Effects of percutaneous transluminal coronary angioplasty: intracoronary thrombolysis with urokinase in acute myocardial infarction.

Coronary angiography and percutaneous transluminal coronary angioplasty (PTCA) were performed in 32 patients with evolving acute myocardial infarction. Of the 25 patients with complete occlusion of an infarct-related coronary artery, in 18 (72%) the occluded vessel was successfully opened by an intracoronary infusion of urokinase. With a small dose of urokinase the successful recanalization was achieved in only 25%; with a larger dose it was achieved in 94%. After PTCA, all patients received glucose-insulin-potassium solution for 76 hours. Repeat angiography 42 days later showed a patent coronary artery in 12 (group A) of 18 patients with successful PTCA. In group A, left ventricular ejection fraction increased from 51 +/- 13% to 72 +/- 10% (p less than 0.01) and regional wall shortening from 4.5 +/- 9.5% to 29 +/- 19% (p less than 0.01). In contrast, these variables did not change significantly in patients with unsuccessful PTCA or late reocclusion of an infarct-related vessel (group B). These data suggest that successful PTCA with sustained patency of an infarct-related coronary artery has a beneficial effect on the salvage of the jeopardized myocardium, and glucose-insulin-potassium therapy may enhance the beneficial effect of PTCA.

Tumor detection with carbon-11-labelled amino acids.

A comparative study of tumor detection with ten 11C-labeled amino acids including four newly synthesized amino acids was carried out to find the most valuable 11C-labeled amino acid for the diagnosis of cancer. 11C-L-methionine showed the highest uptake by the experimental rat hepatoma AH109A (2.7% administered dose/g at 20 min, tumor to blood ratio; 11.4). The second highest uptake was of 11C-aminocyclopentane-carboxylic acid (ACPC). The newly synthesized 11C-DL-methyl-ACPC characteristically showed higher accumulation in tumor than in liver and the tumor to liver ratio reached 3.0 at 60 min after injection. It is suggested that 11C-L-methionine and 11C-DL-methyl-ACPC are useful amino acids for the diagnosis of cancer using positron emission tomography.

Spinal evoked potentials in normal Japanese infants and children.

The spinal evoked potentials on right peroneal nerve stimulation were recorded with surface electrodes in 58 normal children of 41 weeks to 15 years old. Bipolar recordings were performed. In all subjects, tri- or quadri -phasic potentials with poorly defined initial positive phases were recorded. The latency of the evoked potentials increased progressively from the lumbar to cervical location. The peripheral conduction velocity from the stimulating cathode in the popliteal fossa to the lumbar recording location increased until 4 or 5 years, and then reached a plateau. Spinal conduction velocity between the lumbar and cervical locations increased until 7 years, and then remained constant.

A case of lissencephaly (agyria) syndrome with apnea--polygraphic study.

A 4-month-old infant with lissencephaly syndrome who was diagnosed on the basis of clinical symptoms and computed tomography experienced four apnea attacks during three polysomnographies. Apnea attacks were of two types. Type I apnea attacks were thought to be sleep apnea. The cause is unknown. Apnea was followed by tonic seizures due to hypoxia. Type II apnea attacks were thought to be epileptic apneic attacks, because they proceeded from the epileptic discharges in the right anterior temporal region (lead of electrooculogram). The origin of Type II apnea attacks was focused in the limbic system.

Participation of 200K or 150K subunit of neurofilament in construction of the filament core with 70K subunit and promotion of tubulin polymerization by incorporated 200K subunit.

We have already reported that neurofilaments are capable of stimulating microtubule assembly and causing gelation. After separation of each of the triplet proteins of neurofilaments it was demonstrated that only the 200K subunit shows the activity to promote tubulin polymerization (Minami, Y. & Sakai, H. (1983) J. Biochem. 94, 2023-2033). The separation of each subunit protein led us to attempt the reconstitution of filaments from the 200K and 70K subunits or from the 150K and 70K subunits. It was found that both the 200K and 150K subunits independently contribute to the formation of intermediate-sized filaments, provided that each subunit was combined with the 70K subunit before removing urea by dialysis for reconstitution. On the other hand, the 200K subunit alone formed a very short thread-like structure after removal of urea, and the 150K subunit formed a filamentous structure, both incapable of being incorporated into filaments made of the 70K subunit alone. These observations suggest that the 200K and 150K subunits are not peripherally attached to a filament core made of 70K protein, but they take part in the formation of the core. Moreover, both proteins can co-polymerize with the 70K protein at a weight ratio of about 1 : 1 at least, which is in excess of that of the intact neurofilament. We investigated whether or not the 200K subunit incorporated with the 70K subunit into filaments could also stimulate tubulin polymerization. Low-shear viscometry measurements suggested that the 200K subunit retains the activity to initiate tubulin polymerization. This was confirmed by measuring viscosity changes with an Ostwald-type viscometer. In contrast, filaments reconstituted from the 70K and 150K proteins were incapable of increasing low-shear viscosity when mixed with tubulin. These observations suggest that the domain of the 200K protein embedded in the core of intermediate-sized filament is separate from the site responsible for promotion of tubulin polymerization.

An 18K protein from ascites hepatoma cell depolymerizes actin filaments rapidly.

Several actin binding proteins were isolated from ascites hepatoma cells AH7974 by DNase I affinity chromatography. Among them, a protein having a molecular weight of 18,000 was further purified by DEAE cellulose and hydroxyapatite column chromatographies and gel filtration on a Sephadex G-75 column. The 18K protein not only inhibits actin polymerization but also depolymerizes actin filaments. This conclusion was supported by viscosity and fluorescence intensity measurements and the DNase I inhibition assay. A chemical cross-linking experiment suggested that the 18K protein binds to monomeric actin and forms and 18K-actin 1:1 complex. The net depolymerization rate by the 18K protein measured by the DNase I inhibition assay was slower than the rapid reduction of the fluorescence intensity of pyrene-labeled F-actin upon addition of the 18K protein. This result suggests that the 18K protein not only binds to monomeric actin but also binds to actin filaments directly. The sedimentation assay showed that a part of the 18K protein was cosedimented with actin filaments. Electron microscopic observations demonstrated that the 18K protein decreased the amount of actin filaments and the remaining filaments appeared to be decorated and distorted by the 18K protein. The 18K protein had no Ca2+ ion sensitivity and exhibited the same effect on both this tumor actin and muscle actin.

Effects of disturbed sleep on contingent negative variation.

To understand better the pathophysiological aspects of disturbed sleep, six healthy male students underwent a series of sleep recordings. On the experimental night, subjects' sleep was disturbed, and the effects on contingent negative variation (CNV) were studied. Mean CNV amplitude decreased in magnitude in the Fz as well as Cz areas; the decrease in Fz-CNV tended to be greater in those subjects whose sleep was most disturbed. The decrease in amplitude of CNV appears to be a nonspecific phenomenon that occurs after any kind of disturbed sleep. Contingent negative variation is composed of two or more components with different spatiotemporal distribution. Depending on the placement of electrodes, the effect that disturbed sleep has on CNV will or will not be observed. The decreases in CNV amplitude were noted in the Fz and Cz areas. Other than subjective discomforts, psychometric measurements did not change significantly. Our data indicate, from the topographical and constructional analysis of CNVs, the possible involvement of the frontal lobe function after disturbed sleep.

X chromosome inactivation in bone marrow cells of adult mice carrying Searle's X-autosome translocation: occurrence of the early-replicating inactive X chromosome.

Using BrdU labelling-acridine orange fluorescence staining and the expression of PGK-1 isozymes, we attempted to clarify the pattern of X chromosome inactivation in bone marrow cells from adult female mice heterozygous for Searle's X-autosome translocation. The asynchronously replicating X chromosome was always the morphologically normal one, and neither of the translocated X chromosomes showed allocyclic behavior. Unexpectedly, a substantial proportion of the allocyclic X chromosome apparently finished replication earlier than any other chromosomes of the cell. This early replicating allocyclic X chromosomes was judged to be as genetically inactive as the late replicating one, since the Pgk-1b allele on the normal X was never expressed in bone marrow cells. Study of karyotypically normal females and females carrying Cattanach's insertion showed that the early replicating X chromosome is not just a feature of the Searle's translocation in the adult bone marrow cells and may be either paternal or maternal in origin. This type of allocyclic X chromosome deserves attention in assessing X chromosome inactivation in female somatic cells.

Angiographic and pathologic evidence of hemorrhage into the myocardium after coronary reperfusion.

Severe myocardial hemorrhage can occur as a potential adverse effect of reperfusion therapy in evolving myocardial infarction. This report describes a 83-year-old man, who showed angiographic evidence of extravasation of contrast medium from the reperfused right coronary artery into the inferoposterior left ventricular wall. At autopsy, severe hemorrhage was transmurally observed in the inferoposterior wall of the left ventricle. The finding of extravasation is a useful angiographic sign of the production of hemorrhage during coronary reperfusion therapy, and great attention should be focused to the existence of this sign to prevent further hemorrhage.

Electrothrombosis of spontaneous carotid-cavernous fistula by copper needle insertion.

Electrothrombosis by copper needle insertion was used to treat three cases of spontaneous carotid-cavernous fistula. These patients had been treated conservatively for 6 months or more, but their symptoms had not improved. Angiography showed that the feeding arteries were branches of both the ipsilateral internal and the ipsilateral external carotid artery. Frontotemporal craniotomy was performed, and copper needles were inserted transdurally into the cavernous sinus. Closure of the fistula was verified by intraoperative angiography in all cases, and the symptoms improved after the operation. However, one patient had postoperative transient 3rd nerve palsy. Another had Gerstmann's syndrome due to narrowing of the internal carotid artery by a needle and worsening of the preoperative 6th nerve palsy. In one case, the operation was uneventful. Based on our experience, we discuss several problems with this technique.

Aster formation in vitro is nucleated by granules isolated from the mitotic apparatus.

Mitotic apparatuses (MAs) isolated from sea urchin eggs contained clusters of granular material in their centrospheres. After cold treatment and mild agitation, the MA fraction formed asters when combined with tubulin. Many microtubules grew from isolated centrospheres most of which were covered with astral residues. Homogenization of the isolated MA fraction dispersed the centrospheres which broke into fragments or into aggregates of small granules that formed small asters when tubulin was added. Electron microscopy showed that more than ten microtubules were nucleated from a granular aggregate composed of several approximately 90-nm granules. The aster-forming activity was lost with time when the MAs were kept at 0 degree C. Only glycerol stabilized this activity. The aster-forming activity also was heat labile and trypsin sensitive, but it was resistant to RNase treatment. When the dispersed MAs were extracted with a buffer solution of high ionic strength, aster-forming activity was recovered only in the extract; that is, when the extract had been dialyzed against a solution of low ionic strength, the fine granules self assembled and retained their aster-forming ability.

[A case of familial breast cancer].

The occurrence of breast cancer in 5 members of a family is reported. No specificity of the location, staging or histological typing of the lesions was observed in these cancers. But the characteristics of "hereditary breast cancer", early onset and the occurrence of metachronous double cancers, were observed. As for heredity, it is suspected to be autosomal dominant trait; on the other hand it is more appropriate to assume a poly genic pattern. Recognizing the importance of familial cancer occurrence, it may be possible to detect early cancerous lesions in family members.

[An autopsy case of intracranial multiple meningiomas associated with extracranial multiple neurinomas].

An autopsy case of intracranial multiple meningiomas associated with extracranial multiple neurinomas was presented. A 55-year-old woman, who had received surgical treatment for right parasagittal meningioma 20 years ago, gradually showed the sign of increased intracranial pressure and disturbance of the lower cranial nerves. Brain CT scan demonstrated numerous intracranial tumors. At autopsy 124 intracranial meningiomas, and neurinomas in her left cervical and right axillar portion were found. Meningiomas showed the transitional and fibroblastic type, histopathologically, and axillary and cervical tumors were the Antoni A type neurinomas. It is suggested that this case had a tendency of heterogenous and multiple origination of tumors.
