Demonstration of beta 1H globulin in pemphigus.

beta 1H globulin is a plasma protein which regulates the biologic activities of the major fragment of the 3rd complement component, C3b. The role of beta 1H globulin in pemphigus was investigated using immunofluorescence in the present study. Lesional skin biopsies from patients with confirmed pemphigus demonstrated in-vivo deposition of beta 1H in addition to C3 in all of four biopsies. Eight serum samples containing C3 fixing intercellular antibodies were then tested for the capacity to fix beta 1H and other complement components. All eight pemphigus sera showed fixation of beta 1H to the intercellular areas of normal human skin. C1q and C4 fixation by pemphigus sera was also demonstrated in 7 of 8 sera, respectively. The experiment using C2-deficient serum indicated that the fixation of beta 1H by intercellular antibodies requires the activation of the classical complement pathway. These data suggested that beta 1H, a co-factor of C3b inactivator, plays a role in the in-vivo regulation of complement activity and supplies additional evidence for the participation of complement system in the pathogenesis of pemphigus.

Newly established uterine cervical cancer cell line (SKG-III) with Regan isoenzyme, human chorionic gonadotropin beta-subunit, and pregnancy-specific beta 1-glycoprotein phenotypes.

The production of Regan isoenzyme (heat-stable, L-phenylalanine-sensitive term-placental alkaline phosphatase), human chorionic gonadotropin beta-subunit, and pregnancy-specific beta 1-glycoprotein by newly characterized human uterine cervical cancer cell lines, SKG-IIIa and SKG-IIIb, is reported. These cell lines were derived from a moderately differentiated epidermoid cancer partially mixed with epidermoid clear-cell components. At the end of the first 4 months in culture 2 sublines with different morphologies were identified. In nude mice, SKG-IIIa produce clear-cell epidermoid cancer with much glycogen, while SKG-IIIb grew as a moderately differentiated epidermoid cancer rich in tonofilaments. The presence of Regan isoenzyme was established by biochemistry, enzyme cytochemistry, immunocytochemistry, and immunoelectrophoresis. However, the copresence of small amounts of early placental alkaline phosphatase was also demonstrated. The alkaline phosphatase specific activities of SKG-IIIa cells and SKG-IIIb cells were 3.7 and 1.4 nmol per mg protein per min, respectively. The existence was proven by radioimmunoassay of human chorionic gonadotropin beta-subunit (SKG-IIIa, 5.0 mlU/mg protein; SKG-IIIb, 4.4 mlU/mg protein), pregnancy-specific beta 1-glycoprotein (SKG-IIIa, 0.7 ng/mg protein) in the culture media as a tumor cell product. The described cell lines may serve as a more representative model system for studies of regulation of oncodevelopmental genes in gynecological tumors in general and in epidermoid cervical cancer in particular.

[Biochemical studies on heat-stable alkaline phosphatase in uterine cervix and endometrium].

Heat-stable alkaline phosphatase (HS-ALP) has been attracting attention as one of the placental proteins as well as one of the oncodevelopmental proteins. In this paper, biochemical methods were used to examine ALP isoenzyme patterns in both normal endometrium and cervical tissue with special reference to HS-ALP. Results 1) Most of the ALP in both normal endometrium and cervical tissue showed the biochemical characteristics of tissue-unspecific ALP isoenzyme. 2) In normal cervical tissue, a higher level of HS-ALP was found than in normal endometrium. HS-ALP in cervical tissue showed the same biochemical characteristics as Regan isoenzyme. 3) In normal endometrium, very small amount of Regan isoenzyme was found, but some of the HS-ALP showed the same biochemical characteristics of L-L sensitive types as D-variant of normal placenta. From the results of these biochemical tests, it may be concluded that the ALP isoenzyme profiles of endometrium and cervical tissue are different from each other both qualitatively and quantitatively, although the endometrium and cervix developed from the same ancestral "Mullerian duct".

Experimental acantholysis by complement-fixing intercellular antibodies.

Complement-fixing intercellular antibodies were detected in 10 of 17 sera from untreated pemphigus patients. The role of complement in the organ culture system was investigated using these sera. Ten sera possessing complement-fixing intercellular antibodies showed IgG binding to the intercellular substance in the organ-cultured skin and acantholysis-like changes were observed in eight cases. C3 deposition was not seen in any case. However, after treatment of the sections of cultured skin with fresh normal human serum, complement fixation of the intercellular substance by bound IgG was revealed in all the ten cases. No significant differences in the grade of acantholysis-like changes between the complement-depleted system and the complement-supplied system were observed. Complement does not appear to be necessary in the acantholytic process in the in vitro organ culture system, even though we considered the presence of complement-fixing intercellular antibodies.

In vitro complement activation by intercellular antibodies.

By in vitro complement immunofluorescence, 6 sera from pemphigus with intercellular antibodies were tested for their capability to fix C1q, C4, C3, and properdin. All 6 serum samples yielded positive reaction for C3 staining. Three serum samples gave positive staining for C1q, 5 serum samples for C4, and 3 serum samples for properdin, respectively. Substitution of C2 deficient serum as a complement source inhibited C3 and properdin staining but not positive C1q and C4 staining. These results are best explained by the concept that complement activation in vitro by intercellular antibodies occurs via the classical pathway followed by assembly of the C3 amplification mechanism.

The effects of muscle length on intracellular calcium transients in mammalian cardiac muscle.

1. The calcium-sensitive photoprotein aequorin was micro-injected into cells of rat and cat ventricular muscles. The resulting light emission is a function of intracellular free calcium concentration ([Ca2+]i). The transient increases in [Ca2+]i that accompany contraction were monitored. 2. After an increase in muscle length, the developed tension increased immediately and then showed a slow increase over a period of minutes. The peak [Ca2+]i in each contraction was initially unchanged after an increase in muscle length but then showed a slow increase with a time course similar to that of the slow tension change. 3. As a consequence of these slow changes, the shape of the tension-length relation depends on the procedure used to determine it and this change in shape can be attributed to changes in activation. 4. Immediately after an increase in muscle length the calcium transient was abbreviated. 5. When a quick release was performed during a contraction, a short-lived increase in the [Ca2+]i was observed following the release. 6. The two previous observations can both be explained if the binding constant of troponin for calcium is a function of developed tension.

Intracellular Ca++ transients and relaxation in mammalian cardiac muscle.

Calcium sensitive photoprotein aequorin was micro-injected into the cells of cat and rat ventricular cardiac muscles. Light and tension were recorded from the injected muscle in order to clarify the relation between the intracellular Ca++ transient and relaxation. The time course of the falling phase of the light signal and the time course of the relaxation were measured under the various conditions, i.e., changes of stimulation frequency, [Ca++] o, and the administration of adrenaline and caffeine. High stimulation frequency accelerated the time course of the fall of light and the relaxation of tension. Caffeine prolonged the relaxation of tension and the falling phase of light. The time course of relaxation did not change substantially but the falling phase time course of the light was accelerated by increase of [Ca++] o and adrenaline. These results confirm that a fall in [Ca++] i always precedes relaxation but suggest that the decrease of [Ca++] i is not always the rate limiting step of the relaxation in the mammalian cardiac muscle.

Heat-stable alkaline phosphatase in uterine cancer, with special reference to its histochemical heat-stability and the L-phenylalanine inhibition test.

We report a histochemical study of alkaline phosphatase (ALP) in normal cells of the female reproductive system, in pre-cancerous and cancerous lesions of the uterine cervix and in endometrial cancer to ascertain the incidence of ALP and its isoenzyme type. For this purpose, serial sections were subjected to heat stability and L-phenylalanine (LP) inhibition tests. The Regan-like isoenzyme, a heat-stable and LP-sensitive ALP, which has been thought to derive only from cancer or the placenta, was found in uterine cervical reserve cells and endometrial luminal surface lining cells. In contrast, ALP activity in endometrial glandular cells was found to be heat and LP sensitive. Of 183 cases of cervical neoplasia, 60 (33%) manifested non-specific ALP activity. One dysplasia and two invasive cancer cases manifested the Regan-like isoenzyme. The other 36 classifiable lesions had small-intestine ALP-like activity (marked heat and LP sensitivity) or a liver ALP-like isoenzyme (marked heat and slight LP sensitivity). Of 42 cases of endometrial cancer, all cases manifested non-specific ALP activity. Seven endometrial cancers exhibited the Regan-like isoenzyme. The other 19 cases manifested either small intestine or liver ALP-like isoenzyme. Our findings indicate that in the course of uterine carinogenesis, the ALP isoenzyme of reserve cell and endometrial glandular cells undergo a change and that enzyme deviation occurs.

Accessible plasma exchange using membrane filter - a successfully treated case of TTP with repeated plasma exchanges.

Plasma exchanges with a hollow-fiber type plasma filter were attempted in a variety of diseases. The unit was easily assembled and the procedures were safely carried out by our dialysis personnel after some instruction. Approximately 2 liters of fresh frozen plasma were used during each plasma exchange and about the same amount of plasma was removed from the patient in 1.5 to 2 hours. This resulted in 66% replacement of patient's albumin on one occasion. Fifty-four plasma exchanges were done in seven patients. The procedure was well tolerated by all patients with only occasional muscle cramps or itching, which were treated with calcium gluconate infusion or antihistamines, respectively. A successfully treated case of TTP in which we used repeated membrane plasma exchanges combined with conventional treatment is also reported.
