Psoriasiform eruption induced by propranolol.

The appearance of a psoriasiform eruption in a seventy-eight year old patient after one year of treatment with propranolol is presented herein. The histologic picture was not compatible with psoriasis vulgaris, although it contained some of the same features. Immunologic investigation revealed immune deposits at the junction of the dermis and epidermis and in blood vessel walls as well as monoclonal gammopathy. The rash, which was followed by a reduction in tear secretion, is suggested to have been a drug reaction associated with propranolol. This is supported by the positive results of a migration inhibiting factor test towards propranolol, the clearing of the eruption which took place three weeks after withdrawal of the drug, and the negative result of the same test obtained soon afterwards.

Abnormal glucose tolerance associated with lichen planus.

A study was made of 52 patients with lichen planus. Abnormal oral glucose tolerance was found in 19 (36%), including 5 with overt diabetes. The criteria for abnormality were based on an age-related score method. A family history of diabetes was found to be present in 14 (26%). The most common abnormality observed in the glucose tolerance test was an elevation of the blood glucose level 2 hours after administration of the glucose. These results further support the supposition of a disorder in carbohydrate metabolism associated with lichen planus.

Immunofluorescent and electron-microscope findings in the uninvolved skin of patients with systemic lupus erythematosus.

Specimens of uninvolved skin of 20 patients with systemic lupus erythematosus (SLE) were studied using a direct immunofluorescent technique (IF) and the electron microscope (EM). EM examination revealed deposits of electron-dense material below the basal membrane, among collagen fibers in the dermis and in the walls of dermal blood vessels. The direct IF technique revealed granular "band" deposition of immunoglobulins and complement at the dermo-epidermal junction as well as granular deposition in the walls of blood vessels. There was close correlation between the IF and EM findings. It was concluded that the EM is of particular value in precisely delineating the location and ultrastructural pattern of skin deposits. The IF technique is not limited in relation to the size of area which can be studied and makes it possible to identify the specific immune fractions present. It is suggested that these two methods are mutually complementary in providing information concerning the deposition of immunoglobulins taking place in the uninvolved skin of SLE patients.

Beneficial and detrimental actions of histamine H1- and H2-receptor antagonists in circulatory shock.

This study explores the use of both histamine H(1)- and H(2)-receptor antagonists in two different forms of circulatory shock and suggests that histamine may be involved in more than one way in the pathophysiology of circulatory shock. Various single doses of diphenhydramine, chlorpheniramine, promethazine, and burimamide were administered intravenously to Wistar rats subjected to hemorrhagic or bowel ischemia shock. Cumulative survival and mortality, as well as arterial blood pressures and microhematocrits, were monitored. Pretreatment of the animals with the three different H(1)-receptor antagonists exerted significant protection against both forms of shock. Rats pretreated with the H(2)-receptor antagonist, burimamide, demonstrated an exacerbated mortality after induction of shock. Animals pretreated with H(1)-receptor antagonists showed significantly higher mean arterial blood pressure, greater compensatory rebound of blood pressure after induction of shock, and greater responses to transfusion after hemorrhage than control, shocked animals. Similarly, rats pretreated with the H(1)-receptor blockers demonstrated significantly greater compensatory hemodilution which continued late in shock. In marked contrast, rats pretreated with burimamide exhibited opposite effects after hemorrhage and bowel ischemia, i.e., significant falls in blood pressure, lack of compensatory rebound and response to transfusion of shed blood, and a progressive hemoconcentration. This report clearly demonstrates beneficial actions of histamine H(1)-receptor antagonists and detrimental effects of H(2)-receptor antagonists on survival and other parameters in these forms of circulatory shock.

Serum thyroid hormones changes in patients undergoing Caesarean section under general or regional anaesthesia.

The effects of anaesthesia on serum thyroid hormones were studied in 32 pregnant young women undergoing Caesarean section at term. Eighteen patients received general anaesthesia and 14 lumbar extradural blockade. Maternal serum concentrations of thyrotrophin (TSH), thyroxine (T4), triiodothyronine (T3) and reverse triiodothyronine (rT3) were measured using radioimmunoassay at 0, delivery and 24 h. There were no significant changes in TSH in the two groups. T4 concentrations decreased significantly at 24 h in the general anaesthesia group but regional anaesthesia produced a significant decrease only at the time of delivery. T3 concentrations decreased with both techniques. Reverse T3 increased markedly with general anaesthesia only at 24 h.

Postpartum uterine pressures under halothane or enflurance anesthesia.

Postpartum uterine pressures were measured in healthy women with an intrauterine microballoon before, during, and after administration of different concentrations of halothane or enflurane. Arterial blood samples for anesthetic levels were obtained at intervals. Frequency and intensity of contractions diminished markedly when blood levels exceeded the equivalent of 1/2 MAC (minimum alveolar anesthetic concentration which produces immobility in one-half of subjects exposed to a noxious stimulus) of nonpregnant adults, but normal patterns returned promptly on lightening of anesthesia. Response to 10 mU of oxytocin was suppressed at blood levels corresponding to between 3/4 and 1 MAC of the agents. Halothane and enflurane exert equipotent dose-related reversible effects on the activity of the full-term pregnant human uterus.

Lupus erythematosus profundus (Kaposi-Irgang) with monoclonal gammopathy.

The appearance of subcutaneous nodules of lupus erythematosus profundus on the face and neck of a woman with a long history of lupus erythematosus (LE) is reported. Of note were the three distinct stages in the course of the disease: (1) discoid LE, first appearing at the age of 42; (2) systemic LE, appearing 10 years later; (3) LE profundus, appearing 21 years after the diagnosis of discoid LE was made. The finding of monoclonal gammopathy is of particular interest, although its relationship to the basic disease has not been clarified.

Psuedolymphoma syndrome.

A transient dermatologic abnormality which gave a histologic picture compatible with malignant lymphoma is described. The clinical manifestations included diarrhea, hepatosplenomegaly, moderate lymph node enlargement without histopathological changes and evidence of involvement of the reticulo-endothelial system. Both the dermatologic and gastrointestinal disorders appeared approximately 1 year after discontinuation of treatment with diphenylhydantoin, which the patient had taken for 2 years. The possible association between the transient abnormality and the previous diphenylhydantoin therapy justifies the classification of this case as 'pseudolymphoma syndrome'.
