[Esophageal cancer metastatic to the kidney: report of a case].

On December 12, 1988, 51-years-old man was admitted to our clinic with asymptomatic gross hematuria. He had been undergone total esophagectomy for esophageal cancer in the surgery service of our hospital on June 22, 1987, and histopathological diagnosis was squamous cell carcinoma, Stage III, a2, n(-), m0, pl0, ly(+), V(+). X-ray CT revealed a irregular, obscure bordered tumor image in the left kidney and angiography showed a hypovascular tumor. On December 20, nephrectomy was performed. Histologically, it proved to be squamous cell carcinoma metastasized from esophageal cancer. The present case is the 11th of those cases which have been clinically diagnosed as renal metastasis of esophageal cancer in Japan. An extremely small number of clinically reported cases compared with autopsy cases are thought to be ascribable to scarce detectability while living of such metastasis, but not to its rare occurrence. The coming diagnostic improvement may well make such case more and more increasing.

Influence of colchicine on the addition of a sugar to the enamel protein in secretory ameloblasts of cultured germs of rat molar tooth by 3H-galactose radioautography.

The influence of colchicine on the addition of 3H-galactose to the enamel protein in secretory ameloblasts of cultured germs of rat molar tooth was investigated by light- and electron-microscopic radioautography. In tooth germs cultured without colchicine, the reaction products of 3H-galactose were observed over Golgi cisternae at early chase times and then localized over the enamel with time. In tooth germs cultured with colchicine, the silver grains were seen over the Golgi cisternae, condensing granules and accumulated secretory granules. Some grains also appeared with time over the pale granular material precipitated in the intercellular space with colchicine treatment. In quantitative analysis with light microscopic radioautography, values of silver grain counts over the unit area (100 microns2) on ameloblasts and enamel of colchicine-treated tooth germs were significantly lower at both 0 min and 30 min chase after 30 min pulse than those of control tooth germs, respectively. This finding indicates that colchicine diminished the incorporation of 3H-galactose into the secretory ameloblast of cultured tooth germs. It is suggested that colchicine decreases the activity of the Golgi apparatus with regard to the addition of sugar to the synthesizing glycoprotein in the secretory ameloblast.

Triiodothyronine level and triiodothyronine/thyroxine ratio in HBeAg-positive chronic hepatitis patients treated with prednisolone withdrawal.

Twenty patients with hepatitis B e antigen (HBeAg) -positive chronic hepatitis, who received 40 mg of prednisolone per day for three weeks followed by withdrawal, were studied for changes in alanine aminotransferase (ALT), triiodothyronine (T3), thyroxine (T4), and hepatitis B virus DNA polymerase (HBV-DNAp) levels determined before and during prednisolone treatment and after its withdrawal. A decreased HBV-DNAp level of less than 100 cpm/ml three to five weeks after withdrawal was considered a sign of efficacy and was shown in 10 patients (50%). Significant differences were found between ALT levels, between T3 levels, and between the T3/T4 ratios assayed in the third and fourth weeks in total (P less than 0.02) and in the group in which efficacy was demonstrated (P less than 0.01). The T3/T4 ratio in the third week in the effectively treated group was significantly less than that in the noneffectively treated group (P less than 0.05). Prednisolone withdrawal effective for HBV-DNAp was shown in the patients with a decreased T3 level and the T3/T4 ratio at the third week and an increase in the ATL level after the withdrawal. The ALT level increased after the T3 level decreased. Changes in the T3 level or the T3/T4 ratio represent a marker for effectiveness of prednisolone withdrawal and for determination of combination therapy after steroid withdrawal.

Leucine-enkephalin-, neurokinin A- and cholecystokinin-like immunoreactivities in the guinea pig tongue.

The occurrence of these neuropeptides was examined by immunofluorescence. Leu-Enk-like immunoreactivity was seen in nerve fibres associated with the epithelium, blood vessels and lingual salivary glands as well as in ganglionic cells within the tongue. Neuropeptide A-like immunoreactivity was found in nerve fibres associated with the epithelium, taste buds, blood vessels and lingual salivary glands. Cholecystokinin-like immunoreactivity was found in some nerve fibres around blood vessels as well as in ganglionic cells. The coexistence of these neuropeptides and substance P was also demonstrated in some nerve fibres and ganglionic cells within the tongue. Like substance P, these neuropeptides could be involved in blood flow regulation, salivation and as trophic factors for taste buds.

Fine structure of calcitonin gene-related peptide-immunoreactive nerve fibres in the rat temporomandibular joint.

The distribution and fine structure of these nerve fibres was examined by immunoelectron microscopy. CGRP-immunoreactive fibres were seen in the nerve bundles, blood vessels and periosteum around the condyle as well as in the disc. These nerve fibres were unmyelinated and had diameters varying from 200 to 600 nm. They were completely or partially enclosed by Schwann cell cytoplasm and did not form synaptic contact with any cells. CGRP-immunoreactive nerve fibres may be sensory nature and this peptide could be involved in pain transmission and neurogenic inflammation.

The distribution of vasoactive intestinal polypeptides and calcitonin gene-related peptide in the periodontal ligament of mouse molar teeth.

The distribution of VIP- and CGRP-containing nerve fibres was examined by indirect immunofluorescence. There were many such fibres in the lower third of the ligament, some around the blood vessel close to the socket wall. In the middle third of the ligament, some CGRP-containing fibers entered from the lateral wall of the socket; this type of fibre was more numerous in the lower third than in middle third. There were some VIP-containing fibres but no CGRP-containing fibres in the ligament surrounding the furcation of the molar roots.

The effects of a new immunosuppressive agent, FK506, on the glomerular injury in rats with accelerated nephrotoxic serum glomerulonephritis.

The aim of the present work was to study the effects of a new immunosuppressive drug, FK506, in accelerated nephrotoxic serum glomerulonephritis. Glomerulonephritis was induced in female Wistar rats by the preimmunization with normal rabbit IgG (Day-4) and the subsequent intravenous injection of rabbit anti-GBM serum (Day 0). Without treatment with FK506, rats developed proteinuria at Day 6 and onward. Rat anti-rabbit IgG was strongly detected at Day 6 and the titer was maintained through Day 20. Moderate hypercellularity and focal crescent formation were observed at Day 20. Rats injected intramuscularly with 0.3 or 1 mg/kg of FK506 did not develop proteinuria and the anti-rabbit IgG titer was much less or was undetectable throughout the experiments. These data suggest that FK506 is effective in the present model of glomerulonephritis.

Cholera toxin- and forskolin-induced cyclic AMP accumulations of pig skin (epidermis). Modulation by chemicals which reveal the beta-adrenergic augmentation effect.

Effects of cholera toxin and forskolin on pig epidermal adenylate cyclase system were investigated. Both agents increased cyclic AMP levels of epidermis. Marked accumulations were observed in the presence of cyclic AMP phosphodiesterase inhibitor, isobutylmethylxanthine (IBMX). It has been known that hormone-stimulated adenylate cyclase responses are modified by various chemical treatments. Following long term incubation with hydrocortisone, Ro10-1670, and colchicine, the epidermal beta-adrenergic adenylate cyclase response was increased without the alteration of cyclic AMP phosphodiesterase activity. Adenosine-, and histamine-adenylate cyclase responses were unchanged by hydrocortisone treatment, and were decreased by Ro10-1670 and colchicine treatments. Following the long term incubation with these chemicals, effects of cholera toxin and forskolin were investigated. Colchicine-treated skin revealed the increased cholera toxin-, and forskolin-induced cyclic AMP accumulations. Neither hydrocortisone- nor Ro10-1670-treated skin revealed alterations of cholera toxin-, and forskolin-effect. The stimulatory effect of colchicine on the cholera toxin-, and forskolin-effect was observed at doses of the beta-adrenergic augmentation effect. Our results indicate that among the chemicals which reveal the beta-adrenergic augmentation effect, colchicine is unique in that it also increases cholera toxin-, and forskolin-induced cyclic AMP accumulations of epidermis.

Plasma lipid and lipoprotein profiles in pre- and post-menopausal middle-aged runners.

Plasma lipid and lipoprotein profiles were compared in middle-aged trained and untrained women before and after menopause. Subjects were assigned to one of four groups: (1) pre-menopausal trained (Pre-T: n = 17, aged 42 +/- 5 years, body fat 19 +/- 5%, training distance 53 +/- 20 km week-1, VO2max 49 +/- 4 ml kg-1 min-1, mean +/- SD); (2) pre-menopausal untrained (Pre-UT: n = 26, 42 +/- 5 years, 24 +/- 7%, 34 +/- 6 ml kg-1 min-1); (3) post-menopausal trained (Post-T: n = 16, 54 +/- 3 years, 20 +/- 4%, 43 +/- 19 km week-1, 41 +/- 5 ml kg-1 min-1); and (4) post-menopausal untrained (Post-UT: n = 15, 55 +/- 3 years, 25 +/- 6%, 31 +/- 3 ml kg-1 min-1). There were no significant differences in total cholesterol (range 173-194 mg dl-1), triglyceride (56-72 mg dl-1), and HDL-cholesterol (HDLC: 76-85 mg dl-1) among the four groups. LDL-cholesterol (LDLC) in the post-menopausal women (Post-T: 96 +/- 32 mg dl-1; Post-UT: 104 +/- 23 mg dl-1) tended to be higher than in the premenopausal women (Pre-T: 86 +/- 25 mg dl-1, Pre-UT: 81 +/- 23 mg dl-1).(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of fluoride addition on ionized calcium in salivary sediment and in saliva containing various amounts of solid calcium fluoride.

The aim of the present study was to estimate the fluoride concentrations necessary for the precipitation of calcium fluoride or calcium fluoride-like substances in saliva and salivary sediment, in the presence of various amounts of chemically pure calcium fluoride. Fluoride was added in increments to whole saliva to which solid calcium fluoride had been added. The ionized calcium concentration was determined immediately after centrifugation. In salivary sediments and saliva with no calcium fluoride added the ionized calcium concentration was nearly independent of the fluoride concentration added below 10 and 20 mmol/l, respectively, and at fluoride concentration above 15 and 25 mmol/l, respectively, nearly no ionic calcium was detected. In the presence of calcium fluoride the ionized calcium concentration decreased rapidly with increasing concentrations of fluoride. The more calcium fluoride added, the lower the ionized calcium concentration at a certain fluoride level. The results suggested that in the in vivo-situation, caries preventive measures based on frequent oral exposure to low concentrations of fluoride may provide calcium fluoride or calcium fluoride-like substances, which will serve as a reservoir for fluoride.

A monoclonal antibody which identifies an antigen in endothelial cell and epithelial basement membrane.

We isolated a monoclonal antibody which decorates the endothelial cells of normal and lymphatic vessels in formalin-fixed, paraffin-embedded tissue. In addition, the antibody recognizes a previously undescribed substance found in the basement membrane zone and subbasement membrane zone of a variety of epithelial. By ELISA assay, the antigen recognized by this monoclonal antibody is not laminin, type IV collagen or fibronectin. This antibody may be used as a diagnostic tool because it identifies an epitope in neoplasms differentiating towards endothelial cell such as angiosarcoma and Kaposi's sarcoma.

Long-term observation of cardiac function in Duchenne's muscular dystrophy. Evaluation using systolic time intervals and echocardiography.

Cardiac involvement is common in patients with Duchenne type muscular dystrophy (DMD). However, published reports of long-term follow-up studies in the same patients are very rare. The purpose of this study was to clarify the natural course of cardiac dysfunction in DMD, and to analyze the relationship between the deterioration of cardiac function and skeletal muscular function. The cardiac function was serially evaluated for 2-12 years in 34 patients with DMD. The systolic time intervals and echocardiography were used for evaluating the cardiac function. The PEP/ET (pre-ejection period/ejection time) of systolic time intervals significantly deteriorated in 9-12 years (p less than 0.001). The maximal systolic and diastolic endocardial velocity (MSEV and MDEV) and the D/S ratio of echocardiography significantly deteriorated in 5 years (p less than 0.001). The D/S ratio is the ratio of the mean systolic velocity and the mean diastolic velocity of the left ventricular posterior wall. In almost half the patients, the deterioration of these parameters was correlated with that of Swinyard's stage. The Swinyard's functional ability stage demonstrates the grade of the skeletal muscular dysfunction. However, there was no correlation between the deterioration of cardiac function and Swinyard's stage in the other half of the patients. Correlation coefficient in the whole group was 0.32 (p = n.s.) between the PEP/ET and Swinyard's stage, and was -0.15 (p = n.s.) between the D/S ratio and Swinyard's stage. Although the mean left ventricular ejection fraction calculated using echocardiography was slightly decreased for 5 years, no significant change was detected in the whole group. One patient had progressive deterioration of left ventricular function despite mild skeletal muscular dysfunction, and died from congestive heart failure. The cardiac dysfunction was predominant. We named it 'cardiac type' DMD. Since deterioration of cardiac function was not always correlated with deterioration of skeletal muscular function (Swinyard's stage), cardiac function must be carefully monitored in patients with DMD.

Intrinsic reflexes mediated via peptidergic neurons in the smooth muscle esophagus of the hen.

In the extrinsically denervated smooth muscle esophagus of the hen anesthetized with urethane (1 g/kg, i. m.), it was studied whether peptidergic neurons in the intramural plexus are involved in the intrinsic reflex. Ascending and descending contractions, and descending relaxation were induced by electrical stimulation of a narrow segment of the esophagus. Naloxone (1 microM), desensitization to substance P (0.3 microM) and spantide (20 microM) inhibited the ascending and descending contractions, respectively. The degree of the inhibition of the contractile response by a combination of naloxone and substance P was nearly the same as that by a single administration of naloxone or substance P. The ascending and descending contractions were reduced to one-third of the control by hexamethonium (100 microM) and abolished by atropine (10 microM). The descending relaxation was abolished after desensitization to vasoactive intestinal peptide (0.3 microM). Taken together the results suggest that in the hen's esophagus, opioid- and substance P-containing neurons in the intramural plexus may act as preganglionic neurons of cholinergic motor neurons in the ascending and descending excitatory pathways and that vasoactive intestinal peptide-containing neurons are involved in the descending inhibitory pathway.

Effects of far-infrared radiation on lactation.

Massage and warm compresses to the breast have been commonly used for stimulating and/or increasing blood flow to the breasts, and for enhancing lactation consequently. However, more effective and easier remedies seem to be necessary. The vasodilating and warming effects of ceramics far-infrared radiation were studied. Based on the results obtained, the effect of a ceramic disc on lactation, attached to the breast skin, was evaluated in 27 puerperal women who had had poor lactation previously and in 36 with currently poor lactation monthly until weaning. Approximately 3/4 of these puerperal women enhanced lactation significantly one month after attachment and 1/2 of the women were able to breast-feed until weaning. Thus, we found that ceramics far-infrared radiation may be an effective remedy for enhancing lactation.

Significance of exercise and bed rest in pregnancy--study on the lying postures of gravidas during sleep (2).

For the purpose of determining the most reasonable lying posture for pregnant women, we investigated the lying positions of both 247 non-pregnant women and 302 pregnant women during sleep. As for the rate of each position during the entire period of observation, 33.2% of the non-pregnant women were in the supine position, 41.2% in the lateral position, 18.4% in Sims' position and 7.1% in the prone position. In the pregnant group, the rate of supine position, simple lateral position and Sims' position was 34.2%, 52.2% and 12.7%, respectively, but the rate of the prone position was limited to 0.8%. All of the gravidas assuming the prone position were at less than 16 weeks of gestation. Non-pregnant women could sleep in a variety of positions, but pregnant women could assume the prone position during sleep only in the stage when the abdomen is not yet prominent or distended. Pregnant women were restricted significantly, either consciously or unconsciously, with progress in gestation.

A plasma membrane antigen of rat glomerular epithelial cells. Antigenic determinants involving N-linked sugar residues in a 140-kilodalton sialoglycoprotein of the podocytes.

The aim of the present study was to investigate the antigenic make up of the plasma membrane of rat glomerular visceral epithelial cells (GEP). A crude plasma membrane fraction (PM) was extracted by 1% sodium dioxycholate from isolated rat glomeruli. PM was digested with neuraminidase (NRD) and purified by the Helix pomatia agglutinin (HPA)-affinity column. When studied by sodium dodecyl sulfate-polyacrylamide gel electrophoresis, the HPA-affinity purified plasma membrane fraction (HPA-PM) formed two bands, a main band of 160 kilodaltons (kd) and a smaller band of 40 kd. By Western blot analysis, the antibodies raised in a rabbit against HPA-PM (RbAHPA-PM) reacted only with the 160-kd protein of HPA-PM, or with the relevant 140-kd protein of PM when PM was digested by NRD. The 140-kd protein of PM was reactive with wheat germ agglutinin and, when treated with NRD, was reactive with HPA and peanut lectin. The 160-kd protein of HPA-PM was degraded by endoglycosidase-F and lost its reactivity with RbAHPA-PM. These results suggest that RbAHPA-PM react with antigenic sites involving N-linked sugar residues of a 140-kd sialoglycoprotein, presumably podocalyxin. Immunohistochemical studies using normal rat kidney tissues treated with NRD as substratum showed that RbAHPA-PM bound to the free surface of GEP but not with soles of the foot processes or with other structures of the kidney. In rats intravenously injected with NRD and subsequently with RbAHPA-PM, antibodies were rapidly fixed (within 1 hour) to the free surface of GEP. Immunofluorescence study showed that RbAHPA-PM also reacted with human glomeruli after treatment with NRD. These results suggest that GEP express surface sugar residues that are potential targets for direct immunologic attack.
