In vitro maturation of mononuclear phagocytes and susceptibility to HIV-1 infection.

In the present report, we have studied the in vitro transition of normal blood monocytes to macrophages by changes in cell morphology, and the expression of surface antigens with a panel of monoclonal antibodies. The maturation process was accompanied by notable changes in cell-surface markers in a time-dependent manner. The percentage of cells expressing CD11c, ICAM-1, HLA-DR, and Fc receptor class III increased while the CD4 and CD35 expression was markedly decreased. After demonstrating that in vitro monocytes mature to macrophages in a recognizable manner, we studied the susceptibility to HIV-1 infection at time points representing different stages of cell maturation. The results show that monocyte/macrophages are susceptible to HIV-1 infection at all stages of differentiation. However, the kinetics of virus replication depends on the degree of maturation at the time of infection. Two major patterns of replication were observed: Infection of monocytes resulted in efficient virus production measurable by reverse transcriptase activity in culture supernatant, whereas infection of fully differentiated macrophages yielded low but sustained virus release only demonstrable by p24 antigen assay. We were not able to detect differences in the capacity of the virus to infect and replicate in monocyte/macrophages with respect to cellular origin of the virus isolate and whether the viruses were laboratory-adapted strains or low-passaged patient isolates.

Species-specific differences in heterogonic development of serially transferred free-living generations of Strongyloides planiceps and Strongyloides stercoralis.

The direction of free-living development (homogonic vs. heterogonic) in Strongyloides stercoralis and Strongyloides planiceps was examined by successive transplantation of the uterine eggs of free-living females into a test tube culture system containing fresh feces. The eggs from the first-generation free-living females of S. stercoralis did not develop into second-generation free-living adult worms, but all developed into filariform larvae. However, the majority of S. planiceps eggs from the first-generation free-living females developed into second-generation free-living adults. By successive transfer of uterine eggs of each generation, 9 generations developed, and in every cycle more adult worms developed than filariform larvae. However, the number of free-living generations was not infinite; in experiments repeated twice, the number of worms developing from the eggs of eighth or ninth generations was too small to continue further culture. These findings indicate that the pattern of free-living development is different between the 2 species.

[A case of left main bronchial rupture due to bronchocath tube].

A 68-year-old woman underwent a right upper lobectomy for lung cancer. After resection, we noticed the left main bronchial rupture due to bronchocath tube (polyvinyl chloride double lumen tube). The lesion of the rupture was repaired by interrupted sutures with 4-0 prolene. Subsequent course of the patient was uneventful. Tracheobronchial rupture is rare complication of intubation with polyvinyl chloride double lumen tube. There are 6 cases of this complication last five years in Japan. The cause and prevention of this complications are described. It is important to use an adequate tube size, to prevent malposition of the tube and overinflation of the bronchial balloon.

[Pharmacokinetic and clinical studies of flomoxef in perinatal period].

Pharmacokinetic and clinical studies of flomoxef (FMOX) in perinatal period were carried out and the following results were obtained. 1. Concentrations of FMOX in maternal serum, umbilical cord serum and amniotic fluid were determined subsequently to intravenous injection (4 cases) and intravenous drip infusion method (20 cases) of 1 g FMOX. Maternal serum levels were similar to those of healthy adults, and peak levels of umbilical cord sera and amniotic fluids were 12.0 micrograms/ml and 12.05 micrograms/ml, respectively, using intravenous drip infusion. The levels in amniotic fluids were higher than those in umbilical cord sera at 2 hours after treatment in either administration method. Parameters T 1/2 (beta) and AUC were 1.05 hours and 74.1 micrograms.hr/ml, respectively. 2. In the treatment of 4 cases with perinatal infection and in prophylaxis cases, clinical efficacies of FMOX were all good with 1 g twice daily treatment using intravenous drip infusion. No side effects nor abnormal laboratory test values due to the drug were observed in any cases. These results indicate that single intravenous drip infusion of FMOX 1-2 g twice daily is effective for the treatment and the prophylaxis of perinatal infections.

[A case of cardiomyopathy due to allergic bronchopulmonary aspergillosis].

We report a rare occurrence of cardiomyopathy associated with allergic bronchopulmonary aspergillosis (ABPA). A 49-year-old man with a history of bronchial asthma was referred to the Matsuyama Red Cross Hospital for evaluation of the abnormal shadow on his chest X-ray. Laboratory examination showed blood eosinophilia and marked elevation of serum IgE concentration and IgE antibody to A fumigatus. The immediate and late skin reactivities to A fumigatus antigen were both positive. The diagnosis of ABPA was made. Treatment using prednisolone was effective in ameliorating the symptoms. However, he was admitted again due to dyspnea, edema and anorexia 6 months later. Chest X-ray, ECG, UCG and scintigraphy suggested severe cardiac failure. The clinical diagnosis of hypertrophic cardiomyopathy, and the pathohistological diagnosis of endomyocardial fibrosis were made by cardiac catheterization and biopsy of endocardium. Retrospectively, cardiomegaly had gradually increased during the past several months while peripheral blood eosinophilia had continued. All these data strongly suggested that eosinophilia due to ABPA might cause severe cardiac damage.

[Clinical evaluation of fleroxacin in gynecological infections].

Fleroxacin (FLRX), a new quinolone oral antibacterial agent, was studied in the field of obstetrics and gynecology, and the following results were obtained. 1. Clinical efficacy was evaluated in 105 patients (intrauterine infection 38, adnexitis 28, extragenital organ infection 29, others 10), with an exclusion of 9 patients out of a total of 114 patients. FLRX was orally administered at 200 mg or 300 mg once daily. 2. Clinical efficacy rates were 37/38 (97.4%) in intrauterine infection, 26/28 (92.9%) in adnexitis, 29/29 (100%) in extragenital organ infection and 10/10 (100%) in others. 3. Efficacy rates classified by isolated organisms were 23/23 (100%) in single infections by Gram-positive organisms, 11/13 (84.6%) in those by Gram-negative organisms, 8/9 (88.9%) in those by anaerobic organisms and 15/19 (78.9%) in mixed infections. 4. Side effects were observed in 4 cases (3.5%); gastrointestinal disorder with diarrhea and diarrhea in 1 patient each and insomnia in 2 patients. In laboratory examination, eosinophilia and elevation of GOT and GPT were noted in 1 patient each (1.9%). Based on the above results, we consider that FLRX is a useful drug for the obstetrical and gynecological infections.

Protective effects of calcium antagonist (nitrendipine) on calcium ionophore A23187-induced liver cell injury.

Calcium ionophore A23187-induced liver damage was studied in perfused rat liver and isolated hepatocytes. Two groups of calcium antagonists, dihydropyridine-type and nondihydropyridine-type, were examined for their protective effects on A23187-induced liver damage. The former calcium antagonist inhibited cell damage at pharmacological doses, whereas nondihydropyridine-type calcium antagonists were unable to prevent A23187-induced cytotoxicity. Different inhibitors, such as cAMP phosphodiesterase inhibitor OPC3689, calmodulin inhibitor W-7, and C-kinase inhibitor H-7, were tested in isolated hepatocytes to determine whether intracellular signal transduction systems are involved in the liver cell injury produced by A23187. Calcium ionophore A23187-induced LDH activity of the medium was depressed by W-7 and H-7 to 55 +/- 4% and 63 +/- 4% of controls (p less than 0.01), respectively. However, OPC3689 did not show a protective effect. We conclude that A23187-induced liver cell injury was inhibited by dihydropyridine-type calcium antagonists which may interfere with activation of calmodulin and C-kinase.

[A case of localized pleural mesothelioma].

A case of localized pleural mesothelioma is reported. A 49-year-old man was operated for coin lesion detected on routine chest X-ray. A tumor arose from the visceral pleura of right lower lobe and was pedunculated. Size of the tumor was 4 x 3 x 3 cm. Cut surface of the tumor was solid and yellowish white. Pathological diagnosis was fibrous type of the pleural mesothelioma. The patient is well and has no sign of recurrence for three years. There are 123 cases of localized pleural mesothelioma including our case in Japan. Five in 78 cases which was diagnosed to be benign histologically at operation, had recurrence later. It is important to follow up strictly for long term.

Basic fibroblast growth factor suppressed the enhancement of choline acetyltransferase activity induced by nerve growth factor.

We investigated change of choline acetyltransferase (ChAT) activities in rat embryonic (16 days) septal neuron culture in treatment with basic fibroblast growth factor (bFGF) and nerve growth factor (NGF). Total ChAT activity increased by addition of bFGF, NGF, and bFGF plus NGF with dose-dependent manner. NGF showed much enhancement of specific ChAT activities per mg protein, but bFGF or bFGF plus NGF, respectively showed little or slightly enhanced ChAT activities. In histochemical studies with anti-ChAT antibody staining, cholinergic neurons in NGF-treated culture were stained more strongly than those in other conditioned cultures such as control, bFGF-treated, and bFGF plus NGF-treated cultures. These results suggest that bFGF enhances total ChAT activity but not cellular ChAT activity and further suppresses the enhancement of cellular ChAT activity induced by NGF in septal neuron culture.

Effect of basic fibroblast growth factor on neurons cultured from various regions of postnatal rat brain.

Neurons from various brain regions of postnatal (15 days after birth) and fetal (16 days gestation) rats were cultured in the presence of basic fibroblast growth factor (bFGF). bFGF increased the survival of neurons from postnatal septum, striatum, midbrain, and hippocampus. Fetal neurons derived from cerebral cortex, septum, striatum, midbrain, thalamus, and colliculus were far more dependent on bFGF for survival in comparison with postnatal neurons. In contrast, cerebellum neurons of postnatal and fetal rat brain did not respond to bFGF. The increase of postnatal and fetal neuronal survival with bFGF treatment (0.01-10 ng/ml) was dose-dependent and reached 2-4-fold and 5-10-fold more than the control, respectively. Fetal cortical neurons showed almost complete dependence on bFGF since almost all neurons died in control cultures. Nerve growth factor was slightly effective only on postnatal septal and striatal neurons, being ineffective on the other neurons tested. These results indicate that bFGF can function as a neurotrophic factor not only on fetal but also on postnatal neurons of the central nervous system, and that bFGF has great potential for application in vivo.

[Analysis of factors associated with the occurrence of low birth weight infants].

The relationship between low birth weight infants (LBW) and 3 perinatal factors; pre-pregnant maternal Body Mass Index (BMI: Quetelet's index), maternal height and parity was investigated by the log-linear analysis, using the birth records of an obstetric facility at Naha city. In this facility there were 4,894 deliveries from Jan. 1978 to Dec. 1987. Of the 4,894 infants, 4,142 were live singletons with available data for the analysis. The odds ratio of each risk factor was calculated by the log-linear model. Women with lower pre-pregnant BMI (less than 20) had an increased LBW risk by 1.80 (95% CI: 1.32-2.43) compared to women with higher pre-pregnant BMI (20 less than). Lower maternal height (150 cm greater than) was associated with significant LBW risk (1.47; 95% CI: 1.03-2.12). Primiparae had higher LBW risk than multiparae (1.68; 95% CI: 1.25-2.28). In this analysis we evaluated the interaction effects on LBW occurrence of the three factors and found no interaction effects among them.

Granulocyte colony-stimulating factor stimulates human mature neutrophilic granulocytes to produce interferon-alpha.

Granulocyte colony-stimulating factor (G-CSF) is a glycoprotein hormone that specifically stimulates both production and functional activation of neutrophils, while interferon-alpha (IFN-alpha) is known to suppress myelopoiesis, including neutrophil production in vivo and in vitro. On a possibility that IFN-alpha may operate as one of the inhibitory feedback factors in neutropoiesis, we examined whether neutrophils produce IFN-alpha in response to G-CSF. Northern blot analysis showed that messenger RNA (mRNA) for human IFN-alpha 1 became detectable time-dependently in highly purified human neutrophils incubated with purified recombinant human G-CSF (rhG-CSF). But such transcription was not observed either in neutrophils incubated with other neutrophil activators, such as formyl-methionyl-leucyl-phenylalanine (fMLP) or lipopolysaccharides (LPS), or in blood mononuclear cells incubated with rhG-CSF. In addition, radioimmunoassay for human IFN-alpha showed that its levels in culture medium of the rhG-CSF-treated neutrophils rose markedly (up to approximately 100 IU/mL/1 x 10(7) cells) in a time-dependent way, compared with those of nonstimulated neutrophils. These findings suggest that the G-CSF/IFN-alpha system may participate in the feedback regulatory loop of neutropoiesis.

Scanning electron-microscope observations of the perikaryal projections of rabbit spinal ganglion neurons after enzymatic removal of connective tissue and satellite cells.

The true surface of rabbit spinal ganglion neurons has been made directly accessible to scanning electron-microscope observation after removal of both the connective tissue and satellite cells that normally cover it. The neuronal surface is characterized by a profusion of slender projections whose shapes have been determined and whose length and width have been quantified. Controls carried out with transmission electron microscopy demonstrate that the procedure employed in this study satisfactorily preserves neuronal structure.

Production and characterization of a fragment containing the HIV-gp120 binding region of CD4 using a bovine papilloma virus (BPV) vector.

We have used a bovine papilloma virus (BPV) based mammalian cell expression vector consisting of the complete BPV genome and a human cytomegalovirus transcription unit for the production of soluble CD4. Mouse C-127 cells were transfected with vector DNA together with a selectable G418 resistance plasmid. Surviving clones were selected for high production using a solid phase ELISA based on the immobilization of supernatant-derived CD4 onto nitrocellulose paper and subsequent detection with anti-CD4 antibodies. The expressed protein was shown to bind HIV-gp120 and efficiently block HIV-1 infection in vitro. The possibility to use the above system for rapid production of defined glycoprotein fragments harboring defined functional regions, for the further elucidation of the functional role of CD4 in antigen presentation and cell to cell contact, and for possible intervention during HIV infection is discussed.

Characterization of the in vitro maturation of monocytes and the susceptibility to HIV infection.

The in vitro maturation of peripheral blood monocytes to macrophages can be followed morphologically, and by measurement of cell surface antigens (CD4, HLA-DR, and FcR III) and lysozyme production. We used these markers to correlate monocyte maturation with susceptibility to human immunodeficiency virus (HIV) infection. Maturation of peripheral blood monocytes is associated with a decrease in membrane CD4, while HLA-DR and FcR III expression increase along with lysozyme secretion. Cells at all stages of maturation were susceptible to HIV infection, even mature macrophages without CD4 detectably by immunofluorescent staining. Maximal replication was observed in 7-day-old cells.

Bromodeoxyuridine labeling studies on the proliferation of intestinal mucosal mast cells in normal and athymic rats.

The proliferation of mucosal-type mast cells (MMC) in rat small intestine was studied using a bromodeoxyuridine (BrdU)-labeling method. After 24-h cumulative injections of BrdU into adult SD rats, 3.5% of MMC were labeled, while only 0.3% and 0.1% of mast cells were labeled in back skin and ear respectively. From the results, it was concluded that MMC division occurred more than 10 times as frequently as the division of skin mast cells. Similar results were obtained in athymic adult rats (F344/N Jcl-rnu) in which the number of MMC was similar to that in heterologous animals. Thus, thymic factor(s) or T cells may not have an important role in MMC division in normal states. When SD rats were infected with Nippostrongylus brasiliensis, vigorous proliferation of MMC was brought about 13 to 15 days after infection. At that period, 40% of MMC were labeled by a single injection of BrdU, and 85% of MMC were labeled by 9-h cumulative injections of BrdU, with the result that most MMC rapidly proliferated in the intestinal mucosa during this period. Mitotic figures of MMC were sometimes observed. On the contrary, hyperplasia of MMC was not observed in athymic rats infected with nematodes. Therefore, MMC hyperplasia after nematode infection is dependent on thymic factor or T cells, and its mechanism is different from that of MMC division in normal states, in which thymic factor(s) or T cells are not essential.

[Secular changes of the mean birth weight and the mean gestational day at an obstetric clinic in Naha].

In our previous study we reported that the secular change of the mean birth weight (MBW) of Okinawa was different from that of Japan as a whole, that is, the MBW of Okinawa was lower, but the annual increase rate of the MBW of Okinawa was larger than that of Japan as a whole. Through analyses using birth records of several obstetric facilities in Naha, we concluded that this phenomenon must be due to the remarkable improvement of the socioeconomical environment of Okinawa. In this report we analyzed secular changes of mean birth weight (MBW) and mean gestational day (MGD) of 15,847 singleton babies born at an obstetric clinic in the city of Naha for the 33 years from 1955 to 1987. To avoid fluctuations of annual changes in MBW and MGD caused by a small number of babies born in a single year, we also used the moving average of three years. In analyzing birth weight we stratified birth weight by sex, parity and the number of gestational weeks. It is considered that birth weight stratified by the number of gestational weeks reflects the intrauterine growth rate of infants. According to our results, both the MBW of infants with 40 gestational weeks, which reflects the intrauterine growth rate, and the MGD increased during the period from the 1950s to the early 1970s in this clinic. On the basis of these results we concluded that the increase of birth weight in this clinic until the early 1970s was due to increases in both the intrauterine growth rate and the gestational period. Our results also show that the mean height of mothers who delivered at this clinic also increased, which might reflect the development of the socioeconomical environment during their childhood. During the period when MBW increased, the economic growth rate of Okinawa was higher than that of other prefectures. From these results we concluded that the MBW can be used as a community health indicator reflecting the total health condition in a given community.
