Morphology of the release of vitamin A-containing lipid droplets by perisinusoidal lipocytes in the rat liver.

Vitamin A-containing lipid droplets in perisinsoidal lipocytes (or fat-storing cells) of rat liver were found to be exocytotically released from the cells in the form of a 'lipid droplet-protein complex' following intraportal injection of retinol (17, 33, 67 or 100 micrograms). Intraportal injection of retinol produced varied numbers and sizes of vacuoles with a limiting membrane in perisinusoidal lipocytes. The substance within the vacuoles exhibited a meshwork-like configuration in sections from slices incubated in a medium for revealing acid phosphatase activity or the corresponding control medium, and was immunoreactive to retinol-binding protein and proteinaceous in nature. The occurrence and number of the vacuoles depended on the dosage of injected retinol, being greatest at a dosage of 100 micrograms of retinol and becoming progressively less at dosages of 67, 33 and 17 micrograms. The vacuoles appeared to be formed by vacuolization of cisternae of the rough endoplasmic reticulum. Little or no esterase activity was found in lipid droplets in perisinusoidal lipocytes before the intraportal injection of retinol, but after the injection lipid droplets which had fused with the vacuoles became strongly positive for this enzyme activity. This suggests that hydrolysis of retinyl esters may occur in the process of complex formation in perisinusoidal lipocytes.

Persistent wedge-shaped low-density lesions on computed tomography of the kidneys without infarction.

A patient with acute lymphoblastic leukemia, who developed acute renal failure, was imaged by computed tomography (CT). Persistent wedge-shaped areas of low attenuation in the kidneys were shown by sequential scans but not areas of infarction were found at autopsy. This case suggests that a wedge-shaped low-density lesion on CT may occur not only in infarcted areas, but also in areas where patchy vasoconstriction occurs.

[Total replacement of the urinary bladder using an ileal pouch bladder].

We have used the continent ileal bladder as a bladder replacement after radical cystectomy. The ileal bladder is an ileal pouch which is anastomosed to the urethral stumps. The ureters are implanted by a free end ureteroileostomy. The long term results with 26 patients who underwent this procedure are reported. In the early postoperative period, urodynamic and radiographic studies revealed small capacity and high intravesical pressure of the ileal bladder. However, it became a low pressure reservoir with increased capacity gradually. The average bladder capacity was about 250 ml and average residual urine was 30 ml. Most of the patients were continent in the daytime if the voiding intervals were less than 3 hours at night, some patients were incontinent. Urinary leakage was the most frequent complication. VUR and hydronephrosis were still the problems to be solved. The ileal pouch bladder is a valuable procedure in properly selected cases.

[The effect of moxisylyte hydrochloride in the symptomatic treatment of benign prostatic hyperplasia].

The clinical effectiveness of moxisylyte hydrochloride (Moxyl, Fuji Rebio), a selective alpha 1-adrenoceptor blocking agent, was investigated in patients with symptomatic benign prostatic hyperplasia. Moxisylyte hydrochloride was administered at the dose of 90 mg per day orally for four weeks. Residual urine sensation was improved in 9 out of 17 cases (53%), retarded urination in 14 out of 19 (74%), prolonged urination in 12 out of 19 (63%), weak stream in 14 out of 21 (67%) and strained voiding in 11 out of 19 cases (58%). Residual urine volume and residual urine ratio were decreased in 14 out of 21 cases (67%), maximum urine flow rate was increased in 14 out of 21 (67%) and mean flow rate was increased in 15 out of 21 cases (71%). There was, however, no statistical significance between pre- and post drug administration in objective parameters. Our open trial showed that the subjective efficacy of moxisylyte hydrochloride was 82% (18/22 cases), the objective one was 59% (13/22 cases) and overall efficacy was 77%. Side effects were observed in four of all subjects (18.2%), and drug administration had to be ceased in only one of these patients due to mild dizziness. Other side effects were mild nausea, headache and chest discomfort. Decrease in blood pressure was not seen in all but one case.

Gliomas with dural invasion: computed tomography and angiography.

This report is a review of the findings of computed tomography and angiography of eight supratentorial gliomas with dural invasion. Computed tomography revealed localized increases in the thickness of ring enhancements at the sites of dural invasion in four cases, which was attributed to increased blood supplies by the meningeal vessels. Angiographically, feeders from the meningeal vessels entered the tumors mostly through relatively broad areas of tumor-dura attachment. A sunburst type of neovascularity from a meningeal artery was seen in only one case. In most such cases the differential diagnosis from meningioma was possible by the computed tomographic and angiographic findings. Selective external carotid angiography was the most important examination for diagnosing dural invasion by gliomas.

Preoperative diagnosis of ovarian malignancies with intravenous digital subtraction angiography.

Intravenous digital subtraction angiography was performed on 59 patients with ovarian tumors. Dilatation of the uterine artery and its ovarian branch was relatively frequent in both benign and malignant tumor groups, while dilation of the ovarian artery, and hypervascularity from the uterine artery or the ovarian artery were fairly specific to the malignant tumor group. These five abnormal vascular patterns were evaluated by a scoring system and approximately 75% of malignant and 87% of benign ovarian tumors were correctly diagnosed preoperatively by digital subtraction angiography alone. Intravenous digital subtraction angiography can be applied as an easy, safe, and valuable diagnostic method for ovarian malignancies and can replace conventional pelvic angiography.
