Changes of duodenal pH and pancreatic exocrine function after upper G-I intraluminal ethanol administration.

Using 3 outbred dogs with pancreatic, gastric and jejunal fistulas, we examined changes in pancreatic exocrine secretion, duodenal pH and plasma CCK concentration after intragastric or intrajejunal administration of ethanol. Intragastric administration of 20% or 40% ethanol significantly lowered the pH in the duodenal cavity, and pancreatic fluid secretion and bicarbonate excretion were stimulated. Intrajejunal administration of ethanol elicited stronger responses. Plasma CCK concentration was not changed or was only slightly lowered by ethanol administration in the digestive tract. It is assumed that stimulation of pancreatic exocrine secretion was mediated by the secretion of gastric acid.

Immunohistochemical localization of mineralocorticoid receptor in human gut.

Immunohistochemical localization of mineralocorticoid receptor in the human adult gut was examined by using a polyclonal antibody raised against a mineralocorticoid receptor fusion protein. Immunoreactive staining was observed in columnar cells in the middle of the intestinal glands of the proximal colon, but not in goblet cells. Western blot analysis also revealed the presence of an approximately 100 kDa immunoreactive species consistent with the mineralocorticoid receptor protein in the ascending colon as well as the kidney. These results suggest that aldosterone-sensitive sodium transport and water absorption may be mainly carried out in the proximal colon in human adult gut at non-pathological state.

Efficacy of patient-controlled analgesia for management of pain after abdominal operations.

In order to control pain during the early post-operative period, patient-controlled analgesia (PCA) with buprenorphine as an analgesic drug was applied in 23 patients undergoing abdominal operations. With this "on demand" system, the patient was allowed to self-administer narcotic analgesic medication using a programmable infusion pump. Overdose could be minimized with a mandatory lock-out interval between allowable injections. Average total requirement of buprenorphine was 0.355 mg at 48 hr after operation. Nineteen of the 23 (82.6%) patients characterized their pain control as "excellent" or "good". In these patients there existed high correlation between the total number of patient attempts and the number of successful injections. The PCA system was thought to provide improved pain relief at smaller total drug dosages. In addition, earlier and greater spontaneous physical activity was maintained with PCA therapy. The potential for overdose could be minimized, and thereby PCA appears to be an efficacious and safe method of providing for postoperative pain relief.

[Long-term follow-up study after pylorus-preserving gastrectomy for gastric ulcer].

The long-term follow-up study of pylorus-preserving gastrectomy (PPG) for benign gastric ulcer was described in terms of postoperative subjective symptoms related to gastrectomy and recurrent ulceration. Of 134 patients who underwent PPG, 5 patients died within one month after operation, 33 patients died in the follow-up period and 22 patients have not been traced. Seventy-four patients were available for a 3-24 year follow-up (mean 16.6 years) Recurrent ulcer developed in 4 patients, 1, 2, 9 and 11 years after surgery at the greater curvature of pre-pyloric region in all cases. Two patients underwent reoperation. These patients revealed higher gastric acid secretion as compared with that of one month after primary operation. But gastrin hypersecretion and G-cell hyperplasia were not recognized in these patients. According to detailed questionnaires, no patient showed the symptoms of dumping syndrome. The results indicate that PPG is preferable for the surgical treatment of gastric ulcer.

[The growth rates of liver metastases in pancreatic cancer--comparison on growth rates between clinical cases and established human pancreatic cancer cell lines].

By measuring the doubling time both of clinical liver metastases and cultured human pancreatic cancer cell lines, we studied the possibility of occult liver metastases at the time of pancreatectomy in patients with pancreatic cancer. Doubling times of 3 pancreatic cancers were obtained from the measurements of size of liver metastases after pancreatectomy and those were compared with those of cultured human pancreatic cancer cell lines established from two of these cases. Doubling time of liver metastases were about fifteen times as long as those of cultured cell lines. On the assumption that growth rates of liver metastases were constant, the size of liver metastases at the time of pancreatectomy was calculated in three cases. The calculated sizes of these liver metastases were 140 microns, 2.4 mm, and 8.3 mm respectively. These results indicate that occult liver metastases have already existed in patients with pancreatic cancer even though they were clinically undetectable at the time of operation. And some adjuvant therapies against occult liver metastases are necessary for achievement of the better prognosis.

[Hepatic resection under the Pringle maneuver induces endotoxemia and lipoperoxidative attack in the jaundiced rat liver].

Sixty percent hepatectomy under two times of the Pringle maneuver (temporal occlusion of the hepatic inflow of the hepatic artery and portal vein) for 25 min was carried out on rats with obstructive jaundice. Hepatic energy charge which was markedly decreased during the Pringle maneuver did not recover 3 hrs after release of the maneuver. In addition, systemic endotoxemia associated with increased level of malondialdehyde and decreased levels of total glutathione and alpha-tocopherol in the liver was demonstrated. These changes were more dominant in rats with obstructive jaundice for 3 weeks than those for 1 week. It can be said that deteriorated hepatic energy charge after release of the Pringle maneuver may partly be caused by the production of oxygen free radicals which may be enhanced by endotoxemia in resection of the jaundiced liver. In contrast, after release of the Pringle maneuver, deterioration of hepatic energy charge due to oxidative attack was not found in jaundiced rats without hepatic resection.

[Pathogenesis of black gallstones associated with hemolytic disease].

In order to investigate the pathogenesis of black gallstones associated with hemolytic disorders, experimental and clinical studies were performed. Firstly, a one-shot injection of bilirubin and hemolyzed blood, and a continuous injection of bilirubin were performed in mongrel dogs with the aim to analyze the changes of hepatic and gallbladder bile composition. Secondly, gallbladder bile obtained intraoperatively from the patients with black gallstones associated with hemolytic disease, patients with black stones not associated with hemolytic disease and patients with gallbladder polyp were analyzed in order to compare the characteristics of hemolytic patients' bile with those of the model animals. High concentration of total bilirubin and increased proportion of bilirubin monoconjugate (BMC) fraction was observed in the bile of patients with black stone associated with hemolytic disease compared with the bile of the patients of other two groups. Bile pH, total calcium concentration and ionized calcium concentration were similar in all the groups. Total bile lipid concentration of hemolytic patients was decreased but the ratio of total bile acid to total lipid was increased. Both bolus injection of bilirubin and hemolyzed blood produced an increase of total bilirubin concentration and BMC in hepatic bile. Continuous bilirubin injection for 7 days induced an increase of total bilirubin and BMC in both hepatic and gallbladder bile. These results suggest that the conjugating capacity of the hepatocyte is surpassed by the excessive amount of bilirubin produced after hemolysis, producing therefore, an increased BMC and unconjugated bilirubin content in bile. This increased content can precipitate in bile as black gallstones.

Undifferentiated sarcoma of the liver in a 21-year-old woman: case report.

A successful surgical case of malignant undifferentiated (embryonal) sarcoma of the liver (USL), a rare tumor normally found in children, is reported. The patient was a 21-year-old woman, complaining of epigastric pain and abdominal fullness. Chemical analyses of the blood and urine and complete blood counts revealed no significant changes, and serum alpha-fetoprotein levels were within normal limits. A physical examination demonstrated a film, slightly tender lesion at the liver's edge palpable 10 cm below the xiphoid process. CT scan and ultrasonography showed an oval mass, confined to the left lobe of the liver, which proved to be hypovascular on angiography. At laparotomy, a large, 18 x 15 x 13 cm tumor, found in the left hepatic lobe was resected. The lesion was dark red in color, encapsulated, smooth surfaced and of an elastic firm consistency. No metastasis was apparent. Histological examination resulted in a diagnosis of undifferentiated sarcoma of the liver. Three courses of adjuvant chemotherapy, including adriamycin, cis-diaminodichloroplatinum, vincristine and dacarbazine were administered following the surgery with no serious adverse effects. The patient remains well with no evidence of recurrence 12 months after her operation.

[Effect of total biliary diversion (cholecysto-jejuno-cystostomy) on gut hormone release and pancreatic structure in dogs].

In order to elucidate the effect to total biliary diversion (TBD) on gut hormone release and pancreatic structure, we performed cholecysto-jejuno-cystostomy (CJC) in 6 mongrel dogs using a small interposed intestinal segment between the gallbladder and the urinary bladder with ligation of the common bile duct. Twelve weeks after CJC, CJC was converted into cholecysto-jejuno-duodenostomy (CJD) to normalize luminal bile flow. Fat rich meal loading tests were carried out before and after these procedures. Plasma concentrations of GI hormones (CCK, PP, GIP) were measured by radioimmunoassay and morphological changes of the pancreas were evaluated by light and electron microscopic study. CJC significantly enhanced the basal levels and fat-stimulated release of both CCK and PP, however after CJD, these changes returned to the normal levels. Following CJC, fat-stimulated GIP release was completely inhibited, but recovered after CJD. Following CJC, hypertrophy of pancreatic acinar cells with profuse and dilated rough endoplasmic reticulum was observed, while after CJD this change returned to the pre-operative state. These results suggest that a feedback regulation may exist between luminal bile flow and CCK secretion, and pancreatic hypertrophy after TBD is, at least partly, due to the increased plasma CCK levels.

[Role of bile and pancreatic juice in regulation of gut hormone secretion].

To elucidate the role of bile and pancreatic juice in regulation of gut hormone secretion, an experimental study was performed creating models of biliary and pancreatic juice diversion in conscious dogs with reference to gastric acid and pancreatic exocrine secretions. The results were obtained as follows. 1) Diversion of bile from the duodenum to the jejunum, the ileum and urinary bladder (UB) did not affect the postprandial gastric acid and gastrin secretion, except slight suppression of gastric acid in model of bile diversion to the ileum and UB. 2) Postprandial GIP secretion was completely diminished and total-GLI secretion was significantly increased after bile diversion to the ileum and UB, whereas the jejunal diversion did not affect both GIP and total-GLI secretion. 3) A marked hypertrophy of pancreatic acinar cells was seen in conventional histopathological investigation and hyperfunction indicated by microelectroscopical findings was observed after bile diversion to UB with significant hypersecretion of CCK. 4) In the model of bile diversion to UB, hypersecretion of insulin was observed after intravenous glucose infusion test. 5) Diversion of pancreatic juice from the duodenum to the jejunum induced significant postprandial hypersecretion of gastric acid and hyposecretion of GIP.

[Influence of 50% proximal or distal small bowel resection on gut hormone release after test meal loading in dogs].

The effect of proximal and distal small bowel resection on gut hormone release after test meal loading in dogs was studied. Ten beagle dogs were subjected to 50% proximal or distal small bowel resection, and test meal loading was performed after one night fasting to examine gut hormone release. Fasting levels of plasma gastrin were not changed after both proximal and distal resection, but response to test meal was increased at 18 weeks of postoperative period in 50% proximal resection. Postprandial release of plasma GIP was significantly decreased in both proximal and distal resection compared with preoperative period. Postprandial release of enteroglucagon was increased at 4 and 8 weeks in proximal resection. In distal resection, it was increased at 4 weeks but returned to preoperative levels at 8 weeks. Villus height of middle part of the intestine was increased in both proximal and distal resection, and significant change was observed in the duodenal mucosa of proximal resection at 4 weeks. These findings suggest that part of the resection of small bowel influences gut hormone release, and these may play an important role in intestinal adaptation.

[Surgical results in Crohn's disease--an analysis in view of cumulative risk or recurrence and reoperation].

Forty nine cases with Crohn's disease operated at our department were analyzed in view of cumulative risk of reoperation and recurrence. Cumulative risk of reoperation was 22% at five years and 40% at ten years. Cumulative risk of recurrence after "curative" resection in thirty four cases was 53% at five years and 73% at ten years. Both risks were the highest in ileal type and the lowest in colonic type. Cumulative risk of relapse in ileocolic type after non-"curative" resection was 67% at five and ten years, which showed no significant difference with cumulative risk of recurrence after "curative" resection. Cumulative risk of recurrence after "curative" resection with proximal resection margin longer than 10 cm had no significant difference from the group with shorter margin. Cumulative risk of recurrence and reoperation after reoperation was a little higher than after initial operation, but they showed no statistical difference. In conclusion, there is no need to stick to "curative" resection, and it is enough to have the resection margin no longer than 10 cm. It is essential not to spoil the quality of life by excessive resection.

[Effects of mitochondrial lipoperoxidation on liver regeneration after partial hepatectomy of the cirrhotic rat liver].

Lipoperoxide levels in mitochondrial fraction and mitochondrial respiratory activity during hepatic regeneration were studied serially in normal (N) and cirrhotic (C) rats following one-third (I) or two-third (II) partial hepatectomy. Recovery of liver weight after 14 days was about 90% in each group except C-II group, in which it was 71.3%. ATP synthetic activity (ATP-S) was always higher in N-II group than N-I which reached maximum on the first postoperative day (POD) and returned to the preoperative levels on the 14th POD. Lipoperoxide peak value of N-II group was obtained sooner than that of N-I group, in addition the peak value of N-II group was higher than that of the N-I group. ATP-S of C-II group reached maximum levels on the third POD and thereafter decreased progressively. In contrast, ATP-S of C-I group returned to preoperative level on the 14th POD. Peak lipoperoxide levels were obtained on the first POD in both groups, but that of C-II group was 1.6 times higher than that of C-I. Moreover, it was observed that inhibition of lipoperoxidation by alpha-tocopherol administration improved respiratory activity significantly. From these results it can be said that hepatectomy-induced lipoperoxidation in massive resection of the cirrhotic liver may inhibit activation of mitochondrial respiration and hepatic regeneration.

Growth-inhibitory effect of combination chemotherapy for human pancreatic cancer cell lines.

Sensitivities to anti-tumor drugs, mitomycin C (MMC), aclarubicin hydrochloride (ACR), doxorubicin hydrochloride (ADR), cisplatin, and 5-fluorouracil (5FU), were examined using PK-1, -8, -9, -12, -14, and -16 cell lines derived from human pancreatic cancer. These cell lines showed different sensitivities to each of the above anti-tumor drugs. The concentrations required for 50% growth-inhibition (IC50) after 2 hours of exposure were 0.096 to 0.35 micrograms/ml for MMC, 0.0074 to 0.0076 micrograms/ml for ACR, 0.033 to 0.23 micrograms/ml for ADR, 0.35 to 1.9 micrograms/ml for cisplatin, and 21 to 42 micrograms/ml for 5FU, IC50 of each anti-tumor drug decreased significantly after 48 hours of exposure. The combination of any two out of MMC, ACR, and 5FU showed synergistic inhibition of the growth of PK-1 and PK-8 cell lines. These results show that MMC, ACR, ADR, cisplatin, and 5FU have sufficient anti-tumor effect against six human pancreatic cancer cell lines even at clinically achievable concentrations and exposure times, and chemotherapy for pancreatic cancers requires naturally effective drug delivery into cancer tissues.
