Sphingomyelinase activity in the bovine and human eyes.

We examined biochemically sphingomyelinase activity in the bovine and human ocular tissues, using trinitrophenylaminododecanoylsphingosylphosphorylcholine as substrate. The enzyme activity in the crude extracts of neuroretina, retinal pigment epithelial cells, and optic nerve of the bovine eyes was proportional to protein concentration. The activities in these tissues were little activated with Mg2+ at pH 5.1 but was activated with Mg2+ at pH 7.5. The enzyme activity at pH 5.1 was also detected in the iris and ciliary body, neuroretina, retinal pigment epithelium and choroid, and optic nerve of the human eyes. It was highest in the macula, compared with other areas of the human ocular fundus.

Combination chemotherapy with cisplatin and etoposide for malignant intracranial germ-cell tumors. An experimental and clinical study.

Antitumor activity against intracranial malignant teratoma by combination chemotherapy with cisplatin and etoposide was evaluated in experimental and clinical studies. A human teratoma cell line (Tera 2) was exposed in vitro to cisplatin and/or etoposide, after which cell growth inhibition and alterations of deoxyribonucleic acid (DNA) histograms were observed. The results indicated that a synergistic cytotoxic effect was achieved by use of both agents in combination. Four cases of recurrent intracranial germ-cell tumor (three malignant teratomas and one germinoma) were treated with cisplatin and etoposide. With this combination therapy, regression of the tumor was observed in all four cases (three complete and one partial), for a total response rate of 100%. During a follow-up period of 9 to 22 months, no recurrence or progression has been noted in three of these cases.

Postoperative instillation of mitomycin C in the treatment of recurrent pterygium.

We examined retrospectively 61 eyes of 61 patients with recurrent pterygia who had been treated with excision, with or without 0.02% mitomycin C instillation, 3 to 7 years after treatment. The rates of recurrence (5% to 9%) and complications in eyes that had undergone excision and mitomycin C instillation were less than those in eyes that had had excision only. These data suggest that the postoperative instillation of 0.02% mitomycin C, twice a day for five days, may be effective and safe in the treatment of recurrent pterygium.

[An autopsy case of carcinomatous sensory neuropathy associated with gastric adenosquamous carcinoma].

A 61-year-old man was admitted on May 1986 with complaints of hypesthesia and pain in the both legs, and of progressive difficulty in walking. Physical examination was unremarkable. On neurological examination, deep tendon reflexes were decreased in all extremities without pathological reflexes. Vibration sense was decreased severely at the medial malleolus and moderately at the anterior superior iliac spine. Joint sensation of the toes was moderately decreased. Light touch, temperature discrimination, and pinprick sensation were slightly decreased on fingers bilaterally and distal to the middle part of both legs. Muscle strength was normal. His gait was unsteady and Romberg's sign was positive. Finger to nose test and heel to knee test were mildly disturbed bilaterally. The sural nerve action potential was not elicited on electrical stimulation. Laboratory studies for malignancy showed gastric cancer. Only July 4, he underwent subtotal gastrectomy. Histologically it showed adenosquamous carcinoma. Postoperatively gait disturbance and pain in both legs improved slightly. Peak latencies of P2 of SEP following right and left posterior tibial nerve stimulation were 47. 9 msec and 48.8 msec on February 14, and 44.5 msec and 43.9 msec on October 6, 1986, respectively, and their postoperative shortening was evident. He died of multiple liver and lung metastasis of the gastric cancer in November 28, 1986. At autopsy, tumor metastasis were noted in liver, lung and perigastroduodenal and retroperitoneal lymph nodes.(ABSTRACT TRUNCATED AT 250 WORDS)

Oxalate crystallization in the kidney in the presence of hyperuricemia.

It has been a long time since uric acid was suggested to be a promoting factor in calcium oxalate stones, and a number of in vitro studies have been carried out on the relationship between uric acid or urate and calcium oxalate. Concerning in vivo studies, urate or calcium oxalate stone-forming diets were given alone in most cases, and diets that induce formation of stones with different composition have not been given in combinations. We administered a low-concentration oxalemic diet, and a mixed diet containing oxalic acid and uric acid, and biochemically and histologically studied the effects of oxalate and uric acid on kidney stone formation. In the kidney of the animals given the mixed diet, formation of crystalloids of uric acid or urate was evident when no crystallization was noted in the kidney of those given the low concentration oxalemic diet alone. The morphological differences in the uric acid and urate crystalloids in the kidney and the process leading to crystallization of calcium oxalate were examined under transmission and scanning electron microscopy. Histological examination indicated that these uric acid crystals and urate crystals serve as seeds and induce formation and epitaxial growth of calcium oxalate crystals. Our in vivo study provides additional evidence that uric acid is a promoting factor in calcium oxalate stone formation.

[Relation of surface texture of fine finishing diamond point for composite resin and polished surface of composite resin].

Distribution of diamond grain size of seven fine finishing diamond points was measured by a digital image analyzer. Also influence of diamond grain size of fine finishing diamond points on finished surface of two types of visible light cured composite resins (semihybrid type and submicron filler type) were investigated. Diamond grain size was almost from 10 to 100 microns 2 in area. Diamond grain size was closely related to the surface roughness of semihybrid type composite resin, although it was not related to that of submicron filler type composite resin. Surface roughness of a submicron filler type composite resin finished at a low speed was less than that at a high speed. Grain size of diamond point and revolution speed may play an important role in surface texture of composite resin.

[Milk of calcium renal stone: a case report].

A case of milk of calcium renal stone is presented. A 29-year-old woman complained of fever up and left lateral abdominal pain was diagnosis of milk of calcium renal stone. Ultrasonography showed hemispheric hypoechoic area and horizontal high echoic line with acoustic shadow. So-called "milk of calcium renal stone" is relatively uncommon and reports of ultrasonographic features are rare.

[The retroperitoneal liposarcoma: a case report].

A 62-year-old woman was admitted to our hospital because of a mass in the right abdomen. Ultrasonography, aortography and computed tomographic (CT) scanning revealed the right kidney displaced by a huge and avascular mass containing fatty tissue. Also magnetic resonance imaging (MRI) indicated that it was contained of lipomatous tissue and demonstrated the mass occupying the entire right abdominal cavity. The tumor including the right kidney was resected, following diagnosis of the retroperitoneal tumor. The exercised tumor measured 28 X 22 X 18 cm and weighed 2,010g. The histological diagnosis was well-differentiated liposarcoma. She has been healthy for three months following postoperative chemotherapy. Many reports have been made on the ultrasonographic and CT appearance of retroperitoneal liposarcoma, but few have been made on the MRI appearance. According to our case, imaging diagnosis by MRI also seems to be useful in making a histological diagnosis of liposarcoma.

[Combined chemotherapy of CDDP and etoposide in intracranial germinomas].

Two cases with pineal germinomas were treated by the combined chemotherapy of CDDP and etoposide. The first case was a 15-year-old boy who had a pineal tumor of 2.2 x 1.8 cm in diameter. He was given intravenously 25 mg/body of CDDP and 30-60 mg/body of etoposide daily for five days. The tumor had already decreased at two days after starting chemotherapy and disappeared completely just after the first course of chemotherapy. The patient received another course of chemotherapy and focal radiation of 40 Gy to prevent tumor recurrence. The second case was 19-year-old man who had a pineal germinoma associated with CSF dissemination and hydrocephalus. He was also treated by 30 mg/body of CDDP and 80 mg/body of etoposide daily for five days. The tumor size decreased more than 50% just after the first course of chemotherapy, and complete remission was obtained by additional radiation of 6 Gy. From these results, this combined chemotherapy is useful for the treatment of intracranial germinoma, and this makes it possible to discontinue radiation therapy or at least reduce the radiation dose and radiation field.

Phagocytic activity of hemocytes of M-line Biomphalaria glabrata snails: effect of exposure to the trematode Echinostoma paraensei.

The phagocytic activity of hemocytes from 6-8-mm M-line Biomphalaria glabrata snails was studied in an in vitro assay using glutaraldehyde-fixed sheep erythrocytes (SRBC) as target cells. For individual snails, the percentage of hemocytes ingesting SRBC during a 1-hr interval, termed the phagocytic activity index (PAI), was determined. Hemocytes from snails infected for 1 day with Echinostoma paraensei had a slightly elevated PAI, but at both 8 and 30 days postexposure (DPE), hemocytes from infected snails had a significantly lower PAI than controls. Hemocytes taken from snails at 8 DPE also had a low PAI using rabbit erythrocytes and yeast as target cells. The low PAI at 8 DPE is attributed to the presence of large numbers of poorly spreading hemocytes with low phagocytic activity. Hemocytes from snails with 30-day infections were well spread but nonetheless had a low PAI. The presence of plasma from 8-day infected snails did not alter the PAI of hemocytes from control snails, nor was the PAI of hemocytes from infected snails changed by plasma from control snails. SRBC preincubated for 60 min in plasma from various groups of M-line snails did not elicit an increase in PAI when presented to hemocytes from control snails; in some cases, as with plasma from 6-8-mm control snails, such preincubation significantly reduced the PAI below levels obtained using SRBC preincubated in culture medium. As compared to hemocytes from snails with normally developing, 8-day-old intraventricular sporocysts (IS), hemocytes from snails exposed to infection but subsequently lacking IS had a significantly higher PAI.(ABSTRACT TRUNCATED AT 250 WORDS)

Postoperative instillation of low-dose mitomycin C in the treatment of primary pterygium.

Eighty patients (99 eyes) with primary pterygia were treated with excision, with or without additional therapy, and were followed up for three to eight years after treatment. Of 29 eyes that underwent excision and postoperative instillation of 0.02% mitomycin C, there were only two recurrences. The other eyes were treated with excision only, excision and radiation, or excision and 0.04% mitomycin C. Instillation of 0.02% mitomycin C reduced the recurrence rate significantly (P less than .01). Only one of the 29 eyes (3%) treated with excision and 0.02% mitomycin C had a complication, the lowest rate of all groups postoperatively. We found the postoperative instillation of 0.02% mitomycin C, twice a day for five days, to be effective and safe in the treatment of primary pterygium.

Establishment and characterization of a new human renal cell carcinoma cell line (KRC/Y).

A new renal cell carcinoma (RCC) cell line (KRC/Y) has been established from a surgical specimen of a 41-yr-old Japanese female patient with RCC composed of both clear cells and granular cells. This cell line has been maintained for more than 15 mo. through 45 passages with a stable growth. KRC/Y cells have clear or eosinophilic polygonal cytoplasm and round to oval nuclei with one or two nucleoli, and proliferate in a pavementlike cell arrangement with a lack of contact inhibition. By electron microscopy, these cells contain abundant fat droplets and glycogen granules or well-developed organelles or both, which were also observed in the original tumor. The doubling time of these cells at the 15th passage was 73 h. The chromosome number was from 37 to 45 with a hypodiploid modal number of 42. Tumorigenicity was identified by tumor formation after subcutaneous injections of KRC/Y cells in nude mice, which showed close resemblance to the original tumor by light and electron microscope observations.

Ca2+-dependent activation of the malate-aspartate shuttle by norepinephrine and vasopressin in perfused rat liver.

The role of Ca2+ in stimulation of the malate-aspartate shuttle by norepinephrine and vasopressin was studied in perfused rat liver. Shuttle capacity was indexed by measuring the changes in both the rate of production of glucose from sorbitol and the ratio of lactate to pyruvate during the oxidation of ethanol. (T. Sugano et al. (1986) Amer. J. Physiol. 251, E385-E392). Asparagine (0.5 mM), but not alanine (0.5 mM) decreased the ethanol-induced responses. Norepinephrine and vasopressin had no effect on the ethanol-induced responses when the liver was perfused with sorbitol or glycerol. In the presence of 0.25 mM alanine, norepinephrine, vasopressin, and A23187 decreased the ethanol-induced responses that occurred with the increase of flux of Ca2+. In liver perfused with Ca2+-free medium, asparagine also decreased the ethanol-induced responses, but norepinephrine and vasopressin had no effect. Aminooxyacetate inhibited the effects of norepinephrine, A23187, and asparagine. Regardless of the presence or absence of perfusate Ca2+, the combination of glucagon and alanine had no effect on the ethanol-induced responses. Norepinephrine caused a decrease in levels of alpha-ketoglutarate, aspartate, and glutamate in hepatocytes incubated with Ca2+. The present data suggest that the redistribution of cellular Ca2+ may activate the efflux of aspartate from mitochondria in rat liver, resulting in an increase in the capacity of the malate-aspartate shuttle.

Changes in hepatic functional reserve after transcatheter embolization of hepatocellular carcinoma. Assessment by maximal removal rate of indocyanine green.

To clarify the influence of transcatheter arterial embolization (TAE) on hepatic function, the maximal removal rate of indocyanine green (ICG-Rmax), which represents the hepatic functional reserve, and the plasma disappearance rate of indocyanine green (k-ICG) were measured serially before and after 15 TAE procedures performed on 13 hepatocellular carcinoma (HCC) patients with underlying hepatic diseases. Compared to the values before TAE, ICG-Rmax values did not change or gradually decreased during 4 weeks in seven of the 13 patients but markedly decreased in the remaining six by as much as 50% during the first week. k-ICG values remained almost unchanged at any time after TAE. Albumin and prothrombin time were serially measured before and after 24 TAE procedures performed on 21 HCC patients with underlying hepatic diseases in whom no plasma products had been used for therapy. Albumin decreased by up to 75% in one of the 21 patients during the first week but did not change or gradually decreased in 20 of the 21 patients. Prothrombin time showed no obvious changes. This study showed that prominent changes occurred in ICG-Rmax, i.e., in the hepatic functional reserve, after TAE.

The vectoral role of several blackfly species (Diptera: Simuliidae) in relation to human onchocerciasis in the Sierra Parima and Upper Orinoco regions of Venezuela.

Experimental and natural infections with Onchocerca volvulus were studied in several anthropophilic blackfly species present in the endemic area of the Upper Orinoco region of Venezuela. When fed on four different Yanomami volunteers in the Sierra Parima, the total infection rate was 31.4% for Simulium guianense in comparison with 7.5% for S. limbatum. The proportion of ingested microfilariae that entered the thorax during the first 24 hours and completed development to the infective stage was also much lower in the latter than in the former species. There was no larval development of O. volvulus in S. antillarum, but one female harboured in its head six infective larvae of a zoonotic filaria. When S. oyapokense s.l. was fed on an infected volunteer in the Upper Orinoco most flies ingested few microfilariae, and further development reached only the L1 stage. Natural infection and infectivity rates for S. guianense in two localities of the Sierra Parima were 0.2-4.0% and 1.3-10.2%, respectively, more than 50% of the L3 larvae being in the head. The corresponding data for S. oyapockense s.l. in the Upper Orinoco were 0.1-0.5%, although the only L3 larva found did not correspond to O. volvulus. It is suggested that S. guianense is the main vector of onchocerciasis in the Sierra Parima and that S. limbatum could play a secondary role. Simulium oyapockense s.l. replaces them in the Upper Orinoco and may maintain a degree of transmission, but its epidemiological importance remains to be assessed.
