The Danish oral health care services for children began its development at the beginning of this century. The aims and main principles of the service were formulated in the 1960s and in 1971 the Danish Parliament passed the Act on the Children's Oral Health Care System. By 1987 a complete oral health care service as a decentralized public health enterprise will be fully established covering all Danish children from 0 to 16 years of age. The oral health care programme is founded on health education and prevention. Treatment services are considered to be a safety net for disease not yet prevented. The oral health care service for children is described in detail and future developments are discussed.
The proportion of subjects homozygous for DNA restriction fragments of a large size class (U alleles), in the polymorphic region flanking the 5' end of the insulin gene of chromosome 11, was higher in a group of non-insulin-dependent diabetic (NIDDM) patients than in normal controls. This finding confirms that the U allele is associated with at least one form of NIDDM. The U alleles were also found to be strongly associated with macroangiopathy in diabetic as well as in non-diabetic subjects. Thus, DNA sequences contained in the U alleles might be associated with the development of atherosclerosis in an as yet unknown way.

There is restriction-fragment length polymorphism in the 5'-flanking region of the insulin gene on the short arm of chromosome 11 in man. The polymorphic DNA sequences in 53 members of a large family were analysed by means of the restriction endonuclease BglI. In this family, the BglI restriction fragments were found in four sizes--2.8 kilobases (kb), 2.9 kb, 4.6 kb, and 4.8 kb. They segregated as alleles. The two larger BglI restriction fragments (U alleles) were associated with high haemoglobin Alc levels in both blood-related and unrelated non-diabetic members of the family.

The Danish oral health care delivery system is a publicly funded, predominantly school based system of care for children and adolescents and a private practice copayment national health insurance mode for adults. Study of this dualistic system can provide useful information to dental public health practitioners. Interviews with experts in the Danish oral health care system and a selected review of the literature demonstrate that improvement trends are occurring in the oral health status of both adults and children. These improvement trends can be directly related to a combination of treatment and preventive measures undertaken by the country as a whole, and to specific interventions for certain subpopulation groups, such as preschoolers. Four key variables can be defined which have contributed to the success of the Danish oral health care delivery system (DOHCDS). These include 1) the role of private sector input into the development of the DOHCDS, 2) demography, 3) the Danish value system, and 4) the structure of the DOHCDS.
A number of reports during recent years have suggested that dental caries is decreasing in several populations. In order to substantiate this, recordings of dental caries collected over a 10-year period were analyzed. All children received systematic preventive and restorative care from kindergarten to 9th or 10th grade (age approximately 16 years). Each cohort consisted of 50-60 children, and data on dental caries were available on all children in 1st through 7th grade over a 9-year period. Mean DMFS had decreased with 72% for 1st graders, 65% for 4th graders and 69% for 7th graders. Analyses of the distribution of children according to DMFS showed that in spite of the general decrease in mean DMFS, some children still showed high DMFS-values. The variability according to DMFS was increased during the same period as indicated by the coefficient of variation. Furthermore, children with a given, high DMFS account for a decreasing fraction of all DMF-surfaces in the population as the mean DMFS decreases. It is suggested that the effect on mean DMFS for the entire group of children of a hypothetical, individualized preventive program for high-risk individuals would be limited. In the light of these findings the appropriateness of special preventive programs for "high-risk" individuals is questioned.

Eighteen dentists working in the same School Dental Service participated in a calibration trial with the purpose of reducing interexaminer disagreement on radiographic diagnosis of approximal caries. The calibration program was in two stages: firstly, discussion in small groups and, secondly, the use of a reference standard. The effect of the training programs on interexaminer disagreement was limited, but a statistically significant decrease in the number of D-surfaces recorded per child was observed. The finding that the major part of the error variance was due to random error, rather than systematic error, may explain why the calibration program was unsuccessful at reducing error.
A longitudinal study on the effect of a preventive program based on bimonthly professional toothcleaning and hygiene instruction was carried out on 293 schoolchildren from the 1st, 4th and 7th grades. The prophylactic paste used contained 0.8% sodium monofluorophosphate. The control groups brushed with a 0.2% sodium fluoride solution four to five times during each school year. After 2 years the Visible Plaque Index and the Gingival Bleeding Index were significantly reduced for all grades in the test groups as compared to the controls. The percentage of children without new caries lesions increased significantly from an average of 50% in the control to 71% in the test group. The mean caries increments during the 2 years, excluding occlusal surfaces, were 0.22, 0.28 and 1.20 in the three test groups. This represents an average caries reduction of 65%, compared to the control group. The increment reductions reached borderline significance, using the chi-square test.

This investigation was carried out in order to collect information regarding oral health in a group of schoolchildren in Kandy, Sri Lanka. 614 children of 6.9, 12 and 15 years were selected by stratified random sampling and examined under field conditions. The primary dentition showed a high rate of caries on all types of surfaces. Caries in the permanent dentition was mainly located to pits and fissures below the age of 12 years, after which smooth surface caries developed. The D-component was by far the largest component of the DMF-index in both the primary and the permanent dentition. The results are discussed in the light of the existing dental care delivery system in Sri Lanka.

The aim of the present study was to test the caries-preventive effect of a bis-GMA type fissure sealant (Concise Enamel Bond System) in a public dental health service for children. The study compared one group of children, in which all sound permanent teeth were sealed, and a control group in which no sealants were placed. During the study period all children received systematic restorative care, while sealing was performed on the basis of diagnoses made at subsequent examinations by one of the authors. After 2 years a total of 187 children were available for re-examination. Sealant retention was comparable to that obtained in previous studies, but no statistically significant reduction in overall caries increment could be demonstrated. Further analysis showed that only 223 sites had been sealed, while the majority of all erupting sites (394) were either carious or filled before they were screened for the purpose of sealing. It is therefore assumed that only teeth less susceptible to occlusal decay were sealed, thereby explaining the lack of benefit derived from the sealing program.
