Risk factors of cerebro-cardiovascular disorders in mild diabetes.

The previous history of maturity onset diabetics is often difficult to piece together since most of the patients, being asymptomatic in the early stages, are not predisposed to undergo routine examinations. We made a prognostic study of such asymptomatic mild diabetics in order to examine the relationship between diabetes and cerebro-cardiovascular disorders. The subjects consisted of 5,170 individuals who had undergone biennial routine examinations at the Nagasaki RERF since 1958. Of these, 424 persons were diagnosed during the follow-up period as having diabetes. Eliminating those receiving insulin or oral hypoglycemic agents, there were 392 patients. It is shown that 49% of these mild diabetics died from cerebro-cardiovascular disorders. The mortality rate for mild asymptomatic diabetics due to CVD and CHD was significantly higher than that for non-diabetics. To elucidate the risk factors of these disorders, we studied the effects of age, blood pressure (BP), serum total cholesterol (TC), relative body weight and four other factors. In CVD as well as in CHD, age and systolic blood pressure were the major risk factors of statistical significance. It was also noted that diabetes, however mild, was an important factor. Consequently, Bp is felt to be the greatest of the four risk factors above, and the maintenance of normal BP as well as glucose control is important even in mild asymptomatic diabetics.

Combination chemotherapy in retroperitoneal poorly differentiated myxoid liposarcoma: a report of a case.

We describe a case of a 47-year-old man with retroperitoneal poorly differentiated myxoid liposarcoma. The retroperitoneal invasion of the sarcoma caused hydroureter-nephrosis, rectal stenosis and also exaggerated steroid induced diabetes mellitus. A remarkable regression was achieved by systemic combination of chemotherapy with prednisolone, cyclophosphamide, vincristine sulfate and actinomycin D. This combination chemotherapy is worth trying if a patient has inoperable liposarcoma. It can also be used to reduce local recurrence rate after surgical or radiation therapy, or both, against liposarcoma.

Partial purification and characterization of glycylprolyl dipeptidyl aminopeptidase in porcine pancreas.

This study reports the presence of glycylprolyl dipeptidyl aminopeptidase in porcine pancreas, and its partial purification and some properties. Crude enzyme preparation was obtained by extraction from acetone-dried powder of the pancreas at pH 7.6. For solubilization of enzyme, freezing and thawing were carried out. Crude enzyme extract was fractionated with ammonium sulfate precipitation, gel filtration on Sephadex G-200 column and ion-exchange chromatography on DEAE-cellulose. Partially purified enzyme showed 2897-folds purification. The enzyme activity on polyacrylamide gel electrophoresis showed good agreement with a main protein band stained with Coomassie brilliant blue. Molecular weight of this enzyme from the pancreas was estimated to be 300000 by gel filtration on Sephacryl S-300 column. Optimum pH was between 8.5 and 9.0, and Km value for glycylproline-p-nitroanilide tosilate was 0.33 mM. This enzyme from the pancreas was a serine enzyme and was relatively stable to heat at 60 degrees C for 10 min.

Herpes simplex encephalitis treated with adenine arabinoside.

We report a case of a 5-year-old girl with herpes simplex encephalitis (HSE) who was treated with adenine arabinoside (ara-A). The characteristic symptoms consisted of headache and vomiting followed by progressive disturbance of consciousness. CT scan revealed a translucent area in the left temporal lobe. Seven days after the onset vigorous treatment including ara-A was initiated. She recovered without apparent toxicity or sequelae except for mild motor aphasia. Our experience suggests that ara-A is effective in the treatment of HSE if given early enough.
