Hyperinsulinemia in rats with hypothalamic obesity: effects of autonomic drugs and glucose.

The present study examined the effects of autonomic drugs and glucose on the insulin and glucose concentrations of sham-operated rats and of rats with ventromedial hypothalamic (VMH) lesions and obesity. In the basal condition both epinephrine and atropine significantly decreased serum insulin levels in VMH-lesioned but not sham-operated rats. During glucose stimulation of insulin secretion in VMH-lesioned rats, epinephrine inhibited the increase of insulin by 83% and atropine inhibited it by 42%; whereas in sham-operated rats, epinephrine inhibited it by 70% and atropine inhibited it by 34%. Epinephrine with atropine completely blocked the increase of insulin in response to glucose in both VMH-lesioned and sham-operated rats. In the basal condition, epinephrine together with propranolol significantly decreased serum insulin levels in VMH-lesioned but not sham-operated rats. Epinephrine with phentolamine, on the other hand, markedly increased insulin in the VMH-lesioned rats and to a lesser degree in the sham-operated rats. During glucose stimulation epinephrine with propranolol inhibited the increase of insulin in both groups. Epinephrine with phentolamine or isoproterenol markedly increased serum insulin in VMH-lesioned rats. These results suggest that stimulation of the vagus nerve and increased sensitivity of the beta-receptors on the beta-cells of the islet contribute to the development of hyperinsulinemia. The sympathetic contribution may also be through suppression of alpha-adrenergic receptors.

Pancreatic polypeptide and insulin contents in diabetic and nondiabetic human pancreas and their relationship to the stability of the fasting serum glucose.

The amounts of insulin and pancreatic polypeptide (PP) in twenty four autopsied diabetic and nineteen nondiabetic human pancreases were determined and their relationship to the stability of the fasting serum glucose level was investigated. The PP content of the tail of the pancreas in diabetic and nondiabetic subjects was 9.55 +/- 2.41 and 7.71 +/- 1.52 micrograms/g pancreas, and that of the head of the pancreas was 16.86 +/- 5.51 and 15.82 +/- 5.38 micrograms/g, respectively. No significant differences in content were found between diabetic and nondiabetic pancreases. The PP content of the head pancreas of some diabetics and nondiabetics was higher than that of the tail. The insulin content of the tail of the diabetic pancreas was lower than that of the nondiabetic pancreas. In those diabetics where there was less than 0.5 U/g of insulin in the tail pancreas, the stability of the fasting serum glucose was very poor, indicating an unstable type of diabetes. There was a significant inverse correlation between standard deviation of FBS and the amount of insulin, but the PP content of the pancreas had no relation to the stability of the fasting serum glucose.

The relationship between diabetes mellitus and coronary heart disease.

The present study was aimed at investigating the relationship between diabetes and coronary heart disease. 100 g oral glucose tolerance test was performed on 32 well-defined male non-obese patients with coronary heart disease (myocardial infarction and angina pectoris, 16 patients each; average age 47.5 years) and 12 non-obese controls (average age 46.2 years). The results were as follows: 1) Glucose tolerance was impaired in 19 of 32 patients. 2) Insulin response to glucose in patients with normal glucose tolerance was similar to that in controls, while insulin response in patients with impaired glucose tolerance showed a delayed peak. 3) The insulinogenic index of patients with normal glucose tolerance was the same as that of controls, while the insulinogenic index of patients with impaired glucose tolerance was lower, and 4) overnight fast serum cholesterol and triglycerides in patients with normal glucose tolerance showed similar levels to those in controls, but were elevated in patients with impaired glucose tolerance. These results suggest that impaired glucose tolerance observed in patients with coronary heart disease may result in part from "latent diabetes."

[Immunochemotherapy of recurrent and advanced uterine and ovarian cancer using cisplatin].

A combination immunochemotherapy regimen containing cisplatin (20 mg, days 1-5), peplomycin (20 mg, days 2, 9, 16), +/- vinblastine (5 mg, days 1, 2), and picibanil (3-5 KE/week) was performed in twelve patients with advanced or recurrent uterine and ovarian cancers under intravenous hyperalimentation (IVH), except one patient receiving peplomycin by a continuous infusion method using IVH bag (10 mg/day for 5 days). This regimen was repeated every three weeks. Five (71.4%) of seven evaluable patients showed partial response. No patients yielded the complete disappearance of disease. No severe and lethal pulmonary or renal dysfunction occurred and all was well tolerated. In a regimen without vinblastine, myelosuppression, especially thrombocytopenia, occurred later compared to the regimen including vinblastine.

[Mixed mesodermal tumor of the uterine body].

During the 6-year period of 1976 to 1981, we studied 6 cases of mixed mesodermal tumor of the uterus. The incidence of occurrence of the tumor was 0.4% of all the gynecological inpatients, 1.4% of all the uterine malignancies, and 9.5% of the uterine body malignancies, and was higher than those reported by others. The patients were aged 49-75 years (mean 64 years), and 5 out of 6 patients were postmenopausal. Four patients had heterogeneous tumors (3 rhabdomyosarcomas and 1 chondrosarcoma). Two of the 6 patients are now living 34 and 14 months after treatment without clinical evidence of recurrence, respectively. Histologically, one of them showed only benign glandular structures as a component of epithelial elements, and the other one had no epithelial element. These findings suggest that the malignancy of the epithelial components may be one of the most important prognostic factors in the mixed mesodermal tumor of the uterus.

An aspect of metabolic bone disease with idiopathic osteonecrosis of the femoral head.

In spite of vast varieties of investigations on osteonecrosis, there is no report concerning the serum vitamin D metabolite concentrations and quantitative assessment of histological parameters of undecalcified section of bone. In this study, we examined the relations between biochemical and histological parameters in four patients with idiopathic osteonecrosis of the femoral heads. Serum 25-hydroxyvitamin D3 concentrations showed lower value within normal range. However, 1, 25-dihydroxyvitamin D3 and 24, 25-dihydroxyvitamin D3 concentrations were far below the normal value in all patients. Serum immunoreactive parathyroid hormone concentrations were within normal range. Histomorphometric parameters of iliac crest biopsy specimens were as follows. Relative osteoid volume, osteoid surface, and resorption surface were increased in all patients. Calcification front was decreased in all patients. Mean osteoid seam width was increased in two patients. These results suggest that microfractures created in such fragile trabeculae of femoral heads in our patients with deficiency of active vitamin D metabolites, will not be repaired well.
