[Effect of somatostatin on portal and hepatic hemodynamics and portal venous pressure in cirrhotic patients with portal hypertension].

OBJECTIVE: To investigate the effect of somatostatin on hepatic and portal hemodynamics and portal venous pressure in cirrhotic patients with portal hypertension. METHOD: Colorful doppler ultrasonography was used to measure the diameter and maximum hemokinetic velocity of portal vein and hepatic vein before and after intravenous administration somatostatin, and thus their hemokinetic volume was calculated respectively. Among 15 surgical patients, changes of the portal venous pressure were observed via a catheter before and after the administration of somatostatin. RESULT: One hour and 1.5 hour after the administration, the pressure of portal vein significantly decreased from 2.77 +/- 0.26 kPa to 2.42 +/- 0.27 kPa and 2.40 +/- 0.27 kPa, respectively (P < 0.01). The systolic pressure, diastolic pressure and heart rate showed no significant changes. After the administration, the sectional area of portal vein decreased by 7.28% but without significance. The maximum hemokinetic velocity of portal vein significantly decreased by 18.96% (from 19.72 +/- 7.75 cm/sec to 15.98 +/- 7.26 cm/sec, P < 0.01). The mean total hemokinetic volume increased from 1786.22 +/- 926.37 ml/min to 1836.17 +/- 844.24 ml/min, but without significance (P > 0.05). CONCLUSION: Continuous instillation of somatostatin via peripheral vein can decrease the portal venous pressure in cirrhotic patients with portal hypertension. The mechanism may be the decrease of hemokinetic volume caused by the decrease of hemokinetic velocity of the portal vein.

[Direct vision mitral valve reconstruction: a long-term follow-up of 46 cases].

OBJECTIVE: To improve the long-term results of direct vision mitral valve reconstruction. METHOD: From July 1983 through July 1995, 46 patients with mitral valve disease were treated by a direct vision reconstructive techniques. Rheumatic valve stenosis and incompetence were noted in 39 (patients), nonrheumatic insufficiencies in 5, and congenital disease in 2. According to leaflet pliability, type I normal leaflet motion was notedin 17 patients, type II leaflet prolapse in 3 and type III restricted leaflet motion in 29. Cardiac function (NYHA classification) showed class II in 2, class in 25 III, and class IV in 19. Valvuloplasty and annuloplasty were performed with various techniques according to the pathanatomical changes of the valve. Prosthetic obturators self made were employed in 29 patients. RESULT: The operative mortality rate was 6% (3/46). In the mitral valve replacement group it was 10% (27/265). Two to 10 years (mean 65 months) follow-up showed grade I cardiac function in 6 patients, grade II in 17, grade III in 8, and grade IV in 3. CONCLUSION: Good long-term results can be achieved by valve reconstruction with lower mortality and incidence of thromboembolism than.

[Pathophysiology and surgical treatment on Relaxed Pelvic Floor Syndrome].

OBJECTIVE: To study the pathophysiology of the "Relaxed Pelvic Floor Syndrome (RPFS)" and to assess the value of the "Pelvic Cavity Fixation Operation (PCFO)" for the syndrome. METHOD: 11 patients, who were diagnosed as having RPFS, underwent standardized PCFO. All the patients were followed for a mean time of 19 months (5 - 72 months). RESULT: The main syndromes and signs included difficulty in defecation, inability to empty the rectum properly, fullness of perineum, protrusion of perineum at bearing down, back-inclining uterus, relaxation of the rectum on digital examination. The anorectal dynamics showed that the sequence pressure and rest pressure of the anal cannel were lower and the time of contraction of the anal cannel decreased than that of the normal controls. The rectum sensation was damaged slightly. Defeacography showed that there were relaxing changes in several systems in the pelvic cavity. Operatively, it was easy to see the relaxation of the supporting structure of the colon, rectum, uterus and the lower position of the peritoneal cavity. PCFO has achieved good results. Seven patients were free of symptoms completely and 4 improved. CONCLUSION: The relaxing changes of the supporting tissue of many viscus fefer to several systems are the pathophysiologic of the RPFS. "Pelvic Cavity Fixation Operation" is useful in the treatment of the syndrome.

[Surgical treatment of aortopulmonary septal defect].

OBJECTIVE: To study the surgical treatment of aortopulmonary septal defect. METHOD: From January 1983 to December 1996, 5 patients with aortopulmonary septal defect (APSD) (2 male and 3 female) aged from 6 to 34 years (mean 17.4 years) underwent surgical treatment. According to Mori's were classification, 5 patients belonged to type I, 2 type II and 1 type III. The diameters of the defect ranged from 1.0 to 3.0 cm. The operations were performed under moderate hypothermic cardiopulmonary bypass. Pulmonary artery incision was performed, in 3 patients while aortic incision in the other 2. All defects were repaired by Dacron patch. RESULT: There were no operative deaths and postoperative complications. During follow-up of 10 months to 12 years, no late complications and deaths were found. CONCLUSION: The operation should be done early as possible whenever the diagnosis is established. The surgical repair should be taken under cardiopulmonary bypass. To chose an incision on the aorta is more reasonable than on the pulmonary artery. The use the dacron patch can prevent stenosis and recanalization after operation.

[The effects of different instrumentation on scoliosis].

OBJECTIVE: To investigate the effectiveness of the treatment of scoliosis with different surgical methods. METHOD: 125 cases of scoliosis less than 100 degrees treated during 14 years (1984 - 1997) were analyzed. Different operative methods were compared for their advantages and disadvantages, i.e. results and complications 10 months after operation and follow-up. RESULT: Harrington instrumentation was less effective in scoliotic curve correction and had more complications than other procedures. Luque technique needed longer operation time for wire passage at each level and had risk of neurological damage. CD and Zielke method showed 10% - 20% curve correction. No dislodgment of hook and fractured rods was found in the CD group. Anterior release plus posterior instrumentation was superior to other techniques. CONCLUSION: Treatment of scoliosis with anterior release plus posterior CD is practical for children before a new technique without spinal fusion is developed.

[Morphometrical study on inhibitory effect of vitamin B6 and banana-stem extract on calcium crystallization].

OBJECTIVE: To search for the anticrystallization mechanism by vitamin B(6) and banana stem extract. METHOD: Thirty-six male China-1 mice were divided into 4 groups: normal, crystallization, vitamin B(6) and banana-stem extract groups. Calcium oxalate crystallization was induced by 1% glycol and 1% ammonium chloride in the last 3 groups. Vitamin B(6) was then given to the 3rd group and banana stem extract to the 4th group for 3 weeks. At the end of 3 weeks, all the mice were sacrificed and the kidneys were taken for morphometrical studies using image analysis system and assayed for calcium oxalate. RESULT: Morphometrical area density and numerical density of crystallization was significantly less in the vitamin B(6) group and the banana-stem extract group as compared with the crystallization group, being the least in the banana-stem extract group. Oxalate in renal tissue was also significantly reduced. Both vitamin B(6) and banana stem extract had no effect on renal tissue calcium. CONCLUSION: Vitamin B(6) and banana-stem extract may be useful agents in the treatment of patients with hyperocaluric urolithiasis.

[Laser in situ keratomileusis for correction of myopia].

OBJECTIVE: To evaluate the efficacy and safety of laser in situ keratomileusis (LASIK) for the correction of myopia. METHODS: A total of 429 eyes with preoperative refraction of -2.00 to -28.00 diopters were treated with LASIK and followed up for more than 12 months. Based on the refraction, the patients were divided into 3 groups: Group I, -2.00, to -8.00 diopters, 179 eyes; Groups II, -8.25 to -15.00 diopters, 137 eyes; Group III, -15.25 to -28.00 diopters, 113 eyes. The examinations included cycloplegic refractions, Goldman tonometry, corneal sensitivity, contrast sensitivity, specular microscopy, pachymetry and corneal topography. The parameters of LASIK were: the thickness of corneal cap was 130 or 160 microns, and the maximum diameter of ablation zone was 6.8 mm. After surgery, fluorometholone was given to the patients topically for 1 or 2 months. RESULTS: The vision and refractions tended to be normal 3 months after operation. The contrast sensitivity returned to normal 3 to 6 months after surgery. The endothelium did not show any changes. At 12 months, respectively 88.8%, 79.6% and 70.8% in the above groups were within +/- 1D, 73.7%, 65.0% and 46.0% were within +/- 0.5D; the uncorrected visual acuity was 0.5 or better in 96.6%, 81.8% and 54.9% of the patients, 84.4%, 58.4% and 15.0% had vision of 1.0 or better. CONCLUSIONS: LASIK is effective and safe for the correction of mild, moderate and high myopia. However, it is more complicated than PRK and more dependent on the surgical skills.

[Treatment of intraocular fibrinous membrane by anterior chamber injection of tissue plasminogen activator after IOL implantation].

OBJECTIVE: To investigate the method of treatment of intraocular fibrinous membrane by tissue plasminogen activator (t-PA) after intraocular lens (IOL) implantation and evaluate its clinical results. METHODS: 58 eyes of 43 patients after cataract surgery were divided into three groups, and received t-PA anterior chamber (AC) injection or combined with YAG laser therapy at different times after the operation. Group 1 included 26 eyes which received 4 microg t-PA AC injection at the end of operation. In group 2, 22 eyes whose fibrinous membrane was formed 5 - 7 days after the operation were injected with 4 microg t-PA at that time. In group 3, 10 eyes with thicker membrane were given YAG laser treatment at postoperative 5 - 9 days and then 4 microg t-PA was injected into the AC. RESULTS: In group 1, hyphema occurred in 4 eyes, in 8 eyes at the pupillary region the fibrinous membrane was formed which was completely lysed after t-PA AC injection at postoperative 3 - 5 days. In group 2, the membrane was lysed within 24 hours in 20 of 22 eyes and no hyphema was found. In group 3, the membrane was lysed within 4 - 24 hours after t-PA injection in all cases, and no hyphema occurred. CONCLUSION: It is very effective to treat fibrinous membrane formation after IOL implantation by t-PA AC injection at postoperative 1 week when the AC inflammation is well controlled. The combined use of YAG laser with t-PA AC injection is much more effective for the treatment of such a membrane.

[The choice of intraocular lens before cataract extraction for cases with high myopia].

OBJECTIVE: To study the choice of diopter and size of optical portion of intraocular lens (IOL) before cataract extraction for cases with high myopia. METHODS: Extracapsular cataract extraction (ECCE) or phacoemulsification with IOL implantation was performed on 60 cases (94 eyes) with high myopia (axial length > 26mm). The posterior chamber IOL selected and inserted was the one with the diopter and size of the optical portion suitable for the recipient to have a postoperative low myopia and easy to be examined with an ophthalmoscope. RESULTS: The postoperative eyes were of low myopia, -2.07 +/- 1.95 D. The larger size of optical portion was more suitable for ocular fundus examination. Great care was taken for phacoemulsification for cataractous nucleus below III grade. CONCLUSIONS: For patients with axial myopia, the IOL implanted should create low myopia postoperatively. Larger optic size is better for fundus examination. Phacoemulsification can produce less astigmatism and damage, but the indication must be controlled strictly.

[Phacoemulsification and foldable lens implantation through a temporal clear corneal tunnel incision].

OBJECTIVE: To investigate the therapeutic effects, advantages and disadvantages of phacoemulsification and foldable lens implantation through a temporal clear corneal incision. METHODS: Eight hundreds and twelve patients (845 eyes) underwent phacoemulsification and foldable lens implantation through a temporal clear corneal tunnel incision which has a groove of 0.30 mm deep, a tunnel length of 1.75 mm, an internal opening of 3.20 mm and an external opening of 3.50 mm. The postoperative uncorrected visual acuity, the changes of corneal topography, astigmatism and endothelia were observed. RESULTS: The proportions of uncorrected visual acuity of 0.5 or better were 61.30% and 79.05% respectively at one day and a month after the operation. Postoperatively, topographical changes displayed temporal flattening near the incision in the early stage. The change of central corneal astigmatism was mild. Endothelial cell loss was 8.23%. The complications included burn of the incision (0.40%), tear of the posterior capsule (1.07%), subluxation of the lens (0.12%), endophthalmitis (0.12%) and cystoid macular edema (0.24%). CONCLUSION: The phacoemulsification and foldable lens implantation through a temporal clear corneal incision is convenient and effective.

[The diagnostic value of high field MRI for choroidal melanoma].

OBJECTIVE: To clarify the characteristic magnetic resonance imaging (MRI) features of choroidal melanomas by the high field MRI to improve the accuracy of diagnosis and localization and to differentiate choroidal melanomas from other choroidal lesions. METHODS: 12 patients with choroidal melanomas were examined by GE 1.5T magnet and heat coil, fat suppression technique and GD-DPTA as contrast agent were also used. Ten patients underwent operations and the histopathologic diagnosis was confirmed. RESULTS: The melanomas of the 12 patients had relatively high signals on T(1) weighted image and relatively low signals on T(2) weighted image. This is because of the paramagnetic properties of melanin. The melanomas with melanin can produce stable free radicals, both T(1) and T(2) relaxation times are shortened, with this we can differentiate melanomas from other choroidal lesions. CONCLUSION: Sagittal, coronal, axial and oblique scanning with small feild of view for the orbit by MRI can be made to show the exact position of melanomas and differentiate them from other choroidal lesions.

[Serologic testing of hepatitis C virus infection].

We tested anti-HCV, anti-HCV IgM and HCV-RNA in serum samples from general adults, blood donors, acute hepatitis patients and liver cirrhosis patients by the methods of ELISA and RT-PCR so as to study the relationship among serum markers. Results showed that the detected positive rates of anti-HCV are 3.57%, 8.58%, 6.25% and 48.38% in normal adults, blood donors, acute hepatitis patients and liver cirrhosis patients respectively; agreement rates between anti-HCV and HCV RNA are 11.43%, 61.11%, 80.0%, and 73.33% in the four groups of people above; agreement rates between anti-HCV IgM and HCV RNA are 75.0%, 90.9% 81.81% and 100% in the same groups of people. Authors pointed out that anti-HCV IgM is superior to anti-HCV in the aspect of revealing HCV viremia. In comparison with RT-PCR, testing anti-HCV IgM is easier and does not need special instruments, so it is suitable to develop in the basic hygiene units. Testing anti-HCV IgM instead of anti-HCV, the morbidity of post-transfusion hepatitis C can be decreased further.

[Hemorheologic change after strong blood flow disturbance caused by gunshot and its influence on the prognosis of the wounded animals].

Three types of experimental models were used in this study: 1. Both thighs of dogs were wounded by 5.56 mm bullet. 2. Femoral artery and vein were tied at the time of wounding. 3. Shock caused by simply bleeding the dogs' femoral arteries. We observed the characteristics of change hemorheology and analyzed its effects on the wounded dogs. The results showed that strong blood flow disturbances always brought about hemorheologic changes such as rapidly increased blood viscosity and red cell's aggregation index and the simultaneous phenomenon of blood concentration. These may be related to extensive endothelium injury and destruction of a number of blood cells caused by disturbance wave. The above change may give rise to abnormality of microcirculation and disturbance of matter transmission; in that sense, it may provide the physiopathologic basis for the occurrence and development of the complication.
