A case of primary malignant melanoma of the vulva associated with pregnancy in a 31-year-old patient is reported. The primary tumor was first found at week 14 of pregnancy and biopsied on delivery at week 41 of pregnancy. Microscopically, the depth of invasion was Level V. Radical vulvectomy and bilateral inguino-femoral lymphadenectomy were performed and the right inguinal lymph node was positive for metastatic tumor. Chemoimmunotherapy with DTIC, ACNU, VCR and OK432 was undertaken as treatment for this malignant melanoma. Moreover, a comparison of scanning and transmission electron microscopic studies of the original tumor was carried out. This patient is now living 23 months after diagnosis without clinical evidence of recurrence.

A case of congenital esophagobronchial fistula in a 46-year-old woman was described. The patient had presented with repeated attacks of pulmonary infection from her childhood and had been diagnosed pneumonia at least five or six times after the age of 18 years. Preoperative examinations including esophagogram by barium swallow revealed the existence of a fistula between middle thoracic esophagus and left B6 bronchus. The fistula was recognized as Type II of Braimbridge and Keith's classification, and successfully resected with left lower pulmonary lobe. Sixty-eight cases with congenital esophagobronchial fistula in adult including our own case have been reported in Japanese literature and about 70 cases in the literature of western countries. From the analysis of these cases, diagnostic basis on congenital origin of the fistula, useful diagnostic methods, types of the fistula and appropriate surgical procedures were discussed.
Thromboplastic and fibrinolytic activities of 8 lines of cultured human gastric cancer cells were estimated both in cell lysate and serum-free supernatant fraction. Furthermore, cell lysates with differing levels of thromboplastic activity were injected intravenously into congenitally athymic nude mice and the role of this activity in thrombus formation was examined. Thromboplastic and fibrinolytic activities of the cell lysate and the serum free-supernatant fraction varied from one line to another. There was no apparent correlation between these activities and the degree of histological differentiation of the original tumor. The thromboplastic activity was factor VII-dependent and factor IX-independent, indicating that it was attributable to tissue thromboplastin, although some factor VII-independent thromboplastic activity was also included in the cell lysate of two lines. Intravenous injection of the cell lysate with high thromboplastic activity produced more thrombi in the lung than that with low thromboplastic activity. This suggests that thromboplastic activity of cancer cells might play a significant role in the development of the hypercoagulable state in patients with gastric cancer.
External skeletal fixation using self-curing acrylic resin was applied by inserting A-O cortical screws into the diaphyseal region and A-O cancellous screws into the epiphyseal and/or metaphyseal regions. The position of the screws was designed to secure three-dimensional fixation. Points of insertion are selected that avoid infected bone, open wounds, thick muscles, and skin that is relatively mobile during joint motion. After reduction the acrylic resin rather than conventional metal linking devices is used for fixation of the screws. Once the fixation is completed, it is adjustable and so rigid that exercises for joint function are possible. The apparatus is light in weight and sufficiently compact to fit under clothing. In the 204 patients treated by this method, open tibial fractures occurred in 123; nonunion or delayed union in 28; metaphyseal and epiphyseal fractures in 30; femoral fractures in ten children; and miscellaneous fractures and other conditions in 13. Nearly all of the operations were successful. Three patients had refractures after removal of the fixation device.

The tissue activator was extracted with 2 M ammonium thiocyanate and purified by L-arginine methyl ester, concanavalin A and ion-exchange chromatographies, and Sephacryl S-200 gel filtration in buffers containing Triton X-100 and/or ammonium thiocyanate. The final preparations had specific activities of 25 000-40 000 IU/mg protein and were shown to be a single band with an apparent molecular weight of 54 00 by SDS-polyacrylamide gel electrophoresis with or without reducing agent. When subjected to isoelectric focusing, its major component had an isoelectric point of approx. 8.2 with minor components. (7.8-8.6). The purified tissue activator was a serine protease, dissimilar to urokinase in some respects including antigenicity, strong affinity to insoluble fibrin monomer and hydrolytic activities for synthetic substrates. The crude extract contained another plasminogen activator with antigen identity to urokinase, which constituted approx. 15% of the total activity in crude extract. These findings indicated that human kidney would produce at least two plasminogen activators, namely, the tissue activator as a major plasminogen activator and urokinase.

The regenerative capacity of limbs was investigated by amputation of limbs at the zeugopodium in postmetamorphic froglets and adults of various sizes in four species of Japanese frogs, all of which showed some regeneration at these ages. In Hyla arborea japonica and Rana brevipoda porosa most young froglets regenerated their limbs well; however, the rate of regeneration decreased with the age of amputation, and the limb became nonregenerative in adults. Limbs of adults in Rana rugosa and R. japonica, on the other hand, exhibited good regeneration. All of the regenerates in the four species were heteromorphic, consisting histologically of well-developed cartilaginous rods surrounded by connective tissue and skin. Limited development of muscle was apparent in regenerates of the three ranid species. The relations between body size, innervation of limbs, and regenerative capacity are discussed.
