Validity of a dietary interviewing method for use in caries prevention.

A suitable method for determining the usual diet of an individual is a prerequisite for dietary counseling in caries prevention. The purpose of this study was to develop a method that was able to obtain relevant information on the dietary habits of an individual and to test the validity of this method. During a series of interviews a dietary history interview was therefore developed with special emphasis on dietary factors known to be important in caries etiology, such as frequency of eating and frequency and duration of sugar intake at meals and in-between meals. The validity of the method with regard to caries-related factors was tested by interviewing 49 caries-active and 55 caries-inactive 14-yr-old Danish schoolchildren. It was possible to detect a higher frequency of food intake and a more frequent and long-lasting use of sugar in the caries-active group. No difference was found in the consumption of sticky sugars between the two groups. With the present method it should be possible to pinpoint dietary problems in caries-active persons, so that dietary advice can be given in quantitative terms.

Prevalence of dental fluorosis after fluoride-gel treatments in a low-fluoride area.

The goal of the present investigation was to study whether periodic use of fluoride-gel treatments resulted in an increased prevalence of dental fluorosis. 269 children, from 14 to 16 years of age, who had received semi-annual or quarterly fluoride-gel treatments were examined for possible fluoride-induced enamel changes. Further, in order to determine the amount of fluoride retained during the gel treatment, the expectorate was analyzed in 18 younger children presently participating in the prophylaxis program. It was found that the retention of fluoride averaged 7.3 mg per treatment (up to 0.5 mg fluoride per kg body weight). An increased prevalence of dental fluorosis was not observed, even after up to five yearly treatments were given during tooth formation periods. The background prevalence of dental fluorosis in a low-fluoride area in Denmark is described.

The effects of phenytoin (diphenylhydantoin) on the extrathyroidal turnover of thyroxine, 3,5,3'-triiodothyronine, 3,3',5'-triiodothyronine, and 3',5'-diiodothyronine in man.

The extrathyroidal metabolism of T4, T3, rT3, and 3',5'-diiodothyronine (3',5'-T2) was studied before and after treatment with 350 mg phenytoin (DPH) daily for 14 days in six hypothyroid patients receiving constant L-T4 replacement. The total and free serum concentrations of the four iodothyronines were reduced by approximately 30% during DPH treatment, whereas the free fractions in serum were unaltered. Concomitantly, serum TSH increased 137% (P less than 0.02). The production rate (PR) of T4 decreased 16% (P less than 0.005), indicating decreased intestinal absorption (bioavailability) of oral L-T4 during DPH treatment. The fractional rate of 5'-deiodination of T4 to T3 increased from 27% to 31% (P less than 0.05), whereas the rate of 5-deiodination of T4 to rT3 decreased from 45% to 25% (P less than 0.05). The urinary excretion of free and conjugated T4 was 2.3% of the T4 PR and was unaffected by DPH. Thus, the amount of T4 metabolized through nondeiodinative pathways apart from urinary excretion increased from 25% to 44% (P less than 0.05). The apparent distribution volume (Vd) of T4 increased (P less than 0.05), whereas the pool size was unchanged. The PR of T3 did not change during DPH treatment, nor did the mean transit time or the cellular clearance. The rT3 PR was reduced by 54% (P less than 0.02) during DPH treatment. Concomitantly, the transit time increased 10-fold (P less than 0.05), whereas Vd and pool size increased 5-fold (P less than 0.01 and P less than 0.05, respectively). The turnover of 3',5'-T2, in contrast to that of the other iodothyronines, did not change significantly during DPH treatment. T3 formation from T4 was measured in liver microsomal fractions from rats treated for 8 days with DPH and was almost identical to that in untreated animals. The data demonstrate that DPH in therapeutic concentrations did not affect serum protein binding of the iodothyronines. DPH reduced the intestinal absorption of T4 and increased the nondeiodinative metabolism of T4. The resulting decrease in total and free serum T4 and T3 was associated with an increase in serum TSH, demonstrating reduced negative feedback on the pituitary. Our data do not support the assumption that DPH induces increased hepatic deiodinating enzyme activity.(ABSTRACT TRUNCATED AT 400 WORDS)

Adductor transfer versus tenotomy for stability of the hip in spastic cerebral palsy.

Two groups of children with cerebral palsy are compared with respect to the migration of the hip before and after 36 adductor transfers and 29 adductor tenotomies. The results in the two series are positive, with no significant difference (4 and 7%/year, respectively). Adductor transfer is a more stressful intervention for the child, with no proven advantage as yet. Therefore, we have for the time being withdrawn this procedure in favor of adductor tenotomy.

Caries clinical trial of fluoride rinses in a Danish Public Child Dental Service.

365 2nd through 4th graders completed a 3-yr clinical trial on the caries-preventive effect of rinsings every second week during the school year with 10 ml of an 0.2% neutral solution of sodium fluoride. All children received regular dental examinations and treatment in clinics established by the municipality in which the study took place. The trial was performed under double-blind conditions. The caries increment on teeth erupted at baseline was 1.75 DMFS in the fluoride group and 1.83 DMFS in the placebo group (P greater than 0.05; 95% confidence limits for percentage caries reduction: -20.7% and 29.5%). The caries increment on teeth erupting during the trial was 0.73 DMFS in the fluoride group and 0.99 DMFS in the placebo group (P greater than 0.05; 95% confidence limits for percentage caries reduction: 1.0% and 51.6%).

Development of a Danish preventive program for preschoolchildren based on cooperation with key-persons.

Plans to expand public child dental services to include preschoolchildren and the recently established training of dental hygienists in Denmark form the background for the present study. The study was performed in a rural municipality located 35 km north of Aarhus. Public child dental services for schoolchildren (6-16 years of age) had been established for several years and were being planned to include preschoolchildren. In cooperation with the personnel in the Child Dental Service a full-time employed dental hygienist developed a communication network with different categories of key-persons (health nurses, private daycare mothers and nursery school teachers). Close records and diaries were kept during the entire 3-year study period. The paper summarises the experience gained from co-operation with a large number of key-persons and illustrates some of the conditions necessary for a successful process. The effect of the program on dental caries is discussed.

Dental health of Vietnamese Boat People on Pulau Bidong, Malaysia.

The influx of refugees from Vietnam to the industrialized countries has attracted a certain interest to studies describing the oral health status of these population groups. The present study comprises 361 refugees arriving in Malaysia from Vietnam and collected immediately at the refugee camp on Pulau Bidong. Dental caries, calculus, gingival bleeding and loss of periodontal attachment were recorded. Mean dmft increased from 1.3 for 0-2-yr-olds to 7.4 for 3-5-yr-olds. For 6-9-yr-olds mean DMFT was 2.4 while it ranged between 8.5 and 10.10 for the older age groups. The frequency of secondary lesions was high for all age groups. Calculus increased consistently with age, while gingival bleeding was common even in the youngest age group. Loss of periodontal attachment greater than or equal to 6 mm was rare in all age groups except the oldest (45 yr or older). A strategy for oral health care for these population groups is discussed.

Reduction of formation of denture plaque by a protease (Alcalase).

The efficacy of a protease preparation (Alcalase, Novo, Denmark) in preventing accumulation of plaque on the fitting surface of complete upper dentures was examined in a double-blind study. Seventeen denture wearers with clinically healthy oral mucosae participated in the study. The protease was dispensed in dissolvent tablets containing 15, 30, 60, 100, 300, 500, or 1000 mg of the enzyme. Enzyme tablets or placebo tablets were dissolved in 150 ml water at 50 degrees C, and the dentures were soaked once daily for 15 min. Denture soaking was performed with or without brushing. The study consisted of 14 1-week periods. At the start of each period the dentures were cleaned by brushing until they showed no visible plaque after staining. At the end of each period plaque was stained and the dentures were photographed. The photographs were projected, and the percentage area of the fitting surface showing plaque deposits was scored with 10% intervals. Statistical analysis showed that there was a significant reduction of plaque scores with increasing enzyme concentration. The most marked reduction in plaque formation was seen when mechanical cleansing was combined with immersion in a 500-mg enzyme solution. At a concentration of 300 mg (or more) the enzyme immersion was as efficient as mechanical cleansing in preventing denture plaque accumulation.

Evaluation of two methods of scoring denture plaque.

In a previously published study on the effect of an alcalase on development of denture plaque, two different methods were used to quantify plaque on the fitting surface of maxillary dentures. Both methods were based on scoring of photographs at X 10 magnification. In one of the methods the percentage area was estimated with 10% intervals from 0% to 100%. By the other method dentures were ranked according to cleanliness based on area and on degree of staining of plaque. The number of dentures in each of the 14 treatment periods was 14 or 15. The present study was conducted to determine the reliability of the two methods. Reliability of the two methods was determined by using Pearson's correlation coefficient and Kendall's tau. Both intra- and inter-examiner reliability was high. A high correlation between the two methods was also demonstrated.
