Corneal endothelial changes after posterior chamber intraocular lens implantation in patients with or without diabetes mellitus.

We examined 96 patients (111 eyes) who underwent extracapsular cataract extraction with the implantation of a posterior chamber intraocular lens. Of 96 patients 24 (29 eyes) with non-insulin-dependent diabetes mellitus had no or simple retinopathy; 72 patients (82 eyes) served as non-diabetic controls. The central corneas of all subjects were photographed by a specular microscope preoperatively and three, six, and 12 months postoperatively. No significant differences in the endothelial cell density, coefficient of variation, or cell loss were noted between diabetic patients and age matched non-diabetic subjects during the observation period.

Causes of subconjunctival hemorrhage.

We examined prospectively 8,726 patients in outpatient eye clinics. A total of 225 (2.9%) patients had subconjunctival hemorrhage. No sexual or age predilection was found. The most common causes for the condition were minor local trauma, systemic hypertension, and acute conjunctivitis. Subconjunctival hemorrhages resulting from local trauma were frequent in the summer, and those associated with systemic hypertension were noted most often in older patients. Blood pressures should be examined in patients with subconjunctival hemorrhages, particularly in older patients.

Pericentral retinal degeneration deteriorates during pregnancies.

A 25-year-old pregnant woman complained of a transient decrease of vision in her left eye. No ring scotoma was described. During her second pregnancy at age 27, the patient noticed subtle ring scotomas in both eyes. During her third pregnancy at age 30, the scotomas enlarged. A grayish zone with a few small areas of pigmentation around the macula was seen bilaterally. At age 37, the retinal degeneration progressed slightly: results of visual function and electrophysiologic examinations revealed mild rod and cone dysfunction. We believe that this patient represents a rare case of deteriorating pericentral retinal degeneration during pregnancies.

Myelinated retinal nerve fibers: prevalence, location and effect on visual acuity.

We examined fundus photographs of 5,789 patients (11,578 eyes) and found 39 patches of myelinated nerve fibers in 35 eyes of 33 patients. The prevalence was 0.57%. The lesion was more frequent in females than in males. The patch occurred more often at the superior sectors of the nerve head and in the inferotemporal retina than in other areas. No myelinated nerve fibers were discovered in the macula. One patient (case 1) had 3 patches in 1 eye. Two patients (cases 2 and 3) had unilateral myelinated nerve fibers, bilateral myopia and ipsilateral amblyopia. In case 3, the patch was associated with an epiretinal membrane. Another patient (case 4) had branch vein occlusion and neovascularization from the patch of myelinated nerve fibers. The fundus conditions observed in our cases 1-4 may be uncommon.

Abnormally distributed branches of the retinal vessels, enlarged macular arteries and long cilioretinal arteries.

We examined fundus photographs of 3,506 eyes and noted uncommon retinal vessel patterns in the temporal quadrants of the posterior fundus. Of 1,753 subjects, 20 patients (21 eyes) had unusual retinal vessel patterns. Seven eyes had an abnormally distributed branch of the inferotemporal artery; 8 eyes had an enlarged macular artery. Of these 20 patients, 1 had visual complaints associated with the unusual vascular patterns.

Multiple congenital anomalies associated with Duane's syndrome.

A 33-year-old man had Duane's retraction syndrome, microphthalmos, mixed hearing loss, short fingers and toes, and horseshoe kidneys on both sides and a right epibulbar lipodermoid. The patient had undergone surgical procedures to correct a cleft lip and palate and a left undescended testis. His parents were first cousins. Results of a chromosomal study revealed a normal male karyotype. The association of multiple congenital malformation in this patient may be rare.

Anomalous macular vessels: case report and review of the recent Japanese literature.

We examined a 19-year-old woman with large macular vessels who had temporarily impaired visual acuity. The anomalous macular vessels had arteriovenous shunts and were associated with abnormal capillary networks. In a review of the recent Japanese literature, we found another eight patients with anomalous macular vessels who had undergone fluorescein angiography. We prefer the term anomalous macular vessels to describe this condition.

Inverted eyelashes in patients with type 1a glycogen storage disease.

We examined three adult Japanese patients who had a history of decreased hepatic glucose-6-phosphatase activity. All three patients had increased bilateral subcutaneous (SC) fat in the lower eyelids and inverted eyelashes. One patient additionally showed retinal hemorrhages and microaneurysms in both fundi. The inverted eyelashes may have been related to type 1a glycogen storage disease.

Epiblepharon with inverted eyelashes in Japanese children. I. Incidence and symptoms.

Epiblepharon commonly occurs in Japanese infants and tends to disappear spontaneously with age. We examined 4449 Japanese children aged 3 months to 18 years for epiblepharon associated with inverted eyelashes touching the cornea. The condition was evident in 441 cases. We found that the incidence of epiblepharon decreased with age, but about 2% of high school students still had the condition. No sexual predilection was found. Lower eyelids were commonly involved bilaterally. Most cases of epiblepharon produced no or mild symptoms.

Epiblepharon with inverted eyelashes in Japanese children. II. Surgical repairs.

We used lid bracing sutures, buried sutures, and skin resection operations to correct epiblepharon in Japanese children. Epiblepharon and ocular irritation in 103 children (254 eyelids) were treated first with lid bracing sutures. Postoperatively the inversion of eyelashes resolved in 73 patients (196 eyelids). Of these 103 patients 30 (58 eyelids) developed recurrences, which included 11 who also complained of foreign body sensation and ocular pain. These 11 patients (22 eyelids) underwent a second surgical procedure (buried sutures or skin resection) before they became free of symptoms. No severe complications were noted in any of the 103 patients. We believe that lid bracing sutures correct most cases of mild epiblepharon in children and that buried sutures or skin resection are valuable for more severe conditions.

Lysosomal enzyme activities in ocular tissues and adnexa of rabbits.

We examined biochemically the distribution of lysosomal enzyme activities in the ocular tissues and adnexa of albino rabbits using p-nitrophenyl derivatives. The pink and white lobes of Harderian gland disclosed the highest specific activities of acid phosphatase, N-acetyl-beta-D-glucosaminidase, and alpha-L-fucosidase. The lacrimal gland showed moderately high specific activities of acid phosphatase and N-acetyl-beta-D-glucosaminidase. The optic nerve, brain, ciliary body plus iris, and retina plus choroid exhibited nearly the same specific enzyme activities. The tear fluids showed higher lysosomal enzyme activities than did the serum. To our knowledge, this is the first reported measurement of lysosomal enzymes in Harderian gland, lacrimal gland, and optic nerve of the rabbit.

Sphingomyelinase activity in the bovine and human eyes.

We examined biochemically sphingomyelinase activity in the bovine and human ocular tissues, using trinitrophenylaminododecanoylsphingosylphosphorylcholine as substrate. The enzyme activity in the crude extracts of neuroretina, retinal pigment epithelial cells, and optic nerve of the bovine eyes was proportional to protein concentration. The activities in these tissues were little activated with Mg2+ at pH 5.1 but was activated with Mg2+ at pH 7.5. The enzyme activity at pH 5.1 was also detected in the iris and ciliary body, neuroretina, retinal pigment epithelium and choroid, and optic nerve of the human eyes. It was highest in the macula, compared with other areas of the human ocular fundus.

Improvement of tear lysosomal enzyme levels after treatment with bone marrow transplantation in a patient with I-cell disease.

We examined the lysosomal enzyme levels in tear fluids from a patient with I-cell disease after treatment by bone marrow transplantation. Acid phosphatase, beta-D-glucuronidase, N-acetyl-beta-D-glucosaminidase, alpha-L-fucosidase and alpha-D-mannosidase activities were reduced to normal or nearly normal levels after the treatment. We believe that the measurement of tear enzyme levels in I-cell disease is useful for knowing the efficacy of the bone marrow transplantation.

Bilateral macular coloboma associated with progressive atrophy of the peripheral retina.

An 11-year-old girl with poor visual acuity since birth was found to have nearly complete defects of the retinochoroidal tissue in the central areas of both fundi and almost normal peripheral retinas. Her parents were first cousins. At age 24, the patient reported night blindness. An examination showed geographic atrophy of the peripheral retina and stationary macular lesions in both eyes. Her condition appeared to be a rare case of bilateral macular coloboma associated with progressive peripheral retinal atrophy.

Persistent hyperplastic primary vitreous in the right eye and congenital grouped pigmentation of the retina in the left.

We have recently treated an 8-year-old boy who had leukocoria, microcornea, cataract, and falciform retinal fold in the right eye and multiple grouped patches of pigmentation in the left retina. These were diagnosed as persistent hyperplastic primary vitreous in the right eye and congenital grouped pigmentation of the retina in the left. This patient had a different rare congenital anomaly in each eye.

Postoperative instillation of mitomycin C in the treatment of recurrent pterygium.

We examined retrospectively 61 eyes of 61 patients with recurrent pterygia who had been treated with excision, with or without 0.02% mitomycin C instillation, 3 to 7 years after treatment. The rates of recurrence (5% to 9%) and complications in eyes that had undergone excision and mitomycin C instillation were less than those in eyes that had had excision only. These data suggest that the postoperative instillation of 0.02% mitomycin C, twice a day for five days, may be effective and safe in the treatment of recurrent pterygium.

Aging changes in lipofuscin, lysosomes and melanin in the macular area of human retina and choroid.

Biochemical examination was carried out on the lipofuscin content, lysosomal enzyme activities and melanin level in the retina and choroid of normal human eyes. Lipofuscin content, expressed as fluorescence intensity, in the macular retinal pigment epithelium (RPE) and choroid was two-to-three fold higher than that in other areas, and increased with aging. High activities of lysosomal enzymes in the macular area were found, but did not change with aging. Melanin level was two-to-three fold higher in the macular RPE and choroid than in other areas, and showed no alteration with aging.

Unilateral vaso-occlusive retinopathy in quiescent condition of systemic lupus erythematosus.

A 24-year-old woman with systemic lupus erythematosus had decreased visual acuity, retinal cotton-wool spots, venous dilations, and multiple arteriolar occlusions around the optic disc in her left eye. The right eye showed good visual acuity and a few small retinal hemorrhages. Although abnormal serologic findings and systemic manifestations quieted with corticosteroid therapy, the retinopathy in the left eye progressed. After treatment with focal argon laser photocoagulation, progression of the retinopathy stopped.
