Scalp topography and analysis of intracranial sources of face-evoked potentials.

Several reports have described that a positive vertex peak of an evoked potential varied in amplitude and latency specifically when images of faces were the eliciting stimulus. The scalp topography and the possible underlying dipole sources of this peak are the subject of this report. We presented black-and-white photographs of human faces, flowers and leaves to 16 healthy subjects and recorded the evoked brain potentials from 31 scalp electrodes. We found the previously described higher amplitude of the positive vertex peak when faces were the crucial stimulus, but the latency of this peak was the same (180 ms) for all three categories of stimulus. At the posterior temporal electrodes, the face waveforms showed a negative peak at 175 ms, which was only rudimentary in the waveforms elicited by the other stimuli. Since in most previous reports a mastoid reference was used, it is most likely that the previously described latency shift of the positive vertex peak associated with face stimuli was due to the interaction with this posterior temporal peak. The dipole analysis of the possible generators of the recorded potentials suggested the sequential activation of occipital, lateral temporal and mesio-temporal brain structures during the perception of a human face.

Improved automated leucocyte counting and differential in newborns achieved by the haematology analyser CELL-DYN 3500.

Automated leucocyte counts in newborns generated by the impedance principle are artificially affected by the high osmotic resistance of some newborn RBC and possibly by the high normoblast numbers present during the neonatal period. Erroneously high WBC counts may result. The haematology analyser CELL-DYN 3500 (Abbott Diagnostika GmbH, Wiesbaden-Delkenheim, Germany) has two different channels for the WBC count, an electrical resistivity (impedance) channel and a laseroptical channel. In combination with facultative extended lysis of resistant RBC before WBC count, this instrument is claimed to be very suitable for newborn blood analysis. We measured the WBC count and differential of 165 blood samples from newborns and cord blood on the CELL-DYN 3500. Reticulocyte count and manual differential including normoblasts were determined. Furthermore, some technical aspects of neonatal blood analysis were evaluated: precision, cell stability, effect of incorrect blood-anticoagulant ratio of small blood collecting tubes. The internal decision making process of the CELL-DYN 3500 selects the result either from the optical channel (identifies and excludes normoblasts) or from the resistivity channel (eliminates resistant RBC). This instrument gives a reliable and accurate WBC count and differential of neonatal samples even in blood samples with normoblasts and lytic resistant RBC. The result given by the CELL-DYN 3500 can be confirmed by a subsequent run in extended lyse mode.

Rotational vertigo in embolic stroke of the vestibular and auditory cortices.

We describe the unique case of a woman with embolic infarction within the right middle cerebral artery territory who had nonepileptic rotational vertigo, nausea, and unsteady gait that gradually resolved within a week. A well-demarcated lesion of the posterior insula (homologous to parieto-insular vestibular cortex in monkeys) was probably the cause. This is supported by a contraversive tilt of perceived vertical and by involvement of the adjacent auditory (Heschl's) cortex. Dipole source analysis of late auditory evoked potentials revealed a decreased amplitude of a dipole source in the right insular cortex, whereas two other dipoles situated in the temporal lobe showed symmetric activity.

[Laparoscopic colonic surgery].

Eight patients were treated with laparoscopically assisted surgery for colorectal cancer. The technique, results and complications are presented. Conversion to laparotomy was necessary in two patients. There was one postoperative death, which could not be attributed to the operative procedure. It is concluded that laparoscopically assisted surgery will probably be a suitable treatment for some patients with colorectal diseases.

Molecular analysis of a leu2-mutant of Candida maltosa demonstrates the presence of multiple alleles.

Three different alleles of the beta-isopropylmalate dehydrogenase gene were cloned and sequenced from a leucine auxotrophic mutant, G587, of Candida maltosa. The cloning of functionally-intact wild-type genes from this mutant strain suggests the presence of silent gene copies. An interallelic-divergence comparison has provided evidence for new regulatory mechanisms. Sequence data and karyotype analysis argue for a highly-aneuploid genome of C. maltosa. An interpretation for the spontaneous auxotrophy-prototrophy-auxotrophy sequence of mutations in C. maltosa is suggested.

Palliation of rectosigmoid neoplasms with Nd:YAG laser treatment.

PURPOSE: The aim of this study was to evaluate the effect of Nd:YAG laser treatment as palliation for rectosigmoid neoplasms. METHODS: Indications for laser therapy, the degree and duration of symptom relief, complication rate, and survival time were recorded in consecutive patients. RESULTS: Seventy-four patients entered the study. Poor general health in older patients, and disseminated or complicating disease were the most frequent indications for therapy. Fifty-five (74 percent) patients experienced good symptomatic effect from the treatment. Six complications occurred: five cases of perforation and one case of moderate bleeding. There was no mortality. The median survival was seven months (range, 14 days-39 months). CONCLUSION: Laser treatment is a good palliative method in patients with colorectal cancer, especially in patients with local recurrence or symptoms from non-resected tumors.

Serum tryptase and urinary 1-methylhistamine as parameters for monitoring oral food challenges in children.

To study the usefulness of urinary 1-methylhistamine and serum tryptase concentration as monitoring parameters in clinical settings, we investigated 32 children with atopic dermatitis and suspected food allergy during oral food challenges with eggs and cow's milk. Urinary 1-methylhistamine (MH) excretion increased significantly within 1 h after positive oral food challenges (p < 0.006), but showed considerable variation in negative challenges. MH seems to be a sensitive parameter (92.8%), but its specificity is insufficient (37.7%). In the group of 16 positive oral food challenges serum tryptase concentration increased significantly (p < 0.02) directly after provocation and remained elevated up to 2 h after provocation. No variation was observed in negative challenges or nonatopic controls. Serum tryptase concentration seems to be specific for marked clinical reactions after oral food challenges (100%), but its sensitivity was low (25%) and not superior to evaluation by clinical means. We conclude that, despite positive results for the group of children, MH and serum tryptase concentrations are not useful parameters for monitoring oral food challenges in an individual child due to insufficient sensitivity and specificity.

[Palliative treatment of rectal and sigmoid cancer with Nd-YAG laser].

Nd-YAG laser treatment was employed as palliative treatment in 47 patients with colorectal cancer. Thirty-eight patients (81%) experienced good symptomatic effect from the treatment. Four complications occurred: three cases of perforation and one case of moderate bleeding. There was no mortality. It is concluded that laser treatment is a good palliative method in patients with colorectal cancer, especially in elderly patients with complicating diseases and in patients with local recurrence or symptoms from nonresected tumours.

Effect of combined prednisolone, epidural analgesia, and indomethacin on the systemic response after colonic surgery.

Twenty patients undergoing colonic resection were randomized to either conventional postoperative pain treatment with morphine chloride and acetaminophen (group 1, n = 9) or methylprednisolone sodium succinate 90 minutes before surgery plus intraoperative neural blockade, with a postoperative analgesic regimen with combined bupivacaine hydrochloride-morphine and indomethacin sodium for systemic effect (group 2, n = 11). Assessments of pain, pulmonary function, convalescence, and various injury factors were done several times until 8 days after surgery. Postoperative pain and hyperthermic response were eliminated in group 2. Conventional reduction in pulmonary function measures was improved in group 2, and fatigue and mobility were less pronounced. Prostaglandin E2, interleukin 6, and C-reactive protein levels increased in both groups, but significantly less in group 2. These results suggest that a combined neural and humoral blockade may more effectively inhibit the global stress response to elective surgery than previously observed with neural blockade with or without indomethacin.

Efficient electropulse transformation of intact Candida maltosa cells by different homologous vector plasmids.

Conditions for efficient and quick transformation by electroporation were developed in Candida maltosa. To investigate the efficiency of transformation with integrative as well as with autonomously replicating plasmids, a series of vectors was constructed for homologous transformation of this species. Transformants were obtained with different plasmids as covalently closed circular molecules and as linearized DNA. The influence of recipient strain and plasmid type as well as of cell number and parameters of the supplied electrical pulse on the transformation efficiency have been investigated. A maximum of 7000 transformants per 100 ng of plasmid DNA was reached. The efficiency of transformation was compared with that of the LiCl method.

Diurnal variation of urinary histamine and 1-methylhistamine excretion in healthy children.

In order to establish a noninvasive method of monitoring immediate hypersensitivity reactions in children, we studied the diurnal variation of urinary histamine and 1-methylhistamine excretion and the influence of food intake in a group of 14 healthy nonatopic children (aged 2-16 years). Histamine and 1-methylhistamine in spontaneous urine samples were determined by radioimmunoassay. Mean variation of 2-h intervals was much higher for urinary histamine than for 1-methylhistamine (45% of base-line level versus 24%). There was no circadian rhythm or influence of food intake. The short half-life of histamine released into blood circulation may be the main reason for the higher variation of histamine excretion. In children, urinary 1-methylhistamine is less influenced by diurnal variation and is therefore more suited to monitor immediate hypersensitivity reactions than urinary histamine itself.

Hormonal and neural blockade prevents the postoperative increase in amino acid clearance and urea synthesis.

The combined effect of continuous blockade of glucagon and cortisol by somatostatin and etomidate and thoracic epidural analgesia on hepatic conversion of amino nitrogen was studied in eight patients who underwent elective cholecystectomy on day 1 after operation and was compared with 16 patients who underwent operation without blockade. Surgery increased the plasma clearance of total alpha-amino nitrogen from 5.2 +/- 0.3 to 6.6 +/- 0.3 ml/sec (mean +/- sem; p less than 0.05). This increase was due to increased elimination by the liver, because the hepatic effectiveness for amino nitrogen conversion measured by the functional hepatic nitrogen clearance increased from 9 +/- 2 to 16 +/- 4 ml/sec (p less than 0.05). In contrast, during the combined neural and hormonal blockade, surgery decreased the plasma clearance of amino nitrogen from 5.3 +/- 0.3 to 3.9 +/- 0.3 ml/sec (p less than 0.05), and the blockade prevented the postoperative increase in functional hepatic nitrogen clearance. The results suggest that glucagon, cortisol, and afferent neural reflexes are mediators of the hepatic contribution to catabolism after operation.

Influence of local anaesthesia and local hypothermia on leucocyte, temperature, and transferrin response to surgery.

Leucocyte counts, rectal temperature, P-cortisol, P-glucose, P-transferrin, P-orosomucoid, P-IgM and hematocrit were investigated in 24 men undergoing inguinal herniotomy. The patients were randomised to epidural analgesia (n = 8), local infiltration anaesthesia (n = 8) or local infiltration anaesthesia plus local wound hypothermia with ice bags (n = 8). The rectal temperature rose (p less than 0.01) in patients randomised to epidural analgesia but not in the other groups. However, blood granulocyte and acute phase protein changes were similar in all groups. The neural blockade was sufficient in all patients as judged by lack of increase in P-cortisol and P-glucose. Thus, local infiltration anaesthesia and wound hypothermia could not prevent the inflammatory response to small clean surgery.

Transurethral ultrasound scanning in the evaluation of invasive bladder cancer.

One hundred and sixty-six patients suspected of having bladder tumours underwent transurethral ultrasound scanning before cystoscopy. Forty-nine patients had no tumour either at cystoscopy or on ultrasonography. Nine of the remaining 117 patients (8%) had false positive results and in a further 8 (7%) the ultrasound failed to show the tumour. The diagnostic accuracy for superficial tumours was 73%, whereas the accuracy in assessing muscle infiltrating and more advanced tumours was almost identical, 58-59%. It is concluded that transurethral ultrasound scanning does not add to the information obtained from cystoscopy and bimanual palpation in patients with bladder tumours.

Palliation of oesophagogastric neoplasms with Nd:YAG laser treatment.

The palliative effect of endoscopic Nd:YAG laser treatment of malignant strictures of the oesophagus and cardia was prospectively evaluated in 30 patients. Most (n = 28) were primarily treated for dysphagia, and swallowing was improved in 21 and normalized in 14 of these patients. Two patients were successfully treated for bleeding. Six patients developed complications (including two perforations), and four of these were fatal. The median survival was 4 months (range, 17 days to 14 months). It is concluded that in a great number of patients with inoperable cancer of the oesophagus and cardia, laser treatment is better than other palliative procedures.

Effect of combined prednisolone, epidural analgesia and indomethacin on pain, systemic response and convalescence after cholecystectomy.

Twelve patients undergoing elective cholecystectomy received as analgesic medication a single dose of methylprednisolone (30 mg/kg) preoperatively and thoracic epidural analgesia with plain bupivacaine for 48 hours + epidural morphine 4 mg and systemic indomethacin 100 mg, both every 8 hours for 96 hours. Assessments of pain, various parameters of response to injury, peak flow and subjective fatigue were made preoperatively, before and 3 and 6 hours after skin incision and 1, 2, 4 and 8 days postoperatively. These patients were matched with 24 from a previous study who were treated with either intermittent nicomorphine and acetaminophen or with epidural analgesia + systemic indomethacin as analgesic medication. Preoperative methylprednisolone resulted in improved pain relief, with concomitantly reduced need for epidural bupivacaine, prevention of hyperthermic response, improved postoperative pulmonary function and lessened fatigue, while the leucocytic and acute phase responses were unmodified. There were no side effects. These results may be explained by inhibition of various trauma-induced inflammatory mediators.
