Bullous keratopathy accompanying essential iris atrophy: response to lowered intraocular pressure.

We present a patient with essential iris atrophy who was normotensive and yet had corneal edema and bullous keratopathy which cleared upon lowering the intraocular pressure. It is proposed that in this patient the endothelial functional status was so compromised that it could not handle the amount of fluid driven into the cornea by a normal intraocular pressure. A more favorable balance between intraocular pressure and endothelial functional status was obtained by lowering the intraocular pressure with acetazolamide and timolol.

Current status of pressure chamber treatment.

Forty-five patients with unilateral and one patient with bilateral Meniere's disease were exposed to underpressure in a pressure chamber during an acute attack that was primary in 24 and recurrent in 23. The patients were instructed to avoid active equilibration during reduction of the pressure, in order to induce an overpressure in the middle ear. Treatment was promptly followed by improvement of hearing in 20 patients (21 ears) and by a remission for more than three years in seven cases. No side effects were observed. We therefore believe that this procedure may be useful in the prevention of damage to the cochlear hair cells with secondary permanent loss of hearing.

Auditory evoked potentials and auditory behavior following prenatal and perinatal asphyxia in rhesus monkeys.

Two types of asphyxia were studied in monkeys, total asphyxia during mid-pregnancy (94--98 days gestation) and combined partial and total axphyxia at term (165 days gestation). Auditory evoked potentials and the acquisition of 2 auditory discrimination tasks were studied in asphyxiated animals as well as in group of controls. The brains of all asphyxiates were examined histologically. No auditory discrimination deficit was found in the asphyxiated animals; however, the auditory evoked potentials differentiated between control and asphyxiated animals, especially those with verified inferior colliculus damage.

[Simplified multiscreen millimeter-layer ear-tomography-makeshift solution or more? (author's transl)].

The experiences of a simplified multiscreen millimeterlayer ear-tomography technique are reported. The number of patients examined 1969-1978 was 2000 and of these 730 were subjected to microsurgery. The accuracy of the tomographic method was evaluated by a correlation between the preoperative radiographic diagnoses and the peroperative findings. The diagnostic accuracy of multiscreen tomography was much better than expected. The advantages of the method are discussed. It is concluded that multiscreen millimeterlayer eartomography may profitably be used not only in larger departments but also in smaller units.

Hearing improvement in attacks of Meniere's disease treated with pressure chamber.

A method to reduce endolymphatic pressure by exposing patients with Meniere's disease to underpressure has earlier been described. In this work the possible mechanisms are discussed. A material of 36 patients with acute attacks, treated in this way, is now presented. 15 out of these patients experienced a rapid hearing improvement. In 11 cases the remission has lasted for more than 1 year. The method seems to be of great value for those who improved, since at least an early permanent damage to the cochlea can be avoided.

Middle ear mechanics and Eustachian tube function in tympanoplasty.

In 100 patients with chronic otitis media, tubal function and volume was tested before operation. In a postoperative study on hearing and healing there was no positive correlation between these findings and the preoperative results. In 50 patients with healed ear drums, tubal function was retested in a pressure chamber with a flow volume technique. An improvement in Eustachian tube function could be demonstrated. These findings indicate that poor tubal function in chronic otitis media might be secondary to other factors responsible for the disease.

Pulsatile tinnitus and therapeutic embolization.

Fifteen patients with disabling pulse-synchronous tinnitus were investigated with super-selective angiography demonstrating an arteriovenous malformation in 8 cases, chemodectoma of the jugular bulb in 3, and a local arterial stenosis in one case. In 12 of these cases the murmur could be registered objectively, while in the 3 cases with a negative angiographic finding no such murmur could be heard, an observation which may be of importance when selecting patients for further angiographic examination. The cases with a tumour and those with an arteriovenous malformation were all treated with gelatin sponge embolizatioected cases embolization is recommended alone or in combination with surgery.

An improved technic for examining urinary casts and a review of their significance.

Casts represent an important, diagnostic component of urinary sediment, and may signal renal parenchymal disease in asymptomatic individuals. Accurate, precise identification of certain casts may be difficult due to poor visualization by current technics. Casts from freshly voided urine specimens, cytocentrifuged and stained by the Papanicolaou method, can be visualized optimally from permanent slide preparations. The genesis of these casts and their diagnostic potential related to renal disease and acute renal allograft rejection are discussed.

Enhancement of growth of allogeneic mouse tumor by the IgG1 fraction of alloantibody preparations.

Because staphylococcal protein A binds all the known subclasses of mouse IgG except IgG1, ethanol-fixed staphylococci were used as an adsorbent to prepare IgG1 fractions of anti-BALB/c alloantibody-containing globulins and normal globulins of the same strains. The loss of more than 99% of the IgG2 as a result of this adsorption was demonstrated by immunodiffusion. The IgG1 fractions of C3H and CBA anti-BALB/c globulins were tested for their effect on growth of the BALB/c plasmacytomas MOPC-315 and MOPC-460 in C3H and CBA mice by incubation with the tumor cells before transplantation and by injection periodically thereafter into the hosts. With alloantibody-containing globulins that showed slight enhancement of growth of these tumors, or none, the IgG1 preparations caused considerable enhancement of tumor growth. Control preparations of normal C3H or CBA globulins, or IgG1 fractions similarly prepared from the normal globulins, showed no enhancing effect on the growth of these tumors.

Tympanoplasty and eustachian tube function.

In an attempt to assess the value of preoperative tubal testing in the selection of cases most likely to respond favourably to surgery, 100 ears with a central perforation following chronic otitis media were investigated. Tubal function was tested manometrically with different known tests. Postoperatively the healing and hearing results were correlated with the preoperative test results. No positive correlation was found between healing or hearing and tubal function. Postoperatively 50 ears were re-tested in a pressure chamber with a flow volume technique. It was found that the tubal function improves postoperatively in spite of no visible pathological changes in the aural end of the tube. Possible explanations are offered.
