8), epidural analgesia (E, n = 8) or epidural analgesia + indomethacin (E + I, n = 8). The rectal temperature rose significantly after surgery, except in the E + I group, and increase in blood granulocytes was significantly less in these patients than in the GA group. Group E showed intermediate changes. P-transferrin changes were similar in all three groups. Glucose and cortisol showed only slight increase in the GA group and constant values in groups E and E + I. It is concluded that prostaglandins may play a role in mediating the surgical stress response, but that factors other than neural stimuli and prostaglandins are important in releasing postoperative granulocytosis.

Inverted papilloma of upper urinary tract.

Two new cases of inverted urothelial papilloma in the upper urinary tract are described and added to the 22 cases previously reported in the literature. In both cases inverted papilloma was localized beneath macroscopic normal surface, and in one of the cases the changes were found scattered widely in the upper urinary tract. The possible etiology and the symptomatology are discussed, and the need for follow-up of these patients is emphasized.

Pancreatic pseudocysts during first attack of acute pancreatitis.

One hundred and forty-eight patients admitted with their first episode of acute pancreatitis were examined by ultrasonography. During the acute attack 1 or more pseudocysts were found in 19 patients (13%), pancreatic abscess in 2, whereas 127 had a normal or swollen pancreas. Two small cysts resolved spontaneously, eight were cured after ultrasonically guided needle aspiration or catheter drainage, and cystogastrostomy was necessary in four cases. One patient refused treatment. Abscesses requiring surgical drainage developed in four of the patients with pseudocysts. The study showed that pseudocysts may appear as early as within 1 week of the first episode of acute pancreatitis. Some pseudocysts may resolve spontaneously, and ultrasonically guided aspiration or drainage may cure approximately half of the pseudocysts.

Fetal behavioural states and controlled sound stimulation.

The aim of this study was to investigate responses of human fetuses near term (37-42 wks) (increase/decrease of fetal heart rate and/or fetal motility) to acoustic stimuli. The study group consisted of 43 patients and the control group of 27 patients. Polygraphic recordings of 5 different fetal variables were carried out synchronously for the categorization of the actual fetal behavioural state according to Nijhuis et al. (Nijhuis, J.G., Prechtl, H.F.R., Martin, C.B., Jr. and Bots, R.S.A.M. (1982): Early Hum. Dev., 6, 177-195). Controlled acoustic stimulation was performed either with sine wave tone (2 kHz, 120 dB and 5 s duration) or modulated sine wave tone (0.5-2 kHz, 'sawtooth' modulation, 120 dB and 5 s duration). An additional group of patients received 'sham' stimulation. Overall 84 acoustic stimulations have been carried out. In 26 out of 84 stimulations (30%) a fetal response could be observed within 5 s after acoustic stimulation. In general, there were far fewer fetal responses in sleep states than in states of wakefulness. Whereas in state 1 F only 1 out of 28 acoustic stimulations was followed by an immediate fetal response, more responses were observed in state 2 F. An obvious increase in fetal reactivity to acoustic stimulation was noted in states of wakefulness (states 3 F and 4 F). Comparison between the true and sham stimulations revealed a relatively high level of spontaneous fetal activity (high variation of fetal heart rate and fetal motility) present in states 2 F and 4 F. This must be taken into account in all assessments of fetal responses.(ABSTRACT TRUNCATED AT 250 WORDS)

Femoral hernia in children.

The records of 26 children with femoral hernia are reviewed. In five patients, inguinal herniotomy had been performed previously. The correct diagnosis was made preoperatively in 7 (27%) of the cases only. The operation included a repair narrowing the femoral canal in all but 5 cases. At follow-up 1/2-19 1/2 years no recurrence was found. The importance of a careful pre- and peroperative examination is emphasized.

Acute epididymitis with abscess formation in an infant.

A case of non-specific epididymitis with acute abscess formation in a 3-month-old infant is presented. Excision of the abscess was curative. The need of further examinations in the presence of urological symptoms is emphasized.

Prospective controlled vagotomy trial for duodenal ulcer: results after five years.

A prospective, randomized, controlled trial was performed to study truncal vagotomy and drainage (TV), selective vagotomy and drainage (SV) and parietal cell vagotomy (PCV) as elective treatment for duodenal ulcer. Five years postoperatively, 233 patients were available for study: 73 TV, 81 SV and 79 PCV. The ulcer recurrence rates were 13.6, 19.8 and 30.4 per cent respectively. The incidence of severe dumping was 4.9, 10.9 and 1.9 per cent; of severe diarrhoea 4.9, 6.3 and 1.9 per cent; of severe pain/dyspepsia 3.3, 6.3 and 3.8 per cent and of severe nausea/vomiting 0, 1.6 and 0 per cent respectively. Women suffered more postvagotomy symptoms than men. After treatment of recurrences and postvagotomy symptoms, more patients after PCV achieved excellent results than after TV and SV, as recurrences were easier to treat than severe postvagotomy symptoms. The only factor found contributing to the high ulcer recurrence following PCV was the large number of surgical trainees operating in the trial.

Management of bronchopleural fistula following pneumonectomy.

Bronchopleural fistula developed in 28 (12.5%) of 225 pneumonectomies performed for pulmonary carcinoma of non-small cell types during a 10-year period. The incidence of fistula apparently decreased significantly when chromic catgut was replaced by Dexon for closure of the bronchial stump. The fistula presented as an emergency in nine cases and was subacute in 19. The overall mortality from bronchopleural fistula was 28.6%. Conservative treatment, i.e. bronchoscopic application of silver nitrate to destroy the epithelium in the bronchial stump and induce granulation, achieved closure of fistula in all the surviving patients. In the seven patients with sterile pleural cavity the pleura was not drained. The results justified our principle of conservative management when a bronchopleural fistula does not present as an emergency. In emergency situations, however, or if the pleural fluid is purulent, pleural drainage should be instituted.

[Complications in pregnancy following abortion induced with PGF2 alpha ].

The influence of induced abortion on the subsequent pregnancy by intermittent intrauterine extraamniotic application of PG F2 alpha was studied in 159 secundigravidae. The results are compared with courses of pregnancy after conventional interruption and after induced abortion by extraamniotic application of glucose (Kovács). The investigations demonstrate the efficiency of cervical dilatation with PG F2 alpha for interruption in primigravidae.

Association between duodenal bulb ulceration and reduced exocrine pancreatic function.

Seventy two patients admitted to a medical department with dyspepsia but without a previous diagnosis of peptic ulcer disease or chronic pancreatitis were studied consecutively. A pancreatic function test (Lundh meal test) and an upper endoscopy was made in all patients. There was no difference in age, sex ratio, occurrence of upper abdominal pain or chronic alcoholism between the groups of patients with reduced pancreatic function (20) and the group with normal function (52). Seven duodenal ulcers were found, two in patients with normal pancreatic function (2/52 = 3.8%; 95% conf lim: 0.5-13.2) and five in patients with reduced pancreatic function (5/20 = 25%; 95% conf lim: 8.7-49.1). This difference was statistically significant (p less than 0.01). Duodenitis occurred with equal frequency in the two groups.
