Estimation of urinary 24-hr creatinine excretion by body size and dietary protein level: a field survey based on seasonally repeated measurements for residents living in Akita, Japan.

We aimed to investigate the modifying effect of dietary protein level on the estimation of 24-hr creatinine excretion using anthropometric measurements, in a group of 40 healthy Japanese adults, during usual daily life. To evaluate the variability of each measurement during non-restrictive life, we assayed 24-hr urinary excretions of creatinine and urea-N and measured heights and weights once in every three months in a total of 4 times during a year. Urinary urea-N was used as a marker of dietary protein level. Seasonal variability of anthropometrics and urinary excretions was observed on the individual basis. In comparison to the rather small individual variability of weight (mean of CV = 1.2 for males, 1.7 for females), variability of urinary creatinine (CV = 11.7, 8.4) and urea-N (CV = 18.9, 17.3) took larger values. The creatinine excretion was estimated by the following two sets of predictor variables; (I) age, height and weight, (II) age, height, weight and urea-N. Although, a statistically significant estimation was obtained by (I), estimation (II) resulted higher regression coefficient than (I), and contribution of urea-N to the estimation of creatinine was shown. Average level of seasonal variability of each predictor variable was further applied to the regression equation (II). Therefore, the possible difference of 24-hr creatinine excretion caused by the seasonal variability of dietary protein level was estimated to be 9.2% for males and 3.6% for females in the given population. Thus, when one predicting formula of 24-hr creatinine by the use of anthropometrics is applied to other populations, the difference of protein nutritional status should be carefully considered.

Short-term memory in the macaque monkey: coding response during delay interval.

The possibility that macaque monkeys code the position of a cue by color stimuli was investigated. The monkeys were trained on a delayed response task and a conditional position discrimination task based on color stimuli, and were tested while the color stimuli were presented during the delay interval of the delayed response task. The monkeys performed the task better when the colors were presented during the delay. However, the monkeys did not spontaneously choose an appropriate color to code the position of the cue in a choice situation, although they could be trained to show the coding response.

Atrial natriuretic peptide, angiotensin, norepinephrine and electrolyte in cerebrospinal fluid of essential hypertension.

We determined concentrations of atrial natriuretic peptide (ANP), angiotensin (Ang), norepinephrine (NE) and electrolyte in plasma and cerebrospinal fluid (CSF) to study possible roles of these substances within the brain in human hypertension. Blood and CSF samples were obtained from 10 patients with mild to moderate essential hypertension (EHT) aged 40-65 y and 10 age-matched normotensive subjects (NT) on a regular salt diet (8 g/day). Levels of ANP, NE, Na, K, Ca and Cl in CSF and plasma were comparable between EHT and NT. Plasma renin activity, plasma and CSF Ang II were lower in EHT than NT. CSF Ang III tended to be lower in EHT. There was no correlation between CSF and plasma ANP, or between CSF and plasma Ang II. Our results indicate that CSF levels of ANP may not be altered in middle aged patients with mild to moderate hypertension. It is also suggested that Ang II, NE and sodium in the central nervous system may not have important roles in hypertension of those patients.

Alpha-form of atrial natriuretic peptide released from dog atrium during atrial distension.

To assess the released form of atrial natriuretic peptide (ANP) during atrial distension, we compared the form of ANP before and during atrial distension in anesthetized dogs, using radioimmunoassay combined with high performance liquid chromatography (HPLC). When increasing the left atrial pressure (LAP) from 2.2 +/- 0.9 mmHg to 9.4 +/- 1.3 mmHg, the plasma ANP concentration in the coronary sinus was increased by 190% over the control, during left atrial distension. The major form of the released ANP extracted in the blood sample obtained from the coronary sinus was alpha-form. However other unidentified immunoreactive peaks, preceding and following the main peak were also discernible. These results suggest that the major form of ANP released during left atrial distension was the alpha-form.

Epidemiologic studies on schistosomiasis japonica in a newly found habitat of Oncomelania snails in Japan.

Epidemiologic studies' on schistosomiasis japonica were carried out in the Obitsu River basin of central Boso Peninsula, a newly found habitat of Oncomelania snails in Japan. Forty-five (4.1%) of 1,102 schoolchildren and 64 (10.4%) of 618 junior high school students were positive for the skin test with veronal buffered saline-extracted Schistosoma japonicum antigen. None was positive by the enzyme-linked immunosorbent assay, complement fixation test, double diffusion test in agar, and circumoval precipitin test. About one fifth of 356 adult inhabitants showed positive reactions in the enzyme-linked immunosorbent assay and complement fixation test as well as in the skin test, although only 25 individuals (7%) were positive for the circumoval precipitin test. All of 70 individuals who had shown positive reactions in at least two of the immunological tests were negative for a series of 5 stool examinations with the AMS III method. Release of S.japonicum cercariae was not observed in 15,000 O. hupensis nosophora collected in this area. Repeated examinations by dipping mice into irrigation ditches were negative. These observations, with the fact that all of 15 patients reported from nearby hospitals to have schistosome eggs in their tissues were over 45 years old, suggest that schistosomiasis has become a disease of the past in this area.

Cytogenetic findings and clinical features in acute leukemia and transient myeloproliferative disorder in Down's syndrome.

Cytogenetic, immunologic, and electron microscopic studies were performed on the blast cells of 28 pediatric patients with Down's syndrome, 13 with acute leukemia (DS-AL) and 15 with transient myeloproliferative disorders (DS-TMD). Clonal chromosome abnormalities were found in the cells of all patients with DS-AL but not those with DS-TMD. The younger ages and higher hemoglobin concentrations, platelet counts, and WBC counts of DS-TMD patients provided a clinical contrast with the frankly leukemic cases. Myelodysplastic syndrome, characterized by a small percentage of leukemic blast cells, was observed in 11 of the 13 patients with DS-AL compared with none in the DS-TMD group. Electron microscopy disclosed a positive platelet peroxidase reaction in each of the 11 DS-TMD patients and in nine of the 13 DS-AL patients. Immunologic studies revealed antiplatelet-megakaryocyte antigens on the blast cells of the majority of patients in both study groups. Our findings suggest that the blast cells in cases of DS-AL and DS-TMD arise from cells of the megakaryocytic lineage or from a myeloid progenitor with the capacity for megakaryocytic differentiation. The high risk of the development of AL in patients with DS who are less than 3 years old may be related to increased megakaryocyte proliferation in this age group.

[Neocarzinostatin therapy of advanced renal cell carcinoma].

Fifteen patients with advanced renal cell carcinoma underwent chemotherapy of neocarzinostatin (NCS). First, 6-8 mg of NCS was infused into renal artery at angiography; second, 6-8 mg was infused by the same route just before ligation of renal artery at nephrectomy, if possible; and third, 2 mg was given intravenously at 2-week intervals, about 10 times. One patient who suffered from metastatic renal cancer on rs. tibia was treated by femoral arterial injection of 2 mg NCS 5 times. By treatment of NCS for advanced renal cell carcinoma 2 out of fifteen patients achieved complete regression, one patient showed partial regression and four a minor response. Effective rate (CR + PR) of NCS for metastatic renal cancer was 20%, and the response rate (CR + PR + MR) was 47%. We consider that NCS is presently the most effective drug for renal cell carcinoma.

Use of cyclophosphamide and total lymphoid irradiation combined with cyclosporine in bone marrow transplantation for transfused severe aplastic anemia.

Between February 1984 and August 1987, 10 patients with severe aplastic anemia were treated with bone marrow transplantation from HLA-identical sibling donors after preparation with cyclophosphamide (CY) 200 mg/kg and total lymphoid irradiation (TLI) 750 cGy. Ages ranged from 5 to 28 years (median 14 years). All patients were previously transfused. Median number of transfusions was 16 (range, 3-886). For post-transplant immunosuppression all patients were given cyclosporine and the last three patients received additional immunosuppression with short-term methotrexate. All patients had initial engraftment and survived for more than 3-46 months after transplantation. One patient developed significant acute graft-versus-host disease (GVHD) and three of nine recipients who survived more than 100 days developed chronic GVHD. One male patient who had received 21 transfusions from his marrow donor before transplantation suffered from persistent granulocytopenia. Otherwise all have Karnofsky performance scores of 90-100%. Although the number of patients is small, it appears that allogeneic bone marrow transplantation with the regimen of CY + TLI for preparation combined with cyclosporine (+ short-term methotrexate) for post-transplant immunosuppression is a promising modality for treatment of previously transfused patients with severe aplastic anemia.

[Magnetic resonance imaging in spinocerebellar degeneration].

Magnetic resonance imaging (MRI) was evaluated in 11 patients with non-familial spinocerebellar degeneration (6 olivo-ponto-cerebellar atrophy (OPCA) and 5 late cortical cerebellar atrophy (LCCA]. MRI was carried out using a superconducting magnet of 0.256 tesla (VISTA-MR) and an inversion recovery pulse sequence of repetition time 2.08 sec and inversion time 0.5 sec. The degree of atrophy was assessed with regard to ponto-cerebellar system (basis pontis and middle cerebellar peduncle) and cerebellum in the sagittal and coronal images. In the mid-sagittal images, the width of ventral pons, dorsal pons, tegmentum and tectum of midbrain, and the height of fourth ventricle were measured. Especially, the degrees of atrophy of basis pontis in the mid-sagittal image and middle cerebellar peduncle in the coronal image were divided into 4 grades and evaluated respectively. On the other hand, atrophy of cerebellum was judged from enlargement of cerebellar fissures and reduction of cerebellar volume in the sagittal and coronal images. Atrophy of ponto-cerebellar system was found in OPCA, but not in LCCA. In OPCA, atrophy of middle cerebellar peduncle in the coronal image, which was likely to begin in an inferior part of pons, was more marked than, or equal to, atrophy of basis pontis in the mid-sagittal image. With regard to cerebellar vermis, the superior faces were more atrophic than the inferior faces in both OPCA and LCCA, but in OPCA, atrophy on the superior faces was dominant in the posterior lobe including declive and folium as against dominance in the anterior lobe in LCCA.(ABSTRACT TRUNCATED AT 250 WORDS)

[Treatment of complicated urinary tract infections by ofloxacin].

To study the treatment effects of ofloxacin (OFLX) on the patients with complicated urinary tract infections (UTI), OFLX at a daily dose of 600 mg divided into either two or three doses per day, was administered for 14 days to 114 patients with complicated UTI. Among the 114 patients, 67 patients satisfied the criteria for clinical evaluation of complicated UTI. One hundred and one organisms were isolated from these patients' urine. Among them, 82 organisms were eliminated following OFLX administration. Twenty organisms appeared newly by the replacement of organisms. Among them, 6 organisms belonged to Candida sp. As to the overall efficacy of OFLX, an excellent response was seen in 37.3% and sum of efficacy rate was 65.7%. Side effects were observed in 3 of the 114 patients. These results indicate that OFLX is both safe and effective enough to be used for the treatment of the patients associated with complicated UTI.
