Subcorneal pustular dermatosis (Sneddon-Wilkinson disease) after a diagnostic echogram. Report of two cases.

We reported two cases of subcorneal pustular dermatosis (Sneddon-Wilkinson disease) which developed several hours after performance of an echography. To the best of our knowledge, there are as yet no reports in the literature relating performance of echography with the appearance of this rare disease. The possible explanation of the relation between the dermatologic disorder and the performance of echography is discussed.

Subcorneal pustular dermatosis--an unusual course.

In a patient with subcorneal pustular dermatosis (SPD) the appearance of new pustular lesions characteristic of the disease was triggered by two episodes of drug eruption (induced by dapsone and quinidine sulphate respectively) and by the intra-dermal injection of the recall antigens Candida and streptokinase-streptodornase, performed for evaluation of delayed hypersensitivity. Both episodes of drug eruption were probably the result of an immediate-type hypersensitivity reaction towards the offending drugs, as indicated by positive mast cell degranulation tests.

Comparison of lidocaine and 2-chloroprocaine in paracervical block: clinical effects and drug concentrations in mother and child.

2-Chloroprocaine (CP) has recently been recommended as a less toxic alternative to amide-type local anesthetics due to its rapid metabolism. A double-blind, randomized study comparing CP to lidocaine when used for paracervical block was carried out. Twenty-nine patients received CP, while 31 received lidocaine. None of the 60 mothers developed adverse side effects. Adequate pain relief was achieved in 28 cases in each group, with a mean duration of 40 min regardless of the anesthetic. No change in uterine activity was observed. In the CP group one fetus had mild bradycardia, while two in the lidocaine group had severe, and three mild bradycardia within 5-7 min after the block. Low concentrations of CP were detected in the venous blood of 2 of 29 mothers and in the umbilical venous blood of their babies. Measurable amounts of its metabolite, 2-chloro-4-aminobenzoic acid (CABA), were found in all 13 samples of maternal blood 5 min after PCB and in 6 of 27 maternal samples at birth. Traces of CABA were found in umbilical venous blood in three neonates; in a fourth, a level of 1,000 ng/ml was found. In contrast, unmetabolized lidocaine was found in all maternal samples and in all but one of the cord samples at birth. Concentration of lidocaine in cord blood at delivery ranged from less than 100 to 4,000 ng/ml and were similar for both arterial and venous samples. No correlation could be demonstrated between levels of local anesthetics in the cord samples and the frequency or severity of fetal bradycardia regardless of the anesthetic.

Macrophage migration inhibitory activity in the sera of patients undergoing peritoneal dialysis for psoriasis.

A study of macrophage migration inhibitory activity (MIA) in serum was carried out in four psoriatic patients with normal renal function who were undergoing peritoneal dialysis (PD) as a method of therapy. Before weekly PD treatment, the MIA was negative in these patients. whereas immediately thereafter it was positive. In two of these patients the appearance of MIA paralleled the clinical improvement observed. The increase in MIA observed after each PD was of short duration, usually being followed by a subsequent decrease prior to the next PD. Seven uremic patients who were similarly studied for the presence of MIA before and after PD did not show this pattern of response. It is suggested that the appearance of MIA in psoriatic patients undergoing PD as a method of therapy reflects the extraction of a "dialysable serum factor", which may be associated with cell-mediated immunity as well as with the clearing of skin lesions observed in these patients.

Dialysis therapy for psoriasis. Report of three cases and review of the literature.

Three patients with psoriasis, who were resistant to all modes of accepted therapy, underwent 32 hours of peritoneal dialysis weekly for ten weeks. In two patients, there was a clearing of 80% of the psoriatic lesions after completion of therapy. In one of these patients, there was a recurrence of the lesions two months later, but the other patient is still in relative remission after 12 months. The third patient had clearing of 50% of the lesions, but there was recurrence two months after cessation of treatment. This experience as well as a review of the literature indicates that dialysis may have a positive effect on psoriasis and that the effect obtained is more prominent with peritoneal dialysis than with hemodialysis.

Neonatal thyroid function and anaesthesia.

Thyroid function in the neonate and its response to hexobarbitone anaesthesia were studied in newborn Wistar rats between 24 h and 42 days old. 14C-labelled mixed with unlabelled hexobarbitone was injected i.p. in anaesthetic concentrations. Serum T3, reverse T3 (rT3) and T4 and serum and whole brain hexobarbitone concentrations were measured on wakening. A decrease in serum T3 and rT3 and an increase in T4 concentrations were observed after hexobarbitone at 21 and 30 days. This appears to be a critical period in the neonatal rat. A significant negative correlation was found between the changes in serum thyroid hormone concentrations and whole-brain concentrations of hexobarbitone in 30-day-old male rats. It is suggested that similar effects may occur in the human fetus or neonate subjected to anaesthesia.

The sign of Leser-Trélat in association with lymphocytic lymphoma.

The sign of Leser-Trélat was observed in a patient with multiple seborrheic keratoses and hemangiomata. The increase in these lesions of the skin preceded the development of a poorly differentiated lymphocytic lymphoma, a type of malignancy which has not yet been reported in association with this sign.

HLA system in relation to carbohydrate metabolism in lichen planus.

HLA antigens were determined in a group of fifty-two patients with lichen planus. In the group as a whole the frequency of A28 was increased but not significantly so. When the patients were subdivided according to the results of an oral glucose tolerance test, however, a highly significant increase in the frequency of A28 was found among the non-diabetic patients.
