[The use of Ovestin to overcome the side effects of treatment with GnRH agonists (Zoladex)].

The LHRH analogues are used very much in gynecological practice, mostly for a long treatment periods. The menopausal side effects occur often and causes the patients to withdraw from treatment. The GnRh analogues are used for the treatment of endometriosis, uterine fibroids, for breast cancer, and pre and post operatively as hormonal therapy for endometrial cancer. 12 patients were given combined vaginal Ovestin and GnRh analogues (Zoladex). Another 12 patients were taking GnRh analogues alone without Ovestin creme, and this group searched as a control group. The group with the Ovestin had less side effects than the control group. This difference was statistically significant about the cervicovaginal symptoms (p less than 0.01). The treatment with vaginal estrogenes can better and improve the tolerance to therapy with GnRh analogues.

[The incidence and structure of perinatal mortality in Bulgaria].

An analysis of the rate, structure and the principle causes of the perinatal infant mortality during the last 7 years in Bulgaria is presented. The values of the analysed indices are compared with those of the period 1985-1990 year. The main causes in the structure of the perinatal infant mortality during the transitional period in Bulgaria are discussed.

[The Multiload--a modern intrauterine device].

In the first part of publication the authors make a profound review of history of development and qualities of Intra Uterine Devices/IUD's/. There are reflected the first steps dated from the ancient times to come to modern IUD's (Multiload, Copper T. Slimline). In Table 1 and 2 (on W.A.A. van Os) are represented single cases (pregnancy, expulsion and extracted IUD's) related to the modern intrauterine devices: MLCu 250 and MLCu 375 for the periods of 1, 3 and 5 years. In the second part the authors represent their scientific and practical results which has been obtained at the Bulgarian Family Planning and Sexual Health Association and the Ob./Gyn. Department of Medical University-Sofia. A retrospective research has been conducted on 406 women who delivered between 18 and 42 years of age and to which MLCu 375 have been inserted. The so analyzed and presented results show very good qualities of MLCu 375 IUD. The cases' frequency corresponds to the one announced by the world leading practice of contraception.

[Current monophasic hormonal contraception].

Data are presented from an investigation of the use of monophasic contraceptive pills chosen by different age groups. Analyses are made of the data obtained from the Bulgarian Association for Family Planning (BAFP) card index for the last two years. Information is given on the number of observed cycles, type of applied contraceptive pill, aims of their application (contraception, treatment of acne, menstrual disturbances etc.), age of patients, as well as any possible adverse effects--change in blood pressure, bodyweight changes, intermenstrual bleeding, hairiness, breast problems, paraclinical changes, etc. The observed cycles show a very small percentage of adverse effects with hormonal contraception, mainly during the first 2 or 3 cycles, with predominance of intermenstrual bleeding. No bodyweight changes or increased hairiness were observed.

Novel alleles of the chemokine-receptor gene CCR5.

The CCR5 gene encodes a cell-surface chemokine-receptor molecule that serves as a coreceptor for macrophage-tropic strains of HIV-1. Mutations in this gene may alter expression or function of the protein product, thereby altering chemokine binding/signaling or HIV-1 infection of cells that normally express CCR5 protein. Indeed, homozygotes for a 32-bp deletion allele of CCR5 (CCR5-delta 32), which causes a frameshift at amino acid 185, are relatively resistant to HIV-1 infection. Here we report the identification of 16 additional mutations in the coding region of the CCR5 gene, all but 3 of which are codon altering or "nonsynonymous." Most mutations were rare (found only once or twice in the sample); five were detected exclusively among African Americans, whereas eight were observed only in Caucasians. The mutations included 11 codon-altering nonsynonymous variants, one trinucleotide deletion, one chain-termination mutant, and three synonymous mutations. The high predominance of codon-altering alleles among CCR5 mutants (14/17 [81%], including CCR5-delta 32) is consistent with an adaptive accumulation of function-altering alleles for this gene, perhaps as a consequence of historic selective pressures.

[The treatment of acute mycotic colpitis with ciclopiroxolamine in patients using oral contraceptives].

The author reveal the results of treatment with ciclopiroxolamine of women suffering from mycotic colpitis and using OC's. The research covers 26 women users of monophasic OC's with mycotic colpitis during the last 2-3 weeks before the research. The cause for the colpitis in 92% of cases is Candida albicans. The treatment was successful in 84.6% of the cases after the first 6-day-course with Batrafen creme. With 7.7% the treatment course with Batrafen creme was repeated, and with another 7.7% was included oral antimycotic (itraconazol). After treatment was performed microbiological check-up that confirmed the positive result of treatment. The authors' conclusion is that Batrafen creme is an effective device suitable for application on patients, CO's users.

[Tocolysis with the beta-adrenomimetic Partusisten in premature labor].

A total of 46 pregnant women with premature delivery and preserved amniotic sac were examined. The effect of tocolysis applied was recorded at different control periods of time with the aid of obstetrical monitor. The basic frequency of uterine contractions was more than 3--84.78%. In the 120th min after tocolysis with Partusisten 50% of the cases did not show any labour activity: after 120 min the inhibition was very successful--82.61%. This success was mainly recorded in the cases with uterine contractions over 4 for 10 min (31.61%) followed by those with 3-4 contractions (34.78%) and those with up to 2 uterine contractions (15.22%). On the basis of the results obtained it is concluded that the quicker and earlier the tocolytic was applied, the quicker and lasting was the effect. The statistical analysis revealed that all uterine contractions in the second and particularly in the third group depending on the time of tocolysis gradually passed into the first and second group.

[Perinatal problems and the prevention of premature labor].

An analysis of the principal indices, associated with the perinatal mortality rate of premature infants and the level of prematurity during the last 3 years, compared with those of 1989 and the similar indices in the developed countries of Europe and North America is presented. The main trends in prenatal and intranatal prevention of the premature delivery and the complications associated with it are discussed. The most common neonatal morbidity of the premature infants with very low birthweight and the contemporary possibilities of prevention are described.

[The CO2 laser treatment of extensive skin changes to the vulva caused by the human papillomavirus (HPV) and refractory to other types of therapy].

The authors describe three cases of large vulvar lesions caused by HPV. One patient has condylomata covering entirely the labia majora and minora, the clitoris, the perineum and the perianal area. The lesions made the structures of the vulva hard to distinguish. She was treated with podophyllin, Solcoderm and diathermy without effect. After 2 applications of Laser vaporisation 40 days apart she was completely cured. The other patient had two big squamous papillomata 3.5 by 3.5 cm and 0.5 cm thick situated on both sides of commissura posterior. She was also treated unsuccessfully with Podophyllin, Solcoderm and surgical excision. She was completely cured by a single-stage CO2-laser treatment. The third patient had condylomata acuminata of the labia minora and majora, fossa navicularis, the perineum and the perianal area. No other treatment had been attempted before. After two sessions of laser vaporisation she was cured, too.

[Family planning instruments].

Under the contemporary conditions family planning is being connected with the problems of family and reproductive health. Family planning is realized spontaneously or deliberately using definite ways, called methods and instruments of family planning (demographic instruments, sociological and economical instruments, technical instruments of medico-biological character). It has been given a great significance of the sociological and the economical ways and methods, since it has been pointed out the role of the economical determinants of the reproductive behaviour (encouraging activities; restrictive sanctioning measures). At present these measures are not effective in Bulgaria. At the best case the spouses have know each other partially affected in their behaviour mainly to shorten the interval between births (wanted and unwanted). Technical devices of family planning in Bulgaria have been gradually coming into use. The effects and their utilization is still on a comparatively lower level. This is due to several important moments (administrative level, pronatalism, lack of currency, misconception in govern policy, etc.).

[Hemorheological parameters in the prognosis of the risk of fetal retardation in pregnancy with arterial hypertension].

The haemorheological theory creates the notions of the interrelationship between the haemodynamic alterations in the blood flow and the whole-blood viscosity. The whole-blood viscosity is a variable parameter in close relation to the vessel geometry and blood flow rate, the blood plasma viscosity, the volume concentration of the blood cells, mostly of the red blood cells (the haematocrit) as well as to the hydrodynamically effective haemorheological phenomena of the aggregation and deformability of the erythrocytes. In the onward development of pregnancy in Bulgarian women with pre-eclamptic signs an increase of the haematocrit, the haemoglobin concentration and the erythrocyte aggregation were found. These data could be of benefit as reference point of possible risk-conditions for the mother during the pregnancy advance and in the same time of manifestation a retardation and/or stillbirth or neonatal death.

[The medicosocial aspects of contraception for teenagers].

The authors study and examine the influence of some medical and social factors on the teenagers use of contraceptives. The study is based on the questionnaires fulfill by 568 teenagers (aged 13-19). In the enquêtes are shown the role of the family status of the parents the involvement of the family and the school in the sex education, and also are indicated the favorite teenager's contraceptive methods. The positive attitude on contraception and on this kind of researches is very characteristic for 94% of the analyzed teenagers. The authors made a conclusion on optimizing the use of modern contraceptives for teenagers.

[Tocolytic substituent treatment with Magnerot in threatened abortions and premature labor from the 16th to the 36th gestational week].

The Mg's low level in the sera and the urine correlate with clinical symptoms of uterine contraction in pregnant women from the 16th-36th gestation week. The authors present their experience with the use of removal tocolytic curing with Magnerot tabl. in the cases of 60 pregnant women with uterine pain and contractions from 16th to 36th gestation week and Pelvic Score after Bishop under 4. The effect of the curing is remarkable till from the beginning in the first 4-7 days of it, which is demonstrated with disappearing of the objective and subjective contractions (overviewed in obstetric monitor) and there is normalising of the paraclinical levels of the Mg in the sera and the urine, with normal pregnancy and birth of a healthy and alive child.

Rapid assay of dinitrophenyl derivative of taurine by high-performance liquid chromatography.

A rapid and simple method for the determination of taurine (2-aminoethanesulphonic acid) in complex samples is described. It is based on the HPLC separation of the dinitrophenyl (DNP) derivative of taurine. The reaction conditions are selected to allow complete derivatization of taurine within 15 min. DNP-taurine samples are stable for at least 3 days. DNP-taurine was separated by reversed-phase liquid chromatography within 12 s. The recovery of taurine was 102 +/- 3% (S.D. = 2.5%, n = 6) and the detection limit was 10 pmol for taurine (signal-to-noise ratio of 10). The method was applied to the determination of taurine levels in different samples including marine products, infant formulas and fermentation media of different bacterial species.

[A comparative study of the potentials of a cytological method, the progesterone test and echography in the early detection of endometrial carcinoma in women in the postmenopause].

An attempt was made to examine and compare the abilities of the cytological method, the progesterone test and the uterus sonography for early finding and screening of endometrial cancer in postmenopausal women. There were studied three groups of patients, each group of them consisted of 900 women, for the period of 10 years. The uterus sonography and progesterone test showed the best results. The cytological smears method was found not suitable for screening programme.
