[Dysgerminoma of the ovary: a clinical retrospective analysis of 60 cases].

OBJECTIVE: To study the treatment methods and the prognostic factors of pure dysgerminoma of the ovary. METHODS: Sixty cases histologically confirmed pure ovarian dysgerminoma treated in our hospital, from May 1959 to Oct. 1992 were retrospectively analysed. Factors mainly studied were: age, symptoms, clinical staging and treatment methods. RESULTS: The median age of this series was 21. The main symptoms were abdominal mass, pain and distension. Thirty two out of 60 cases were stage I, 2 were stage II, 5 were stage III and 21 with recurrences or persistence of the disease. All cases excepting one case received surgery alone were treated by surgery combined with external radiotherapy or chemotherapy. Nine cases died of the disease. The actual 5-year survival rate was 85.17%. The survival rate for patients with postoperative adjuvant chemotherapy is 95% and that for patients with radiotherapy is 76.1%; no statistic significance was shown. CONCLUSIONS: In pure ovarian dysgerminoma surgery combined with radiotherapy or chemotherapy showed high complete remission rates and excellent survival rates. For younger patients and those with gestation desires as well as patients with advanced diseases, adjuvant chemotherapy following surgery might be a better choice.

[Long-term effect of extended parietal cell vagotomy on patients' nutrition and digestive function].

In this study, 36 patients undergoing extended parietal cell vagotomy (EPCV) were compared with 30 patients with subtotal gastrectomy 7-14 years after surgery. The patients' nutritional status, gastric sectretion, gastrointestinal motility and intestinal absorption function were evaluated. The nutritional status was generally less satisfactory in 33. 3% patients with anaemia after gastrectomy and decrease of body weight in 54%. The patients after EPCV were all in good nutritional status, with no anemia; 56% of them had a body weight increase of more than 3kg. Therefore, the rationality of EPCV was estalished.

[Intra- and extra-cranial arterial lesions in TIA patients].

Ninety-six patients with transient ischemia attacks were examined by transcranial Doppler (TCD), carotid color-coded ultrasonic imagine, brain computed tomographic scan (CT), compared with digital subtract angiography (DSA), MRI and MRA were done in some cases. The abnormality of CT and MRI was 30.2% and 74.1%, respectively. The most common findings were lacunar infarction or cerebral atrophy. Sixty-two cases fulfilled the TCD criteria of definitive arterial stenosis (64.5%) and three cases of arteriovenous malformation, among whom 27 patients (43.5%) carried out the examination of DSA. There was an excellent coincidence between the DSA and TCD findings, although DSA showed more vessels involved in some patients. Of the 96 cases, the intracranial arterial stenosis or occlusion was in 48.5% and the extracranial in 17.8%. The younger patients (age < 60 years) were subject to involvement of unilateral intracranial artery, but the older had predominantly the bilateral intracranial arterial and extracranial carotid or subclavian arterial lesions.

Multimodality therapy including surgical resection for limited small cell lung cancer.

From 1975 through 1990, 199 patients with limited small cell lung cancer (LSCLC) were subjected to multimodality treatment including surgical resection combined with chemotherapy or chemoradiotherapy in our department. The median postoperative survival time of the 199 patients was 39 months, and the 5-year survival rate was 26%, which was decreased with increase of tumor-stage. In comparison of the survival time of patients in Stage I and those in Stage IIIa, there was a significant difference (P < 0.01). There were no significant differences in survival rate of 3 and 5 years between the patients receiving chemotherapy prior to or after surgical resection. The improvement in survival was documented by surgical resection combined with chemotherapy or chemoradiotherapy for LSCLC. The effect of multimodality treatment is correlated with tumor P-TNM staging, the involvement of lymph node, especially that of the mediastinal lymph node, is a negative factor influencing the prognosis. Surgical resection is an initial management, followed by chemotherapy or chemoradiotherapy may be indicated in LSCLC patients of Stage I, Stage II and some Stage IIIa as the cancer can be resected completely.

Proliferative inhibition of rabbit lens epithelial cell--preliminary investigation for prevention of after cataract.

PURPOSE: To study the ability of Homoharringtonine (Hh), 5-Fluorouracil (5-Fu), and Adriamycin (ADM) on inhibiting the proliferation of rabbit lens epithelium. METHODS: Whole rabbit lenses were removed from freshly enucleated eyes under sterile condition. The rabbit lens epithelia (RLE) were isolated and cultured: (1) The passage RLE were placed in 24-well tissue culture plates and incubated for 48 hours, then exposed to different concentrations of Hh, 5-Fu, and ADM for 24 and 72 hours; (2) The passage RLE and Hh (0.084 microgram/ml), 5-Fu (0.058 microgram/ml), ADM (0.45ng/ml) were placed and cultured for 24 hours; (3) The morphological changes of RLE exposed to different concentrations of Hh, 5-Fu and ADM were studied under light microscope. RESULTS: The ID50 of Hh, 5-Fu and ADM exposed to RLE for 24 hours were 0.84 microgram/ml, 0.58 microgram/ml and 4.50ng/ml, respectively, and those for 72 hours were 0.49 microgram/ml, 0.33 microgram/ml and 3.85ng/ml. The attachment rate of RLE after being cultured for 24 hours with Hh, 5-Fu and ADM were respectively 83.6%, 89.1% and 87.3%. The morphological changes of RLE demonstrated that obvious changes in the cell membrane and cytoplasm were found even in lower concentration, but changes in the nuclei could only be found in higher concentration of these drugs. CONCLUSION: Hh can not only inhibit the proliferation of RLE but also reduce the number of attached cells. It is suggested that Hh may be more useful for the prevention of after cataract than 5-Fu and ADM.

The relation of the location of haptics of posterior chamber intraocular lenses and peripheral anterior synechia.

PURPOSE: To investigate the occurrence, outcome and influencial factors of the peripheral anterior synechia (PAS) following implantation of posterior chamber intraocular lenses for finding a way to reduce the PAS. SUBJECTS: 40 eyes of 38 senile cataract patients with normal chamber angle and intraocular pressure (IOP) preoperatively were examined. METHODS: Extracapsular cataract extraction was performed under microscope with insertion of a posterior chamber lens implant vaulted anteriorly by 10 degrees. Gonioscopy and slit-lamp examination and photography of the operated eyes were performed 3 to 6 months postoperatively. RESULTS: PAS were found in 20 (50%) of the 40 eyes. Among the 20 eyes with PAS the locations of 23 haptics in 17 eyes corresponded with those of the PAS. PAS were seen more frequently with vertically sulcus-fixated haptics than with the horizontally capsular-fixated haptics, showing a significant difference (P < 0.05). 88% of the eyes with PAS had pupillary deformation, but their visual acuities and IOP were not affected. CONCLUSIONS: PAS is more likely to occur with vertically sulcus-fixated IOL. Rotating the capsular-fixated haptics to the horizontal position may reduce the incidence of PAS. Gonioscopy should be a routine follow-up examination.

Pandysautonomia: clinicopathological. Report of 5 cases.

Five cases of pandysautonomia were reported. Pupils, salivary and sweat glands, gastrointestinal tract, urinary bladder and heart were impaired in all cases, indicating that the peripheral parts of both sympathetic and parasympathetic nervous systems were involved, especially the cholinergic post ganglionic efferent fibers. Besides, two cases showed sensory impairment of the distal parts of the extremities, slight atrophy of small hand muscles and weak or absence of deep tendon reflexes. Three cases showed elevation of CSF protein without cellular reaction. Sural nerve biopsy was performed in 3 cases. Neuropathological changes included the reduction of myelinated neurofibers, atrophy of Schwann's cells and the presence of degeneration products and myelin-like structures. One case also revealed small vasculitis among neurofascicles. It seems that the disease entity is a result of autoimmune dysfunction. The prognosis of pandysautonomia in the present group was good, four cases have some improvement during 1-4 years of follow-up study.

[Homology of the recombinant DNA of plasmid pDJH2 with the recombinant DNA probe of L. alstoni and analysis of its expression in Escherichia coli].

Dig-labeled recombinant DNA probe of L. alstoni which contains the entire structural OmpL1 gene was hybridized with the recombinant DNA of the plasmid named pDJH2 of the gene library of L. interrogans serovar lai strain 017. The result showed a high degree of homology among them; expression of recombinant DNA of pDJH2 was achieved by beta-D-galactosidase (IPTG) induction in E. coli. The molecular weight of this product is 68kd. Then they were treated with proteinase K and subjected to SDS-PAGE. The results showed it is a protein in nature. Using the specific monoclonal antibody E4B7D5 on immunoblotting and specific polyclonal antibody on dot-ELISA assay, we investigated the immune reaction and noticed that protein 68kd might be an antigen in character. E. coli which contains the recombinant plasmid pDJH2 were injected into BALB/c mice. Then the mice were challenged by leptospires of the strong virulence strain 017, but all the infected mice survived. In this paper, we first report the expression of recombinant DNA of L. interrogans serovar lai strain 017 in E. coli when injected with IPTG, and immunoprotection of BALB/c mice which were injected with the expression against the infection of L. interrogans serovar lai strain 017. pDJH2 may be the first recombinant for which the gene has been cloned and its expression product 68kd may be the immunoprotective antigens.

A chimeric tRNA(Lys3)-ribozyme inhibits HIV replication following virion assembly.

Co-localization of ribozymes with their appropriate target is one method utilized to increase their effectiveness in vivo. Effective antiviral ribozymes will likely rely on mechanisms which direct the ribozyme to the genomic or subgenomic RNAs. Exploiting the fact that a specific host cellular tRNA primer is bound by viral proteins and co-packaged with viral genomes in newly synthesized virions, ribozymes were fused to the 3'-terminus of tRNA(Lys3) in an attempt to direct their activity to cleave the HIV-1 genome. This chimeric ribozyme is catalytically active in vitro, and is efficiently recognized and bound by HIV-1 reverse transcriptase with affinities similar to tRNA(Lys3). The intragenic RNA polymerase III promoter entity of the tRNA allows for high levels of expression of the tRNA-RBZ and the preferential localization of transcript within the cytoplasm in transfected cells. This ribozyme was effective in reducing the infectivity of a viral stock which was produced from transiently transfected cells bearing the chimeric gene. These results demonstrate the feasibility of using tRNAs as a means of co-localizing ribozymes with their viral genomic RNA targets. The possibility exists to fuse stable RNAs to ribozymes as a means of increasing their stability and localizing them to their appropriate target sites.

[Experimental study on targeting therapy of human hepatocellular carcinoma in nude mice using adriamycin-anti-transferrin receptor conjugate].

Conjugate between adriamycin and anti-transferrin receptor monoclonal antobody WuT9 was prepared with Dextran T10 as a bridge, and it was proved to retain both the drug cytotoxity and antibody activity. The conjugate or its individual components were administered to athymic mice bearing SMMC-7721 human hepato-cellular carcinoma (HCC) cells. These agents were given 2, 5, 8 days after inoculation of tumor cells (1 x 10(7) per mouse). Tumor size was measured daily (geometric mean diameter, GMD) and latent period recorded. All the animals were killed on day 35 to measure tumor weight and natural killer (NK) activity of splenic lymphocytes. The results showed that the latent period in the conjugate treated group was much longer than that in Adr treated group (33.0 days vs 25.0 days), and that GMD on day 35 in the conjugate treated group was much smaller than that in Adr treated group (3.80 +/- 1.9 mm vs 6.69 +/- 2.3 mm, P < 0.05). In addition, NK activity in the conjugate treated group was higher than that in Adr treated group (28.2 +/- 8.2% vs 15.1 +/- 3.6%, P < 0.05). The results suggest that adriamycin-anti-transferrin receptor conjugate might be useful in the treatment of human HCC.

[Evaluation of combined test of sequential fecal occult blood and albumin in the screening of colorectal neoplasms].

Fecal albumin test was developed and the combined use of sequential occult blood test and albumin test (SA) was evaluated and compared with sequential fecal occult blood test (S) in 365 cases diagnosed clinically and pathologically and in 883 asymptomatic individuals. In the former group, the sensitivity of detection of colorectal cancer was: 50.7% (37/73) with S and 87.7% (64/73) with SA (P < 0.001), and that of the adenoma was: 8.1% (6/74) with S and 36.5% (27/74) with SA (P < 0.001). In the latter group 2 of 3 cancer patients were detected with S while all with SA, and the detection rate of adenoma was: 4.7% (4/85) with S and 42.4% (36/85) with SA (P < 0.001) it is suggested that the additional use of fecal albumin test is of help in detecting precancerous inflammatory bowel lesions without bleeding.

Improved antitumor activity of cis-Bis-neodecanoato-trans-R,R-1,2- diaminocyclohexaneplatinum (II) entrapped in long-circulating liposomes.

Cis-bis-neodecanoato-trans-R, R-1,2-diaminocyclohexaneplatinum (II) (NDDP) has previously been formulated in conventional liposomes and shown to be nonnephrotoxic in humans, not cross-resistant with cisplatin in different in vitro and in vivo systems, and more active than cisplatin against murine models of experimental liver metastasis. The activity was attributed to the avid uptake of liposomes by the liver. To extend to the treatment of solid tumors outside the liver, NDDP was formulated, in this study, in long-circulating liposomes composed of egg phosphatidylcholine (PC), cholesterol (Chol) and polyethyleneglycol conjugated to phosphatidylethanolamine (PEG-PE). In vitro, PC/Chol/NDDP liposomes were barely toxic to murine melanoma cells B16-F0 (IC50 = 195.6 microM) as evaluated by tetrazolium dye colorimetric assay. Inclusion of PEG3000-PE into PC/Chol/NDDP liposomes significantly enhanced their cytotoxicity to a level that was comparable to that of DMPC/DMPG/NDDP liposomes (7.3 vs. 7.9 microM). Biodistribution study indicated that PC/Chol/PEG3000-PE/NDDP liposomes could preferentially localize in s.c. melanoma in C57BL/6 mice. PC/Chol/PEG3000-PE/NDDP liposomes were much more efficient in inhibiting tumor growth in vivo than free cisplatin, free NDDP or NDDP formulated in liposomes of other lipid composition. Compared with cisplatin, the in vivo toxicities of PC/Chol/PEG3000-PE/NDDP liposomes were also significantly reduced. Furthermore, if the tumor was treated with local hyperthermia after injection of PC/Chol/PEG3000-PE/NDDP liposomes, the tumor uptake of liposomes increased by 60%. The tumor inhibitory effect of PC/Chol/PEG3000-PE/NDDP was also significantly improved when combined with local hyperthermia.

[Computed sonography observation on the effects of cimetidine on the liver blood flow in cirrhotic patients with portal hypertension].

The effects of cimetidine on the hepatic and portal vein blood flow were investigated in 20 cirrhotic patients with portal hypertension by computed sonography. Cimetidine given intravenously at a dose of 0.4g significantly increased the hepatic vein blood flow (60.2% +/- 63.6%, P < 0.01), at the same time its calculated cross section area increased by 40.2% +/- 81.3% (P < 0.01). The portal vein blood flow increased by 53.2% +/- 52.3% (P < 0.01). While its cross section area was 16.8% +/- 17.3% (P < 0.01). These results showed that the histamine H2-receptor antagonist cimetidine may dilate liver vascular bed and reduce portal blood flow resistance in cirrhotic patients with portal hypertension.

[An experimental study on HSV-1 corneal latency by in situ nucleic acid hybridization].

An experimental study was performed on animal models for evaluation of the possibility of herpes simplex virus-1 (HSV-1) corneal latency by in situ nucleic acid hybridization. 20 normal New Zealand white (NZW) rabbits were used, and 3 x 10(6) PFU/ml of McKrae strain HSV-1 was inoculated bilaterally into the corneal stroma in 14 rabbit eyes. Of the 28 eyes, 22 developed typical herpes simplex keratitis (HSK). On the postoperative 60th day, 4 corneas with latent infection were transplanted into one unilateral eyes of each 4 non-infected NZW rabbits respectively and removed 2 weeks postoperatively. The corneal buttons were individually detected for the presence of HSV-1 antigen and nucleic acid sequences by using clonal IgG HSV-1 antibody and biotinylated HSV-1 DNA probe respectively. The results showed that the HSV-1 DNA sequences retained only within the corneal stromal layer with negative HSV-1 antigen staining. These results strongly suggest that the cornea be capable of harboring latent HSV-1.

Ultrastructure of parietal cells before and after extended parietal cell vagotomy in patients with duodenal ulcer and their complications.

Since 1979 we have treated 149 patients with duodenal ulcer by extended parietal cell vagotomy (EPCV). Electron microscopy of parietal cells from gastric body mucosa and gastric acid secretory test were carried out in 22 randomly selected patients before and after EPCV. It was shown that before EPCV the ultrastructure of parietal cells presented the feature of vigorous section identical to the hyperfunctional secretion of gastric acid. The ultrastructure of parietal cells presented the feature of acute depression of secretory function, which was identical to a significant reduction of secretory function of gastric acid 0.5-6 months after EPCV. Two to 10 years after operation, however, especially over 10 years after EPCV, the ultrastructural changes of parietal cells gradually returned to normal, but gastric acid secretion remained at a low level, without preoperative secretory capacities. The results of this study provide a histological basis for the application of extended parietal cell vagotomy in the treatment of duodenal ulcer and its complications.
