Epidemiological follow-up study of Japanese Thorotrast cases--1980.

An epidemiological follow-up study was conducted on 272 Thorotrast-administered war-wounded ex-servicemen to determine their status as of 31 December 1980, after a lapse of 35-43 yr from Thorotrast injection. In the 251 cases who had been intravascularly injected with Thorotrast in the past, 40 malignant hepatic tumors, 3 blood diseases, 2 lung cancers, 1 osteosarcoma, 22 other malignant tumors, and 13 cases of liver cirrhosis were found. The mortality rates due to hepatic and other malignant tumors, blood diseases, and cirrhosis of the liver as well as the overall mortality rate were significantly higher in the intravascular-Thorotrast group than in the controls. In the remaining 21 cases, who had been given Thorotrast by a route other than intravascularly, none of the deaths was found to be related to Thorotrast administration.

Statistical analysis of Japanese Thorotrast-administered autopsy cases--1980.

In 193 cases autopsied between 1945 and 1980, all persons who had been intravascularly injected with Thorotrast in life, the authors found 131 malignant hepatic tumors, 20 liver cirrhoses, 6 myeloid leukemias, 4 erythroleukemias, 5 aplastic anemias, 4 lung cancers, 1 mesothelioma and 1 osteosarcoma. The causes of death in the Thorotrast-administered autopsy group (193 cases) were compared with those of a non-Thorotrast-administered autopsy group (95,000 cases) of the same sex and age at death as recorded in the Annals of Japanese Pathological Autopsy cases from 1958 to 1978. This comparison revealed that the frequencies of malignant hepatic tumors, liver cirrhosis, erythroleukemia, and aplastic anemia were significantly higher in the Thorotrast-administered group than in the non-Thorotrast-administered group.

Enzyme histochemical study on human thymus and its age change.

Human thymuses, ranging in age from newborn to 62 years old, were studied enzyme histochemically. The thymic epithelial cells covering cortical surface and bordering vascular areas in the medulla were positive for 5'-nucleotidase, but not for other enzymes. The thymic epithelial cells composing Hassall's corpuscles were positive for acid phosphatase, esterases, beta-glucuronidase, and alkaline phosphatase, regardless of age, but totally negative for 5'-nucleotidase and ATPase. All enzymes examined except for beta-glucuronidase were demonstrated in some of the thymic epithelial cells scattered in the medulla, although the pattern of distribution and the degree of positivity were different by enzymes. These findings suggest that the thymic epithelial cells are composed of functionally heterogenous subpopulations. Acid phosphatase was demonstrated in thymocytes in both cortex and medulla, but 5'-nucleotidase and ATPase were observed in some thymocytes in the medulla of young thymus.

Cushing's syndrome with bilateral multinodular adrenal hyperplasia. Ultrastructural, histochemical, and immunohistochemical study.

An unusual case of Cushing's syndrome of a 59-year-old man with bilateral multinodular adrenal hyperplasia and microadenoma of the pituitary gland is presented. Failure to suppress plasma cortisol with large doses of dexamethasone may suggest autonomous growth of hyperplastic nodules of the adrenals, which were at first induced by prolonged stimuli of ACTH from the microadenoma of the pituitary gland. ACTH could not be detected in the microadenoma cells on paraffin sections, while Crooke's cells were strongly positive for ACTH. The interrelation between bilateral multinodular adrenal hyperplasia and pituitary microadenoma is discussed.

Histochemical location of 17 beta-hydroxysteroid oxidoreductase in the adult and infantile human testis.

17 beta-Hydroxysteroid oxidoreductase in the human testis was investigated histochemically using tissues obtained from seven patients with undescended testis or varicocele at the time of orchiopexy or high ligation of spermatic vein. Formazan precipitates were formed from nitro-blue tetrazolium in the tissue utilizing hydrogen released by oxidation of testosterone, which is catalyzed by the reductase function of 17 beta-hydroxysteroid oxidoreductase. The precipitates were formed specifically in the presence of 17 beta-hydroxy-C19-steroids under the conditions employed in the present study. In infantile testes, the precipitates were formed in cytoplasm of immature Sertoli cells, while in pubertal or adult testes, marked formazan precipitates were found in cytoplasm of both Sertoli and Leydig cells. The results indicate the presence of two distinct 17 beta-hydroxysteroid oxidoreductases in the human testis; one in Sertoli cells and detectable independent of age and the other only in functional Leydig cells.

Morphometrical analysis of autopsy hearts of congestive cardiomyopathy.

To analyse the morphological changes of 25 autopsy hearts of 18 males, 7 females including 3 children with congestive cardiomyopathy (COCM), measurements of the heart weight, chamber volume, wall thickness, circumference of the valvular annulus, thickness of the valvular cusp and transverse diameter of the muscle fiber were performed. The mean onset of heart failure was 38 years of age, the mean duration 60 months, the mean age of death 43 years, the mean heart weight 513 g and the mean total chamber volume 286 ml, the mean chamber volume being 89 ml in the left ventricle (LV), 72 ml in the right ventricle (RV), 71 ml in the right atrium (RA) and 55 ml in the left atrium (LA). Various kinds of correlation among these measured parameters were analysed statistically. There was a significant correlation between the heart weight and the total chamber volume (P less than 0.05), the volume of LV and RV (p less than 0.05), the volume of RV and RA (P less than 0.01), the volume of RA and the circumference of the TV annulus (P less than 0.0001) and the volume of LA and the circumference of the MV annulus (P less than 0.001). These data were also compared according to the mode of death (arrhythmia versus heart failure) and the duration of survival (more than 5 years versus 5 years or less). No statistically significant differences between the above groups were found except for the total chamber volume.

A scanning electron microscopic study of the boundary zone of the human spleen.

The structural characteristics and cellular elements of the boundary zone between the white and red pulp of the human spleen were studied by SEM and TEM. The boundary zone consisted of both the perifollicular region and the region surrounding the periarterial lymphoid sheath. The perifollicular region was further subdivided into two, equally thick layers. The inner half layer of the perifollicular region outside the mantle zone of the lymph follicle was composed of tightly packed medium-sized lymphocytes, interspersed by a small number of reticular cells. The outer half layer was composed of a reticular cell meshwork containing blood cells in vessels, which communicated with the splenic cords of the red pulp. Intermittent rows of reticular cells distinguished the outer from the inner half layer. The region surrounding the periarterial lymphoid sheath revealed the same type of reticular cell meshwork as the outer half layer of the perifollicular region. Capillary ends opened into the reticular cell meshwork, which suggested the presence of an open circulation in the human spleen. A deep lymphatic vessel which communicated with the periarterial lymphoid sheath was noted.

Mucoepidermoid carcinoma of the thymic region.

A case of mucoepidermoid carcinoma in thymus in a 59-year-old Japanese female is presented. She died of cardiac tamponade due to tumor invasion ater a 5 years' clinical course. At autopsy the main tumor was found in the thymic region with metastases to the sternum, regional lymph nodes, pericardial, and left pleural cavity. The mucoepidermoid carcinoma might be probably originated from a hen's egg-sized cyst which was located in the upper posterior aspect of the tumor-involved thymus. No teratomatous components were present. The cyst was most likely to be of thymic or bronchogenic cyst origin, though it was not determined, in view of the lining with pseudostratified ciliated columnar epithelium of the cystic wall and the surrounding with the thymic tissue outside. Moreover, there was thymic hyperplasia with germinal center that was compatible with SLE-like symptoms in her past history and autoimmune nature of the autopsy findings of pulmonary fibrosis.

Freeze-preservation of staining solutions in enzyme histochemistry.

Staining solutions of acid phosphatase and esterase group (naphthol-AS-D-chloroacetate esterase, naphthol-AS-acetate esterase and alpha-naphthyl-acetate esterase) were stored for 6 months using the freezing facilities, and the staining results were compared with those of each set of controls. The results showed that the staining solutions of all enzymes were stable for long-term preservation (at least 6 months) in the deep freezer (-80 degrees C) when they were stored appropriately in separated forms. In such a freezing procedure, the staining results for each enzyme did not show any decrease of positive intensity nor any non-specific staining.

Experimental study on the drainage of so-called intravenous fat pigment from the liver. Long-term observation with histologic, enzyme-histochemical and electron microscopic analysis.

The drainage and decrease of so-called intravenous fat pigment (i.v. fat pigment), which was a lipoid-pigment complex and deposited in the reticuloendothelial system after repeated infusions of Intrafat (intravenous fat emulsion), was observed in the liver of the rabbit by histological, histochemical and electron microscopic methods. As a result, the i.v. fat pigment was mainly found in Kupffer cells immediately after the repeated injections of Intrafat. After then, depending on the period when no injection were given, it decreased gradually and disappeared finally from Kupffer cells. On the other hand, in the interstitium of Glisson's sheath, many phagocytes, extremely swollen and laden with i.v. fat pigment, appeared during the postinfusion period. Furthermore, these phagocytes laden with i.v. fat pigment were found in Glisson's sheath for a long period, although they showed a tendency of decrease. Histochemical and electron microscopic characteristics, quite similar to those of Kupffer cells, suggested that these phagocytes corresponded to the extremely swollen histiocytes or macrophages having some role in the drainage or the metabolism of the i.v. fat pigment.

A method for assessing chronic effects of toxic substances on the midge, Paratanytarsus parthenogeneticus-effects of copper.

A method was developed for the assessment of the chronic effects of copper on midge larva. Growth and reproduction of the midge was related to copper concentrations greater than 0.08mg/L; 50% reproductive impairment was apparent at a copper level of 0.37 mg/L. Growth and amount of food uptake by larvae were inversely related to copper concentration. Accumulation of copper in green algae was proportional to copper concentration in the water.

Morphological changes of myofibril in the carcinogenetic course of 20-methylcholanthrene-induced rhabdomyosarcoma.

In this study 20-methylcholanthrene (MC) was embedded in skeletal muscle tissue of mice. The resulting morphological changes were observed by light and electron microscope until the occurrence of rhabdomyosarcoma. The injected region of the muscle tissue in which MC was embedded went through three stages: destruction and degeneration, repair and appearance of atypical cells. The morphologic figures of the myofibrils seen in the skeletal muscle cell during destruction and degeneration were similar to nonspecific. The muscle tissue then regenerated in the same manner as seen during experimentally induced muscle tissue regeneration in both humans and animals. Abnormal mitotic figures were found 60 to 70 days after the injection of MC. Immature muscle cells, with tuft-formed myofibrils present, underwent abnormal mitosis in which one of the nuclei divided. The rhabdomyosarcoma cells produced by this abnormal mitosis began to appear in the lesion 70 days after the initial embedding of MC. The rhabdomyosarcoma cells in the induced tumor formed undifferentiated myofibrils in various degrees of maturation, and were therefore considered to play a part in the abnormal myofibrillar formation.

Demonstration of various antigens on paraffin sections of formalin-fixed tissues: trypsin-treated, indirect peroxidase-labelled antibody technique.

Detection of various antigens was performed on the formalin-fixed, paraffin-embedded sections by indirect peroxidase-labelled antibody technique after pretreatment with trypsin digestion. It was revealed that the optimal condition for trypsin digestion of the paraffin sections differed according to the character of antigen and type of tissue, and, under the optimal condition, this method was easily applicable to the regular paraffin sections obtained in the laboratories for routine examinations. This method was also recommended for the detection of the antigens which were usually stained even without trypsin digestion, since it could not only remove the background staining but also uncover the otherwise masked antigen.
