[The prognostic significance of nuclear DNA content in non-small-cell lung carcinoma].

The nuclear DNA content of paraffin-embedded specimens of primary non-small-cell lung carcinomas was analysed using flow cytometry in 210 patients (80 squamous cell carcinomas, 99 adenocarcinomas, 19 large cell carcinomas and 15 others). The relationship between nuclear DNA content and prognostic factors was studied using multivariate analysis with Cox's proportional hazard model. 1) The frequency of DNA aneuploidy was 77.3% among 210 patients, and it significantly (p less than 0.05) increased with advanced stage and the presence of lymph node metastasis. 2) The patients with DNA aneuploid tumors had a significantly (p less than 0.001) less favorable prognosis than those with DNA diploid tumors among 179 patients with non-small-cell lung carcinomas. Similar results were demonstrated in 79 patients with stage I carcinomas and in 85 patients who underwent absolute curative resection. 3) Multivariate analysis using Cox's proportional hazard model showed that DNA ploidy was an independent prognostic factor for survival. Especially in the patients with absolute curative resection, DNA ploidy was the most important prognostic factor. In conclusion, flow cytometric nuclear DNA content analysis provided useful biological information, and DNA ploidy was an important and major independent prognostic factor in non-small-cell lung carcinoma.

[Two cases of pulmonary leiomyosarcoma].

Pulmonary sarcoma is a rare tumor of the lung. There are 81 cases in the literature for review, including our two cases. Our first case who was a thirty-seven year old male who was admitted to our hospital with complaints of left back pain. His chest X-ray showed an 11 X 14 cm tumor shadow in the lower left lung area. A left upper lobectomy was performed, and the histological diagnosis was leiomyosarcoma of the pulmonary vein. He died due to mediastinal recurrence 29 months later. Our other case was a fifty-six year old female. Her chest X-ray showed an abnormal shadow that was a tumor shadow in the right central lung area of 4 cm in diameter. As the diagnosis of the transcutaneous lung biopsy was benign tumor, partial resection of lung was performed. The histological diagnosis was leiomyosarcoma of the bronchus, and she is alive and well at 29 months after the operation. As the rate of diagnosis in leiomyosarcoma detected by transcutaneous lung biopsy is low, the most acceptable method of diagnosis is a total examination, including cytology of sputum, bronchoscopic findings, and roentgenoscopy. Probe thoracotomy would also be necessary if malignancy could not ruled out. The treatment of lobectomy should include lymph node resection.

[Relationship between the nuclear DNA content and the prognosis in squamous cell carcinoma of the lung].

Analysis of the nuclear DNA content from paraffin-embedded specimens using flow cytometry was performed on 86 patients in squamous cell carcinoma of the lung. There were no relationships between the site of tumor origin and the nuclear DNA content or the survival of the patients. The frequency of DNA aneuploidy was 74.4% among tumors, and it significantly (p less than 0.05) increased with advanced stage. The patients with DNA aneuploidy were significantly (p less than 0.01) less favorable prognosed than those with DNA diploidy. Similar results were demonstrated in patients of absolute or relative curative resections. These results indicate that DNA aneuploidy is of higher grade malignant intensity than DNA diploidy in squamous cell carcinoma of the lung. In patients with absolute or relative curative resection, DNA aneuploid tumors occurred much more often in distant metastasis. In conclusion, there were no relationships between the site of tumor origin and the biological features or the prognosis of patients in squamous carcinoma of the lung. DNA ploidy pattern is related to relapse and prognosis of the tumors. If the DNA ploidy pattern of patients on whom absolute curative resection was performed is DNA aneuploidy, adjuvant therapy should be strongly performed and carefully followed up as long as possible.

[Tracheal T-tube treatment following tracheoplasty--post-intubation tracheal obstruction and stenosis].

A 26-year-old man with post-intubation tracheal obstruction and stenosis was treated at our department. The lesions were granulomas, showing almost complete obstruction of the tracheal lumen at the stomal level, circumferential cuff stenosis at the cuff level and pressure necrosis on the trachea directly behind the brachiocephalic artery. Circumferential resection of the trachea including 13 tracheal cartilages with end to end tracheal anastomosis was performed. The site of the anastomosis was covered with pedicled omentum retrosternally. Seven days following the tracheal resection, a granulomatous lesion resulting in dyspnea was found at the distal trachea of the anastomosis. Subsequently, a tracheal T-tube was inserted, he had a remarkable improvement of the patient's overall condition. Pedicled omental wrapping of tracheal anastomotic site was found quite useful, particularly in the case of possible anastomotic insufficiency. Treatment of post-intubation stenosis with tracheal T-tube proved effective when the tracheal resection was not possible.

Allergen control. An indispensable treatment for allergic contact dermatitis.

Patch testing is performed to find out the causative allergens for patients, and once they are found, to indicate the exclusive usage of daily necessities that do not contain the causative allergens is necessary, such as when the contact dermatitis is recurrent, persistent, or complicated by secondary hyperpigmentation. Such a procedure has been called "allergen control." Sufficient success has been obtained in the categories of soaps, shampoos, cosmetics, underwear, and some dental metals in Japan with cooperation of industries, and summaries of these investigations are reported.

Peptidergic innervation of the temporomandibular disk in the rat.

The peptidergic innervation of the temporomandibular disk was investigated in the postnatal young rat by using an indirect immunofluorescence method. Calcitonin gene-related peptide-containing nerve fibers were located around the blood vessels and terminated as free nerve endings in the disk. These nerve fibers may be of a sensory nature.

Intrahepatic biliary cysts after hepatic portoenterostomy in four children with biliary atresia.

We report our experience with 4 cases of cystic dilatation of intrahepatic bile ducts following hepatic portoenterostomy for biliary atresia. Two of the cases did not achieve satisfactory bile excretion and all four cases developed recurrent cholangitis after hepatic portoenterostomy. The attacks of cholangitis seemed to be associated with the presence of intrahepatic cysts. Although one case resulted in death from hepatic failure, three other cases are now outpatients. Patients who develop recurrent cholangitis following hepatic portoenterostomy, should be examined to exclude the presence of intrahepatic biliary cysts. Ultrasonography, computed tomography and percutaneous transhepatic cholangiography were all effective in detecting cysts and provided valuable information for planning treatment. Percutaneous transhepatic or surgical drainage of the bile ducts was effective in reducing jaundice, and recurrent cholangitis.

Localization of two distinct acid phosphatases in secretory ameloblasts of rat molar tooth germs.

Acid phosphatases were examined histochemically at the light- and electron-microscopic levels using para-nitrophenyl phosphate (pNPP) and beta-glycerophosphate (beta-GP) as substrates. By light microscopy, there was intense activity with pNPP in supranuclear and distal regions of the secretory ameloblast, and moderate or slight activity respectively in those regions with beta-GP. These enzyme activities were less at the late secretory stage of amelogenesis and disappeared at the transitional stage. By electron microscopy, acid phosphatase activity was seen in the trans side cisternae of the Golgi apparatus, in lysosome-like granules, and in small vesicles in the Tomes' processes. The activity with pNPP but not beta-GP was also localized at the plasma membrane (proximal, lateral and distal surface). Activity with beta-GP was completely inhibited by 1 mM sodium tartrate and by 1 mM NaF; activity with pNPP was inhibited by 1 mM NaF and 10 mM sodium tartrate, but not by 1 mM sodium tartrate. Thus there are at least two different acid phosphatases, one tartrate-sensitive and the other 1 mM tartrate-resistant, in the secretory ameloblast; the tartrate-resistant enzyme is plasma-membrane bound.

Experimental glomerulonephritis induced in rats by a lectin and its antibodies.

The aim of the present study was to investigate the events following interaction of antibody with an antigen planted on the surface of glomerular endothelial cell (GEN). A lectin, Helix pomatia agglutinin (HPA), was planted at the surface of rat GEN by the perfusion of the isolated left kidney with neuraminidase (NRD) and HPA. A subsequent perfusion with IgG fraction, but not with Fab fragments, of rabbit anti-HPA serum induced formation of granular immune deposits (IDs) on the surface of GEN. When the left kidney was revascularized after initial formation of IDs, acute glomerulonephritis ensued. Fifteen minutes after revascularization, granular IDs were observed at the subendothelial space. Two days later, there was decrease of subendothelial IDs with concomitant appearance of subepithelial IDs. At the seventh day, IDs were mainly localized in the subepithelial space. The results suggest that this model of experimental glomerulonephritis is characterized by: (1) initial formation of HPA immune complexes (ICs) at the surface of GEN; and by (2) subsequent movement of ICs from luminal side to subepithelial area with local formation of IDs. The results are relevant to the understanding of the kinetics of ICs resulting from interaction of antibodies with exogenous antigens "planted" in the glomerular capillary walls, and of the local formation of IDs.

DR antigen expression on vascular endothelium and duct epithelium in fresh or cultured human fetal pancreata in the presence of gamma-interferon.

The cells expressing MHC class II antigens play an important role in allograft rejection. We examined the expression of HLA DR antigens in human fetal pancreata ranging in gestational ages from 8 to 24 weeks. DR antigens were detected by immunohistochemical techniques using H4 and 40D MoAbs with immunoperoxidase staining and the ABC procedure. Vascular endothelium was identified by goat antibodies directed to human factor VIII-related antigens. DR expression on endothelium was further confirmed by double staining of tissue sections with H4 and anti-FVIII antibodies. In pancreata younger than 18 weeks of gestation, DR antigens were found on single cells that were randomly distributed throughout the pancreatic parenchyma. These cells resembled dendritic cells in morphology, as reported previously. Some vascular endothelial cells located in the intralobular connective tissues expressed DR antigens, but those in the pancreatic parenchyma were DR-negative. In 18-22 weeks, some endothelia of small vessels in the parenchyma became DR-positive. By 24 weeks, DR antigens were also expressed on medium-sized blood vessels, whereas intraislet capillaries and duct epithelia were DR-negative. The number of DR-positive cells increased rapidly from 11.2 cells/field (x400) in 12-14 week pancreata to 42.8 cells/field in 18-22 week pancreata. Most DR-positive cells in pancreata earlier than 18 weeks were dendritic-like cells, while those in older pancreata were endothelial cells. When pancreatic tissue fragments were cultured for two days in the presence of rIFN-gamma, vascular endothelium and duct epithelium, both of which were otherwise DR-negative, had become DR-positive. Even with this increase, DR-positive cell numbers were fewer in pancreata younger than 18 weeks of gestation as compared with those in older pancreata. We also detected clusters of epithelial-like cells that were clearly stained for DR antigens. These cells were located in the parenchyma where insulin positive cells were generally found. Their DR-antigens had not changed during the 3-day rIFN-gamma culture.

Effect of amniotic fluid volume on umbilical cord length.

To study effect of amniotic fluid volume on umbilical cord length, 939 pregnant women at 24-41 weeks of gestation, including 10 twin cases, 51 preterm premature rupture of the membranes (PROM) and 70 premature delivery cases, were examined for their amniotic fluid volume within a week of delivery by the ultrasound procedure and these 949 newborns were also examined for umbilical cord length and incidence of fetal distress. In the polyhydramnios group (pocket size of more than 8 cm), 8 of 20 infants (40%) had longer cords and 6 infants (30%) had shorter cords, while only one of 49 infants (2%) with oligohydramnios (pocket size of less than 2 cm) had a longer cord and 36 infants (73.5%) had shorter cords than the mean cord length (+/- SD) of the normal group. The 20 twins had significantly shorter cords than those of the normal group (P less than 0.05) and the polyhydramnios group (P less than 0.01). The PROM group showed significantly higher incidence of oligohydramnios (P less than 0.05) and fetal distress (P less than 0.01), which suggests that intrauterine space availability is important for assuring fetal well-being.

Cytochemical demonstration of lysosomes in the chicken pineal gland: changes induced by light-dark cycle.

Light and electron microscopic demonstrations for acid phosphatase (AcPase) activity in the chicken pineal gland were studied with special reference to the changes induced by the light-dark cycle. AcPase-positive lysosomes and Golgi cisternae and vesicles located in the pinealocyte in the light period are well developed and more numerous than in the dark period. In a few cases, a type of lysosome wrapping mechanism is present in the pinealocyte during the light period. Therefore, the lysosomes in the chicken pinealocyte appear active during the light period. In addition, the lysosomes may have functional relationships with the lipid-like inclusions seen in the dark period.

Histochemical localization of calcium in the enamel organ of rat incisors in early-stage amelogenesis.

The localization of calcium in the enamel organ of rapidly-frozen, freeze-substituted rat incisors in early-stage amelogenesis was examined by a histochemical calcium-staining method. In secretory ameloblasts, glyoxal bis(2-hydroxyanil) (GBHA) staining revealed intense red reactions in mitochondria and tubulovesicular structures located throughout the cytoplasm, while no reaction was seen in the nucleus and cytosol, nor along the plasma membranes of the respective cells. No significant GBHA reaction was observed in the intercellular compartment and other cells of the enamel organ. Some granular reactions were localized in the cells of the adjacent connective tissue. Control tests confirmed the specificity of GBHA reactions for calcium. Thus, the present observations provide histochemical evidence indicating an exclusive localization of calcium in mitochondria and tubulovesicular structures of the secretory ameloblast, and support their contributions to the translocation of calcium from the proximal to the distal pole of the cytoplasm.

A case of nephrotic syndrome with glomerular lipoprotein deposition with capillary ballooning and mesangiolysis.

A 33-year-old man with nephrotic syndrome and rapid deterioration of renal function showed curious glomerular morphological abnormalities. Ballooning of the glomerular capillaries due to a substance accumulated in the capillary lumina and mesangiolysis were prominent histological features. The deposits in the capillary lumen were positive for Sudan III staining, and also for beta-lipoprotein, apoprotein B and apoprotein E by immunofluorescent technique. The staining of beta-lipoprotein in a flower leaf pattern was a striking characteristic, while such staining was negative when studied in 20 patients with nephrotic syndrome who were used as controls. Based on these findings, the morphological abnormalities in this case were considered to be related to lipoprotein deposition in the glomeruli. This case is thought to be the first reported in a complete form in the literature which could be classified as a new kind of disease related to lipoprotein metabolism abnormalities.
