Unusual case of metastatic carcinoma of eyelid.

A case of metastatic squamous cell carcinoma of the eyelid from the parotid gland in a 61-year-old woman is presented. Open biopsy of the cervical lymphadenopathy revealed the metastatic lesion of squamous cell carcinoma in the nodes. Histopathological examination of extirpated eyelid tumor also showed secondary squamous cell carcinoma probably from the squamous cell carcinoma in the parotid gland. Including this case presented here, 3 reported cases of the metastatic tumor from the parotid gland are mentioned and discussed.

Fertilization potential in golden hamster eggs consists of recurring hyperpolarizations.

The fertilization potential, or activation potential, has been demonstrated in the eggs of various species, and it has been shown to block polyspermy in echinoderm, echiuran and frog eggs, but no studies have been reported of electrical phenomena occurring when mammalian eggs are fertilized. We report here the fertilization potential of golden hamster eggs in vitro. To correlate the change of potential with the interaction between sperm and egg, only one sperm was attached to each egg. We found that a sperm induces recurring hyperpolarizations, constituting a fertilization potential which differs from that in the eggs of other species.

Precaution on use of hydrochloride salts in pharmaceutical formulation.

Previous reports suggested that the formation of hydrochloride salts does not always enhance solubility due to the common ion effect. The extent of the common ion effect seems to be related to aqueous solubility, with slightly soluble hydrochlorides being more sensitive to the common ion, i.e., chloride ion. The relationship between solubility in water and the extent of the common ion effect was examined, and a high correlation was found, suggesting that hydrochlorides possessing solubilities in water at least of the order observed for papaverine and demeclocycline hydrochlorides (approximately 32 mg/ml at 25 degrees and 42 mg/ml at 37 degrees) are less soluble than the corresponding free base at gastric pH.

Sequential combination chemoimmunotherapy for various malignant tumors: clinical and laboratory results.

A chemoimmunotherapy program designed on the bases of the results of animal experiments was designed for 139 patients with various advanced malignant tumors. The treatment regimen consisted of cyclophosphamide (CY) 200 mg intravenously on day 1, Vibrio cholerae neuraminidase (VCN) treated autologous tumor cells admixed with BCG 5 to 10 mg intradermally on day 4 and mitomycin C (MMC) 10 to 16 mg and 5-fluorouracil (FU) 500 mg intravenously on day 7, of each course. Thereafter, maintenance treatment was started on day 14 with tegafur (FT) 600 mg and immunostimulants, such as OK432, PSK or levamisole. In the group of patients with gastric carcinoma, as compared to historical control patients, this treatment significantly improved survival in those with stage III disease (p less than 0.02), stage IV disease (p less than 0.05) and recurrent or unresectable tumor (p less than 0.001). Immune reactivities as measured by PPD skin test and PHA lymphocyte blastogenesis increased slightly following the therapy. After completion of the treatment, cellular immunity to autologous tumor extract could be detected in vitro by macrophage migration inhibition technique in 12 of 42 patients. The relationship of immune reactivity before or after therapy to patient survival was examined. Analysis of survival curves according to PPD skin test showed significant prolongation of survival in patients with positive reaction as compared to those with negative response following the treatment.

Multiple aneurysms of the vertebrobasilar system associated with fenestration of the vertebral artery.

A rare case of multiple aneurysms of the vertebrobasilar system associated with fenestration of the intracranial vertebral artery is reported. A search of the world literature revealed 56 other cases of the vertebral artery fenestration, mainly in reports from Japan. Only 11 cases of this association with intracranial aneurysms were reported, and none of these aneurysms were located on the posterior circulation. Association of multiple vertebrobasilar aneurysms and vertebral artery fenestration is rare.

False aneurysm with subdural hematoma and symptomatic vasospasm following head injury.

A rare case of cortical false aneurysm in a subdural hematoma and symptomatic vasospasm following head injury without skull fracture is reported. Angiography led to the correct preoperative diagnosis and successful surgical treatment, although the patient's symptoms of acalculia, agraphia, and astereognosis, mainly due to the vasospasm of the proximal portion of the middle cerebral artery, remained unchanged. The mechanism of such a cortical false aneurysm and symptomatic vasospasm is discussed briefly.

Apparent brain atrophy and subdural hematoma following ACTH therapy.

A case of subdural hematoma following ACTH-Z therapy for infantile spasms was presented. A female baby of 5 months old showed little clinical evidence of cerebral dysfunction associated with subdural hematoma. There have been several reports about the relationship between steroid treatment and apparent brain atrophy on the CT brain scans. Then, we studied the CT brain scans before and after ACTH-Z therapy for infantile spasms, atonic seizure or Lennox syndrome and showed some relationship between apparent brain atrophy on the CT brain scans and ACTH-Z treatment. We also discussed the possible etiology of apparent brain atrophy and subdural hematoma, and stressed the necessity of extreme caution with long-term ACTH-Z administration.

The evolution of visual and auditory evoked potentials in infants with perinatal disorder.

The evolutionary changes of evoked potentials (EPs) were studied from the neonatal period up to 1 year of age in 41 infants with various perinatal disorders. Abnormal EPs in the first week of life recovered quickly. In infants with normal outcome, abnormal EPs became normal within a month. In infants with cerebral palsy (CP) or mental retardation (MR), EPs recovered within 2-3 months of age. Infants with more severe neurological damage showed abnormal EPs even beyond 6 months of age. Abnormal EPs beyond 2 weeks of age indicated poor prognosis. As for the wave form of EPs, absent responses or abnormal wave form reflected more severe brain dysfunction. AEPs tended to show more profound abnormalities than VEP. However, some infants with absent AEP in the first week of life had a favorable prognosis. AEPs seemed to be more easily affected by brain dysfunction.

Development of the motor behavior during sleep in newborn infants.

Body movements during sleep were examined in eight non-eventful preterm babies and two full-term babies using the polygraphic methods including superficial EMG. Typical movements in quiet sleep were startle (St) or generalized phasic movement (GPM) and increased tonic activity in chin muscles. In active sleep, instead of total simultaneous patterns observed in quiet sleep, more un-coordinated and localized movements such as generalized movement (GM), localized tonic movement (LTM), localized phasic movement (LPM) and clonic movement (CM) occurred. Longitudinal study of movements revealed a decrease of St or GPM, GM and LPM. LTM did not decrease with increasing conceptional age. The differences of decreasing tendency among these movements may indicate the differences of maturational changes in the different part of CNS.

Prognostic factors of infantile spasms from the etiological viewpoint.

We investigated the prognostic factors for mental and physical development and seizure control by dividing the subjects into various etiologic groups in 200 patients with infantile spasms, all of whom (except 48 who died) were aged six years or older. The results were as follows: 1) Intermediate (4-12 mos) onset was found to be a favorable prognostic factor for seizure control in cryptogenic cases, although there was no relation between the age of onset and prognosis in other etiologic groups. 2) There was a significant correlation between the treatment lag and long-term prognosis for mental and physical development only in cryptogenic cases. A short treatment lag (0-2 mos) was associated with a good prognosis. All cryptogenic patients who had no relapse after ACTH therapy developed normally, although in symptomatic cases, there was no correlation between the relapse and the outcome.

Peripheral neuropathy in Marinesco-Sjögren syndrome.

A case of Marinesco-Sjögren syndrome manifesting cataract, short stature, psychomotor retardation, hypogonadism, and progressive ataxia was described. The result of electrophysiological study reflected the presence of peripheral nerve involvement, which was clearly confirmed by sural nerve biopsy. The conspicuous abnormality observed in the peripheral nerve was the presence of segmental demyelination. The process of axonal degeneration was not remarkable. Results confirmed that Marinesco-Sjögren syndrome is a disorder which may involve both central and peripheral nervous system.

Long-term prognosis after infantile spasms: a statistical study of prognostic factors in 200 cases.

A follow-up study was made on 200 children (115 boys, 85 girls) who had had infantile spasms, in order to compare their present condition over the age of six years with various prognostic factors. 48 of the children (30 males and 18 females) had died, and all the rest were aged six years or older at the time of final follow-up. 139 of the children had received ACTH therapy: at final follow-up, spasms had ceased in 43.5 per cent, and about the same proportion showed normal physical development; 23 per cent had normal mental development and 15.4 per cent were attending ordinary schools. Complete recovery (normal mental and physical development and attending ordinary schools) was achieved in only 19 cases (9.5 per cent). Of the cryptogenic cases, 44.4 per cent had made a full recovery. The poor prognostic factors for continuing seizures were evolution into other types of fits, relapse of seizures after ACTH therapy, seizures concomitant with spasms, and convulsions before the onset of spasms. Poor prognostic factors for physical development were delayed development before the onset of spasms, neurological abnormalities, PEG abnormality, symptomatic aetiology, neonatal convulsions, low birthweight, perinatal asphyxia and being female. Poor prognostic factors for mental development were delayed development before the onset of spasms, neurological abnormalities, PEG abnormality, prenatal and perinatal aetiology, relapse after initial ACTH therapy, laughing attacks, and evolution into other types of fits. Only in the cryptogenic cases was there significant correlation between the delay in treatment and the long-term prognosis for mental development. Poor prognostic factors for educability were very similar to those for mental development. In spite of conflicting views as to the long-term effects of ACTH, prompt treatment seems to be mandatory, at least in cryptogenic cases of infantile spasms.

Brief atonia associated with electroencephalographic paroxysm in an infant with infantile spasms.

The loss of muscular tone or muscular inhibition associated with sharp waves observed during abnormal tonic posture in a patient with infantile spasms of early onset is described. The latency between the beginning of a sharp wave and the muscular inhibition varied from 30 to 700 msec, and the duration of the inhibition ranged from 100 to 400 msec. These brief muscular inhibitions may sometimes precede tonic seizures, and these phenomena might be noticeable only in the state of steady tonic muscular contraction.

A circadian variation in the excretion of urinary kinin, kallikrein and prostaglandin E in normal volunteers.

The present study shows that there is a circadian variation in the excretion of urinary kinin, kallikrein and prostaglandin E (PGE) as well as urine flow, urinary sodium excretion and urinary potassium excretion in recumbent healthy volunteers. The present data, that the acrophase of urinary kallikrein excretion was 1.5 hr later than that of plasma aldosterone concentration (PAC) and that a significant positive correlation was found in circadian variations between urinary kallikrein and PAC, suggest that the circadian variation of urinary kallikrein excretion may be regulated by aldosterone. The pattern of circadian variation of average urinary kinin excretion coincided with those of average urine flow, urinary sodium excretion and urinary PGE excretion, suggesting that renal kinin and PGE may be involved in the regulation of water-sodium excretion. The present data that the phase of circadian variation of urinary kinin was delayed from that of urinary kallikrein and that there was a mirror image between the circadian variation in urinary kinin and kallikrein suggest that there may be a feedback mechanism in renal kallikrein-kinin system.
