Transient paraplegia caused by acute aortic dissection--case report.

A case of acute aortic dissection (AAD) presenting as sudden, transient paraplegia and severe back pain is reported. The patient was a 66-year-old male with a 10-year-history of hypertension. The pain characteristically migrated from the back to the neck and then returned to the back. He showed complete transverse myelopathy at the level of the 9th thoracic cord. Computed tomography disclosed internal displacement of aortic intimal calcifications, without abnormalities in the spinal canal, and myelography showed no spinal canal block or stenosis. Electrocardiography and chest x-ray indicated nonspecific changes of high amplitudes and mild cardiomegaly, respectively. Together, these findings suggested acute aortic dissection with spinal cord ischemia. The initial systolic blood pressure of 220 mmHg was lowered with medication, and the pain was controlled with morphine. He recovered fully and was discharged 80 days after the onset of symptoms, with no neurological deficits. AAD carries a very poor prognosis unless treated immediately. Therefore, it is very important to promptly differentiate this disorder from spinal vascular conditions that also produce back pain and paraparesis.

Acute promyelocytic leukemia associated with insulinoma: a case report.

We report a case of acute promyelocytic leukemia (APL) coexisting with insulinoma in a 61-year-old female. Two years before the onset of APL, she was diagnosed as having insulinoma and underwent resection of the body and tail of the pancreas, but no insulinoma was found in the resected pancreas. The clinical symptoms of hyperinsulinism, however, continued after the operation and were treated with diazoxide (100 mg daily, 37 g in total) for about two years until the onset of APL. During the induction therapy for APL, she died of hemorrhage and infection; autopsy revealed the presence of insulinoma (13 X 9 mm in size) in the residual pancreas head. This is the first case report of coexistence of APL and insulinoma.

[Pharmacokinetics and clinical evaluations on aztreonam in perinatal infections in obstetrics and gynecology. A study of aztreonam in the perinatal co-research group].

Pharmacokinetics and clinical studies on an injectable monobactam antibiotic aztreonam (AZT), were carried out in perinatal infections in obstetrics and gynecology and the obtained results are summarized as follows. 1. Pharmacokinetic study (1) Upon one-shot intravenous injection of AZT 1 g before delivery, maternal serum concentration of AZT was 89.0 micrograms/ml immediately after the injection and a half-life (T 1/2) of 0.96 hour was observed. Umbilical-cord serum concentration showed a peak value of 16.5 micrograms/ml at 1.26 hours after the injection and gradually decreased with a T 1/2 of 1.91 hours. The transfer into amniotic fluid was observed and the peak value of AZT in amniotic fluid reached 12.9 micrograms/ml at 5.57 hours after the injection and slowly decreased thereafter with a T 1/2 of 4.42 hours. Transfer and disappearance in one-shot 2 g intravenous injection and 1 g intravenous drip infusion (1 hour) of AZT were very similar to the results obtained with the one-shot 1 g intravenous injection. (2) The residual serum concentration in neonates after one-shot 1 g intravenous injection of AZT to the mother was almost below the detectable limit. Transfer of AZT into milk was scarcely recognized. 2. Clinical studies (1) AZT was injected to 47 cases with various perinatal infections and it was more than "effective" in 45 cases with an efficacy rate of 95.7%. Also, all the 12 cases to which AZT was administered for prophylaxis of infections showed prophylactic effect. Bacterial eradication was obtained with 25 strains out of 29 aerobic Gram-negative bacteria, but 1 strain "persisted" and for 3 strains results were "unknown", hence an eradication rate of 96.2% was obtained. However, AZT treatment resulted in a little lower eradication rate against Gram-positive bacteria. (2) One case (1.3%) of minor degree of urticaria was found as a side effect, and one case each of eosinophilia and elevation of GOT, GPT and Al-P was observed as abnormal laboratory value. From the above results of pharmacokinetics and clinical evaluation, it has been concluded that AZT is a useful and highly safe drug in various perinatal infections and prophylaxis.

[Isotonic contraction of rabbit superior rectus muscle].

It is well known that the mammalian extraocular muscle has slow muscle fiber morphologically. But the contraction speed of slow muscle fiber has not been mentioned in previous reports. We studied the isotonic contraction of slow muscle fiber and compared it with fast muscle fiber. Each fiber bundle was isolated from the rabbit superior rectus muscle. Both light microscopic findings and the reaction to isotonic contraction in Ca2+ free Ringer's solution could identify each of the two muscle fiber types. Contraction speed depended on the frequency of stimuli (40-200 Hz) in both slow and fast muscle fiber. Maximum velocities of slow and fast fiber bundles were 4.3 +/- 1.53 mm/sec and 26.9 +/- 3.48mm/sec at 200Hz, respectively. It was suggested that the relaxation of slow muscle fiber did not interfere with the contraction of the fast muscle fiber. The relation between contraction velocity and afterload showed an approximately right angle hyperbolic curve.

[A case of surgical treatment of multiple bilateral pulmonary arteriovenous fistulas in a senile patient].

A 65-year-old woman was admitted with exertional dyspnea. She had a history of syncopal attack and cerebral infarction. On admission, her chest radiography ++ revealed two mass shadows in the right upper and lower fields. Pulmonary angiography and right heart catheterization were performed. Six arteriovenous fistulas in the right lung and five in the left were observed. The percent of R-L shunt was 51.5%. Because of exertional dyspnea, large shunt and history of cerebrovascular disorders, excision resection of two fistulas in right upper and middle lobes and right lower lobectomy were performed. Her symptoms improved and the percent of the R-L shunt markedly decreased after the operation. There is no sign of enlargement in size of the remaining arteriovenous fistulas.

[A case of extraskeletal Ewing's sarcoma of the chest wall].

A case of extraskeletal Ewing's sarcoma arising in the chest wall of a 15-year-old male is reported in this paper. His complaint was chest and back pain. Physical examination and laboratory data disclosed no abnormality. Radiographs and computed tomography revealed chest wall tumor, and resection of the tumor with a portion of the eighth rib was performed. Histologically, there was no involvement of the rib at all. The tumor tissue was composed of solidly packed tumor cells which were uniform in size and shape. Individual tumor cells were small, round-shaped, and had scanty, ill-defined cytoplasm. The diastase-digested, PAS positive material was found in the cytoplasm. NSE (neuron specific enolase) was positive, but EMA (epithelial membrane antigen) LCA (leucocyte common antigen), and keratin were negative. With these results, we got a diagnosis of extraskeletal Ewing's sarcoma. Combination chemotherapy with vincristine, cyclophosphamide, adriamycin, actinomycin D was administered after operation, and there is no evidence of recurrence after 12 months of follow-up.

Anatomical variation in mast cell nerve associations in the rat small intestine, heart, lung, and skin. Similarities of distances between neural processes and mast cells, eosinophils, or plasma cells in the jejunal lamina propria.

Several studies have indicated that mast cells occur in close proximity to enteric nerves in the gastrointestinal tract of rats, man, and other mammalian species, and such intimate associations have been proposed as one of the anatomical bases of communication between the immune and the nervous systems. However, the specificity of anatomical associations between enteric nerves and mast cells, as opposed to other bone marrow-derived or lymphoid cells normally present in mucosal sites, is unclear. We used transmission electron microscopy to quantify the distances between mast cells and neural processes (nerve terminals or axons) in the small intestinal mucosa, right atrium, skin, and pulmonary parenchyma of normal rats, and in the small intestinal mucosa and lung parenchyma of rats that had undergone hyperplasia of the mast cell populations in these sites as a result of infection with the nematode Nippostrongylus brasiliensis. In the jejunal mucosa of normal rats, 8.0% of mast cells occurred within 100 nm of neural processes and an additional 11.0% between 101 and 500 nm of these structures; the corresponding figures for eosinophils were 3.3% (N.S. vs. mast cell value) and 23.3% (p less than 0.05 vs. mast cell value) and for plasma cells were 8.5% and 14.6% (N.S. vs. mast cell values). In the right atrium, 1.2% of mast cells occurred within 100 nm and an additional 13.4% within 101 and 500 nm of neural processes, whereas no mast cells were observed within 500 nm of neural processes in the pulmonary parenchyma or ear skin. Infection with N. brasiliensis increased by 61% the proportion of mast cells within 500 nm of neural processes in the jejunal mucosa and resulted in the appearance of mast cells in close association with these structures in the jejunal muscularis propria, but had no effect on the proportion of mast cells within 100 or 500 nm of neural processes in the pulmonary parenchyma. Acetylcholine esterase staining demonstrated dense networks of neural processes in the three sites where some mast cells were closely associated with these structures (jejunal mucosa and muscularis, right atrium) but not in the pulmonary parenchyma or ear skin. Taken together, our findings indicate that mast cells occur in close proximity to neural processes in sites where these structures are abundant, but that anatomical associations as close as those between mast cells and neural processes can also occur between such structures and other bone marrow-derived cells (eosinophils) or lymphoid cells (plasma cells) resident in the small intestinal mucosa.(ABSTRACT TRUNCATED AT 400 WORDS)

[Clinical analysis of renal trauma].

Since the start of the urological service in our hospital in 1986, we have experienced six male and one female patients with renal trauma. These cases were clinically analyzed and compared with hepatic trauma treated during the same period in our hospital. There were two cases of renal contusion, laceration and injury to the pedicle, and one of hemorrhage into the cyst in polycystic kidney. The causes of the trauma were mainly traffic accidents. Among them, 4 patients had injuries to other organs such as liver and brain and to the thorax. Two cases were able to retain both their kidneys, but the rest were treated with single nephrectomies. Six of the seven patients are alive. This proved that computerized tomography was very useful to decide the severity of injury, presence of lesions to other organs, and the treatment method. No close relation was observed between the time from injury to operation and the amount of blood transfusion. Moreover, hematological and biochemical analysis did not express the patient's status.

[Transplacental transfer and clinical application of aztreonam].

Preclinical and clinical studies on aztreonam (AZT) in the perinatal period were carried out and the results are summarized below. 1. Concentrations of AZT in maternal serum, umbilical cord serum and amniotic fluid were measured after intravenous injection of AZT 1 g and 2 g, and intravenous drip infusion of AZT 1 g. As results, the transfer of AZT into umbilical cord serum started to increase in 1 to 2 hours, and the transfer of AZT into amniotic fluid started to increase after an elapse of 2 hours. Upon the intravenous injection of 2 g, AZT concentration in amniotic fluid was as high as 27.1 micrograms/ml even at 10 hours 30 minutes after the injection, and it was still 6.9 micrograms/ml at 20 hours or more after the injection. 2. AZT 2 g/day (b.i.d.) was administered by intravenous drip infusion to 1 perinatal infection case with pyelonephritis. It was clinically effective and neither side effect nor abnormal laboratory test value was observed.

Human granulocyte colony-stimulating factor specifically binds to murine myeloblastic NFS-60 cells and activates their guanosine triphosphate binding proteins/adenylate cyclase system.

The specific binding of human granulocyte colony-stimulating factor (G-CSF) to its receptors on NFS-60 cells acts as a primer for cellular proliferation. There are approximately 400 binding sites per cell, with a binding constant of about 100 pmol/L. Before the proliferative response, the affinity constant of the membrane particulate fraction to 35S-labeled guanosine triphosphate-gamma-S (35S-GTP gamma S) and the intracellular cyclic adenylate monophosphate (cAMP) level increased in the presence of G-CSF to about 2.5-fold and about fivefold higher, respectively, than the levels seen in the absence of G-CSF. The increases were time-dependent, with a peak occurring 15 minutes after the addition of G-CSF at 37 degrees C. These findings suggest that, following the binding of the G-CSF to its surface receptors, the activation of the guanosine triphosphate (GTP)-binding protein/adenylate cyclase system may be involved in the proliferation of immature myeloid cells.

Lens protein glycation and the subsequent degree of opacity in streptozotocin-diabetic rats.

We measured how much glycated protein there was in rat eye lenses with different degrees of cataract, using an antibody against glucitol-lysine. Streptozotocin-diabetic (STZ) rats were in some cases treated with insulin (STZ + INS); control rats were normal. We graded the cataracts from 0 (transparent) to 3 (entirely opaque). STZ rats had significantly more grade 3 cataracts, and STZ + INS rats more grade 1 cataracts, than other groups. Grade 3 lenses had significantly more glycated protein than those of grade 0 (10.8 +/- 2.7 vs. 1.0 +/- 0.4 nmol/mg protein), grades 1 and 2 being intermediate. Glycosylated hemoglobin levels correlated similarly with severity of cataract. These data are consistent with the greater incidence of cataract among diabetics than among non-diabetics, and suggest that lens protein glycation contributes to the development of cataract.

New crystal form of recombinant murine interferon-beta.

Although we have reported (Matsuda, S., Kawano, G., Itoh, S., Mitsui, Y., and Iitaka, Y. (1986) J. Biol. Chem. 261, 16207-16209) that recombinant murine interferon-beta produced in Escherichia coli was crystallized in an orthorhombic space group C222(1) using polyethyleneglycol 8000 as precipitant, the crystals had an insufficient resolution and a marked tendency for orientational disorder around the c axis. We now report that another form of murine interferon-beta crystals with little disorder was obtained in the presence of dioxane using ammonium sulfate as precipitant. The new crystals belong to a hexagonal space group P6(1) or P6(5) with a = b = 71.4 A and c = 79.6 A having only one murine interferon-beta molecule in an asymmetric unit. The crystals are reasonably stable to x-rays and significantly diffract up to 2.2 A resolution when a synchrotron beam is used.

Disseminated infection of Pneumocystis carinii in a patient with the acquired immunodeficiency syndrome.

This report describes the histopathology of a disseminated Pneumocystis carinii infection in a 24-year-old Japanese male haemophiliac diagnosed as having the acquired immunodeficiency syndrome. He developed respiratory symptoms, and Pneumocystis carinii pneumonia was confirmed by transbronchial lung biopsy. On the 70th day of hospitalization the patient died. Autopsy findings revealed Pneumocystis carinii not only in the lungs but also in the stomach, jejunum, ileum, colon, mesoappendix, abdominal lymph nodes, diaphragm, and thyroid gland.

Proliferative activity of autoimmune MRL mouse serum IgG to interleukin 3-dependent cell line through autocrine mechanism.

Serum of an autoimmune MRL/Mp-lpr/lpr (MRL/l) mouse supported the proliferation of interleukin 3 (IL-3)-dependent cell line, FDC-P2. This IL-3-like activity initially appeared at 1 month of age and increased with age. Females showed higher titers than did males. MRL/Mp-+/+ mouse sera also exhibited such activity, though somewhat later in life only in female. Other autoimmune mice, NZB, NZB/NZW F1, and BXSB, demonstrated no such activity in either males or females, young and old. The active component of MRL/l sera was shown to be IgG. F(ab')2 or Fc fragments of MRL/l-IgG lost such activity. Not all IL-3-dependent cell lines, however, responded to MRL/l-IgG. We subcloned MRL-IgG responding and nonresponding clones from FDC-P2 cells and both were still dependent to IL-3. Such nonresponding IL-3-dependent cell lines, however, could be stimulated by the culture supernatant of the responding cell line, FDC-P2/185-4, after being stimulated with MRL/l-IgG. In this culture supernatant, IL-3 was found, thus the existence of an autocrine system was suggested in the IL-3-dependent MRL/l-IgG responding cell line.

Induction of twinning in crossbred heifers by ipsilateral frozen embryo transfer.

Induction of twinning by means of nonsurgical ipsilateral transfer of two frozen embryos or of demi-embryos was attempted in 297 virgin crossbred dairy heifers that were kept at the following locations: in stables at our breeding station (Group 1), in a mountain field (Group 2) and at a private farm (Group 3). Of these, 187 heifers (63%) were diagnosed pregnant at 25 d after embryo transfer and 134 heifers (45%) were diagnosed pregnant at 60 d after transfer by ultrasonic echography or by palpation per rectum. At calving, 118 heifers produced 46 sets of twins and 72 single calves, for a total of 164 calves sixteen pregnant heifers aborted fetuses between 2 and 7 mo. The pregnancy rate of Group 1 (39%) was lower than that of Group 2 (48%) or Group 3 (50%). Abortion and mortality rates for Group 3 (8% and 8%, respectively) were significantly lower (P<0.05) than for Group 1 (16% and 12%) or Group 2 (13% and 18%). Twin calves had lower birth weights (P<0.05) than singles, but at 270 to 330 d of age, there was no significant difference between the weights of single and twin calves. Dystocia or difficult delivery was not observed heifers producing twins. Retention of the placenta was observed in twin calving heifers, with placenta retention levels the highest in Group 2 (33%) than followed by Group 1 (14%) and Group 3 (10%). When gestation periods were compared between single and twin calves in Groups 1 and 2, the gestation period of single calves was 5.0 to 7.0 d shorter than that of twin calves. No significant difference was observed between the gestation periods of single and twin births in Group 3.

Polyclonal increase of HTLV-I provirus-carrying lymphocytes in HTLV-I-carrier rabbits transplanted with Shope carcinoma cells.

Fifteen HTLV-I carrier rabbits of two inbred strains and their F1 hybrid were inoculated with Shope carcinoma cells. The peripheral blood leukocyte counts in all these rabbits increased to 2-4 times the preinoculation level with a concomitant increase in abnormal lymphocytes. Southern blotting analyses of peripheral blood leukocytes revealed the polyclonal integration of HTLV-I proviral genome in all these animals. However, lymphocytic infiltration without any apparent leukemic infiltration was observed in major organs of these animals; a similar but slight lymphocytic infiltration was also observed in non-carrier control animals inoculated with Shope carcinoma cells. These results are interpreted as indicating that the growth of squamous cell carcinoma in HTLV-I carrier rabbits induced the polyclonal expansion of provirus-carrying cells, which may correspond to the intermediate state between healthy carrier and smouldering ATL.
