Significance of exercise and bed rest in pregnancy--pregnancy and walking (1).

Prior to evaluating the significance of bed rest in managing obstetric diseases, characteristics of physiology of pregnant women were examined on the basis of 24 hours heart rate changes in eight pregnant women at the third trimester and also distances covered in walking by a total of 53 multiparas and of 87 primiparas during free-living daily activities were measured by means of a pedometer. A significantly higher mean heart rate was observed in those pregnant women even during sleep than in non-pregnant control. The primiparas showed significant decreases in the pedometer readings in all stages except for 15-27 weeks of gestation, while the multiparas of less than 28 weeks exhibited no decrease as compared with non-pregnant control. The decrease in the pedometer readings were marked in both primiparas and multiparas, the value being much lower than that of any other week of gestation. However, the equation of weight x distance covered in walking remains rather constant throughout gestation.

[Estrogen dependent plasminogen activator in breast cancer cells; experimental and clinical studies].

Hormonal regulation of plasminogen activator (PA) in rat mammary tumor induced by 7,12-dimethylbenz (a) anthracene (DMBA) was studied both in vivo and in vitro. PA activity in DMBA-tumor was markedly decreased by ovariectomy, and recovered in a dose-dependent fashion upon estradiol administration. This estrogen-stimulated production of the enzyme was prevented by actinomycin D, cycloheximide and tamoxifen. Furthermore DMBA-tumor cells in primary culture displayed similar estrogen-dependency toward the production of the enzyme without any cell proliferation. This indicates that estrogen might regulate de novo synthesis of PA at a transcriptional level via an estrogen receptor system, and that this hormone might support the growth of DMBA-tumor into adjacent tissues by inducing PA in a direct manner via a route distinct from a prolactin pathway. To examine whether PA reflects the functional state of estrogen receptors in human breast cancer, the enzyme activities were determined in extracts prepared from 160 breast cancer specimens and compared on qualitative and quantitative bases with the levels of steroid receptors. The results strongly suggest that PA can be used as an effective functional marker for hormone dependence in human breast cancer.

Heparan sulfate proteoglycan of murine glomerular basement membrane: relationship between localization of the core protein and localization of fixed negative charges.

Antibodies to a 250,000 molecular weight heparan sulfate proteoglycan (HSPG) core protein purified from PYS-2 cells were prepared in the rabbit. After absorption with laminin, the anti-HSPG serum (AHSPGS) was specific for HSPG by enzyme-linked immunoabsorbent assay (ELISA). Immunoelectron microscopy showed that AHSPGS reacted with the mouse glomerular basement membrane (GBM) or, more precisely, with three HSPG rich layers, i.e., (i) the lamina rara interna in a discrete or interrupted-linear pattern, (ii) the interface between the GBM and the soles of the foot processes, and (iii) the lamina rara externa in a finely discrete pattern. The localization of HSPG in the GBM was very similar to that of the fixed negative charges detected with polyethyleneimine (PEI). When mice were treated with heparitinase, binding of PEI was seen only on the cell surface including the foot process soles (ii) but not in the GBM (i and iii), while the localization of HSPG core protein was unaltered. These results suggest that the localization of the HSPG core protein was correlated with that of the fixed negative charges in the GBM (i and iii). The foot process base (ii) seems to be a complicated zone since the PEI binding in this area was less affected by heparitinase treatment, while this area has been reported to have a lower sialic acid content than the foot process surface facing the urinary space.

Glomerular injury induced by antibodies against heparan sulfate proteoglycan (HSPG) derived from PYS 2 cells.

Kidneys of Balb/c mice intravenously injected with rabbit antibodies to heparan sulfate proteoglycan (HSPG) derived from PYS-2 cells were studied for 14 weeks. Antibodies were found to bind to the glomerular basement membrane (GBM) as early as 15 min after the injection. Binding of antibodies was observed over the whole thickness of the GBM. The lamina rara interna and foot process base appeared to be more intensely stained. This pattern did not change throughout the experiments. Mild inflammatory changes (infiltration of polymorphonuclear cells and swelling of the glomerular endothelium) were seen at the initial stage (approximately 1 day), and mesangial expansion followed (1-4 weeks). In mice pre- and booster-immunized with normal rabbit IgG, a moderate autologous response was observed by immunofluorescence microscopy, but no significant inflammatory changes were noted. At the late stage (6 weeks approximately), irregular GBM thickening was observed. The thickening was due mainly to expansion of the lamina rara externa. These findings suggest that the anti-HSPG antibodies caused mild glomerulonephritis at the initial stage, and later caused thickening of the GBM possibly by disturbing the assembly, production and degradation of GBM components.

[Clinical application of the omentum in chest surgery].

The benefit of the omentum in chest surgery was experimentally evaluated on the basis of findings in relation to recanalization of the bronchial artery in bronchoplasty with and without omental wrapping. The arterial blood supply at the anastomotic sites was observed in the early stage following bronchoplasty using the omental wrapping. Clinical application of the omentum in chest surgery was performed by wrapping anastomotic site at tracheobronchoplasty in 3 cases, bronchial stump coverage after lobectomy or pneumonectomy in 2 cases, repair of bronchial fistula following pneumonectomy in 1 case and repair of bronchopleurocutaneous fistula in pulmonary tuberculosis in 1 case. Six of these clinical cases could have favorable postoperative results. Based on these experimental and clinical results, we consider that the omentum is very useful tissue for wound healing in chest surgery.

[Anti-glomerular basement membrane antibody-mediated glomerulonephritis remarkably improved by pulse therapy with methylprednisolone, plasmapheresis and continuous heparin infusion].

The patient, a 28 year-old-man, was admitted to a hospital because of general fatigue and fever. He was pointed out renal dysfunction and was transferred to Nagasaki University Hospital. The laboratory data on admission showed moderate azotemia (BUN 43 mg/dl, Cr 5.4 mg/dl). A percutaneous renal biopsy on admission revealed a diffuse crescentic glomerulonephritis. A direct immunofluorescence of renal biopsy showed a linear pattern for IgG along the glomerular basement membrane. Radioimmunoassay of his serum for circulating anti-GBM antibody was strongly positive. Aggressive treatment with pulse therapy (methylprednisolone), plasmapheresis, and continuous heparin infusion was performed. He had markedly recovered from renal failure and escaped hemodialysis. The patient is making satisfactory process after 3 years.

[Measuring periodontal pockets with contrast media. 1. Preparation of contrast media].

Periodontal pocket is main clinical symptom of periodontitis. Measurement of periodontal pocket depth is the most important examination. The purpose of this study was to get contrast medium radiography of periodontal pocket. The present study was compared with contrast media of varying concentration. Contrast medium-sample containing 40% Sodium Iothalamate and 30% Meglumine Adipiodone had slight higher radiopacity than the others. These contrast media-sample were adequate to clinical practice over 40% concentration. Using Sodium Iothalamate and Meglumine Adipiodone, radiopacity was able to differ from radiopacity of teeth at 60%-100%.

[A case of sick sinus syndrome, considered to develop to atrial standstill].

A 25 year old man was admitted to our hospital because of dizziness and bradycardia. Physical examination was normal except for an irregular pulse of 90 beats/min. Chest X-ray film showed no cardiomegaly and no pulmonary congestive changes. ECG showed 2:1 or 3:1 atrial flutter on admission. After atrial flutter was terminated spontaneously, ECG revealed absent P waves, a QRS interval of 0.10 seconds, and A-V junctional rhythm with irregular R-R interval ranging from 1.20 to 2.12 seconds. At times, cardiac arrest was noted. Esophageal electrocardiogram also failed to demonstrate atrial activity while A-V junctional rhythm continued. Mitral valve echocardiogram lacked A point, and then ä waves were absent in both the right atrial and pulmonary capillary pressure recordings. Transient atrial standstill was suspected from these findings, so that electrophysiological study was performed. Right atrial electrogram revealed complete absence of atrial activity. His bundle electrogram revealed H-V prolongation [80-83 msec]. Right atrial pacing was attempted at several atrial sites, including the high lateral, middle lateral, low lateral right atrium, and low right atrial septum. Atrial activity could be elicited with stimulus strength of 3 to 5 volts. These atrial pacing thresholds were mild or moderately higher than usual. After the cessation of atrial pacing at 90 beats for two minutes, the recovery time of the first beat prolonged to 8.46 second. We considered that this case was sick sinus syndrome in young adult which revealed generalized disturbance of conduction system including the atrial muscle, and would develop to atrial standstill in the near future.

The effect of alpha-interferon on the liver in a patient with hairy cell leukemia: light and electron microscopic studies.

A daily dose of 3 x 10(6) or 6 x 10(6) units of alpha-interferon was given during two 4- to 6-month periods to a 65-yr-old male patient with hairy cell leukemia, reducing splenomegaly and decreasing the number of hairy cells. Liver biopsy specimens taken during treatment revealed predominantly decreased hairy cell infiltration in the dilated sinusoids and enlarged or vacuolar nuclei of hepatocytes, compared with those in the liver before treatment. The ultrastructure of hepatocytes in specimens taken during treatment showed cytoplasmic vacuoles, weakly stained glycogen particles, and conspicuously decreased endoplasmic reticulum. Liver tests revealed decreased serum cholinesterase and total cholesterol levels in the early stage of treatment, low levels of total protein and albumin during treatment, and a very low value in the [13C]aminopyrine breath test. No clinical reports have been made on the decreased microsomal function during treatment with interferon. alpha-Interferon damaged the endoplasmic reticulum of hepatocytes, although it was effective for the reduction of hairy cells in the liver of hairy cell leukemia.

[Tuberculosis in patients undergoing hemodialysis].

The incidence of tuberculosis is very high in patients treated by hemodialysis particularly in the early stage of hemodialysis. The diagnosis of tuberculosis in dialysis patients was obscured as symptoms were nonspecific and extrapulmonary involvement was seen frequently. We investigated cell mediated immunity in dialysis patients in relation to the period of dialysis. The data indicate that dialysis patients show the following immunological impairments; 1) lymphopenia, 2) decreased B cell, 3) alteration of T cell subset, 4) decreased reaction of PPD skin test, 5) decreased T cell activity, 6) decreased IL-2 production, 7) decreased PHA induced lymphocyte blastogenesis, 8) decreased NK cell. Decreased immunologic host defence mentioned above may contribute to the high incidence of tuberculosis in the early stage as well as in the maintenance phase of dialysis.

Phasic coronary artery flow velocity determined by Doppler flowmeter catheter in aortic stenosis and aortic regurgitation.

Phasic coronary artery flow velocity was recorded in 14 patients with aortic regurgitation (AR), 4 with aortic stenosis, 61 with other heart diseases and in 2 normal subjects by means of a bidirectional Doppler flowmeter catheter. The normal pattern of the phasic coronary artery flow velocity was characterized by a small forward flow during systole (S wave) and a large forward flow during diastole (D wave). The phasic coronary artery flow velocity in patients with AR showed increased S wave and decreased D wave. The area under the S-wave curve divided by the area under the D-wave curve (S/D ratio) in patients with AR increased (left coronary artery flow velocity 0.66 +/- 0.39, p less than 0.05; right coronary flow velocity 0.79 +/- 0.36, p less than 0.01) as compared with the S/D ratio in patients with other heart diseases (left coronary flow velocity 0.32 +/- 0.12; right coronary artery flow velocity 0.38 +/- 0.17). There was a tendency toward a relative positive correlation between S/D ratio values and AR cineangiographic grades. Decreased S/D ratios were observed in 4 patients with aortic stenosis. It is believed that no reports exist on phasic coronary flow velocity recorded in conscious patients who had aortic valve disease.
