Renal function and growth after antireflux surgery in children.

The outcome of antireflux surgery for vesico-ureteral reflux (VUR) was studied in 39 children (65 renal units). The T max of the renogram improved significantly after operations in 51 (78.5%) of the 65 renal units due to a reduction of the back pressure. Improvements in scarring were observed in only 3 (7.3%) of 41 scarred kidneys. The growth of 38 kidneys was examined using the renal ratio, more than 2 years (mean, 3 years and 1 month) after the operation. The normal renal ratio was restored in 1 of 12 small kidneys with a preoperative size of less than the mean -2SD. The other 11 kidneys remained small, but 6 of these grew with a normal growth curve. The growth of the remaining 5 kidneys was unremarkable. Of the 26 kidneys with a normal renal ratio, 4 became small after the operation, but all the others had growth within the normal range.

Evaluation of the therapeutic effect of percutaneous nephroureterolithotomy by Tc-99m diethylenetiaminepentaacetic acid (DTPA) renal scintigraphy--alteration of the renal fraction of blood flow, split-GFR, and renal mean transit time.

To evaluate the therapeutic effects of percutaneous nephroureterolithotomy, the renal function of eleven patients with renal calculi was studied, pre- and post-intervention. Renal function was determined, by renal scintigraphy with the renal agent, Tc-99m diethylenetriaminepentaacetic acid (DTPA). In each renal scintigram the renogram curve was analyzed and the following were determined by deconvolution analysis; the renal fraction of blood flow (RFBF), DTPA-glomerular filtration ratio (GFR), and the renal mean transit time (MTT). The successful results in percutaneous nephroureterolithotomy (PNL) was proven using the radionuclide technique in most cases. From these results it can be concluded that renal scintigraphy is an effective procedure to evaluate the effect of PNL for treating renal calculi and secondary hydronephrosis.

Histopathological studies on the cystic formation of the human urothelium.

Histopathological examination of pyeloureteritis cystica and cystitis cystica revealed the mechanisms for the cystic formation of the urothelium. Chronic stimulation with inflammation or physical stimulation with crystals or calculi causes the urothelium to form an inflammatory crypt. The crypt is isolated as a result of an adhesive occlusion of the urothelium at the orifice of the crypt. This crypt is an immature cyst that cannot be clinically detected. A von Brunn's cell nest represents a cut surface of the immature cyst. The inflammatory cyst isolated from the urinary tract, i.e., the immature cyst, gradually grows into a complete cyst, i.e., a clinically visible mature cyst, because of a hydrodynamic flow between the surrounding tissue and neogenetic capillaries, and inflammation. These findings indicate that von Brunn's cell nest, and glandular and cystic formation, occur during development from an inflammatory crypt to an immature cyst and then, a mature cyst.

Acute-phase kidney transplants show early peak appearance in Tc-99m DTPA renogram.

Kidney transplants have been evaluated using a radionuclide technique to determine perfusion index (PI), mean transit time (MTT), and the parameters in renogram curve (Tmax, T1/2, and T2/3). The values for Tmax, T1/2, and T2/3 were also determined in normal volunteers. The results indicated that the Tmax value was less than 2 minutes, one week after transplantation. The other data were within normal limits. The appearance of Tmax was very early. The early time activity curve (TAC) was quite similar to the blood disappearance curve, TAC did not have the functional-phase, so-called, 2nd-phase. These data suggested that there could be an abnormality of the renal circulation in the transplanted kidney. The suggestion offers the new finding in a field of renal transplantation using radionuclide technique.

[Drug information in recent 5 volumes of the Year Book of Dentistry].

The Year Book of Dentistry appears annually, providing a review of dental articles. We surveyed the abstracts of the recent 5 volumes of the Year Book of Dentistry, from 1985 to 1989. Two hundred and eighty articles were abstracted in each volume, divided into about 13 categories. The proportion of abstracts concerned with drugs and drug therapies (here termed drug information) was almost 20% in all volumes. Most of the drug information appeared under the heading "Oral Medicine and Oral Pathology" (63%). The original articles for the abstracts were from many journals, but 70% of the drug information articles were from JADA, Oral Surgery, and 10 other journals. The classification of the drugs in the drug information were: drugs for dental treatment, antibacterials, antiinflammatories, hemostatics, anesthetics, and others. Oral manifestations of systemic diseases or side effects of drugs used for these diseases were also abstracted. There were numerous articles of systemic diseases, due to dental treatments, including infective, hematologic diseases.

[In vitro cytotoxicity of Palfique Bonding Agent].

Cytotoxicity of Palfiqu Bonding agent including Universal and Catalyst was examined by a cell culture study using HeLa cells. The Catalyst was cytotoxic to the cell, but Universal revealed no such cytotoxicity. This depended on time and concentration of materials immersed in culture medium.

[Combined operation for renovascular hypertension and ischemic heart disease].

We have experienced two patients of ischemic heart disease associated with renovascular hypertension. Patient 1 (60-year-old man) underwent LV aneurysmectomy and triple aortocoronary bypass grafting (saphenous vein to diagonal branch, left internal mammary artery to obtuse marginal branch, and right gastroepiploic artery to right coronary artery). Seventy five days after the initial cardiac surgery endarterectomy for the left renal artery and bifurcated Dacron graft implantation for the iliac artery obstruction were performed. Patient 2 (62-year-old woman) underwent simultaneous operation of both right nephrectomy and triple aortocoronary bypass grafting (saphenous vein grafts to obtuse marginal branch and right coronary artery, and left internal mammary artery to left anterior descending artery). Their postoperative courses were uneventful except unstable and high blood pressure for four to seven days after the operation. It appears that it should be decided to achieve either simultaneous or two stage approach for ischemic heart disease associated with renovascular hypertension based on the preoperative cardiac function. And both postoperative cardiac function and hypertension should be carefully managed until the blood pressure becomes stable after the surgery.

[A polypoidal adenosquamous cell carcinoma of the esophagus with a pseudosarcomatous stromal reaction].

A case of a polypoidal carcinoma with a pseudosarcomatous stromal reaction is presented. Histologically, the polypoidal tumor of the lower esophagus was revealed to be an adenosquamous cell carcinoma associated with the proliferation of atypical stromal cells, osteoclastic-like giant cells, and histiocytes. Using an immunohistochemical stain, these stromal atypical cells were found to be positive for vimentin, with some cells positive for alpha 1-antichymotrypsin. However these atypical cells proved negative as epithelial markers (keratin and EMA). Thus, it was felt that such atypical stromal cells may be a reactive proliferation of the stromal cells to an adenosquamous cell carcinoma.

[A case of involuntary movements probably produced by low doses of phenytoin intoxication].

A 49-year-old woman, who presented gait disturbance, orofacial dyskinesia, choreoathetosis and slightly cloudy consciousness, was admitted to our hospital on February 7, 1986. She had a slight fever and sore throat for the previous ten days. She had been treated for hypothyroidism as well as migraine with abnormal electroencephalogram since age 47, and was given a daily dosage of 70 mg phenytoin, 80 mg phenobal, and 125 mg dried thyroid. On admission, she was somnolent, and her speech was slurred. There were choreoathetosis of all extremities, orofacial dyskinesia, horizontal nystagmus, and dysdiadochokinesis with impaired heel-knee and finger-nose test. She could not only walk but also stand by herself. The plasma level of phenytoin was above 40 micrograms/ml (normal: 10 to 20 micrograms/ml). The plasma level of phenobal was normal. T3 was 0.76 ng/dl (normal: 0.96-1.92). T4 was 3.3 micrograms/dl (normal: 5.1-12.8). Biochemical screening, liver and kidney function tests were normal. Cerebrospinal fluid, ECG, chest X-rays and brain CT were normal. Electroencephalogram showed 5 to 6 Hz moderate voltage theta waves with artifacts of electromygram due to orofacial dyskinesia. After phenytoin was discontinued, the dyskinetic movement and gait disturbance disappeared, and her consciousness became alert in parall with reduction of plasma level of phenytoin. We suggested that acute phenytoin intoxication due to low dosages of phenytoin might be precipitated by upper respiratory infection and that involuntary movements in this case might be related to hypothyroidism.

Localization of coronary artery narrowings by applying R-wave amplitude correction to exercise-induced ST depression in angina pectoris and single-vessel coronary artery narrowing.

This study investigated whether coronary artery narrowings can be localized by applying R-wave amplitude correction to exercise-induced ST depression in multiple unipolar precordial lead electrocardiography using 20 electrodes covering the left chest wall. Ten normal subjects and 29 patients with stable angina pectoris and single-vessel coronary artery narrowing (greater than or equal to 75% luminal diameter stenosis in only 1-vessel) participated. Of the 29 patients, 5 had left main coronary artery disease (CAD), 14 had left anterior descending CAD, 4 had right CAD and 6 had left circumflex CAD. The exercise-induced ST depression with R-wave amplitude correction was defined as the exercise-induced ST depression divided by the R-wave amplitude. The 20 points of the lead system were divided into 4 areas: the left main, left anterior descending, right and left circumflex coronary arteries. Coronary artery narrowing was supposed to be in an artery corresponding to the area where the maximal value of the exercise-induced ST depression with and without R-wave amplitude correction was situated. By applying R-wave amplitude correction, the diagnostic ability of localization of coronary artery narrowings was improved significantly from 52% to 86% (p less than 0.005). In particular, localization of the left main coronary artery narrowing was correctly diagnosed in 100% (5 of 5) of angina pectoris patients with left main CAD.

Inverted eyelashes in patients with type 1a glycogen storage disease.

We examined three adult Japanese patients who had a history of decreased hepatic glucose-6-phosphatase activity. All three patients had increased bilateral subcutaneous (SC) fat in the lower eyelids and inverted eyelashes. One patient additionally showed retinal hemorrhages and microaneurysms in both fundi. The inverted eyelashes may have been related to type 1a glycogen storage disease.

Antipyrine clearance per unit liver volume in cirrhotics with and without hepatocellular carcinoma indicating a correlation with histological change of the liver.

The antipyrine metabolizing capacity was studied in 12 patients with cirrhosis of the liver and 12 with cirrhosis and hepatocellular carcinoma (HCC). Antipyrine clearance (Cl) and liver volume (LV) were measured and the antipyrine clearance per unit liver volume (Cl/LV) was calculated. The patients with HCC showed a significantly lower Cl value than those without HCC but there was no significant difference in Cl/LV between the two groups. This suggested that the lower Cl values in the HCC patients resulted from a decrease in residual liver mass. Cl/LV showed positive correlation with % parenchymal cell mass as an indicator of residual parenchymal cell mass per unit volume of liver. This result showed a correlation of Cl/LV with histological change of the liver in cirrhotics.

Effects of infection with Echinostoma paraensei on the circulating haemocyte population of the host snail Biomphalaria glabrata.

Circulating haemocytes from Echinostoma paraensei-infected M line Biomphalaria glabrata snails, or from age- and size-matched control snails, were studied on plastic slides with phase-contrast optics. Granulocytes, hyalinocytes, and round cells were consistently present; granulocytes were further categorized as 'fully spread' (FS) or 'partially spread' (PS). Among control snails, the relative percentage and estimated number/mm3 of round cells declined significantly with increased snail size, and the corresponding values for both categories of granulocytes increased. At 1 day post-infection (p.i.) with E. paraensei, overall composition of the haemocyte population was relatively unaffected, but by 8 days p.i. infected snails had significantly higher relative percentages of round cells and PS granulocytes than controls. Because a marked increase in the number of circulating haemocytes is also evident by 8 days p.i., infected snails had approximately 12 times more round cells and PS granulocytes/mm3 of haemolymph than did controls. At 30 days p.i. the relative and absolute abundance of PS granulocytes was still significantly elevated, but otherwise haemocyte populations did not differ from control snails. Alterations in granulocyte size in infected snails were also noted. Infection with E. paraensei has a striking impact of circulating haemocyte populations and also increases the relative concentration of haemocytes with less ability to adhere to a foreign surface.

Experience with 647 consecutive tumors of the duodenum, ampulla, head of the pancreas, and distal common bile duct.

Between 1946 and 1987, 647 patients with periampullary tumors were diagnosed at the University of Chicago Medical Center. These included 549 tumors located in the head of the pancreas, 40 in the distal common bile duct, 29 in the duodenum, and 29 at the ampulla of Vater. Ninety-eight per cent of all tumors were adenocarcinoma, with 93% of the remaining being duodenal carcinoid or sarcoma. Operability rate ranged from 81% to 97%, according to the tumor location and histologic type. A combination of laparotomy, biopsy, and bypass was performed in 433 patients and only one survived 5 years (0.2%). Resectability rate ranged from 16.5% for pancreatic adenocarcinoma to 89.3% for ampullary tumors. Of the 133 resections, 80 were pancreatoduodenectomies, 29 total pancreatectomies, 7 duodenectomies, 2 gastrectomies, 8 common bile duct resections, and 7 local excisions. Overall 19% of patients who underwent radical resection died in the immediate postoperative period, although mortality has decreased to 5% since 1981. Mortality was 20% after a standard pancreatoduodenectomy and 24.1% after a total pancreatectomy. Five-year actuarial survival rates, including perioperative deaths, were 8.8%, 20%, and 32% for pancreatic, duodenal, and ampullary adenocarcinoma, respectively. One half of patients with sarcoma and two-thirds with carcinoid of the duodenum survived 5 years. No patient with distal common bile duct adenocarcinoma achieved a 5-year survival rate. Multivariate analysis on all patients operated on (n = 566) revealed that the 5-year survival rate was significantly related to intent of operation (palliative 0.2%, curative 12%; p less than 0.001), histologic type (adenocarcinoma 2%, carcinoid and sarcoma 31%; p less than 0.0001), and site (ampullary and duodenal 21%, biliary and pancreatic 0.9%; p less than 0.001). A second multivariate analysis, evaluating only those patients with adenocarcinoma who survived the perioperative period of the radical resection (n = 97) analyzed the influence of tumor size and differentiation, lymphatic, capillary, and perineural microinvasion, lymph node status, and type of procedure (pancreatoduodenectomy vs. total pancreatectomy) on 5-year survival. None of these additional variables was significantly associated with long-term survival rates. In addition we evaluated the presence of local or distant recurrence after resection by analyzing the findings from all autopsies performed on these patients (n = 49): 29.4% of patients died with local recurrence alone, 23.5% with distant recurrence alone, and 47.1% had both local and distant recurrences.(ABSTRACT TRUNCATED AT 400 WORDS)

Usefulness of rate responsive atrial pacing in patients with sick sinus syndrome.

We studied heart rate response and blood lactate changes during treadmill exercise testing in 10 patients with sick sinus syndrome (SSS) and normal atrioventricular (AV) conduction in whom an activity detecting multi-programmable rate responsive pacemaker was implanted for atrial pacing (AAI pacing). Treadmill exercise testing was performed twice in each patient under the two different AAI pacing modes: rate responsive function off (fixed rate 60 pulses per minute [ppm]; RR-off) and rate responsive function on (rate response 10, activity threshold medium, basic rate 60 ppm, and maximum rate 150 ppm; RR-on). Chronotropic response and blood lactate change during exercise were compared among patients under each pacing mode and eight healthy volunteers of ages matching those of the patients. In patients under the RR-off pacing mode (RR-off patients), chronotropic response was lower (P less than 0.01) and blood lactate level was higher (P less than 0.05) than those in patients under the RR-on pacing mode (RR-on patients) and volunteers during exercise. In RR-on patients, however, both chronotropic response and lactate level changes during exercise were comparatively improved, with no significant differences between RR-on patients and volunteers. These data suggested that improvement of chronotropic response by rate responsive pacing, in comparison with nonrate responsive pacing, increased the blood supply to the working muscles and resulted in less lactate production during exercise. It was concluded that rate responsive AAI pacing which could maintain AV synchrony and normal intraventricular conduction was an optimal physiological pacing mode for SSS patients with normal AV conduction.

Epiblepharon with inverted eyelashes in Japanese children. I. Incidence and symptoms.

Epiblepharon commonly occurs in Japanese infants and tends to disappear spontaneously with age. We examined 4449 Japanese children aged 3 months to 18 years for epiblepharon associated with inverted eyelashes touching the cornea. The condition was evident in 441 cases. We found that the incidence of epiblepharon decreased with age, but about 2% of high school students still had the condition. No sexual predilection was found. Lower eyelids were commonly involved bilaterally. Most cases of epiblepharon produced no or mild symptoms.

Epiblepharon with inverted eyelashes in Japanese children. II. Surgical repairs.

We used lid bracing sutures, buried sutures, and skin resection operations to correct epiblepharon in Japanese children. Epiblepharon and ocular irritation in 103 children (254 eyelids) were treated first with lid bracing sutures. Postoperatively the inversion of eyelashes resolved in 73 patients (196 eyelids). Of these 103 patients 30 (58 eyelids) developed recurrences, which included 11 who also complained of foreign body sensation and ocular pain. These 11 patients (22 eyelids) underwent a second surgical procedure (buried sutures or skin resection) before they became free of symptoms. No severe complications were noted in any of the 103 patients. We believe that lid bracing sutures correct most cases of mild epiblepharon in children and that buried sutures or skin resection are valuable for more severe conditions.
