Molecular characterization by RNA-RNA hybridization of a serotype 8 human rotavirus with "super-short" RNA electropherotype.

A human rotavirus isolate (the 69M strain) with a "super-short" RNA electropherotype has been classified as serotype 8, a new human serotype [Matsuno et al., 1985]RNA-RNA hybridization using 32P-labeled transcription probes was used to assess the genetic relatedness of the 69M strain to a panel of human and animal rotaviruses. The 69M strain showed a medium level of homology with subgroup 1, serotype 2 human rotaviruses with short RNA electropherotypes. However, the 69M strain was not significantly related to any other human rotavirus strains tested in this study, including recently identified serotype 9 strains (represented by the WI61 strain) and subgroup 1 human rotaviruses with long RNA electropherotypes (represented by the AU-1 strain) that are shown to be genetically distinct from other human rotavirus strains. Unexpectedly, a medium level of homology was also found between the 69M probe and the double-stranded RNAs from bovine rotavirus strains (represented by the NCDV strain).

Transcatheter arterial embolization and chemoembolization induce the production of free radicals in the normal rat liver.

Superselective transcatheter arterial embolization (TAE) of the left and median lobes of the normal rat liver was performed using gelatin sponge (1.5 mg/ml). Superselective transcatheter arterial chemoembolization (TAC) of the same lobes was done adding mitomycin (MMC) at a dose of 1.6 mg/ml to the embolic solution. In another group of rats MMC alone was injected intraarterially into those lobes. It was found that both embolic procedures and the arterial injection of MMC decrease the hepatic total glutathione content. Glutathione peroxidase activity and alpha-tocopherol content remained unchanged. Lipoperoxides were detected 3 and 6 h after TAC in chemoembolized and non-chemoembolized lobes. Similar findings in both injected and noninjected lobes were found after MMC injection. The TAE lobes only showed elevated lipoperoxide content 6 h after embolization, this parameter remained unchanged in the non-TAE lobes. These findings suggest that free radicals are formed after TAE, and that the addition of MMC to the embolic solution increases the oxidative attack and/or that the oxidative reactions after TAC are mainly mediated by lipid peroxidation due to the presence of MMC.

Continuous removal of middle molecules by hemofiltration in patients with acute liver failure.

In patients with acute liver failure and hepatic coma, an increase in the abnormal "middle molecules" seen on the chromatograms of the sera is suspected of playing an etiologic role in the coma. A pilot study of continuous hemofiltration using a high-performance membrane was conducted in 16 such patients in an attempt to decrease the serum levels of the middle molecules. The procedure was used alternately with plasma exchange. High-performance liquid chromatography showed a notable removal of the substances in the filtrates and a sequential removal from the serum by hemofiltration. Eight (50%) of the 16 patients had amelioration in level of consciousness and were weaned successfully from hemofiltration. Although only three of the 16 patients survived the acute illness, 13 others lived an average of 15 days and five patients survived greater than 3 wk. While the continuous removal of middle molecules from the serum may not reverse liver failure, this procedure used in conjunction with plasma exchange may provide a means of life support, e.g., for patients awaiting a liver transplant.

Sequence analysis of gene 11 equivalents from "short" and "super short" strains of rotavirus.

The molecular basis for the aberrant migration pattern of the gene 11 equivalent in rotaviruses with "short" (human DS-1) and "super short" (human 69M and bovine VMRI) electropherotypes was investigated. The mRNAs of these viruses were synthesized in vitro, and the entire gene 11 equivalent of each of these viruses was sequenced with specific synthetic oligonucleotide primers. These sequences were compared with previously published sequences of "long" pattern rotavirus gene 11 segments. The increased lengths of the gene 11 equivalents of DS-1, 69M, and VMRI are due to a prolonged, 3' untranslated region in this gene segment. The 3' untranslated region of the VMRI gene 11 equivalent contains a clear duplication of a portion of its coding sequence. A stretch of 18 consecutive nucleotides within the 330-nucleotide, 3' untranslated region of 69M is identical to a section of UK coding sequence. The DS-1 and the remainder of the 69M 3'-end additional sequences are similar to each other, but neither is similar to any other currently available rotavirus gene sequence. This finding suggests that a process other than homologous duplication is involved in the evolution of these sequences. The widespread occurrence of human and animal rotaviruses with short and super short electropherotypes provides evidence that intragenic and possibly intergenic recombinational events associated with an error-prone viral RNA polymerase may play a role in increasing the genetic repertoire of rotaviruses.

[Perfusion study of the pulmonary hilar region by SPECT--normal cases].

Alveoli in the hilar region comprise the peripheral area containing daughter branches from subsegmental or one more divisional peripheral bronchi. Pulmonary perfusion in hilar region was examined by SPECT (single photon emission CT) in ten normal volunteers. ROI (region of interest) in the axial images were set in the hilar region, the upper, middle and lower lung fields with 10.8 mm thickness. Counts/one pixel (C/P) were calculated in these ROI. There was a tendency of C/P increase from upper to lower lung field. And there were no significant differences in C/P increase between hilar region and other axial fields. In the chronic obstructive pulmonary diseases, however, ventilation studies in the previous reports using Xe dynamic CT or PET (positron emission tomography) showed differences between outer region and hilar region. This method will be expected to evaluate the pulmonary perfusion not only in the whole lung but in different lung areas, including the hilar region in the chronic obstructive pulmonary diseases.

[Effect of peripheral pancreatic duct obstruction on the progression of chronic pancreatitis].

Experimental chronic pancreatitis model were made in five dogs with chronic pancreatic fistula by injection of microspheres into peripheral pancreatic duct. Sequential changes of pancreatic exocrine and endocrine functions also with morphology were studied. Significant decrease of volume, bicarbonate output, and amylase output of pure pancreatic juice (PPJ) separately collected with stimulation of secretin were detected. While, viscosity of PPJ was significantly increased with concomitant increase of hexosamine concentration. Chronic pancreatitis was demonstrated morphologically also. These results suggest that concentrated PPJ caused by decrease in volume in PPJ and increase in viscosity of PPJ with concomitant increase in hexosamine concentration bring about the progression of chronic pancreatitis in this experimental model.

[Effect of continuous arterial infusion of protease inhibitor on experimental acute pancreatitis induced by closed duodenal loop obstruction].

The effectiveness of continuous arterial infusion of protease inhibitor on acute experimental pancreatitis was investigated. Acute hemorrhagic pancreatitis was induced by closed duodenal loop obstruction using mongrel dogs. The obstruction was released at 16 hr, and dogs were divided into three groups; Group I: non-treated control, Group II: nafamostat mesilate (FUT-175) was admitted intravenously (5 micrograms/kg/min), Group III: FUT-175 was admitted via celiac artery. At 24 hr, the concentration of FUT-175 in the pancreatic tissues in group II and III were 905 and 4453 ng/g, respectively. The trypsin like activities in the pancreatic tissues in group I, II and III were 2.1, 1.4 and 0.7 nmol/min/mg protein, and the extent of necrosis of pancreatic parenchyma in each group were 49.5, 25.6 and 12.4%, respectively. Serum calcium, amylase and lipase levels were significantly improved in group III. These results suggest that continuous arterial infusion of protease inhibitor markedly decreases the extent of pancreatic necrosis in severe acute pancreatitis.

[Three-dimensional morphology of intrahepatic bile ducts in patients with intrahepatic gallstones].

Eight livers surgically resected for intrahepatic gallstones were submitted to graphic reconstruction from serial histological sections to correlate the peculiar ductal changes with the pathogenetic aspects of this disease. Reconstruction was aided by a microcomputer system we have developed. Further, morphometric analysis was added to relate the obstruction of portal veins with parenchymal atrophy. Three-dimensionally, the ducts in these livers were strikingly transformed into a chain of ampullar dilatations interposed by either bending or strictures. Morphometrically, even a stenotic segment was wider than a corresponding normal one, showing that it was a relative stricture. The parenchymal lobule was atrophic or even completely destroyed due to the frequent thrombotic obstruction of portal veins about 0.5mm diameter. These 3-D morphology of intrahepatic bile ducts provided little evidence of congenital bile duct anomaly for the origin of intrahepatic gallstones. The basic ductal changes were dilations, alternated with "stenoses" that were more apparent than real. On the other hand, both the frequent obstruction of small portal veins and the lobular atrophy reflected a severely reduced portal blood flow. Not only the recurrent cholangitis that is apt to occur upon dilated ducts, but this functional incompetence of a stone-harboring, atrophic liver lobe strongly suggests a surgical indication for hepatic resection.

[Effects of intragastric and intrajejunal ethanol administration on canine pancreatic exocrine secretion and duodenal pH].

The effect of ethanol on pancreatic exocrine secretion and intraduodenal pH was investigated in 3 dogs, after making pancreatic, gastric, and jejunal fistula. After intragastric and intrajejunal administration of ethanol the intraduodenal pH went down and pancreatic juice, pancreatic bicarbonate output was increased. The integrated value of pancreatic juice, pancreatic bicarbonate and pancreatic protein output was greater in case of intrajejunal administration rather than intragastric administration of the ethanol. After intragastric and intrajejunal administration of the ethanol the plasma concentration of CCK was unchanged. The decrease of intraduodenal pH and increase of pancreatic exocrine secretion was though be mainly due to the secondary action of gastric acid secretion stimulated by ethanol.

[Right ventricular outflow tract reconstruction with monocusp patch].

Two kinds of monocusp patch were used for reconstruction of right ventricular outflow tract in patients with a variety of congenital heart defects. A bovine pericardial patch with monocusp (MVOP) was used in 11 patients (group M), and a separate transannular outflow patch (STOP) with turned-down equine pericardial monocusp was used in 10 patients (group S). Mean age at operation was 9 years 5 months in group M, and 7 years 9 months in group S. There was one operative death in group M. Survivors have completed a mean follow-up of 19 months. There were no late deaths. In early postoperative period, mean peak systolic pressure gradient between right ventricle and main pulmonary artery was 21 +/- 11 mmHg in group M and 21 +/- 11 mmHg in group S, which were not statistically significant. During follow-up period, no significant increase of peak systolic pressure gradient was noted in both groups. In late postoperative period, pulmonary regurgitation was moderate in 7 and severe in 1 in group M, while it was non-significant in 1, mild in 6 and moderate in 2 in group S. The difference between two groups was significant (p < 0.005). In conclusion, both types of patches equally relieved right ventricular outflow obstruction, but STOP prevented postoperative pulmonary regurgitation more effectively.

The development and extension of hepatohilar bile duct carcinoma. A three-dimensional tumor mapping in the intrahepatic biliary tree visualized with the aid of a graphics computer system.

Computer-assisted three-dimensional (3D) reconstruction of the biliary tree including extrahepatic and intrahepatic parts was performed from surgical or autopsy materials from 12 patients with hepatohilar bile duct carcinoma in an effort to visualize three-dimensionally the distribution of carcinoma and dysplasia. In each case, material including a hepatic lobe was reduced to serial slices 1 mm thick using a ham slicer, then the tumors, ducts with carcinoma in situ, and those with dysplasia were submitted to reconstruction. In a 3D map of biliary tree reproduced on a display, a dysplastic zone was shown in most cases surrounding a focus of carcinoma, justifying the assumption of a dysplasia-carcinoma sequence. The carcinoma itself proved to form multiple foci along bile ducts in as many as 42% of the patients, in some of whom the foci were independent without any intervening dysplasia. These results suggest that as extensive a surgical measure as possible should be taken in designing a strategy against this tumor.

Long-term survival in carcinoma of the biliary tract. Analysis of prognostic factors in 146 resections.

In 146 consecutive patients undergoing resection for carcinoma of the biliary tract, various tumor characteristics that affected long-term survival of the patients were studied. Patients with gallbladder carcinoma whose tumors had no serosal infiltration or vessel invasion were grossly papillary, or were papillary or well-differentiated adenocarcinoma histologically survived longer than those without these tumor characteristics. In upper-third lesions, patients whose tumors showed no serosal infiltration or vessel invasion, were grossly papillary, were papillary or well-differentiated adenocarcinoma histologically, or were treated with hepatic lobectomy had a higher chance of long-term survival. Patients with the middle-third lesions, whose tumors were grossly papillary or nodular or whose margins were tumor-free, were apt to survive longer. Long-term survival for patients with lower-third lesions was obtained most often in patients without lymph node metastasis or vessel invasion.

A long-term follow-up study of total hip replacement with bone graft. Correlations between roentgenographic measurement and hip mobility.

Fifty total hip replacement treating the acetabular deficiency with autologous bone graft from the femoral head were performed on 44 patients with congenital dislocation of the hip. The average age was 55 years. The average follow-up period was 5 years and 2 months, ranging from 3 years, 8 months to 6 years, 11 months. Solid union was demonstrated in all acetabular grafting. No loosening of either acetabular or femoral components occurred. Roentgenological measurements used as a parameter of good results showed that better results were obtained using the true acetabulum (ideal position) with appropriate bony coverage.
