[Detection of the p53 protein accumulation and p53 gene mutation in squamous cell carcinoma of the lung].

OBJECTIVE: To investigate the effects of p53 protein accumulation and p53 gene mutation in the pathogenesis of squamous cell carcinoma of lung. METHODS: Immunohistochemistry and PCR-SSCP techniques were used to detect p53 protein accumulation and p53 gene mutation. RESULTS: p53 protein accumulations was found in 52.5% (63/120) of the samples. p53 gene mutation was detected in 56.7% (68/120) of the same specimens by PCR-SSCP. In 24 cases (35.3%, 24/68), the mutations was locating on exon 5, 11 cases (16.2%, 11/68) on exon 6, 18 cases (26.5%, 18/68) on exon 7 and 15 cases (22.1%, 15/68) on exon 8 respectively. The rate of p53 gene mutation/overexpression was 60%. p53 gene mutation/overexpression rate of moderately (63.6%, 28/44) or poorly (63.5%, 40/63) differentiated pulmonary squamous cell carcinoma was significantly higher than that of the well differentiated carcinoma (30.8%, 4/13). In comparing with non-lymph node metastasis group (43.1%, 28/65), p53 gene mutation/overexpression rate in metastasis group (80%, 44/55) was significantly higher. p53 gene mutation/overexpression rate of pulmonary squamous cell carcinoma in advanced stage (76.3%, 45/59) was significantly higher than that in the early stage (44.3%, 27/61). p53 gene mutation/overexpression rate in heavy smoking group (72.4%, 55/76) was significantly higher than that in non-smoking group (38.6%, 17/44). No statistically significant correlation was found between p53 gene mutation/overexpression and sex, age, tumor location or invasion of the surrounding tissue. CONCLUSIONS: p53 gene mutation/overexpression may play an important role in the pathogenesis of squamous cell carcinoma of lung and can be considered as a predicting marker of prognosis.

[Studies on genes associated with biologic behavior in human lung cancer].

OBJECTIVE: To study alteration of several genes in the process of primary lung cancer. METHODS: A series of 59 lung cancer specimens were analyzed for p53, myc oncogene family and mdrl gene by DNA/PCR sequencing, immunohistochemistry and RT-PCR methods. RESULTS: p53 mutation or/and protein accumulation were found in 37 of 57(65%) cases. Overexpression of myc family oncogenes and mdr1 gene were 27/46 (59%) and 15/48 (31%), respectively. The results also showed that there was no significant correlation between p53 alteration and tumor size, metastasis, stage and relapse, but there was a significant correlation between overexpression of myc family oncogene and these factors. Overexpression of mdrl gene was detected in NSCLC, especially in adenocarcinoma, and was not associated with metastasis and stage of lung cancer. It was also found that aberration of both p53 and myc family oncogenes occurred in 19/30(63%) cases; the relapse rate was 76%. Both overexpression of mdrl and myc gene was 62%; the relapse rate was 83%. CONCLUSION: p53, myc and mdrl genes in cooperation may be involved in the process of lung cancer, but prognostic determinant is myc gene overexpression.

[Effect of adrenomedullin on bronchoconstriction responses induced by histamine].

OBJECTIVE: To observe the effect of synthetic human adrenomedullin (AM) on anesthetized guinea pigs bronchoconstriction induced by histamine. METHOD: The guinea pigs were divided into saline group; isoproterenol groups at the dosages of 0.03 microgram/kg, 0.3 microgram/kg, 3 micrograms/kg and 30 micrograms/kg; adrenomedullin groups at the dosages of 0.02 microgram/kg, 0.2 microgram/kg, 2 micrograms/kg and 20 micrograms/kg. There were seven guinea pigs a group. Guinea pigs were anesthetized by sodium pentobarbital, normal saline group, isoproterenol and adrenomedullin at different dose i.v., while histamine (10(-4) mol/L) were vasolated and delivered into the airway. Body plethysmogroph was used to measure airway resistance and lung compliance. RESULT: The increment of airway resistance and decrement of lung compliance induced by histamine were inhibited significantly by isoproterenol at the dosages of 0.3 microgram/kg, 3 micrograms/kg and 30 micrograms/kg (P < 0.05). The increase of airway resistance was inhibited significantly by adrenomedullin at the dosages of 2 micrograms/kg and 20 micrograms/kg (P < 0.05). The decrease of lung compliance induced by histamine was inhibited significantly by adrenomedullin at 0.2 microgram/kg, 2 micrograms/kg and 20 micrograms/kg (P < 0.05). CONCLUSION: Adrenomedullin inhibited the bronchoconstriction induced by histamine significantly.

[The morphological and functional changes of pulmonary intravascular macrophages induced by LPS].

OBJECTIVE: To study the roles of pulmonary intravascular macrophages (PIMs) on infective acute lung injury (ALI). METHOD: Porcine pulmonary blood vessels were flushed by modified Morton's method, PIMs were isolated by adhesion method and identified with the features at both light and electron microscopic level. The activity of IL-1 beta, and content of IL-6, IL-8 and TNF alpha in the culture supernatants were measured by thymocyte proliferation or ELISA, respectively. RESULT: Enlarged and increased pseudopods, and increased amount of lysosomes and phagosomes were found in the PIM stimulated with lipopolysaccharide (LPS, 10 micrograms/ml); the releases of TNF alpha, IL-1 beta, IL-6 and IL-8 were increased significantly as compared with the level of pre-stimulation of LPS (P < 0.01), and reaching their peaks at 1 h, 2 h, 4 h and 6 h after LPS stimulation, respectively. CONCLUSION: The isolation of porcine PIMs can be completed with modified Morton's method; the phagocytosis and secretion of PIMs are more active after LPS stimulation. In the pathogenesis of ALI, TNF alpha and IL-1 beta may play an important role at its early stage; however, the IL-6 and IL-8 may be associated with the pathophysiological changes at the later stage of ALI.

[The tuberculin test in intrathoracic sarcoidosis].

OBJECTIVE: To highlight the diagnosis and treatment of intrathoracic sarcoidosis. METHOD: The clinical and laboratory data of 35 cases of intrathoracic sarcoidosis in Tianjin thoracic hospital from 1991 to 1996 were reviewed. Tuberculin tests for 35 cases of sarcoidosis were carried out and the results were evaluated. Simultaneously, the results have been compared with that of literature published. RESULT: Of 35 cases of intrathoracic sarcoidosis 17 cases were tuberculin positive, 4 were strongly positive, positive tuberculin reactivity (60%) was significantly higher than that published. CONCLUSION: The results showed a considerably higher tuberculin positive rate, which may be due to the difference of positive tuberculin reactivity in different country or regions, age of patients and expression of gamma delta T cell receptor. The authors suggested that "sarcoidosis should be diagnosed with negative tuberculis reastivity" has to be reconsidered.

[Occurrence of hyperventilation syndrome in Chinese patients].

OBJECTIVE: To evidence the occurrence of hyperventilation syndrome in Chinese patients. METHOD: Case report and literature review. RESULT: Three cases with manifest hyperventilation syndrome were reported. The diagnosis was based on the presence of several suggestive complaints occurring in a context of stress, and the reproduction of the most important complaints by the hyperventilation provocation test. Organic diseases as a cause of the symptoms had been excluded. Breathing therapy reducing the tendency to hyperventilate by acquiring an abdominal breathing pattern, with slowing down of expiration, markedly reduced complaints in the three patients. CONCLUSION: Hyperventilation syndrome occurs also in Chinese patients.

[The pre-experiment on space crystallization of pig heart mitochondrial F1-ATPase].

ATP synthase (H(+)-ATPase) plays a key role in oxidative phosphorylation. Its catalytic moiety F1-ATPase is under intense structural study because of different crystallographic results and diverse hypotheses of its catalytic mechanism. To obtain high quality crystals, F1-ATPase sample is going to be carried to outer space by recovable satellite for crystal growth under microgravity. And to make it suitable for space crystal growth, F1-ATPase was purified from pig heart and the conditions were optimized for its maintenance, transportation, and crystallization. By using hanging drop vapor diffusion method and sitting drop vapor diffusion method, the crystals were obtained within 5 d and with a size of 0.1 mm x 0.1 mm x 0.l mm on the 7th day. The reproducibility of these experiments is also good.

Influence of space conditions on photosynthetic pigment contents and chloroplast ultrastructure of maize leaves.

Dried seeds of maize inbred lines were carried by recoverable satellite to space conditions at an altitude of 175-253 km from sea level for 15 d. The effects of space flight on ultrastructure and photosynthetic pigment contents of maize leaves were studied. Various changes in structure were observed. Chloroplast envelope membrane notch, swelling of thylakoids, less granum stacks and stroma thylakoid membranes, and changes of chloroplast contour were often seen. The contents of chlorophyll a, chlorophyll b, and chlorophyll a+b and carotenoids tended to be lower, but Ca/Cb ratio tended to be higher in the leaves from space flight maize in comparison with ground control.

[Identification of Tougucao derived from Clematis species].

Comparative studies were carried out on the morphology and histology of 5 Clematis species, namely, C. intricata, C. aethusifolia, C. glauca, C. tangutica and C. hexapetala, allegedly used as "Tougucao". New characteristics, a sclerenchymatous ring composed of primary phloem fibers and lignified parenchyma cells, and a parenchymatous tertiary ring were discovered. A parameter representing the ratio of the diameter of cambium ring to the diameter of the stem (RCR) is created. A key for identification of the stems of the above five species is made according to the presence or absence of the sclerenchymatous ring, the diameter and numbers of hair cells, the thickness of fiber wall and the value of RCR, etc.

[Embolization for renal carcinoma].

OBJECTIVE: To decrease bleeding during renal malignancy resection and to get higher resection rate as a paliative treatment for advanced renal carcinoma. METHODS: 50 cases of renal carcinoma were treated with renal artery embolization from 1981 to 1996. In 96% of the cases the tumour size was larger than 7 cm in diameter. Gelatine, spring coil with antineoplastic agent were used as embolizer. RESULTS: After tembolization, the average bleeding of 46 cases in nephrectomy was 254 ml. Resection rate was 10%. Pain and fever varied after embolization in most cases. In 82.6% of the cases, surgery was done within 48 hours after embolization. There was no mortality and morbolity in this series. CONCLUSIONS: Renal artery embolization is an effective and safe procedure. It could be used as an adjuvant therapy for renal carcinoma.

[Esophageal insufflation test in secondary voice reconstruction].

OBJECTIVE: To restore the alaryengeal patients' enunciation function, reduce operative trauma, increase the accuracy of detecting the success of secondary voice restoration and evaluate objectively the practical clinical significance of esophageal insufflation test. METHODS: Esophageal insufflation test and secondary voice restoration were done for 45 alaryngeal patients, especially those with negative results of esophageal insufflation tests. RESULTS: It showed that the success rate of voice rehabilitation in patients with negative insufflation test but no intact pharyngeal constrictor was 94.7%(36/38), in those with positive insufflation test was 100%(7/7) uniformly. The postoperative pronounciation performance was better than that at the test time. CONCLUSION: Esophageal insufflation test is a useful procedure in predicting the postoperative pronunciation, but could be influenced by many factors, so it can not be used as the only test for estimating the postoperative pronunciation.

[Measurement of blood flow in nasal mucosa].

OBJECTIVE: To determine the amount of blood flow in the human nasal mucosa and its change in patients with nasal polyps and allergic rhinitis. METHODS: Using the Ameflow-H2, the amount of blood flow in nasal mucosa was examined in 98 normal adults, 18 cases of nasal polyps and 14 cases of allergic rhinitis before and after application of the vaso constrictor. RESULT: Before the application of the vaso constrictor, blood flow of nasal mucosa of patients with nasal polyps and allergic rhinitis was lower than that of healthy people (P < 0.01). After using the vaso constrictor, the amount of blood flow was decreased in all cases with a significant decrease in patients with nasal polyps. CONCLUSION: Ameflow-H2 may be the best tool to measure nasal blood flow than other devices. The method is technically simple, less traumatic, and repeatable.

[Radionuclide pulmonary perfusion imaging in evaluating pulmonary hypertension in patients with congenital heart disease].

OBJECTIVE: To evaluate the degree of pulmonary hypertension in patients with congenital heart disease (CHD) by using radionuclide pulmonary perfusion imaging. METHOD: The pulmonary perfusion in 81 patients with left to right cardiac shunts was examined by scintigram with macroaggregates of (99m)Tc-labeled human serum albumin (PPS). PPS was analyzed for (1) lung regional radionuclide count (LRC), (2) right upper and lower lung count ratio (RULR), (3) kidney count ratio (KCR). The results were compared with those of cardiac catheterization exam. RESULT: There were correlation between mean pulmonary arterial pressure (mPAP), total pulmonary resistance (TPR), and PPS (KCR, RULR with mPAP gamma = 0.25, 0.35; LRC, KCR with TPR gamma = -0.34, 0.29). Five patients repeated PPS test after early operation. There were no changes in PPS. CONCLUSION: The degree of reversibility of pulmonary hypertension in patients with CHD could be quantitatively analysed by PPS.

[Correction of cardiac defects through a right minithoracotomy in children].

OBJECTIVE: To review the experience of correction of congenital cardiac defects through a right minithoracotomy. METHOD: 319 patients underwent correction of congenital heart malformations through right lateral thoracotomy under cardiopulmonary bypass. The average age was 3.44 +/- 1.59 years (range, 5 months-8 years). The average body weight was 13.66 - 3.98 kg (range, 6 - 26 kg). Cardiac defects repaired included atrial septal defect in 87 patients (1 patient associated with left superior vena cava (LSVC), 6 pulmonary stenosis, 5 partial anomalous pulmonary venous connection), ventricular septal defect in 200 (7 patients with coexisting patent ductus arteriosus, 7 mitral insufficiency, 3 LSVC, 11 right ventricular outflow tract obstruction), Fallot's Tetralogy in 19 (3 patients associated with LSVC, 1 single coronary malformation), partial endocardial cushion defect in 2 and other defects in 11. The mean cardiopulmonary bypass time was 56.07 +/- 24.90 min (range, 20 - 176 min) and the mean aortic crossclamping time was 32.97 +/- 20.38 min (range, 6 - 140 min). The average mechanical ventilation time after operation was 18.75 +/- 24.57 hr (range, 2 - 140.72 hr), and the mean postoperative hospital stay was 7.08 +/- 0.69 days (range, 7 - 17 days). RESULT: No operative mortality and severe postoperative complications were noted. CONCLUSION: The right lateral thoracotomy is a safe and effective alternative to a median sternotomy for correction of cardiac defects. Advantages of this approach include less injury, maintaining the continuity and the integrity of the bony thorax, and preventing postoperative pigeon breast. The cosmetic result is superior to that of median sternotomy or bilateral submammary incision.
