Reversible asymmetric septal hypertrophy in acute myocarditis. Serial findings of two-dimensional echocardiogram and thallium-201 scintigram.

Serial two-dimensional echocardiographic and thallium-201 scintigraphic findings are described in a patient with acute myocarditis diagnosed by endomyocardial biopsy. On the 4th day of illness, just before the onset of congestive heart failure, the echocardiogram showed asymmetric septal hypertrophy (IVS/PW = 16 mm/10 mm = 1.6) and thallium-201 scintigram showed the ventricular septal thickening. On the 8th day of illness, when severe congestive heart failure was seen, asymmetric septal hypertrophy disappeared (IVS/PW = 8 mm/8 mm = 1.0), the left ventricle dilated markedly (LVDd = 63 mm), and the wall motion became poor (EF = 0.24). After one month, when congestive heart failure and clinical inflammatory findings disappeared, the contractility somewhat improved (EF = 0.43), although marked left ventricular dilatation remained. Thallium-201 scintigram showed some scattered persistent perfusion defects, thinning of the ventricular septal thickening, and dilatation of the left ventricle. The right ventricular endomyocardial biopsy revealed the histologic findings of the late stage of acute myocarditis. It is concluded that transient thickening of the ventricular wall may represent early changes in acute myocarditis.

Multiinstitutional evaluation of local injection of absolute ethanol as the new hemostatic method for upper G-I tract bleeding.

Local injection of absolute ethanol was proven effective for hemostasis of gastrointestinal bleeding in our fundamental (Asaki et al. 1983a) and clinical (Asaki et al. 1983b) studies. The purpose of the present paper is to report the results obtained in 258 hemorrhagic foci in 152 unselected cases treated with this method at 6 major hospitals in the Tohoku area in Japan. Sixty (39%) of these 152 cases had shock symptoms due to hemorrhage. Gastric and duodenal ulcers were responsible for the hemorrhage in about 90% of the cases in which our method was applied to hemostasis. Eighty-nine (59%) of the 152 cases had some form of complications such as malignant tumors or liver cirrhosis. Hemorrhage due to postoperative stress ulcer after major surgeries including renal implantation or laryngectomy for resection of cancer was seen in 20 cases (13%). The condition of hemorrhage immediately before the treatment with our technique was classified as spouting hemorrhage for 8 foci (3%), pulsating hemorrhage for 22 foci (9%), adhesion of clot for 179 foci (69%), and hemorrhage from veins and capillaries for 49 foci (19%). In all of these cases temporary hemostasis was obtained. Rebleeding occurred in 17 cases (7%) including 5 cases in which hemostasis was successfully obtained by the repeated use of our method. In 4 of the 17 cases, in which repeated endoscopy was infeasible, emergency surgery was performed. In the remaining 8 lesions in 8 cases, new bleeding or rebleeding occurred more than 1 week after the initial hemostasis had been obtained. Six of these 8 cases died from rebleeding due to DIC syndrome. With our method complete hemostasis was obtained in 144 of 152 cases (95%) including 11 cases (7%) which underwent elective surgical operation.

Bovine serum albumin nephritis in rats. V. Kinetic studies of antigen localization in various organs and the phagocytic role of polymorphonuclear leukocytes.

Kinetic studies of antigen distribution to various organs were performed throughout the course of experimental immune complex (IC) glomerulonephritis induced in rats. By using a paired radiolabel technique and histologic observations, the authors found massive amounts of antigen within both the lungs and the livers of these rats before the development of glomerulonephritis. However, the rate of antigen disappearance from the lung exceeded that from the liver. Polymorphonuclear leukocytes (PMNs) handled almost all the antigen administered, presumably in the form of ICs. Electron microscopy yielded little evidence that Kupffer cells or monocytes phagocytosed these ICs. After glomerulonephritis developed, only minimal amounts of antigen were evident in the lungs and livers. These observations indicate that the mononuclear phagocyte system (MPS) does not participate in the processing of ICs as previously believed, but that PMNs dispose of nearly all the ICs formed in vivo in this model of glomerulonephritis.

Impaired production of interleukin-2 after surgery.

The capacity of peripheral blood mononuclear cells (PBM) to produce interleukin-2 (IL-2) was studied serially before and following operation in patients undergoing various surgical procedures. In patients who had major surgery, significant decrease in IL-2 activity was observed 1, 3 and 6 days after operation as compared to that before surgery, although there was no significant change throughout the post-operative course in patients undergoing minor surgery. IL-2 activity returned to the pre-operative level by the 8th post-operative day. However, it remained significantly depressed 8 days after surgery in patients who had undergone major surgical procedures of more increasing severity. Distribution of T cell subsets, especially OKT4 positive cells, did not differ significantly from the pre-operative value throughout the post-operative course. However, the depressed production of IL-2 3 days after surgery could be abolished when adherent cells were removed from PBM by plastic adherence procedures. These results indicated that adherent cells, but not quantitative change in T cell subsets, might be responsible for the depression of IL-2 production after surgery.

Bovine serum albumin (BSA) nephritis in rats. III. Antigen distribution in various organs.

We established an experimental model of immune complex glomerulonephritis (ICGN) in rats by daily i.v. administration of 2.0 mg of bovine serum albumin (BSA) for 4 weeks (Yamamoto et al., 1978). In the present study, antigen distribution among various organs was evaluated in the whole course of the experimental rat model using the paired radiolabel technique and the direct immunofluorescent method. In the late stage of chronic serum sickness (CSS), antigen distribution to the liver decreased and that to the glomeruli increased remarkedly whereas hepatic distribution of the antigen was the highest among the organs in the early stage of CSS. BSA distribution in the glomeruli correlated positively with the distribution in blood and there was an inverse relationship between glomerular distribution of BSA and hepatic distribution throughout the course of CSS. These observations may show direct evidence that events in the liver influence the development of ICGN.

Preoperative management of pheochromocytoma with the calcium-antagonist nifedipine.

Labile high blood pressure and associated complaints (eg, severe headache, palpitation, and vague discomfort in the chest) in a 28-year-old woman with pheochromocytoma were stabilized by adding nifedipine to the conventional regimen of alpha- and beta-blocking agents. Electrocardiographic (ECG) data (ST depressions, prolonged QT intervals, and giant negative T waves during a hypertensive attack) and findings in biopsied myocardial specimens (slight cell infiltration composed mainly of lymphocytes associated with interstitial fibrosis) had suggested the presence of catecholamine cardiomyopathy. Oral administration of 10 mg of nifedipine alone had rapidly resulted in normalization of blood pressure and complete relief from associated signs and symptoms. Because conventional preoperative treatment with alpha- and beta-blockers did not alleviate the hypertensive attacks, a 20-mg long-acting nifedipine tablet was added to the regimen. The effect of twice-daily administration of a 20-mg long-acting nifedipine tablet (combined with alpha- and beta-blockers) was so prominent that it was possible for the patient to undergo surgery for removal of the right adrenal gland and a 4-cm tumor at the gland. After surgery there were no abnormal ECG findings.
