Congenital choledochal dilatation associated with carcinoma after choledochocystoduodenostomy--case report and a review of the literature.

A 24-year old Japanese woman with congenital choledochal dilatation associated with carcinoma, cholelithiasis and pancreatolithiasis was treated. She had undergone choledochocystoduodenostomy at the age of 5 years. At the present admission, a dilated choledochus was excised and hepaticojejunostomy (Roux-en-Y anastomosis) and pancreatic ductojejunostomy were performed. Adenocarcinoma involved the entire mucosal surface of the resected specimen. We also present a review of the literature on the treatment and prognosis of patients with congenital biliary dilatation associated with carcinoma after bypass operation in childhood. We emphasize that in patients with congenital choledochal dilatation, the dilated choledochus should be excised, even in young children.

Role of the gene 17 protein of bacteriophage T4.

Head-related particles of bacteriophage T4 were examined by using heat leakage scanning calorimetry. The 17- particles showed two endothermic peaks on thermograms during their thermal transition process, while 49- particles gave only a single sharp endothermic peak. Thermograms of 17- particles treated with 23- defective lysate were different from those of nontreated 17- particles, and closely resembled thermograms of 49- particles. The 31- defective lysate was also capable of converting thermal properties of 17- particles. These results suggest that there is a structural difference between 17- particles and 49- particles, and that the product gene 17 of bacteriophage T4 is involved in the structural conversion of prohead to a more completed structure.

Plasma apolipoprotein A-IV levels decrease in patients with chronic pancreatitis and malabsorption syndrome.

An immunochemical method was developed for measurements of serum levels of apolipoprotein A-IV (apo A-IV). Using this technique, we found decreased levels of apo A-IV in patients with chronic pancreatitis and malabsorption syndrome and these low levels of apo A-IV in a patient with malabsorption syndrome were overcome after appropriate oral nutrition. Thus, measurements of apo A-IV may provide a good index for the assessment of fat intake and absorption.

[Clinical significance of the determination of serum carbohydrate antigen 19-9 in patients with carcinoma of the stomach and colorectum].

Serum CA 19-9 levels were measured in 68 gastric cancer patients and 38 cases of colorectal cancer. In patients with stage I-III gastric cancer or Duckes A-C colorectal cancer, the CA 19-9 levels ranged from normal to 127 U/ml, whereas in more advanced cases it was very high. Classified by the mode of recurrence, the highest level of CA 19-9 was observed in liver metastasis. After removal of the main tumor, the CA 19-9 level declined rapidly, increasing with recurrence. Serum CA 19-9 determination in cases of gastric or colorectal cancer was useful to classify the stage of disease, developing a prognosis and identifying a recurrence.

[Effects of delivery positions on the onset of first cry and umbilical blood gas parameters].

Physiological evaluation of sitting delivery position has not been well demonstrated. We measured the duration of 'the first cry occurrence time' both in supine (n = 54) and in sitting (n = 128) delivery positions. Umbilical blood gas analysis data were obtained from 130 pregnant women in sitting and 50 in supine delivery positions. To elucidate the mechanism of fetal blood gas differences due to posture, we also analyzed the maternal arterial blood gas during delivery (n = 145) and prior to labor (n = 100) in both positions. The first cry occurrence time was significantly shorter (p less than 0.01) in the sitting group. A weak negative correlation (r = -0.355, p less than 0.01) was found between the umbilical pH and the first cry occurrence time. Blood gas values for the umbilical vein and artery in the sitting group were significantly higher in pH, Po2, BE and Sao2, and lower in Pco2. Maternal blood gas values not only at delivery but also before labor did not elicit any significant differences between the two groups. It is suggested that the infants who have a high pH in their umbilical vessels cry sooner than those with a low pH. The cause of umbilical blood gas improvements induced by sitting delivery position is not directly due to the maternal blood gas difference, but may be mediated through other factors.

Effects of total-body hyperthermia on metastases from experimental mouse tumors.

To study the effects of total-body hyperthermia (TBH) on metastases from malignant tumors, Lewis lung carcinoma (LLC)-bearing C57BL/6 mice and mouse ascites hepatoma 134-bearing C3H/He mice were immersed in a heated water bath. Rectal temperature was maintained for 30 min at 40 degrees C or 42 degrees C. After treatment, the incidence of lung metastasis was analyzed in LLC-inoculated mice, and the presence or absence of metastasis in affiliated lymph nodes was determined in mouse ascites hepatoma 134-inoculated mice. A significant inhibition in primary tumor growth in LLC- and mouse ascites hepatoma 134-bearing mice treated with 42 degrees C TBH was noted. The incidence of lung metastasis was increased from the control level of 1.6 +/- 0.63 (SD) to 2.4 +/- 0.98 in the 42 degrees C TBH (P less than 0.01) groups but not in the 40 degrees C TBH group. Metastasis to affiliated lymph nodes was similar for the controls and the 40 degrees C and 42 degrees C TBH groups. The increase in lung metastasis in LLC-treated mice subjected to 42 degrees C TBH could be prevented by the combined use of anticancer drugs such as cis-diamminedichloroplatinum(II) (1.0, 3.0 mg/kg) or mitomycin C (0.3, 1.0 mg/kg). Furthermore, the combined use of 42 degrees C TBH and anticancer drugs showed the inhibition of primary tumor growth to a greater degree than did 42 degrees C TBH alone or anticancer drugs alone. Since 42 degrees C TBH may induce tumor metastasis, especially hematogenous metastasis, it seems advisable to use anticancer drugs in combination with clinical thermal applications.

[Use of cefminox in infections in the field of obstetrics and gynecology].

A total of 15 cases of obstetrical and gynecological infections was treated with cefminox (CMNX, MT-141), a new cephamycin antibiotic, with following results. The subjects consisted of 3 cases of salpingitis, 2 cases of parametritis, 5 cases of endometritis, and 1 case each of puerperal fever, inflammation of the pelvic dead space, Bartholin's pyocele, vulvar abscess and suppurative mastitis. In 2 cases of endometritis, pelveoperitonitis and adnexitis were complicated, respectively. As a rule, CMNX was administered intravenously at a dosage of 1 g each twice a day by drip infusion route. The clinical results were rated as excellent in 8 cases, good in 6 cases and poor in 1 case, with an efficacy rate of 93.3%. No subjective or objective side effects were seen nor any abnormal laboratory test results were found.

[Prophylactic and therapeutic continuous hyperthermic peritoneal perfusion for peritoneal metastases of gastric cancer].

Continuous hyperthermic peritoneal perfusion (CHPP) using the physiologic salt solution with the addition of Mitomycin C was performed for patients with peritoneal dissemination of gastric cancer. The result was favorable in some of them but unsatisfactory in survival. On the other hand, prophylactic CHPP for gastric cancer patients with serosal invasion revealed the favorable result in postoperative survival.

[Total-body hyperthermia and combined anticancer chemotherapy].

The present status of extracorporeally induced total-body hyperthermia (TBHT) for far-advanced cancer was presented from data collected from Japanese patients given this treatment. TBHT was most effective in lung cancer. In analyzing the anticancer effects of TBHT according to cancer site, the highest efficacy was observed in patients with their main cancer in the lung or liver, irrespective of whether tumorigenesis was primary or secondary. The anticancer effects were more enhanced when TBHT was combined with dosage of cis-DDP. What is important in combined chemotherapy is to determine what agent(s) should be applied and when. The most useful method for examining the thermosensitivity and thermochemosensitivity of agents for drug selection was the human tumor stem cell assay using individual cancer cells. Further, the usefulness of angiotensin II-induced hypertensive chemotherapy during TBHT in augmenting selective drug delivery to tumors was stressed.

[Metachronous hematogenous metastasis of gastric cancer--histopathologic characteristics of gastric cancer developing of the liver, lung or bone metastasis].

Fifty-seven gastric cancer patients developing metachronous hematogenous metastasis were histopathologically analyzed. In 37 patients who developed metachronous liver metastasis as the main pattern of recurrence, papillary adenocarcinoma was the most frequent histologic type, followed by poorly differentiated adenocarcinoma of the medullary type. In 20 patients with metachronous lung or bone metastasis, poorly differentiated adenocarcinoma of the scirrhous type was the common histologic pattern. From the present data, we assume that liver metastasis of gastric cancer occurs when cancer cell clusters are transported to the liver, where they attach and proliferate, whereas single cells passed through the portal system tend to establish secondary deposits in the lung or bone.
